1.Current situation of and thoughts on the minimally invasive esophageal surgery

Esophageal cancer is one of the most common malignancies in the world.Surgical treatment is the first choice for the treatment of esophageal cancer because it could radically resect the tumor and significantly improve the patients'condition.Minimally invasive esophagectomy (MIE) was rapidly developed because of its advantages of minimal invasion,small volume of blood loss,slight pain,low complication rate and quick recovery of patients.The extent of lymphadectomy and postoperative survival rate of MIE were comparable to those of traditional thoracotomy.MIE has become the treatment of choice for benign esophageal diseases and early esophageal cancer in some medical centers.However,debate still exists regarding the role of MIE in the treatment of esophageal cancer,particularly given the limitations in the widespread implementation of this technically challenging operation.This review summarized the current status of the use of minimally invasive surgery for the treatment of esophageal cancer and some thoughts on the development of MIE were provoked.
2.Effects of ursolic acid on proliferation of transplanted glioma in nude mice and its mechanisms

Objective To investigate the effects of ursolic acid on the expression of extracellular signal regulated kinase(ERK1),C-Jun,C-Myc,Cyclin D1 in transplanted tumor of malignant glioblastoma cell line C6 in nude mice and the related mechanisms.Methods C6 glioblastoma cells were subcutaneously injected into nude mice to establish the subcutaneous model of glioblastoma in nude mice,and then the mouse models were divided into 3 groups: blank control group,ursolic acid group(50 mg?kg-1?d-1,intra-abdominal injection,20 d),PD98059 group(2 mg?kg-1?d-1,intra-abdominal injection,7 d).Survival of nude mice,growth and histopathological changes of the tumors were observed.The expressions of ERK1,C-Jun,C-Myc,and Cyclin D1 in the tumor tissues and the expression of ERK1 mRNA in the tumor tissues were detected by immunohistochemical technique(IHC) and in suit hybridization(ISH),respectively.Results Slow growth of the implanted tumor was found in ursolic acid group and PD98059 group.The mean volume and weight of tumors in the two groups were significantly smaller than those in the blank control group(P

Because of the characteristics of microRNAs (miRNAs) and their important regulative roles in human tumor diseases,they may be novel approaches for gene target therapy.Most studies focus on finding critical regulator miRNAs of oncogenes or tumor suppressor genes.Recent researches show that the expression of miR-221 and miR-222 are abnormal,and they play different roles in different types of human tumors.The regulation of miR-221 and miR-222 may be a novel effective therapy for tumor.
4.Molecular targeted therapy for carcinoma based on Notch signal transduction pathway

The Notch signaling pathway represents control cell proliferation, differentiation and apoptosis during development.Aberrant activation of this pathway contributes to tumorigenesis and genes in the Notch signaling pathway could be potential therapeutic targets.This review involves regulation of Notch pathway and molecular targeted therapy.
5.Laparoscopic Partial Nephrectomy for Children with Duplex Kidney

Objective To explore the value of laparoscopic partial nephrectomy for children with duplex kidney.Methods Between June 2006 and February 2009,we performed laparoscopic partial nephrectomy on totally 21 children with duplex kidney.Three or four trocars were used for the operation.Through transabdominal approach the peri-renal tissues were freed and the vessels supplying the duplicated kidney were ligated.Afterwards,we resected the duplicated kidney,as well as the ureter,and a subperitoneal drainage tube was indwelt.Results The operation was completed in all of the 21 cases without conversion to open surgery.The mean operation time for laparoscopy was 190 min(range,130 to 210 min).The intraoperative blood loss ranged from 5 to 10 ml.No patient received blood transfusion.Liquid diet was given in 8 to 10 hours after the operation.In this series,the postoperative hospital stay ranged from 5 to 7 days with a mean of 6.2 days.Follow-up was available for 3 to 28 months in the cases(mean,20.2 months).During the period,the clinical symptoms disappeared and B ultrasonography showed no abnormalities.Conclusions Providing larger operation space and better exposed surgical field,laparoscopic partial nephrectomy for children with duplex kidney is feasible and safe with quicker recovery.
6.Pneumocystoscopy for Congenital Malformation of the Vesicoureteral Junction:Report of 18 Cases

