1.Influence of Anxiety and Depression in Digestive system Cancer Patients' Prognosis

Objective:To study the effect of anxiety and depression in 46 digestive system cancer patients' prognosis.Methods:The differences of SDS mark?SAS mark and ≥ 50 mark percentage were compared between patients and common mould in nationwide adopting SDS and SAS,moreover the prognosis was compared between varying degrees of anxiety and depression in patients.Results:The SDS mark?SAS mark and ≥ 50 mark percentage in patients were obviously higher than that in common mould in nationwide;the patients with anxiety and depression had bad prognosis.Conclusion:The results suggest that anxiety and deprssion is common feeling in digestive system cancer patients and seriously influence the patients' prognosis,psychotherapy must be conducted and it may contribute to improve the patient's prognosis.

3.The clinical outcomes of nutritional support in children with high nutritional risk

Objective To explore the clinical outcomes of nutritional support in children with high nutritional risk. Methods Improved screening tool for the assessment of malnutrition in pediatrics (STAMP) was used to make nutritional risk score in 1296 cases of consecutively hospitalized patients, and to analysis the effects of nutrition support in clinical outcome. Results In these 1296 hospitalized patients, 379 cases had STAMP score?≥?4 and the detection rate of high nutritional risk was 29 . 24 %. A total of 304 cases were included for further analysis, including 85 cases ( 27 . 96 %) of nutritional support, among whom there were 37 cases of parenteral nutrition (PN), 23 cases of enteral nutrition (EN), 25 cases of combined application of EN and PN. Per capita and daily mean support cost were statistically different among patients with EN, PN and combination of PN and EN (P all?0.001), and EN had least cost. Compared with patients without nutrition support, the infection rate in patients with nutrition support were lower, but there was no statistic difference (P?=?0.095); the treatment fee of infectious diseases in patients without nutrition support was higher than that of patients with nutrition support and the cost of antibiotics was higher as well, and there were statistic differences (P all?0.001). The hospitalization costs in patients without nutrition support were different from that in patients with EN, PN, and PN combined EN (P all?0.001), while the duration of hospital stays were the same (P?=?0.213). Conclusions Improved STAMP is a simple and practical nutritional risk screening tool for pediatric patients; there are differences in the cost among different nutrition support methods. Nutritional support can effectively improve clinical outcomes, especially in reducing infection and reducing the cost for the treatment of infection.
4.Expression and clinical significance of telomerase and hTERT in ovarian neoplasm tissues

Objective To discuss the expression and clinical significance of telomerase and hTERT in ovarian neoplasm tissues . Methods The activities of telomerase and the expression levels of hTERT mRNA were detected in the tissues of malignant ovarian neoplasm ,benign ovarian neoplasm and normal ovarian respectively ,and the results were analyzed .Results Comparing with the tis‐sues of benign ovarian neoplasm and normal ovarian ,the activity of telomerase and the expression level of hTERT mRNA increased in malignant ovarian neoplasm tissues ,and the differences were statistically significant(P< 0 .05) .Conclusion The telomerase plays an important role in the occurrence and development of malignant ovarian neoplasm .The detection of telomerase activity can help in early diagnosis of malignant ovarian neoplasm .
5.Clinical significance of blood lactic acid,B-type brain natriuretic peptide and homocysteine in elderly acute cerebral infarction

