1.Exploration on developing the awareness of anesthesia quality control in interns

Anesthesia quality control is essential to guarantee the safety of patients undergoing surgeries.Emphasis should be placed on developing the interns' awareness of anesthesia quality control.The interns' awareness of anesthesia quality could be preliminary formed by strengthening the orientation,training standard and proficient anesthesia skills,consolidating theoretical knowledge,cultivating professional responsibility as well as excellent psychological quality,which also enable interns to be perfect anesthesiologists in the future.
2.Mutiple Primary Malignant Tumor.

Though the occurrence of multiple primary malignant tumor is a rare finding but the reported cases of it has increased in recent years. We collected multiple primary cancer of different organ, tissue and the multicentric origin of bilaterally paired organs. This paper reports 6 cases of multiple primary malignant tumors which were experienced at Yeungnam university hospital in Taegu during the past 2 years with review of journals. The results were as follows. 1. The incidence of multiple primary cancer was 0.31% for 2 years (1987-1988). 2. The ratio between male and female was 1:1 and mean age of incidence was 54.1 years. 3. The ratio between synchronous and metachronous (interval more than 6 months) was 1:1. 4. The time interval between first and second cancer was average 2.7 years in metachronous cases. 5. The most frequent involved organ was stomach, breast and colon in order of frequency. 6. The incidence of familial cancer associations was found in one out of 6 cases. 7. The test of DNCB, multitest CMI and ratio of T4 to T8 were performed in 4 cases but there was no definitive evidence of abnormality. We concluded that every effort should be made to discover the presence of synchronous malignancies in the patients who are being treated for a known tumor, and also special care should be given to detect new metachronous lesions is required.

3.Infection status of Clonorchis sinensis in residents of Hamyang-gun, Gyeongsangnam-do, Korea.

Oriental liver fluke (Clonorchis sinensis) infection was surveyed among residents of Hamyang-gun, Gyeongsangnam-do, Korea during the period of January 2001 to March 2002. Total 1,041 stool samples were collected from residents who visited Public Health Center and its branches in Hamyang-gun and examined using formalin-ether sedimentation method. The overall egg positive rate was 16%, male showing higher positive rate (21%) than female (10%). The age group of 30 to 50 years had the highest egg positive rate of C. sinensis from 20% to 22%. The positive examinees were treated with praziquantel and educated individually to prevent reinfection. Egg positive rate in this area was decreased when compared with results recorded in the past, however, still remained more than 10%. This study suggests that periodic examination, treatment as well as education of residents should be continued and systematized.

5.Exploratory application of formative assessment in internship education for clinical anesthesia

The formative assessment system has been applied to the internship education for the clinical anesthesia with the aim to improve students' initiative and to evaluate their outcomes more compre-hensively. The students' performance in the shift exchange, case discussion, raising question, solving question at the time points of after the preclinical train, one month and 3 months into the anesthesia internship, and after the completion of internship, and their capability in preoperative patient assessment, condition report, clinical practice, review writing have been evaluated to determine the educational quality and to instruct the improvement of educational approach. Assess process takes into account both the individuality and the gen-eral character of the students and feedbacks the evaluation result to improve the practice teaching The im-plementation of the evaluation can promote students' autonomous learning and comprehensively evaluate students' practice process.
6.Clinical observation of cinobufacini injection used to treat moderate and advanced primary liver cancer

OBJECTIVE: To observe the clinical effect of cinobufacini injection in treating moderate and advanced primary liver cancer (PLC). METHODS: One hundred patients with moderate and advanced PLC were randomly divided into cino-treated group (50 patients) and control group (50 patients). The quality of life, tumor size, some changes of laboratory tests, and survival time were observed. RESULTS: The progressive rate of cino-treated group (18%) was lower than that of the control group (32%). The quality of life of the cino-treated group (80%) was better than that of the control group (72%), but without statistical significance. The survival rate of >12 months of the cino-treated group (30%) was higher than that of the control group (18%). The patients' liver function such as serum total bilirubin and ALT decreased obviously in the cino-treated group while increased a lot in the control group. The level of AFP increased after treatment with statistical significance in the control group while there was no statistical significance in the cino-treated group. CONCLUSION: Cinobufacini injection can not only inhibit the proliferation of cancer, but also protect liver function, improve quality of life and prolong survival time.
7.X-irradiation of contusion site in rats with spinal cord injury improves the recovery of locomotor function

