Objective We generated an experimental canine model of heterogeneous emphysema.The dogs subsequently underwent unilateral bronchoscopic lung volume reduction(BLVR).Observing the postoperative condition of ventilation/perfusion,blood gas analysis,respiratory dynamics,hemodynamic measurement,HRCT and radiologic outcomes,compared with the preoperative level,the correlative mechanism and the effects of BLVR were analyzed.Methods There were 15 healthy dogs that were treated samely with localized papain instillations under bronchoscopic guidance to generate heterogeneous emphysema.The right dorsal lobe was selected as the target area.All dogs were divided into 3 groups randomly.Group A was control group;Group B and Group C received BLVR 6 weeks later while group A was raised as the same way.Group B underwent endobronchial valve insertion(EVI);Group C underwent bronchial blocking with albumin gel.Measurements were made in each animal at 3 time points:prior to papain exposure(base-line),after establishment of emphysema(6 weeks later),6 weeks after BLVR.Data included blood gas analysis(PaO2,PaCO2),respiratory dynamics(respiratory peak pressure,lung compliance),hemodynamic measurement(pulmonary artery pressure,pulmonary capillary vessel wedge pressure),nuclear ventilation/perfusion scan(CTS,CTS/PIX).Dogs were euthanized at 6-week time point followed by autopsy.The data was statistically managed and compared.Results After development of emphysema,all dogs exhibitted aggravation in PaO2,PaCO2,PAP and lung compliance(P0.05).Through ventilation/perfusion scan,CTS/PIX of the target areas reduced(P

Objective To summarize the experiences of ultrasonic liposuction in 204 cases, and to analyze the advantages and disadvantages of this technique. Methods The ultrasonic system (SILBERGEE E.U.A. TM ) and the liposuction equipment (HEROULES TM , American) were applied with the tumescent technique in 204 cases. Results Satisfactory rate was 90.69 %. After operation the highest sucked fat quantity was 6 800 ml, and the fewest quantity 250 ml, with an average of 1 907.6 ml for each case. Average volume of sucked fat in each position was 1 005.5 ml, which exceeded that by the traditional method. Advantages of this technique were larger fat quantity, obvious effect, less bleeding and pain, and safety. Disadvantages and complications were rough and uneven surface, accumulation of the liquid, bilateral asymmetry, slow wound healing, subcutaneous ecchymosis and hardening nodule, and even decreased blood pressure. The corresponding processing technique was recommended after liposuction. Conclusion This method is a simple and convenient, safe and reliable technique. It is suggested that multiple times and less portions of liposuction may be of benefit to patients.

Objective:To detect the levels of TGF-β1 in hypertensive renal function impairment patients and its clinical value. Methods: Primary hypertension patients with no target organs damage were enrolled in control group, primary hypertension patients with renal function stageⅠand stageⅡwere enrolled in renal function stageⅠgroup and stageⅡgroup. Serum TGF-β1 levels and renal function indices of three groups were compared. Results:There were no significant difference of serum creatinine and blood urea nitrogen of three groups(F=23.523, P<0.05);endogenous creatinine clearance rate of renal functionⅠandⅡgroups were lower than that of control group, TGF-β1 levels of renal functionⅠandⅡ groups were higher than of control group; TGF-β1 levels were negatively correlated with creatinine clearance rate. Conclusion:TGF-β1 level is an ideal index to reflect renal function for its level significantly changes in renal damage stageⅠ,Ⅱand has good correlation with Cin.

Objective: To compare the effect of urapidil at different doses on blood pressure and prognosis of patients with acute ischemic stroke (AIS) so as to provide data evidence for its clinical use.Methods: Totally 62 patients with AIS were divided into 3 groups in a prospective study: 20 cases in high dosage group, 21 cases in middle dosage group and 21 cases in low dosage group.All the patients were treated with urapidil and urokinase, and the initial dose of urapidil was 25 mg, 15 mg and 5 mg, respectively.Systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the patients before and after the treatment were observed, and the US national institutes of health stroke scale (NIHSS), modified Rankin scale (mRS) and activity of daily living scale (ADL) were also evaluated in the follow-ups.Results: The time of blood pressure up to standard in the middle dosage group and low dosage group was longer than that in the high dosage group.At the same time, the time of blood pressure up to standard in the low dosage group was longer than that in the middle dosage group, and the difference had statistical significance (P＜0.05).After the treatment, SBP and DBP at T1 in all the groups were notable lower than those at T0,and those at T2 and T3 were lower than those at T1, and the differences had statistical significance (P＜0.05).After the treatment, the SBP and DBP levels in the high dosage group were lower than those in the middle dosage group and low dosage group, and those in the middle dosage group were lower than those in the low dosage group, and the differences had statistical significance (P＜0.05).The NIHSS, mRS and ADL scores of the middle dosage group were lower than those of the low dosage and the high dosage group (P＜0.05), and there was no significant difference between the latter two groups (P＞0.05).Conclusion: With the initial dose of 15 mg, urapidil can ensure good prognosis on the premise of effective decompression.

Bone marrow mesenchymal stem cells (MSC) are the non-hematopoietic cellular component in the bone marrow that have multiple differentiation potency. MSC play a key role in the regulation of bone marrow hematopoietic niche and modulation of immune function through various mechanisms. They are currently recognized as a promising cell source in tissue engineering, a vehicle in gene therapy and a powerful tool in the management of graft-versus-host disease after allogeneic stem cell transplantation. In this review some momentous aspects regarding the current status and potential clinical applications of MSC in hematopoietic stem cell transplantation, aplastic anemia and multiple myeloma were summarized.

