1.Mechanism of Antiasthma by Inhaled Arsenic Trioxide

Objective To investigate the effect of eosinophils(EOS)in asthmatic guinea pigs airway and explore the mechanism of antiasthma by inhaled arsenic trioxide. Methods The guinea pigs asthma models were established with ovalbumin(OVA)challenge method,2 groups received inhaled different doses of arsenic trioxide,and three control groups respectively received normal saline inhaled and high dose of arsenic trioxide by intraperitoneal injection and dexamethasone(DEX)intraperitoneally. EOS invaded and apoptosis with all of the groups were assessed after 7 days intraperitoneal injection or inhalation. Results Compared with group of inhaled normal saline, both EOS invaded and apoptosis in bronchus submuconsa were significantly different(P0.05)among groups of inhaled low dose arsenic trioxide [2.0 mg(kg?d)]and intraperitoneal injection with higher dose arsenic trioxide[5.0 kg/(kg?d)]and DEX[10 mg/(kg?d)]for these two parameters. Conclusions The mechanism of arsenic trioxide antiasthma with arsenic trioxide can decrease EOS amount in bronchial submucosa and accelerate EOS apoptosis, and relieve bronchial inflammation in asthma. For inhaled lower dose arsenic trioxide or intraperitoneal injection with high dose arsenic trioxide ,the effect of EOS is equivalent. The antiasthma effect of inhaled lower dose arsenic trioxide[2.0 mg/(kg?d)]is equivalent to intraperitoneal injection higher dose[5.0 mg/(kg?d)],and may be safe comparatively. The mechanism of antiasthma by inhaled arsenic trioxide is the same with DEX.
2.Clinical Features of Spinal Muscular Atrophy Type Ⅱ: 53 Cases Report

Objective To investigate the clinical features of spinal muscular atrophy type Ⅱ.Methods The data of clinical manifestation,laboratory data, onset, diagnosis, and rehabilitation of 53 outpatients suffering from spinal muscular atrophy type Ⅱ (SMA-Ⅱ) were analyzed. Results Among 53 patients with SMA-Ⅱ confirmed by molecular genetic tests, 27 patients were male while 26 were female. The mean age was 3.35 years (ranged 0.75~7.8 years), and the age receiving gene diagnosis was 17.27 months. Only 15% had a family history,and abnormalities were found in 23% patients' mothers during pregnancy. 83.4% of them had water choke cough, while 87% expectoration weakness. None of them had cough assist machines and had regular spirometry monitoring. 53% of the patients took semi-liquid diet, however,none of them used stomach tube. Symmetrical flaccid paralysis was so remarkable, even muscle strength of lower limbs in 87.7% of these patients were only grades 1-2. 92% had scoliosis, while 83% had tendon contracture. EMG showed extensive neurogenic changes. All children did not accept normal pre-school education, and 85% patients did not accept formal rehabilitation. Conclusion Most of phenotype of SMA-Ⅱ for the children was similar and more severe in China. Most of the patient didn't get formal education, rehabilitation and care.

4.Observation of clinical efficacy of preventing liver injury of lamivudine on hepatitis B virus carriers with anti-tuberculosis drugs

Objective To evaluate the clinical efficacy of lamivudine in preventing liver injury induced by anti-tuberculosis drugs in hepatitis B virus(HBV)carriers.Methods One hundred and ten HBV carriers treated with anti-tuberculosis drugs were randomly divided into lamivudine group and control group.Patients in both groups were treated with conventional anti-tuberculosis drugs (isoniazid,rifampicin,pyrazinamide,streptomycin or ethambutol)for 6-8 months.However, patients in lamivudine group were treated with lamivudine 100 mg orally dairy concomitantly.Before and after treatment,the clinical manifestation,liver function and serum HBV DNA level of patients were evaluated.Statistical analysis was performed using t test and x~2 test.Results During 6-8 months of treatment,the incidence rate of liver injury was 9.1% in lamivudine group,while it was 38.2% in control group(P0.05).Conclusion Lamivudine is effective and safe in reducing liver injury induced by anti-tuberculosis drugs in HBV carriers.

Backgroud and Objective Proline-rich tyrosine kinase2(Pyk2) is a Ca~(2+) sensitive,non-receptor tyrosine protein kinase.Previous reports showed Pyk2 involved in development of left ventrieular hypertrophy. Objective To investigate the role of contrast enhanced ultrasound (CEUS) in evaluating and guiding radiofrequency ablation (RFA) of hepatocellular carcinoma (HCC) and its feeding vessels. Methods From January 2006 to June 2007, 71 patients with 75 hypervascular HCC in Peking University Cancer Hospital who underwent RFA were included in the study. The diagnosis was conifrmed by ultrasound guided biopsy for all patients. These patients were not suitable for transcatheter arterial chemoembolization (TACE) or had poor responds to TACE. They were divided into two groups, which included group percutaneous artery ablation (PAA) combining RFA and group RFA. There were 38 patients with 39 HCC in group PAA combining RFA and CEUS were used to identify the range of HCC inifltration. Firstly, PAA of the feeding vessels was conducted under the guidance of color doplor lfow imaging (CDFI). Then CEUS was performed to evaluate HCC perfusion after blocking the feeding vessels. Finally, the rest of the tumor was ablated by RFA. In group RFA, there were 33 patients with 36 HCC, who did not undertake PAA before RFA. Generally, the RFA was planned based on tumor size and location, and the ablation started with deep part of HCC or portion close to nearby organs. Contrast CT was used as a post-RFA imaging for follow-up at 1, 3 and 6 months post-RFA. T test was used to compare the difference in focal lesions number between two groups, andχ2 tests were used to compare the difference in necrosis rate between two groups after treatment. Results In group PAA combining RFA, post-PAA CEUS showed intratumor perfusion decreased more than 70%in 31 HCC (79.5%, 31/39). Of them, 13 HCC (33.3%, 13/39) showed complete perfusion defect with clear margin, called“solar eclipse sign”. The rest 8 HCC (20.5%, 8/39) showed 40%-70%of perfusion defect. In group PAA combining RFA, CDFI showed 35 (83.3%, 35/42) feeding vessels were blocked, and 3 vessels (7.1%, 3/42) showed signiifcant decreased lfow signal after PAA. There were average 3.18±1.42 ablations per HCC in group PAA combining RFA, and 4.32±1.56 in group RFA. The number of ablations per HCC in group PAA combining RFA was signiifcantly less than group RFA (t=2.524, P=0.015). The tumor necrosis rate at 1 month post-RFA in group PAA (92.3%, 36/39) combining RFA was signiifcantly higher than that of group RFA (66.7%, 24/35) (χ2=8.264, P=0.001). Conclusions With CEUS, PAA can effectively block the feeding vessels of HCC, enhance ablated necrosis in the tumor and signiifcantly increase necrosis rate post-RFA for large hypervascular HCC. CEUS-assisted PAA can improve efifciency of RFA with less ablation number and better result.
