Chinese Journal of Tissue Engineering Research 2006;10(10):184-185
BACKGROUND: The application of apparent diffusion coefficient (ADC)value to the diffusion-weighted image in the diagnosis of cerebral ischemic diseases is confirmed in clinic. The influence of different diffuse gradient direction in different lesions of human brain on diffusion-weighted image has become one of the focus researches of neuroradiology.OBJECTIVE: To explore the changing characters of the ADC value on diffusion weighted imaging with different diffuse gradient directions of human corpora striatum, parietal cortex and corona radiata.DESIGN: A sample survey.SETTING: Medical Imaging Center, the First Affiliated Hospital of Jinan University; Institute of Biomedical Engineering, School of Life Science and Technology, Jinan Univeristy.PARTICIPANTS: Sixty healthy volunteers (37 males and 23 females)aged 18-65 years were selected from the Huaqiao Hospital affiliated to Jinan University between March 2000 and March 2001.METHODS: The 60 volunteers received brain diffusion-weighted MR imaging in different directions (left-right, anterior-posterior, upper-down),and the ADC values of corpus striatum, parietal cortex and corona radiata were measured.MAIN OUTCOME MEASURES: The ADC values of corpus striatum, parietal cortex and corona radiata were measured in the 60 healthy volunteers.RESULTS: All the 60 healthy volunteers entered the analysis of results. There were no significant differences between the ADC value of corpus striatum and that of parietal cortex in different directions; the mean ADC values of corona radiata in left-right, anterior-posterior, upper-down axial were (5.0±0.5), (4.2±0.6), (15.2±1.4)×104 mrn2/s, and there were significant differences between the former two and the latter (P < 0.01).CONCLUSION: Diffuse gradient directions do not affect the ADC values of corpus striatum and parietal cortex, but the ADC value of corona radiata is different. We should delineate the diffusion direction if we want to do quantitative studies on the diffusion of corona radiata.

Objective By observing the clinical efficacy of acupuncture plusBu Shen Jie Yu Qing Xin decoction (herbal decoction for tonifying kidney, anti-depression and clearing heart fire) in treating perimenopausal depression, to discuss about the effect of acupuncture plus medication on Hamilton Depression Scale (HAMD), Self-rating Depression Scale (SDS) and hypothalamic-pituitary-ovarian (H-P-O) axis in perimenopausal depression patients.Method By following a completely randomized design, sixty perimenopausal depression patients were divided into a Chinese medication group (Bu Shen Jie Yu Qing Xin decoction) with 30 cases, and an acupuncture-medication group (acupuncture plusBu Shen Jie Yu Qing Xin decoction) with 30 cases, to receive 12-week treatment in total. The HAMD and SDS were estimated prior to the treatment and in the 4th, 8th and 12th weeks; the serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), testosterone (T), prolactin (PRL), and progesterone (P) contents were detected before and after the treatment.Result Acupuncture plus medication and Chinese medication alone were both effective in treating perimenopausal depression, and the therapeutic efficacy of acupuncture plus medication was superior to that of Chinese medication alone (P<0.05). In the acupuncture-medication group, the HAMD score dropped significantly in the 4th, 8th and 12th weeks compared to that before the treatment, and the score dropped significantly in the 8th and 12th weeks compared to that before the treatment in the Chinese medication group (P<0.01). The HAMD score in the acupuncture-medication group was significantly lower than that in the Chinese medication group in the 4th, 8th and 12th weeks (P<0.01). After the treatment, the items including anxiety, insight, sleep, and despair were significantly changed in the Chinese medication group (P<0.05); 7 items were significantly changed in the acupuncture-medication group (P<0.05). The scores of anxiety, insight, sleep, diurnal-nocturnal, obstruction, and body weight in the acupuncture-medication group were significantly superior to those in the Chinese medication group (P<0.05). The SDS score dropped significantly in the 4th, 8th and 12th weeks in the acupuncture-medication group, and in the 8th and 12th weeks in the Chinese medication group (P<0.01,P<0.05); in the 4th and 8th weeks, the SDS score in the acupuncture-medication group was significantly lower than that in the Chinese medication group (P<0.05), while the between-group difference was statistically insignificant in the 12th week (P>0.05). After 12-week treatment, the contents of FSH and LH declined significantly in both groups (P<0.01, P<0.05), and the contents of E2 and T increased significantly in the acupuncture-medication group (P<0.01,P<0.05). The decrease of FSH and LH, and the increase of E2 were more significant in the acupuncture-medication group (P<0.05,P<0.01).Conclusion Acupuncture plus medication can improve the symptoms of perimenopausal depression, and it takes a faster action than Chinese medication alone. Acupuncture plus medication can mitigate 7 items in HAMD, while Chinese medication can improve anxiety, insight, sleep and despair. Acupuncture plus medication can produce a more significant efficacy in improving anxiety, insight, sleep, diurnal-nocturnal, obstruction, and body weight compared to Chinese medication. For perimenopausal depression, acupuncture plus medication can correct the endocrine disorder and improve depression state by down-regulating the levels of FSH and LH, and up-regulating the contents of E2 and T to improve the function of H-P-O axis.

