1.Missed and misdiagnosis of popliteal artery injury associated with knee dislocations

[Objective]To analyze the reasons of the missed and misdiagnosis of popliteal artery injury associated with knee dislocations, so that the patients can be diagnosed as early as possible.Amputation can be avoided.[Method]Twelve patients admitted in our hospital 1998~2003 who were misdiagnosed in other hospitals.Five patients had compound injured.The misdiagnosis time was 8~50 hours,meaned 22.6 hours.Five patients had an arteriogram after registes,one of them was recanalizated in radiology department.Other 6 patients were oqerated with saphenous vein bypass grafting,2 patients were anastomosed the end to the end and 3 patients were amputated directly.[Result]Three patients were healed completely,2 patients remained ischemic symptoms,4 patients were operated again with amputation.[Conclusion]There are four reasons for the missed and misdiagnosis:a.The doctors were not familiar with the anatomy of the popliteal artery;b.The knee was automatically reduction in the process of transmitting.There was no deformity when arriving at the hospital;c.The patients were severely injuried and the knee dislocations was ignored.d.The physical examination was not carefully done.
2."T" shape autologous bone transplantation in treating giant cell tumor of bone around the knee

[Objective]To introduce a surgical technique used in the treatment of giant cell tumor of bone.[Method]Twenty-one patients were treated in our hospital from 1995 to 2003 for bone tumors of Campanacci Ⅰ~Ⅱ.All patients underwent curettage and "T" shape autologous bone transplantation and phenol.The patients were followed up for 18~24 months.[Result]Eighteen patients were healed on the basis of the duration,all knees presented no function limited.Two patients had local recurrence 8 to 10 months after the treatment.[Conclusion]For the giant cell tumor of bone around the knee,curettage and adjuvant "T" shape autologous bone transplantation is a good alternative method of reconstruction.
4.Surgical treatment of cervical esophageal carcinoma

Objective To explore the application of gastric(enteric)-pharyngeal anastomosis for cervical esophageal carcinoma. Methods The clinical data of 12 cases with surgical management of cervical esophageal carcinoma were retrospectively analyzed. Results The resectability of cervical esophageal carcinoma was 100%,no case complicated with pharyngeal fistula.Swallowing function of all cases was in a good state.The overall follow-up was 1 to 7 years,among them 9 surviving,3 dead.The surviving 5 cases are over 3 years,the ongest beyond 7 years. Conclusions Gastric(enteric)-pharyngeal anastomosis is a good primarily rehabilitating method of the cervical esophageal defect after surgical treatment of cervical esophageal carcinoma.
5.Impact of spironolactone on the expression and activity of ROCK1 in renal tissues of uninephrectomized diabetic rats

ObjectiveTo investigate the expression and activity of Rho-associated kinase 1 (ROCK1) in renal tissues of uninephrectomized diabetic rats,and to explore the possible mechanism of renal protection of spironolactone.Methods The model rats were established by a single intraperitoneal injection of streptozotocin(STZ) after uninephrectomy and randomly divided into sham operation group(N),uninephrectomized control group?,uninephrectomized diabetic group (D),and spironolactone treated group (S).Four and 8 weeks later,biochemical indexes and renal morphology were detected.Expressions of ROCK1 and connect tissue growth factor (CTGF) were examined by immunohistochemistry and real-time PCR.Protein expression of p-MYPT1 was examined by Westem blotting.Results After 4 and 8 weeks,compared with group N and C,blood glucose,systolic blood pressure,24-h urinary protein,kidney weight/body weight (KWI) were significantly increased (P<0.01),and extracellular matrix proliferation and basement membrane thickening were found in group D.After 8 weeks in group D,Alb significantly decreased and Scr significantly increased (P<0.05).In group D,protein and mRNA expression of ROCK1 and CTGF increased significantly and protein expression of p-MYPT1 increased significantly as well with time.Treatment with spironolactone could partially reverse those changes. ConclusionsExpression and activity of ROCK1increase in renal tissues of uninephrectomized diabetic rats and are positively correlated with the expression of CTGF.Spironolactone can protect the kidney of diabetic rats in early stage probably through decreasing the expression of ROCK1,CTGF and inhibiting the activation of ROCK1.
7.Comparative analysis of laser confocal microscopy and corneal smear in the diagnosis of fungal keratitis

