1.Clinical study of prevention of oxaliplatin-induced acute neurotoxicity by calcium and magnesium infusions

Objective To evaluate the efficacy of calcium(Ca)and magnesium(Mg)infusions in pre- vention of oxaliplatin-induced acute neurotoxicity.Methods Fourty-two patients with advanced colorectal carcinoma were eligible for the study:21 were assigned to the Ca/Mg ann and 21 to the control arm.The Ca/ Mg arm and the control arm were comparable for patients'characteristics.Chemotherapy regimen were al- most the same in both arms.Chemotherapy regimen consisted of oxaliplatin 130 mg/m~2 on day 1,given as a 3-hour infusion in 500 ml of 5% glucose,concurrent with rahitrexed 3 mg/m~2 as a 15-minute intravenous (Ⅳ)infusion or calcium folinate(CF)200 mg?m~(-2)?d~(-1),days 1~5,5-Fluorouracil(5-Fu)500 mg?m~(-2)?d~(-1),days 1~5.Therapy was repeated every 3 weeks.The treatment consisted of Ca gluconate and Mg sulfate,1 g each, delivered i.v.over 15 min just before the oxaliplatin infusion and repeated at the same dose after the comple- tion of the oxaliplatin infusion.A specific neurotoxicity scale was used for oxaliplatin-related neurotoxicity. Results Ten patients(47.62%)had acute neurotoxicity in the Ca/Mg arm compared with 19 patients (90.48%)in the control arm(P0.05).Conclusion Ca/Mg infusions seem to reduce incidence and intensity of oxaliplatin-induced acute neurotoxicity,and they do not reduce the clinical activity of oxaliplatin,but dosage and administration schedule could be optimized.

Objective To explore the clinical characteristics ,diagnosis, treatment and prognosis of phaeohyphomycosis. Methods Clinical data were collected, including history, physical examination, cranial and spinal imaging. Brain biopsy was performed. Data of the pathology and incubation of brain tissue were analyzed. Responsiveness to treatment was followed up. Results A previously healthy three and half years old boy was presented to our unit, with a three- month history of recurrent headache, vomiting, progressive paraplegia accompanied by urinary continence and constipation. A computed tomogram scan and magnetic resonance imaging of the brain revealed multiple lesions located in the region of the parietal - occipital lobes, periventricular area and frontal lobe, with prominent surrounding edema and irregular peripheral enhancement of the mass after the administration of contrast materials. A cerebral biopsy was performed and the pathological report was cerebral phaeohyphomycosis. The culture of the tissue and cerebrospinal fluid grew a same fungus identified as exo-phiala dermatitidis. The patient's response to therapy was poor, the parents of the boy gave up therapy, and the boy died 1 month later. Conclusions Cerebral Phaeohyphomycosis caused by Exophiala dermatitidis is rare, but the most serious form of fungus infection. Pathology and incubation of the tissue are essential for diagnosis. There is no curative therapy and the prognosis is poor.
5.Treatment of meniscal injuries of knee joints by arthroscopy

Objective To assess the effectiveness of treatment of meniscal injuries of knee joints by arthroscopy.Methods 33 patients 35 joints were followed up and the parts,types and treatment under arthroscopy were analysed.Results 33 patients were followed up from six months to six years,the mean preoperative Lysholm score was 60.5 points,and the mean postoperative one was 86.7 points.Conclusion The advantage of treating meniscal injuries by arthroscopy was the result of correct examination and little wound of arthroscopy operation,and arthroscopic repair or partial menisectomy could effectively restore the function of the injured knee.

OBJECTIVETo introduce the clinical application of venous nutrition flap pedicled by medial plantar artery of the hallux on the medical aspect of the foot.
METHODSBased on the anastomoses between the medial plantar artery of the hallux and the nutritional vein, the flap was designed with the perforator of medial plantar artery adjacent to the first metatarsal bone as the rotation point. The flap axis was along the vein at the medial aspect of the foot between rotation point and medial malleolus.
RESULTS5 cases were treated with primary healing and complete survival flaps. The patients were followed up for 1-12 months with good match of texture and color.
CONCLUSIONSThe venous nutrition flap pedicled by medial plantar artery of the hallux on the medical aspect of the foot can be transpositioned to repair the defect at forefoot.

9.Pharmacological study on free anthraquinones compounds in rhubarb in rats with experimental acute pancreatitis.

OBJECTIVETo verify the pharmacological hypothesis of prescriptions by studying the targeted distribution of major components in stewed rhubarb in the rat model with acute pancreatitis (AP).
METHODNormal SD rats (control group, n = 5) and the AP model induced with intraperitoneal cerulein (model group, n = 5) were taken as the experimental objects. Rats of the two groups were orally administered with stewed rhubarb granules (20 g x kg(-1)). Their heart, liver, spleen, lung, kidney and pancreas were collected two hours after the administration. Such constituents as emodin, chrysophanol, physcion, rhein and aloe-emodin and their concentrations in each tissue homogenate were detected by high performance liquid chromatography-mass-mass.
RESULTAloe-emodin and physcion in stewed rhubarb whose concentrations in liver and kidney of normal rats were higher than that in pancreatic tissues, while the distribution spectrums and concentrations of the remaining components in pancreatic tissues had no significant difference with that of other organs. The concentrations of emodin, aloe-emodin, rhein and chrysophanol in stewed rhubarb in pancreatic tissues of the AP model group were higher than that in other tissues and organs, while their concentrations in pancreatic, renal and splenic tissues were notably higher than that in the normal group.
CONCLUSIONIn the conditions of AP, effective components in stewed rhubarb show a targeted distribution feature in pancreas, which provides experimental basis for the pharmacological hypothesis of prescriptions.

10.Therapeutic Effect of Bergamots Volatile Oil on Asthma

Objective To explore the effect of bergamots volatile oil on relieving cough,asthma and eliminating phlegm in asthmatic rats and guinea-pig.Methods Bergamot essential oil was extracted by steam distillation,and the experimental guinea-pigs and mice were divi-ded into 3 groups with high,moderate and low doses of bergamot essential oil respectively(1.080,0.540,0.108 g?kg-1for guinea pig,1.753,0.877,0.175 g?kg-1 for mice).The other group were pre-treated with normal saline as model control group,and animals were fed with acute-brochisis-syrup,aminophylline,and xianzhuli oral-liquid as positive drug control groups.After the medication,the guinea pigs were sprayed with citric acid or histamine in a confined glass to induce cough or asthma,while the latent period of cough or asthma attack was detected.The mice were injected phenol red solution through the abdominal cavity,and the tracheal phenol red secretion volume was measured.Results The latent period of cough in high,moderate and low dose of bergamot volatile oil groups and acute-brochisis-syrup group were significantly longer than that of the model control group(Pa