Chinese Journal of Digestive Surgery 2009;8(3):190-192
Objective To evaluate the long-term efficacy of modified loop choledochojejunostomy (MLC). Methods The clinical data of 259 patients who had underwent choledochojejunostomy in First Affiliated Hospital of Sun Yat-Sen University from January 2000 to December 2006 were retrospectively analyzed. Of all the patients, 130 underwent MLC (MLC group) and 129 underwent Roux-en-Y choledochojejunostemy (RYC, RYC group). The changes in incidence of cholangitis and liver function between the 2 groups were compared. All the data were analyzed by t test, chi-square test or Fisher exact probability. Results The levels of alaninetransa-minase and alkaline phosphomonoesterase were (63±42) U/L and (147±147) U/L in MLC group, and (84±52)U/L and (256±201)U/L in RYC group, with statistical difference between the 2 groups (t=1.634, 1.655, P>0.05). The level of gamma-glutamyl transferase in MLC group was (116±91)U/L, which was signifieandy lower than (169±96)U/L in RYC group (t=2.461, P<0.05). Three patients (2.3%) in MLC group and 9 (7.0%) in RYC group suffered from acute cholangitis after operation, with no statistical difference in the incidence between the 2 groups (P>0.05). Of the 12 patients with acute cholangids, 1 in MLC group and 7 in RYC group were hospitalized, with statistical difference between the 2 groups (P<0.05). Conclusions The incidence of acute cholangitis in patients who underwent MLC is comparable to that of RYC. However, the procedure of MLC is simpler than RYC, and patients have milder symptom and lesser frequency of reflux cholangitis onset after MLC.

International Eye Science 2005;5(6):1107-1109
AIM: to assess the effectiveness of a combined procedure ( pars plana vitrectomy with temporary keratoprosthesis, vitreoretinal surgery, and penetrating keratoplasty). in the complicated cases and the risk factors for the surgical failure.METHODS: Restrospectively reviewed charts of patients who underwent penetrating keratoplasty in combination with vitreoretinal surgery between 1990 and 2005, with a follow-up of 3mo to 9a. Analysis was focused on ocular history, indications for surgery, visual acuity (VA), anatomic results, and complications.RESULTS: 18 eyes had light perception or VA of hand motions only. The best-corrected VA improved during the first 3mo, increased in 72.2% of all eyes, remained unchanged in 27.78%, and no decreased. In 3 of 18 eyes (16.67%), VA was better than finger counting and hand motions, and nine eyes(50%) showed useful vision (0.05) postoperatively. 10 eyes showed a clear corneal graft (55.56%). 2 eyes needs the second keratoplasty, Bullous corneal edema was evident in 3 eyes, band keratopathy was evident in a 3 eyes. 10 patients were observed for more than 2a;6 had a clear graft (60%). Two eyes (11.11%) had silicone oil-corneal endothelium contact and all of these grafts failed.CONCLUSION: Although the functional outcome of a combined procedure is limited by primary and secondary tissue destruction, preserving ambulatory vision is possible and thus improves the quality of life, at least in patients with single remaining eyes.
