3.An investigation and analysis on current status of general practice service teams in urban Sbanghai

Objective To investigate current status of general practice service teams in urban Shanghai to provide practical evidence and lay theoretical basis for sustainable development of general practice service teams in the future. Methods A cross-sectional study was carried out for 40 staff members of eight general practice service teams in eight urban districts of Shanghai, as well as 110 local residents, by observation, interview and questionnaire methods during March to April 2008. Results Among 21 general practitioners, 86 percent of them ( 18/21 ) were graduated from medical colleges or universities with three-year curriculum or at levels above it and 52 percent ( 11/21 ) had intermediate professional titles or above.Among the eight general practice service teams, five had full coverage of supervision and management for tuberculosis therapy, six had full coverage of management for tumor patients, four had full coverage of management for cardiovascular and cerebrovascular diseases and four had full coverage of management for patients with mental disorders. In all staff members of eight general practice service teams, 38 percent of them (15/40) were satisfactory with their working environment, 40 percent (16/40) with their work activities, five percent (2/40) with their incomes, and 28 percent (11/40) with their professional future.In 110 local residents, 77 percent (85/110) were satisfactory with service environment, 85 percent (93/110) with quality of health-care service and 89 percent (98/110) with the attitude of service teams.Conclusions Implementation of management for chronic diseases and two-way referral, as well as satisfaction of staff members in general practice service teams should be improved further.
4.Correlation among HIF-1α,ERCC1 genetic polymorphisms and outcome of NSCLC patients receiving platinum-based chemotherapy

Objective To analyze the relationship among HIF-1α, ERCC1 gene polymorphisms and the effects of platinum-based chemotherapy for the treatment of advanced non-small cell lung cancer (NSCLC) Methods HIF-1α C1772T and ERCC1 C118T loci were genotyped with PCR-RFLP method. The relationship between genotypes and the outcome of treatment as well as TTP was investigated. Results HIF-1α and ERCC1 of 102 patients were under the Hardy-Weinberg equilibrium. Patients with HIF-1α CT+TT genotype showed a significantly higher chemotherapeutic efficiency compared with patients with CC genotype; Patients with ERCC1 CC genotype showed a significantly higher chemotherapeutic efficiency compared with patients with CT+TT genotype. The median TTP was 7.0 months for the 98 patients followed-up; 7.0 months for HIF-1α CT+TT genotype, and 6.0 months for CC genotype, and the difference showed no significance; The median TTP was 7.0 months for ERCC1 CC genotype, and 6.0 months for CT+TT genotype, and the difference was statistically significant. Conclusions Patients with HIF-1α CT+TT genotype or ERCC1 CC genotype have showed higher chemotherapeutic efficiency, and patients with ERCC1 CC genotype show longer median TTP.
5.Psychological changes after receiving comprehensive interventional therapy and the effect of psychological intervention on carcinoma patients

Objective To study the psychological changes and the effect of psychological intervention and to discuss the influence of psychological rehabilitation on carcinoma patients after comprehensive interventional therapy.Methods 100 carcinoma patients were divided into two groups randomly:the psychological intervention group(50 casea) received both psychological intervention therapy and comprehensive intervention therapy;the control group (50 cases) received only comprehensive interventional therapy;Curative effects were assessed with the Symptom Checklist90(SCL-90) ,the Self Rating Depression Scale(SDS) and the Self Rating Anxiety Scale(SAS) for each patient. Results For the psychological intervention group, SCL-90 scores after therapy was significantly lower than that before therapy ( P < 0. 05 ), and were also significantly lower than that ( without anxiety agent) of the control group. The scores of SDS and SAS in post-therapy patients were significantly lower than that of pre-therapy patients in the psychological intervention group. The scores of SDS had no statistical difference between post-therapy patients and pre-therapy patients in the control group. The scores of SAS in post-therapy patients was significantly lower than that of pre-therapy patients in the control group. Both scores of SDS and SAS in the control group were significantly higher than that of the psychological intervention group after therapy. Conclusion Psychological intervention for carcinoma patients after comprehensive interventional therapy played an promotional role in psychological rehabilitation in the community.
6.Relationship of BRCA1 andβ-tubulinⅢmRNA expression with efficacy of taxanes for advanced esophageal carcinoma

Objective:To investigate the relationship of the mRNA expression of BRCA1 andβ-tubulinⅢwith docetaxel-resistance in esophageal cancer. Methods:The genes BRCA1 andβ-tubulinⅢwere determined at the mRNA level using RT-qPCR in 36 esophageal carcinoma specimens. Results:The mRNA expression of BRCA1 andβ-tubulinⅢwas determined in the 36 tumor samples using RT-qPCR. The median BRCA1 mRNA expression level in relation to that ofβ-actin was 6.27. The medianβ-tubulinⅢmRNA expression level in relation to that ofβ-actin was 4.44. The patients were divided into two groups using these cutoff values. The BRCA1 mRNA expression level was not correlated with the sensitivity of esophageal cancer patients to docetaxel (P=0.733). The response rate of the tumors with highβ-tubulinⅢexpression was (38.9%), which is significantly lower than in patients with lowβ-tubulinⅢexpression (83.3%) (P=0.015). Conclusion:Theβ-tubulinⅢexpression levels in the tumor tissues were probably an important biomarker for the efficacy of docetaxel chemotherapy in esophageal cancer patients. Our study may provide new insights into taxane chemotherapy for advanced esophageal cancer patients.
7.Effects of processing on piceid and emodin in Polygonum cuspidatum

Object To investigate the effects on the active components in Polygonum cuspidatum Sieb. et Zucc. and its different processed products. Methods The qualitative analysis of piceid and emodin are taken by TLC, and content of emodin was determined by TLCS and HPLC. Results The contents of emodin significantly increased in samples fried with rice wine, vinegar and salt compared with the crude. Conclusion Different methods and ajuvants made a certain influence on the contents of emotion in P. cuspidatum.

10.Establishment of evaluation index system for urban general practice service team in Shanghai

Objective To establish an evaluation index system for Shanghai urban general practice service team to improve standardized and scientific management. Methods The evaluation index system for Shanghai urban general practice service team was established using two rounds of Delphi consultation. Results The established system consisted of 5 fast-grade indicators and 56 second-grade indicators. The 5 first-grade indicators were staffing, infrastructure, quality and quantity of service, continuing education and satisfaction degree of suppliers and demanders, with weight of 0.2153, 0.1978, 0.2087, 0.1869 and 0.1923 respectively. The active coefficient for two rounds of Delphi consultation was both 100%. The authoritative coefficient of the two rounds was 0.852 and 0.883 respectively. The harmonious coefficient of importance of the evaluation index was 0.251 and 0.411 respectively. Conclusion Through using Delphi method scientifically and reasonably, we set up the evaluation index system for Shanghai urban general practice service team, with integral contents, strong operability and easy application.