Objective To evaluate the efficacy of pneumocystoscopy for congenital malformation of the vesicoureteral junction. Methods A total of 18 children (male 11 and female 7, aged from 3 months to 7 years with a mean of 3.6 years) with congenital malformation of the vesicoureteral junction (22 ureters) were enrolled in this study. Among the patients, 9 had vesicoureteral reflux (4 of them had bilateral reflux), 2 had bladder diverticulum complicated with urethral stenosis, and 7 had urethral stenosis and ipsilateral hydronephrosis. After the bladder was insufflated with CO2 through a Foley catheter (16 mm Hg), three trocars were inserted into the bladder, and the bladder was fixed on the abdominal wall using silk thread. By pneumocystoscopy, the Cohen procedure was done through the trocars. A Foley catheter was indwelled at the end of the procedure. Results The procedure was completed in all but one patient, who was converted to open surgery because the contraction of the urethra. The mean operation time was 130 min (110 to 145 min) for unilateral cases (13), and 242 min (225 to 250 min) for bilateral cases. The patients were discharged form hospital 9 to 12 days after the operation, and were followed up fro 6 months to 5 years with a mean of 3.8 years. During this period, MUC showed no ureteral reflux. One of the cases developed mild infection of the urethra, and was cured by circumcision. Conclusion Pneumocystoscopy is effective for congenital malformation of the vesicoureteral junction.

8.Effects of atmospheric fine particulate matter on respiratory diseases and symptoms of community residents in Chun'an County

Objective:
To evaluate the impact of fine particulate matter ( PM2.5 ) on respiratory diseases and symptoms of community residents in Chun’an County, so as to provide the basis for air pollution treatment strategies.
Methods:
Using the cluster random sampling method, the permanent residents in Qiandaohu Town of Chun’an County were investigated from 2017 to 2018. The demographic information, respiratory diseases and symptoms were collected by using Surveillance Plan for the Impact of Air Pollution ( Haze ) on health ( 2016 Edition ). The air pollutants and meteorological data were collected through Qiandaohu station of Hangzhou Ecology and Environment Monitoring Center and Hangzhou Meteorological Information Center. The effects of PM2.5 on respiratory diseases and symptoms of residents were analyzed with generalized estimating equation.
Results:
Totally 1 181 people aged 6 months to 95 years were recruited, including 557 ( 47.16% ) males and 624 ( 52.84% ) females. Acute nasopharyngitis (common cold) and tracheitis/tonsillitis occurred most frequently, with 203 cases, accounting for 3.44%. The median of daily average concentration of PM2.5 was 24 μg/m3, with the standard exceeding rate of 2.80%. The results showed that PM2.5 increased the risk of acute nasopharyngitis ( common cold ), tracheitis/tonsillitis, cough, expectoration, runny nose, sore throat and nasal congestion ( lag 3 days, OR: 1.015-1.022, 95%CI: 1.001-1.037 ); the effect of PM2.5+PM10 ( OR: 1.020-1.040, 95%CI: 1.006-1.070 ) and PM2.5+O3 ( OR: 1.017-1.024, 95%CI: 1.005-1.035 ) was greater than that of PM2.5 alone on respiratory diseases and symptoms.
Conclusion
Atmospheric PM2.5 exposure in Chun’an County increases the risk of respiratory diseases and symptoms among community residents.
9.Regulation of the Expression of NEF3 in RA Induced Differentiation of SH-SYSY Neuroblastoma Cells

Objective To analyze the regulation of NEF3 expression in SH - SY5Y after RA induction. Methods The expression of NEF3 and Brg1 genes was individually detected by realtime RT - PCR and Western blot analyses. Then, the CAT reporter driven by 5' flanking region ofNEF3 gene (pBLCAT3 -2. 8k - NEF3) was co - transfccted with eukaryotic expression plasmids of pBJ5 - Brgl or pBJ5 - Brg1 - NTP into SH - SY5Y respectively. Promoter activity of the NEF3 gene was detected by competitive RT - PCR assay. Results Similar changes in the mRNA expression of NEF3 and Brg1 and that of the protein expression of Brgl were detected in Sh - SY5Y cells af-ter RA treatment for 12 -24h. Wild type Brgl can promote the expression of pBLCAT3 -2.8k -NEF3 promoter after RA induction, but not mutant Brg1. Conclusion Brgl helpes regulating the expression of NEF3 gene possibly via changes in chromatin conformation in the SH -SY5Y cells after RA induction.