AIM: To comparatively analyze the application of laser confocal microscopy and corneal smear in the diagnosis of fungal keratitis.METHODS: Totally 77 patients (77 eyes) diagnosed as fungal keratitis were selected.Laser confocal microscopy and corneal smear examination were performed to observe the characteristics of the images, and the detection rate of fungus were compared between the two methods.RESULTS: Of the 77 patients, 66 eyes (86%) were positive and 11 eyes were negative detected by laser confocal microscopy;51 eyes (66%) were positive and 26 eyes were negative detected by corneal smear examination, the difference was statistically significant compared between two group (P<0.05).CONCLUSION: Laser corneal confocal microscopy is relatively safe and sensitive.If combined the application of laser corneal confocal microscopy and corneal smear examination in clinical, the diagnosis rate of fungal keratitis may be improved.
8.Gel electrophoresis analysis on plasma differential protein in patients with unstable angina of blood-stasis pattern.

OBJECTIVETo seek the plasma differential proteins in patients with unstable angina of blood-stasis pattern (UA-BSS) for exploring the proteomic specialty in them by way of two-dimensional difference gel electrophoresis (DIGE) detection on plasma of patients and healthy persons.
METHODSUsing DIGE and tandem mass spectrometry, comparative proteomic study was conducted on the plasma of 12 UA patients of qi-deficiency and blood-stasis pattern (UA-QBS), 12 UA patients of phlegm-stasis cross-blocking pattern (UA-PSS) and 12 healthy volunteers.
RESULTSPreliminary results showed that Haptoglobin beta chain, DBP, HBB, HBA, Transthyretin, ApoA- I, ApoA-IV were significantly differentially expressed in both patterns, while Haptoglobin alpha1 chain, alpha-1-acid glycoprotein, ApoC-III, ApoA-II, ApoC-II, ApoJ, and Haptoglobin alpha 2 chain were only seen differentially expressed in the UA-PSS patients, alpha1-antitrypsin, Fibrinogen gamma chain, and Fibrin beta were only seen differentially expressed in UA-QBS patients.
CONCLUSIONThe common proteomics characteristics of patients of QBS and PSS patterns may be correlated with inflammatory reaction and metabolic disturbance (including blood lipid and blood oxygen).

9.Study on Helicobacter infection in liver tissue from hepatocellular carcinoma.

OBJECTIVETo investigate the relationship between Helicobacter species and hepatocellular carcinoma(HCC).
METHODSLiver samples resected during operation from 34 patients with HCC diagnosed by histopathology and 20 without primary liver carcinoma as controls were studied. The two groups of sample were cranked out pathologic slice for in situ hybridization of Helicobacter, Helicobacter pylori and Helicobacter hepaticus. Qualitative and quantitative studies were used to assess the correlation of liver tissue Helicobacter infection with HCC.
RESULTS64.71% (22/34). of the samples of HCC showed positive for Helicobacter specific 16S rRNA-mRNA gene by in situ hybridization, while none was positive in controls (P < 0.01).
CONCLUSIONHelicobacter pylori were found in the liver of patients with HCC.

10.Study on effect of oligochitosan in promoting intestinal absorption of protoberberine alkaloids in extracts from Corydalis saxicola total alkaloids.

OBJECTIVETo investigate the effect of oligochitosan in promoting intestinal absorption of protoberberine alkaloids in extracts from Corydalis saxicola total alkaloids.
METHODThe in vitro single-pass intestinal perfusion model in rats was established to study the changes in absorption kinetic parameters of dehydrocavidine, berberine hydrochloride and palmatine chloride in C. saxicola total alkaloids after the addition of different concentrations oligochitosan and evaluate the effect of oligochitosan in promoting intestinal absorption of the drugs.
RESULTThe concentration of oligochitosan had different effects on the absorption rate constant (Ka) and apparent permeability coefficient (Peff) of the three active component in rat intestines. Ka and Peff in 0.5% oligochitosan group significantly increased, indicating a stronger effect in promoting the absorption.
CONCLUSIONOligochitosan has a certain effect in promoting the intestinal absorptions of protoberberine alkaloids in C. saxicola total alkaloids.