Objective To investigate the clinical significance of the combination detection of blood lactic acid,B-type brain natri-uretic peptide(BNP)and homocysteine(Hcy)in elderly acute cerebral infarction.Methods The arterial and venous blood during a-cute onset stage,at 24,48,72 h after thrombolysis and at 12 h after fasting in 85 cases of acute cerebral infarction was collected for detecting blood lactic acid,BNP and Hcy,performing the dynamic monitoring analysis and the comparative observation with the healthy control group.Results The blood lactic acid,BNP and Hcy levels in the acute onset stage group and the recovery group were significantly increased compared with the healthy control group(P <0.05).The blood lactic acid,BNP and Hcy levels during the onset stage in the acute cerebral infarction group were(6.47 ±3.92)mmol/L,(100.52 ±48.96 )pg/mL and(48.96 ±15.13 )μmol/L respectively,which were significantly higher than those at 48,72 h of postoperative recovery,the differences between them were statistically significant(P <0.05).The blood lactic acid,BNP and Hcy levels during postoperative convalescence in the cere-bral infarction group were gradually decreased and declined to the minimal level until postoperative 72 h,which were(2.16±1.83) mmol/L,(33.61±10.42)pg/mL and(18.87±8.27)μmol/L respectively.Conclusion Blood lactic acid,BNP and Hcy are closely related with the occurrence and development of cerebral infarction,the joint detection of these three indicators provide a reliable ba-sis for the early diagnosis of elderly acute cerebral infarction.The active and effective treatment on the patient should be timely a-dopted and the dynamic monitoring should be performed in order to correctly assess the prognosis.
6.Effect of Cx43 Protein on Improvement of Learning and Memory Ability Induced by Enriched Environment in Rats with Traumatic Brain Injury

Objective To explore the effect of Cx43 protein on improvement of learning and memory ability induced by enriched environment (EE) in rats with traumatic brain injury (TBI). Methods TBI model was made by fluid percussion injury (FPI) in Sprague-Dawley rats. The TBI rats were divided into EE group (A), standard housing (ST) group (B), Cx43 specific antisense oligonucleotides (Cx43 ASODN)+EE group (C) and scrambled sequence ODN+EE group (D) with 6 rats in each group. Another 6 normal rats were taken as the control group. Groups C and D were given hippocampal microinjection of Cx43-ASODN (2 μl/d/rat, 1.5 mmol/L) and ScrbASODN (2 μl/d/ rat, 1.5 mmol/L) respectively. Morris water maze was used to evaluated the learning and memory ability. Results The latency was longer and the traversing times was less in group B than in the control group (P<0.05). The latency was shorter in group A than in group B (P<0.05), and there was no significant difference between group A and the control group (P>0.05) from the 9th day after injury. The traversing times was more in group A than in group B and there was no significant difference between group A and the control group (P>0.05). The latency was longer and the traversing times was less in group C than in group A (P<0.05). Conclusion Cx43 protein may participate in the improvement of the learning and memory ability induced by EE in rats with TBI.

8.Effect of glucagon-like peptide-1 agonist on insulin signaling pathway in skeletal muscle of mice with insulin resistance

Objective To study the effect of glucagon-like peptide-1 agonist(exendin-4)on insulin signaling pathway in skeletal muscle of mice with insulin resistance(IR). Methods Thirty male KM mice were divided into normal control group(NC group, n=10)and IR group(n=20). The mice in NC group and IR group were fed with routine diet and high-fat diet for 16 weeks, respectively. After IR models were successfully established, 10 mice in IR group were treated with exendin-4(Ex group, 3μg·kg-1·d-1)for 4 weeks. NC group and IR group were given the same dose of normal saline. Body weight, fasting blood glucose(FBG), and fasting insulin(FINS)were measured before and after the intervention, and homeostasis model assessment for insulin resistance ( HOMA-IR ) was calculated . The muscle glycogen content were measured by Periodic Acid-Schiff Stain ( PAS ) . The expressions of insulinreceptor(IRC), insulin receptor substrate-1(IRS-1), and phosphatidylinositol 3-kinase(PI3K)in the skeletal muscle were measured by Western blot and immunohistochemistry. Results Before intervention, the body weight, FBG, and HOMA-IR of IR group and Ex group were higher compared with NC group(all P<0. 05), but FINS did not rise significantly(P>0. 05). Compared with before intervention, the body weight of three groups after intervention were all increased, but increased more significantly in the IR group. After intervention, the FBG and HOMA-IR in Ex group decreased, compared with IR group and Ex group of non-intervention (all P<0. 05), but FINS did not decrease significantly(P>0. 05). The skeletal muscle cells in NC control were colored extensively and more colored granules, while IR group were less colored, and Ex group were between NC group and IR group. Compared with NC group, the expressions of IRC, IRS-1, and PI3K in the skeletal muscle of IR group were significantly decreased(P<0. 05), not in Ex group(P>0. 05). Compared with IR group, the expressions of IRC, IRS-1 and PI3K in Ex group were higher(P<0. 05). Conclusions Exendin-4 improves insulin resistance by up-regulating the expressions of IRC, IRS-1, and PI3K in the skeletal muscle and promoting the synthesis of muscle glycogen.
9.Clinical Observation of Tripterygium Glycosides Combined with Valsartan in the Treatment of Idiopathic Membranous Nephropathy

