Objective To explore diagnosis,treatment and prognosis of intestinal atresia(IA) in neonates,so as to increase the healing rate.Methods Surgical treatment was given to 29 cases of congenital IA from Apr.2004 to Nov.2008.Comprehensive analysis was carried out on clinical manifestations and flat X-ray.All cases were with vomiting,15 cases were with normal fetal faeces.Stomach and intestinal atresia were observed in the upper abdomen in 19 cases,fluid levels in generous stomach and expended intestines were seen in 21 cases by X-ray.All children underwent operation,including ileum atresia(16 cases),duodenal atresia(7 cases),and jejunal atresia(6 cases).The types of atresia were type Ⅰ(9 cases),type Ⅱ(9 cases),type Ⅲa(9 cases),type Ⅲb(1 case) and type Ⅳ(1 case).Nine cases were treated with septum resection and then anastomosed longitudinally,3 cases underwent end-to-end anastomose and 17 underwent end-to-back anastomose.All cases were served in intravenous hyperalimentation.Results All cases lived postoperatively,and 1 child gave up for postoperatively treatment.Twenty-eight cases were cured.There was 1 case of obstruction of anastomatic,and was cured by non-operatively treatment.There was no anastomatic leakage and wound disruption.Twenty-six cases were followed up for 3 months and up to 4 years.These symptoms had no relapse.Conclusion Early diagnosis,appropriate surgical procedure and surrounding operation management are crucial for improving the cure rate.

Objective To evaluate the value of cholangiography for early diagnosis of ischemic-type biliary lasions(ITBL) after liver transplantation. Methods Two hundred and fifty-three patients with liver transplantaion between Jan 2004 and Oct 2006 were recruited. Initial cholangiography was compared with terminal cholangiography to evaluate the value of initial cholangiography of ITBL. The t test, Chi-square test, sum rank test were used for statistics. Results Based on initial cholangiography, 189 patients were diagnosed with normal appearance, while 64 patients were diagnosed with abnormal appearance. The abnormal initial cholangiography appearances included poor filling in 33 patients and irregularity in 31 patients. Based on terminal cholangiogruphy, 199 patients were diagnosed with normal appearance and 54 patients with ITBL In patients with abnormal initial cholangiography, ITBL was occurred in 39 of 64 patients including 10 of 33 poor frlling patients and 29 of 31 irregularity patients. In patients with normal initial appearance, ITBL was only occurred in 15 of 189 patients. The abnormal initial cholangiography was associated with ITBL significantly (X2 = 79.999, P = 0.000, r = 0.490). Initial cholangiography had an overall sensitivity of 72.22%, and specificity of 87.44%, with positive and negative predictive values of 60.94% and 92.06%, respectively. The abnormal initial cholangiogruphy was a risk factor of ITBL by logistic regression analyses(OR=15.193, P=0.000). Conclusion The abnormal initial cholangiography is associated with ITBL Initial cholangiography, especially minimal irregular of intrabepatic biliary tract, is a sensitive and specific method for the detection of ITBL after liver transplantation.

Objective To investigate the impact of down-regulation of histone methyltransferase enhancer of zeste homolog 2 (EZH2) on RUNX3 expressions,proliferation and apoptosis in human pancreatic cancer.Methods The expressions of EZH2 and RUNX3 in 38 pancreatic cancer patients and human pancreatic cancer AsPC-1,PANC-1 and BxPC-3 cells were detected by immunohistochemistry and western blot,respectively.Cells were transfected with siEZH2 by lipofectamin 2000.Real time-PCR and western blot were used to detect EZH2 and RUNX3 expressions.Cell growth and apoptosis in vitro and vivo were assessed by MTT,flow cytometry and nude mice experiments,respectively.The correlation among the expressions of EZH2,clinical pathological features and overall survival rate were analyzed.Results Elevated EZH2 and decreased RUNX3 expressions were observed in human pancreatic cancer tissues and cells (P ＜ 0.05).Knockdown of EZH2 reduced cell growth and induced apoptosis in vitro and vivo by upregulating RUNX3 protein expression (P ＜ 0.05).In addition,the EZH2 expressions were correlated with tumor stage,lymph node metastasis and poor prognosis (P ＜ 0.05).Conclusions EZH2 expressions were correlated with malignancy and poor prognosis in pancreatic cancer.Tumor cell proliferation was promoted by EZH2 through down-regulation of RUNX3.EZH2 may be a potential therapeutic target for pancreatic cancer.