Objective To investigate the influence of pulmonary microembolism of early stage on hemodynamics, respiratory function and blood coagulation in dogs. Methods Eight mongrel dogs (6 male, 2 female) weighing 15.5-16.5 kg were anesthetized with intravenous atropine 0.02 mg?kg-1 , propofol 2-3 mg?kg-1 and pancuronium 0.4 mg?kg-1 . The animals were intubated and mechanically ventilated with 100% O2. The ventilatory settings were as follows : VT 12 ml? kg-1 , RR 15 bpm and I: E = 1:2. Anesthesia was maintained with intravenous infusion of propofol at 200-300 ?g ? kg-1? min-1 and intermittent iv boluses of pancuronium. A 7.5 F Swan-Ganz catheter was placed via femoral vein for hemodynamic monitoring and injection of microemboli. 50 ml of blood was removed from artery and mixed with methylene blue. The clot was cut into small pieces 1-2 mm in diameter. After being washed with normal saline, 100 microemboli in normal saline 10 ml were rapidly injected into pulmonary artery via Swan-Ganz catheter. BP, HR, pulmonary arterial pressure (PAP), pulmonary arterial wedge pressure(PAWP), arterial blood gases, airway pressure, lung compliance, D-dimer, tissue plasminogen activator (t-PA), protein C and S were measured and recorded before (T0 ) , immediately after (T, ) and 30 min (T2) , 60 min (T3 ) , 120 min (T4) after embolization. Two hours after embolization, chest was opened and lung tissue was obtained for microscopic examination. Results Both systolic and diastolic PAP, PAWP and pulmonary vascular resistance (PVR) were significantly increased immediately after embolization (T1 ), and then decreased to the baseline level (T0) at Ih after embolization (T3) . There were no significant changes in respiratory function. D-dimer was increased at 30 min after embolizatiion (T2 ) and decreased to the baseline level at T4 . Microscopic examination showed that the lung exhibited hemorrhage and consolidation with microemboli in arterioles. Conclusion Pulmonary microembolism induces pulmonary hypertension and change in D-dimer level in the early stage but respiratory function is not affected. It causes injury to the lung parenchyma.

Objective To analyze diagnostic experience of children with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) and assess its surgical results.Methods Six children with ALCAPA had been diagnosed in our hospital from Mar. 2006 to Oct.2007 aged from 6 months to 10 years old(median 43.5 months).The medical data included common clinical presentations,the findings of electrocardiogram(ECG),the echocardiogram,ECG-gated 64-slice computed tomography,the digital subtraction angiogram (DSA),surgical records and the outcome.Results Diaphoresis,fantod,breathlessness and difficult feeding were commonly presented in 5 cases ALCAPA and the 5 children presented with left ventricular failure in infancy.The predominant ECG findings were Q waves and ST segment changes in the anterolateral chest leads in 4 cases,ST changes in 1 case,and normal ECG in 1 case.Cross-sectional echocardiography showed dilated left ventricles with poor contractility in 4 cases,medium mitral regurgitation in 2 cases and severe mitral regurgitation in 2 cases,aneurysmal dilatation of the right coronary artery(RCA) in 2 cases.CT and DSA examinations showed characteristic changes in these children.The surgical reestablishment of a two-coronary system was performed in 5 cases,direct reimplantation in 2 cases and creation of an autologous extrapulmonary tunnel in 3 cases.Simultaneous mitral annuloplasty was performed in 2 cases.After operation,the symptoms of 5 cases were all improved and none died.Conclusions Understanding the clinical manifestations and lab findings of ALCAPA is helpful to making an early diagnosis of ALCAPA.Early surgical treatment can improve the patients′ outcome.

Objective To investigate the effect of chronic intermittent hypoxia (CIH) on myocardial tissue pathology,oxidative stress and apoptosis in rat fed a high-fat diet,and to explore the possible mechanism of CIH induced cardiomyocyte injury.Methods A total of 24 male Wistar rats were randomly divided into 3 groups (n=8 each).The control group was fed common rat forage,the high-fat group was fed high-fat forage,and the high-fat plus intermittent hypoxia group was fed high-fat forage combined with a 7h/d intermittent hypoxia treatment.The changes of myocardial tissue pathology and ultrastructure of cardiomyocyte,and the activities of apoptosis regulating factor cysteine-containing aspartate-specific proteases-3 (caspase-3) and oxidative stress marker myeloperoxidase (MPO) were observed in the 3 groups after 4 weeks of treatment.Results There were significant differences in the activities of caspase-3 and MPO among the three group (F=89.94,71.24,both P＝0.001).The activities of caspase-3 and MPO were lower in the control group than in the high-fat group and in high fat plus intermittent hypoxia group [(0.21±0.06) vs.(0.80±0.11),(1.15±0.21),(3.20±0.58) vs.(10.87±1.96),(13.17±2.22),P＜0.01].The activities of caspase-3 and MPO were higher in the high-fat plus intermittent hypoxia group than in the high fat group[(1.15±0.21) vs.(0.80±0.11),(13.17±2.22) vs.(10.87±1.96),P＜0.01].No abnormal findings in the structure of cardiomyocyte were observed in the control group,while multiple pathologic damages in cardiomyocyte were detected in the high-fat group,and more obvious injuries in the high-fat plus intermittent hypoxia group.Conclusions The pathologic damages to cardiomyocyte are more serious in high fat and intermittent hypoxia group than in the high-fat group.Apoptosis induced by oxidative stress may play an important role in the pathogenesis of these injuries.