OBJECTIVE:To observe therapeutic efficacy and safety of tripterygium glycosides combined with valsartan in the treatment of idiopathic membranous nephropathy(IMN). METHODS:A total of 85 patients with IMN were enrolled and randomly divided into observation group(42 cases)and control group(43 cases). Based on routine treatment(reducing lipid,anti-platelet ag-gregation,protein dietary restriction),control group was additionally given oral valsartan,with initial dose of 80 mg/d,and gradu-ally adding the amount to 160 mg/d if there was no obvious ADR;based on control group,observation group was additionally giv-en Tripterygium glycosides tablet 40 mg,tid,and reducing the amount to 20 mg,tid after complete remission. 2 groups were treat-ed for 12 months. RESULTS:Total effective rate of observation group was 92.86%,which was significantly higher than that of con-trol group(74.42%),with statistical significance(P<0.05). Compared with before treatment,the levels of 24 h urine protein,TC and TG of 2 groups were decreased after treatment,and serum albumin(ALB)of observation group increased;24 h urine protein and TG of observation group was lower than control group,while the serum level of ALB was higher than control group,with sta-tistical significance(P<0.05 or P<0.01). No obvious ADR was found in 2 groups. CONCLUSIONS:Tripterygium glycosides com-bined with valsartan can improve the kidney function and blood lipid level of IMN patients with high safety.
10.Lung cancer Ⅳ recipe combined with chemotherapy in treating non-small cell lung cancer in the elderly: a case-control study

Objective To explore the therapeutic effect of lung cancer Ⅳ recipe combined with chemotherapy on treating middle and advanced stages of non-small cell lung cancer (NSCLC) in the elderly.Methods A total of 77 NSCLC patients in middle and advanced stages aged 70 years and over who had heat-toxin syndrome in traditional Chinese medicine (TCM) were selected from Apr.2013 to Oct.2014 in our hospital.Patients were randomly divided into chemotherapy group (n=39) and chemotherapy + herbs group (n=38).Karnofsky assessment scale (KPS),effective rate,symptom and sign in traditional Chinese medicine,changes in the levels of high-sensitivity c-reactive protein (hs-CRP),carcino embryonic antigen (CEA),neuron specific enolase (NSE),tumor special growth factor (TSGF) were observed before and 1 and 56 days after treatment.Results There was no significant difference in effective rate between the two groups (P>0.05).The KPS and symptom and sign in TCM were improved significantly in chemotherapy + herbs group as compared with chemotherapy group (36.7 % vs.26.5 %,80.0 % vs.58.8 %,respectively (x2 =2.721,6.731,both P<0.05).There were no significant differences in hs-CRP,NSE,CEA,TSGF levels between two groups before and after treatment (P>0.05),but hs-CRP level in both groups was decreased after treatment as compared to pre-treatment and the decvease was more obvious in chemotherapy + herbs group (51.47% vs.23.77%,x2 =19.713,P<0.05).Conclusions Lung cancer Ⅳ recipe can improve the main clinical symptoms,quality of life and tolerance to chemotherapy in NSCLC patients with heat-toxin manifestation in TCM.There is a correlation between heat-toxin syndrome and hsCRP level in NSCLC patients,which shows that hs-CRP probably is the material basis of the heattoxin syndrome in TCM.
