3.Design and manufacture of hypobaric cabin simulator for experimental animals

Objective To design and manufacture a hypobaric cabin simulator for experimental animals.Methods The simulator was composed of a fully transparent working chamber,an electrical chamber,a pressure control component and a program control component,which had its body made of polymethyl methacrylate plastics,the hatch made of metal plate and support by reinforced metal bar.The air inside chamber was exhausted by the diaphragm vacuum pump,the air inflow was controlled by proportional valve,and the inner pressure and the speed of rise and decline were exactly controlled by the balance of inflow and exhaust.Results The new type of hypobaric cabin simulator had the characteristics of accurate pressure control,low fluctuation range and controllable up and down time,low noise as well as adaptable air exchange rate.Conclusion The new type of hypobaric cabin simulator matches the national standard for experimental animals,and can be used in making the animal model of high altitude diseases.
4.Detection of gene mutation in glucose-6-phosphate dehydrogenase deficiency by RT-PCR sequencing.

OBJECTIVESince glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common hereditary hemolytic erythrocyte enzyme deficiency, most cases have single nucleotide mutations in the coding region, and current test methods for gene mutation have some missed detections, this study aimed to investigate the feasibility of RT-PCR sequencing in the detection of gene mutation in G6PD deficiency.
METHODSAccording to the G6PD/6GPD ratio, 195 children with anemia of unknown cause or who underwent physical examination between August 2013 and July 2014 were classified into G6PD-deficiency group with 130 children (G6PD/6GPD ratio <1.00) and control group with 65 children (G6PD/6GPD ratio≥1.00). The primer design and PCR amplification conditions were optimized, and RT-PCR sequencing was used to analyze the complete coding sequence and verify the genomic DNA sequence in the two groups.
RESULTSIn the G6PD-deficiency group, the detection rate of gene mutation was 100% and 13 missense mutations were detected, including one new mutation. In the control group, no missense mutation was detected in 28 boys; 13 heterozygous missense mutations, 1 homozygous same-sense mutation (C1191T) which had not been reported in China and abroad, and 14 single nucleotide polymorphisms of C1311T were detected in 37 girls. The control group showed a high rate of missed detection of G6PD deficiency (carriers) in the specimens from girls (35%, 13/37).
CONCLUSIONSRT-PCR sequencing has a high detection rate of G6PD gene mutation and a certain value in clinical diagnosis of G6PD deficiency.

5.Influence of continuous cropping years on yield and active compounds in Salvia miltiorrhiza f. alba.

Biological characteristic of Salvia miltiorrhiza f. alba in field was studied. HPLC method was used to determine the lipophilic constituents (dihydrotanshinone, cryptotanshinone, tanshinone, tanshinone II (A) and miltione) and hydrophilic constituents (salvianolic acid, rosemarinic acid). The results showed that the fresh weight of S. miltiorrhiza f. alba which cropped for 2 years was decreased by 80.47%, while dry weight decreased by 79.42%. The normal diameter of the root was 0.3-0.5 cm, however, the diameter was 0.2-0.4 cm after 2 years, it was said that the decrease of the root diameter was the main reason for the decrease of the yield. The average contents of dihydrotanshinone, cryptotanshinone, tanshinone, tanshinone II (A), miltione, salvianolic acid and rosemarinic acid were decreased by 35.26%, 32.26%, 19.35%, 3.39%, 64.40%, 66.93% in plant which continuously cropped for 2 years, respectively. The yield and active constituents were mostly effected in the plant of S. miltiorrhiza f. alba, which continuously cropped for 2 years.

6.Urinary tract infections in the neonatal intensive care unit: clinical analysis of 229 cases.

OBJECTIVETo study the clinical features, distribution of pathogens, drug susceptibility, and treatment effectiveness in neonates with urinary tract infection (UTI) and admitted to the neonatal intensive care unit (NICU).
METHODSThe clinical data of 229 neonates who developed UTI during their stay in the NICU were retrospectively studied.
RESULTSThe main clinical manifestations of these children included fever/irregular body temperature, refusing to milk feeding, jaundice, vomiting, diarrhea, poor weight gain, and lethargy. The top three pathogens were Escherichia coli, Enterococcus feces, and Klebsiella pneumoniae. Escherichia coli and Klebsiella pneumoniae were highly resistant to ampicillin and most cephalosporins (≥ 85%), and were highly sensitive to imipenem (100%), meropenem (100%), cefoperazone/sulbactam and piperacillin/tazobactam (>90%). Enterococcus feces were highly resistant to penicillin (100%), rifampicin (84%) and gentamicin (79%), but were sensitive to vancomycin.
CONCLUSIONSThe clinical manifestations of neonatal UTI are often atypical and manifested as systemic symptoms. The main pathogenic bacterium is Escherichia coli, and the isolation rate of enterococci can also be high. Most pathogenic bacteria are resistant to penicillin and cephalosporins, and therefore decision-making on drug administration must be based on the results of drug sensitivity tests.

7.Surgical treatment of Siewert II adenocarcinoma of the esophagogastric junction.

