Journal of Clinical Neurology 2001;0(05):-
Objective To investigate the change of serum C-reactive protein(CRP) levels in patients with acute cerebral infarction(ACI) and ACI with multiple organ dysfunction syndrome(MODS), and the relationship between serum CRP levels and incidence of MODS.Methods The serum CRP levels of 30 normal healthy people and 82 patients with ACI were detected by immune transmission turbidity method. The data from the patients with lacunar cerebral infarction(LCI), acute simple cerebral infarction(PACI) and ACI with MODS(ACI+MODS) were compared. Results The levels of serum CRP in patients with ACI significantly increased compared with the controls( P

Objective To evaluate the value of gated ~(99)Tc~m-methoxyisobutylisonitrile (MIBI) myocardial perfusion imaging in detection of myocardial ischemia in hypertrophic cardiomyopathy. Methods Sixty-nine patients with clinically proven hypertrophic cardiomyopathy were divided into 2 groups using coronary angiogram as "gold standard":positive group(n=19,narrowing≥50%) and negative group (n=50,narrowing<50%).Gated ~(99)Tc~m-MIBI myocardial perfusion imaging was performed and positive in all 69 patients (41 males,28 females, aged 35-75 years).Comparative analysis between the two groups was carried out using t-test.Results In the positive group, reversible and irreversible perfusion defects were detected in 9 and 10 patients, respectively.Left ventricular ejection fraction (LVEF) increased to (69.1±2.8)% in 8 patients and decreased to(42.8±2.1)% in 11 patients. In the negative group, reversible and irreversible perfusion defects were found in 37 and 13 patients, respectively.LVEF increased to(70.8±4.0)% in 38 patients and decreased to(48.9±2.7)% in 12 patients. The values of ischemic area, severity and extent of perfusion defect, and LVEF were significantly different between the two groups(t=9.28,16.51,2.65;P<0.001,<0.001,<0.01,respectively).Conclusions Gated ~(99)Tc~m-MIBI myocardial perfusion imaging is valuable in assessing patients with hypertrophic cardiomyopathy.Detection for the presence or absence of coexisting coronary artery disease and myocardial ischemia has an important prognostic indication and management indication for these patients.

Objective To investigate the effect on and mechansm by which prostaglandin E_1(PGE_1)protects liver functions after hepatectomy. Methods In this study,82 cases undergoing hepatectomy were divided randomly into control group with conventional therapy(41 cases),and PGE_1 treatment group(41 cases)treated with liposomal prostaglandin E_1 in addition to conventional therapy.Postoperative hospital days, urinary volume and abdominal drainage volume were observed.Pre-and postoperative liver functions were measured. Results Postoperative hospital days(median time 22 days)in PGE_1 treatment group were significantly shorter than those(median time 26 days)in control group. The postoperative levels of alanine transaminase, aspartic transaminase, total bilirubin and albumen in PGE_1 treatment group recovered to preoperative levels more quickly, than control group.Postoperative urinary volume in patients of PGE_1 treatment group was significantly more than that in control group, while abdominal drainage volume was markedly less, although there was no significant difference in prothrombin time between the two groups. Conclusion In patients undergoing hepatectomy,PGE_1 is very useful and safe to protect and improve hepatic function, decreasing the level of bilirubin, preventing ascites, formation shortening hospital days, without causing prolongation of prothrombin time remarkably.

Objective To evaluate the results and toxic effects in three dimensional radiotherapy (3DCRT)with late course accelerated hyper-fractionation for advanced esophageal carcinoma. Methods From December 2001 to December 2003,115 patients were randomly divided into two groups:in 3DCRT group,55 patients were given late course accelerated hyper-fractionation radiotherapy to a total close of 66 Gy (2.0 Gy per fraction,5 times per week;to a dose of 36 Gy were changed into 1.5 Gy/f,2 f/d).In CF group,60 patients were given late course accelerated hyper-fractionation radiotherapy to a total dose of 66 Gy.Results The 1-, 2-,3-,4-year survival rates in 3DCRT group were 63.6 %, 50.9 %, 38.2 %, 30.9 % compared to 58.3 %, 38.3 %,33.3 %,23.3 % in CF group(X~2=4.44,P=0.031),and local control rates in 1-,2-,3-,and 4- years in 3DCRT group were 72.7 %,58.1%,47.2 % and 36.3 %,compared to 53.3 %,40 %,28.3 %,and 20 %(X~2=5.33,P=0.013)in CF group. However,in the 3DCRT group, the incidence of acute esophagitis was similar to CF group; hematogenous toxic and acute bronchitis were similar between the two groups. Late course lung injury in CF group was higher than that in 3DCRT group. Conclusion 3DCRT is able to improve the local control rate and survival rate, and unable to improve acute and late radiation toxie effects.

OBJECTIVE To investigate the tendency of bacterial distribution and drug resistance of pathogens in ICU,then instruct clinical application of antibiotics reasonably.METHODS The identification of bacteria, isolated from patients in ICU from Jan 2005 to Dec 2006,and antibiotic susceptibility by ATB expression were used.RESULTS A total of 1197 pathogens strains were isolated,Gram-negative bacilli,Gram-postive cocci and fungi accounted for 64.2%,19.6% and 15.8%,respectively.The most common pathogens of them were Pseudomonas aeruginosa(22.1%).Except Escherichia coli(40%),the rate of the ESBL-producing strains of Serratia marcescens,Klebsiella pneumoniae,Acinetobacter baumannii and Enterobacter cloacae were higher than 70%.The most common pathogens of Gram-postive cocci were Staphylococcus.Meticillin-resistant strains of Staphylococcus aureus accounted for 93.8%.Mainly pathogenic bacteria were multi-resistant to some antibiotics. There were 189 strains fungi. And all the fungi were sensitive to amphotericin B,fluconazole and ketoconazole.CONCLUSIONS The infection and multi-drug resistance of pathogens are serious in ICU.It′s important and urgent to carry out surveillance of bacterial resistance for appropriately using antibiotics.

OBJECTIVE To investigate the influence on air quality in cleaning room of the supply department when the water/gas blast gun is used to clean endoscope,to give basis for valuable protective procedures. METHODS Bacterial number of the air in the supply department was sampled using flat plate settling process pre-and post-operation. Simultaneously, the air purification effect of the laminar purification equipment was evaluated. RESULTS The air bacterial content before the water/gas blast gun ran was significantly different from that when the gun stopped (P

Objective The feasibility and the curative effect of bipolar hemiarthroplasty for hemiplegic side femoral neck fractures in elderly patients were evaluated. Methods The patients suffering from hemiplegic side femoral neck fractures and undergoing bipolar hemiarthroplasty in Beijing Tongren Hospital were followed-up during 2012-2015.The postoperative Harris Score and the complications such as infection, deep vein thrombosis and dislocation were collected. Results Totally 25 patients(25 hips)were followed-up.The average age was 70.1 years, and mean follow-up period was 24.3 months (10-42 months). The mean Barthel index score was higher (77.4 ± 14.7,ranging from 40-95 points) after operation than before operation[25.00 ± 5.95 (15-35 points)] with a statistically significant difference(t=-21.374,P<<0.05).The postoperative Harris Score was 83.5± 8.9(60-95 points).According to Harris score, the excellent and good rate was 80.0%.The patient's satisfaction rate was 88.0%.Conclusions Choosing proper prosthesis and fixing measure, and keeping the function and tension of short extorsion muscles can achieve the satisfactory results for hemiplegic side femoral neck fractures in elderly patients.