Objective To determine whether the X-irradiation of the contusion site can improve the recovery of locomotor function following spinal cord injury in rats. Methods The spinal cords of 70 male Wistar rats at the level of T10 were contused with a weight-drop apparatus similar to the NYU impactor. Ten rats were chosen as control group. The other 60 rats were equally randomized into 6 groups and localized X-irradiation (20 Gy) of the contusion site was performed on rats in these 6 group 20 min, 1,2,4, 7 and 17 d after the injury, respectively. Locomotor function was determined by the Basso, Beattie and Bresnahan Locomotor Rating Scale (BBB). Following 6 w of behavioral evaluation, the spinal cords were received histological examination. Results X-irradiation of the contusion site 20 min, 1 and 2 d after the injury significantly improved the recovery of locomotor function as compared with no X-irradiation (control group, P<0.05); and the improvement in the 2nd and 3rd w were the quickest among all the time points. Increased amount of spared spinal cord tissue in the X-irradiation groups was found as compared with that in the control group. Conclusion X-irradiation of the contusion site following contusion at an early stage can protect the spared spinal cord tissue and help the recovery of locomotor function.
8.The Results of Laparoscopic Cholecystectomy in Acute Cholecystitis.

BACKGROUND: The laparoscopic cholecystectomy (LC) has been accepted as the procedure of choice for chronic cholecystitis. However in cases of acute cholecystitis, the safety and the efficacy of LC has not been fully determined. Thus we performed this study to assess the clinical outcomes of a LC for acute cholecystitis to evaluate it's efficacy and safety. METHODS: The authors retrospectively analyzed 1,164 LCs performed in Yeungnam University Hospital from May 1991 to March 1996. Among the 1,164 LCs, 118 were performed for acute cholecystitis and 1,046 were performed for chronic cholecystitis. The authors compared the mean operation time, the conversion rate to an open cholecystectomy (OC), the reasons for conversion, the complication rate, the postoperative hospital stay, and the postoperative use of analgesics between the patients with acute cholecystitis and the patients with chronic cholecystitis. RESULTS: In the 1046 patients with chronic cholecystitis, the mean operation time was 55.9 minutes, the conversion rate to an OC was 2.7%, the complication rate was 4.0%, the mean postoperative hospital stay was 3.3 days, and analgesics were used in 48% of the patients. However in the 118 patients with acute cholecystitis, the mean operation time was 65 minutes, the conversion rate to an OC was 11.8%, the complication rate was 16.9%, the mean hospital stay was 4.4 days, and analgesics were used in the 60% of the patients. Also the authors found that the longer duration of preoperative symptoms and an advanced state of inflammation (e.g., GB empyema or gangrenous changes) were the two most common causes of conversion to an OC in the case of acute cholecystitis. Although all the analyzed parameters (especially, the conversion rate and the complication rate) were higher in the patients with acute cholecystitis than they were in the patients with chronic cholecystitis, a LC for acute cholecystitis seems to be acceptable because there were no mortalities and there were no life threatening complications. CONCLUSIONS: From, the aspects of safety and efficacy, a LC can be performed in most patients with acute cholecystitis. However, it should be remembered that the prolonged duration of symptoms prior to a LC increases the conversion rate to O.C. and if we confront the advanced cholecystitis (GB empyem or gangrenous change) with difficult Calot's triangle during a L.C., early conversion to an OC should be considered.

9.A Case of Actinomycosis of the Maxillary Sinus Accompanied with Fungal Ball.

Actinomycosis of the paranasal sinuses is rare and usually arises from the oral cavity after a dental procedure or trauma. Actinomycosis of the maxillary sinus is very rare and found only in isolated case reports. We report the case of a 62-year-old patient with actinomycosis accompanied by a fungal ball in the maxillary sinus.