OBJECTIVETo explore the outcomes after surgical treatment of esophagogastric junction carcinoma (EGJC).
METHODSOne hundred and eighty-five patients with EGJC undergoing surgery from October 2000 to September 2006 at the Cancer Hospital of Shantou University were reviewed retrospectively. The clinical outcomes were compared between transthoracic and transabdominal approach.
RESULTSOf the 185 patients, 133 underwent operation via transthoracic approach and 52 via transabdominal approach. The postoperative complication rates were 10.5%(14/133) and 11.5%(6/52) and the 1-, 3-, 5-year overall survival rates were 83.9%, 44.5%, 32.9% and 86.0%, 38.0%, 30.0% in transthoracic and transabdominal groups respectively, and the difference were not statistically significant (both P>0.05).
CONCLUSIONSurgical approach should be individualized for EGCJ.

8.Voluntary HIV counseling and testing among peacekeepers in Liberia: acceptability and influential factors.

OBJECTIVETo investigate the acceptability and influential factors of Voluntary HIV Counseling and Testing (VCT) among the Chinese and Ethiopian peacekeepers in Liberia so as to conduct more effective health education of AIDS and enhance the acceptance of VCT.
METHODSAn anonymous questionnaire was used to investigate the attitudes to VCT among 518 Chinese (aged 23-51) and 648 Ethiopian (aged 25-49) peacekeepers in Liberia.
RESULTSThe rates of those who claimed to have never heard of HIV/AIDS were 8.1% and 7.9% in the Chinese and Ethiopian subjects respectively, with no significant differences (P > 0.05) in between. Among those who had heard of HIV/ AIDS, 407 (85.5%) Chinese and 449 (75.2%) Ethiopians expressed their willingness to accept VCT, with a significant difference (P < 0.05) between the two groups. Statistic analysis indicated that the acceptance of VCT was as- sodiated with schooling and knowledge on HIV/AIDS in both groups as well as with the susceptibility to HIV among the Ethiopian subjects.
CONCLUSIONThe acceptability of VCT is obviously lower among those with less schooling and less knowledge of HIV/AIDS. It is important to carry out information-oriented education to increase VCT acceptability among the peacekeepers in the area with a high incidence of AIDS.

9.Clinical study of osteotome sinus floor elevation without grafting and with simultaneous implants placement.

OBJECTIVETo investigate the clinical results of osteotome sinus floor elevation (OSFE) without grafting combined with simultaneous implant placement.
METHODSA total of 65 patients underwent maxillary sinus floor elevation from alveolus without any bone grafting from January 2000 to December 2008 and 96 implants were placed in the maxillary posterior edentulous region simultaneously. Clinical and radiography examinations were performed. The residual bone height ranged from 5 to 8 mm and the mean bone height was (6.78 ± 1.04) mm. The mean following period was 33.4 months. Statistical analysis was performed by chi square test.
RESULTSNinety-five of 96 implants were clinically stable and functioned without any pain and other complaints. One implant was extracted 15 days after operation because of mobility and the other implants obtained osseointegration. The mean implant protrusion length was 2.6 mm, ranging from 1 to 5 mm. Different degree of new bone formation was observed in 51 (54%) of implants. New maxillary sinus floor outline was observed in 33 (35%) of implants and there was no obvious new bone in 11 (12%) of implants. There was no significant deference between the implant protrusion length and sinus floor remodeling.
CONCLUSIONSUnder strict indications, the clinical results of OSFE without bone grafting combined with simultaneous implant placement were predictable in short term. The new sinus floor formation was not related to the implant protrusion length.

10.Effects of feeding intervention on development of eczema in atopy high-risk infants: an 18-month follow-up study.

OBJECTIVETo assess the preventive effects of different dietary regimens on development of eczema and food allergy in infants at high-risk for allergy.
METHODSForty-six infants whose parents were atopic and umbilical cord IgE > 0.35 kU/L were enrolled in the study. The infants were randomly assigned at birth to one of 2 dietary regimen protocols: those in intervention group (23 cases) were breast fed till more than 4 months of age, then followed by feeding with partially hydrolyzed formula (pHF), combined solid foods avoidance until 4-month of age, egg, fish, shrimp avoidance until 12-month of age. The other 23 cases in non-intervention group were breast fed for less than 4 months, or bottle fed with cow's milk-based formula, egg yolk was introduced at 4-month of age, and egg white at 6-month of age, besides, no any other dietary avoidance was applied. All the infants were followed-up for 18 months. The primary end point was the presence of atopic eczema. Food allergy was detected by fresh food prick-to-prick tests or in vitro sIgE or Fx5E.
RESULTSAt 6 months, 12 months and 18 months, the incidence of eczema in intervention group was 4.3% (1/23), 8.7% (2/23), and 17.4% (4/23), respectively, which was significantly reduced as compared to that of the non-intervention group, which was 26.1% (6/23), 34.8% (8/23), and 39.1% (9/23), respectively. Food allergy was found in 13.0% (3/23) of intervention group and 34.8% (9/23) of non-intervention group by skin prick tests or sIgE. Egg white was the most common offending food.
CONCLUSIONEarly life dietary interventions which included breastfeeding, delayed solid food introducing, pHF feeding, and high risk food avoidance could reduce the risk of atopic eczema and food allergy development, and was probably an effective primary intervention method for infants at high risk for atopy.