Objective To evaluate pericardial devascularization with splenectomy (PCDV) for the treatment of cirrhotic portal hypertension. Methods From January 1994 to December 2004, 177 patients were treated by PCDS, among them posthepatitic cirrhosis was identified in 170 cases, and alcoholic cirrhosis in 7. One hundred and thirty two patients were operated on electively, 25 prophylactically, and 20 emergently. Results The bleeding control rate was 95% , the overall operative mortality rate was 4. 5%. The main causes of death were upper gastrointestinal bleeding, hepatic failure and intra-abdomimal hemorrhage. The mean follow-up time was 3. 6 years. The 5-year survival rate was 90%. The 5-year recurrent bleeding rate was 5. 1% , The rate of postoperative hepatic encephalopathy was 5. 1%. Conclusions This procedure has the advantage of high successful rate of bleeding control, low complication rate, and long term survival.

A design of the wireless ambulatory blood pressure monitoring system has been presented based on ARM embedded microprocessor and GPRS module for the extention of native mobile telemedicine. The whole system framework and flow chart have been introduced in detail. Based on RBF neural networks, the ambulatory blood pressure data curve has been simulated, and the trend curve provides a reliable gist for the prevention, treatment, diagnosis and therapeutic efficacy of cerebrocardiac diseases.

Objective To investigate the value of functional MRI in the diagnosis and differential diagnosis of breast diseases.Methods Sixty-five patients with 68 lesions were enrolled in this study. Conventional T_1 WI and T_2 WI scan,dynamic contrast enhanced MRI,diffusion weighted imaging and ~1H single voxel MR spectroscopy were performed consequently.All lesions were verified by pathology,including 4 cases of breast adenosis,22 fibroadenomas,2 chronic inflammations,3 cysts,33 infitrating ductal carcinomas,1 intraductal carcinoma and 3 cystosarcoma phyllodes tumors.Morphological features,maximum enhancement ratio,time-intensity curve,apparent diffusion coefficient and Choline peak were analyzed. Results The detection rates of T_1 WI and T_2 WI were 14.7%(n=10)and 51.5%(n=35).The sensitivity,specificity,accuracy of dynamic contrast.enhanced MRI for the malignant tumor were 94.6%, 71.4% and 76.5% respectively.Retrospective study showed that diffusion weighted imaging,with the b value from 800 s/mm~2 to 1000 s/mm~2,could be used to differentiate various types of breast lesions.~1H signal voxel spectroscopy had a sensitivity of 51.4%,specificity of 82.6%,and accuracy of 67.6% for the malignent.The sensitivity,specificity and accuracy could reach 97.3%,90.0% and 92.6% respectively by combining conventional scan,dynamic contrast enhanced MRI and MR spectroscopy.Conclusion Functional MRI,with high sensitivity,specificity and accuracy,can be used widely in the diagnosis of malignant breast lesions.

Objective:To assess the expression levels of CD95 and OX40 ligand(OX40L)messenger RNA(mRNA)in hepatocellular carcinoma(HCC),and their clinical values thereof.Methods:The lymphocytes of research objects were mixed with corresponding fluorescence labeled monoclonal antibody(McAb);CD95 expression by CD3 positive T ceils was quantitatively measured by dual color flow cytometry.The expression of OX40L mRNA was detected in peripheral blood mononuclear cells by fluorescence quantitative reverse transcription polymerase chain reaction(FQ-RT-PCR).Results:The CD95 expression by CD3 positive T cells was significantly higher in patients with HCC(34±20)% compared with that of normal controls (20±7)%(t=2.960,P ＜ 0.01).The expression level of OX40L mRNA was significantly decreased in patients with HCC compared with that of normal controls(t=2.302,P ＜ 0.05).Conclusion:These results suggest that abnormal expressions of CD95 and OX40L play crucial roles in peripheral blood of HCC patients at the stage of human hepatocareinogenesis.

Objects To analyze the epidemiological characteristics of outbreaks of brucellosis in recent years in Gansu province,in order to provide a scientific basis for prevention and control of this disease.Methods Data of brucellosis cases from 2005 to 2010 in Gansu province were reviewed and retrospectively analyzed; the data included the outbreaks in different times,locations,groups,source of infection,and clinical manifestations.Results There were 8 brucellosis outbreaks among humans and animals in the nine administrative villages,seven townships (towns),six counties (districts) of the whole province in 5 years,a total of 66 of confirmed new cases.The source of infection was mainly the sheep (Small Tailed Han sheep and Cashmere goat),followed by dairy cow.The outbreak time was mainly in the May to August.Occupation distribution:of all the patients found in the past 6 years,farmers were 59 cases,accounting for 89.39％ (59/66); students 4 cases,accounting for 6.06％ (4/66);and herdsman 3 cases,accounting for 4.54％(3/66).Conclusions Brucellosis outbreaks increase significantly in recent years,and the epidemic situation is not optimistic.
