1.Clinical study of cervical anterior and posterior decompression and internal fixation in the revision of anterior cervical fixation failure.

OBJECTIVETo investigate the clinical effects and prevent the complications of posterior and anterior decompression and internal fixation in the revision of cervical anterior internal fixation failure.
METHODSFrom 2008 January to 2011 December, 17 patients with cervical anterior internal fixation failure were treated with posterior and anterior decompression and internal fixation. There were 12 males and 5 females, aged from 26 to 68 years old with an average of 44.1 years. The lower screw loosening was found in 6 cases, the upper screw loosening in 5 cases, titanium mesh caving in 3 cases, the upper screw breakage in 2 cases, the lower screw breakage in 1 case. Informations of bone fusion were observed by X-ray, CT, MRI. Clinical effects were evaluated by modified JOA score.
RESULTSAll the revision operations were successfully completed. One case with poor blood coagulation function before operation resulted in postoperative hematoma and occurred neurological symptoms; after hematoma removal and fresh frozen plasma infusion later, neurological symptoms of the patient disappeared. All patients were followed up from 6 to 38 months with an average of (22.4±10.0) months. Postoperative at 2 weeks, 3 months, and final follow-up, JOA score had obviously improved and respectively was 13.1±1.6, 13.4±1.6, 14.2±1.5. All internal fixation locations were good after revision,and obtained bone fusion at 10 months after operation, with an average fusion time of 6 months.
CONCLUSIONThe combined posterior and anterior decompression and internal fixation in the revision of cervical anterior internal fixation failure is safe, can achieve thoroughly decompression, maintain the cervical curvature, reconstruct the three column stability, and it may be used for the patients of cervical anterior fixation failure.

2.Clinical and molecular study on Fechtner syndrome--case report and literature review.

OBJECTIVETo identify clinical and laboratory abnormalities and genetic defect of Fechtner syndrome in a Chinese family.
METHODSThe characteristic morphological features of platelets and leukocytes were examined on blood smears with Wright's-Giemsa staining and ultrastructure of platelet and leukocyte were investigated under electron microscope. Genomic DNA was isolated from peripheral blood of the proband and 9 members of his family. All the exons and exon-intron boundaries of the MYH9 gene were amplified by PCR followed by direct sequencing.
RESULTSPatients presented the characteristic clinical features including macrothrombocytopenia, leukocyte inclusions and/or hereditary nephritis. A heterozygous C to T mutation was found in the proband and three members of his family at nucleotide 5981 in exon 40 of MYH9 gene, resulting in a nonsense mutation which encoded truncated protein due to premature termination at the Arg 1933 codon.
CONCLUSIONIt is the first report of a Chinese family with Fechtner syndrome. The Arg (CGA) 1933--> stop (TGA) nonsense mutation in MYH9 gene is a causative genetic defect.

3.Study on the superfine comminution of traditional Chinese medicine.

This paper has introduced the terms, concept and characteristics of superfine comminution of traditional Chinese medicine. The progress in the study on the superfine comminution of the single drug and compound prescription was analyzed, and work principles of equipment in common use for superfine comminution were outlined. The future application of superfine comminution technique in traditional Chinese medicine was forecast and the problems that should be solved during the future research work were also pointed out in the paper.

4.Treatment of Radical Resected NSCLC by Chinese Medicine Combined with Adjuvant Chemother- apy: a Clinical Study.

OBJECTIVETo evaluate the efficacy of Chinese medicine (CM) combined adjuvant chemotherapy in postponing relapse and metastasis of radical resected Ib-IIIa stage non-small cell lung cancer (NSCLC) patients, and to explore its effect in improving their quality of life (QOL) and clinical symptoms.
METHODSWe designed a cohort study of 336 radical resected Ib-IIIa NSCLC patients by analyzing disease free survival (DFS) using Log-rank test. They were randomly assigned to the control group (155 cases, treated by adjuvant chemotherapy group) and the test group (181 cases, treated by adjuvant chemotherapy combined CM). By using controlled method, 60 radical resected NSCLC patients undergoing NP/NC program in 2012 (vinorelbine 25 mg/m2, combined with cisplatin 75 mg/m2 on day 1 and day 8/on day 1 or on day 1, 2, and 3; or carboplatin AUC = 5 on day 1) were assigned to the control group (29 cases) and the test group (31 cases). QOL scores (using EORTC QLQ-LC43 questionnaire) and TCM symptoms scores were compared between the two groups before chemotherapy, peri-chemotherapy (one day before the 2nd course of chemotherapy) , and after chemotherapy (20 days after ending the 4th course of chemotherapy).
RESULTS(1) The median DFS was longer in the test group than in the control group, but with no statistical difference between the two groups (42.73 months vs 35.57 months , P = 0.179). In the subgroup analysis, there was statistical difference in IIIa stage DFS. The median IIIa stage DFS of was longer in the test group than in the control group with statistical difference (27.87 months vs 19. 93 months, P = 0.047). (2) In the control study, repeated measured data indicated there was significant difference in physical functions between the two groups (P < 0.05). Total scores for health states decreased more in the test group than in the control group, but with no statistical difference (P > 0.05). Scores for constipation and CM syndrome scores were higher in the test group than in the control group (P < 0.05).
CONCLUSIONSCM had advantages in postponing DFS of radical resected NSCLC patients, especially in IIIa stage. CM could improve their QOL and clinical symptoms during adjuvant chemotherapy.

5.Ecological surveillance on breeding ground for Oncomelania hupensis snails in the areas prevalent with islet-type schistosomiasis using remote sensing technology.

OBJECTIVETo probe rational indices suitable for ecological surveillance on breeding ground for Oncomelania hupensis snails in areas prevalent with islet-type schisitosomiasis using remote sensing technology.
METHODSThree adjacent islets, prevalent with islet-type schistosomiasis, along the Yangtze River within the boundaries of Dongzhi County, Anhui Province were selected as study field for remote sensing analysis. Multi-spectral data were composed and non-supervisedly classified in computer with Idisi software for remote sensing analysis. Values of the normalized difference vegetation index (NDVI), green vegetation index (GVI), bright index (BI), which reflect the greenness and brightness of landscape, were also calculated. Finally, all the results were comprehensively analyzed, combined with data from the field investigation.
RESULTSNDVI, GVI and BI could depict characteristics of the landscape quantitatively. Values of NDVI, BI and GVI were varied in different types of landscapes, and 95% confidence interval of these values suitable for breeding of snails was 0.0522 approximately 0.3566, 2.4162 approximately 28.2672 and 29.3404 approximately 40.3135, respectively. Classification of NDVI showed that type 5 anf type 6 were main breeding ground for snails, and type 4 with values of NDVI from 0 to 0.1 was potential areas for snail propagation. Classification of GVI showed that types 5, 6 and 7 were main breeding ground for snails, and also type 4 with values of GVI from 2 to 10 was potential areas for snail propagation. Both NDVI and GVI showed type 2 and type 3 were temporarily not suitable for snail breeding.
CONCLUSIONIndex figures of NDVI and GVI re-formed by reasonable classification could reflect not only breeding ground for snails and range of the areas for snail propagation in islets, but also their evolving rules, i.e., status of new marshland formation and vegetation growth.

6.Comprehensive surgical approaches to management of various facial nevi.

OBJECTIVETo investigate the effects of the flexible surgical approaches on therapeutic and cosmetic outcomes of facial nevi.
METHODSFrom August 2002 to January 2008, 16 cases with facial nevi had been treated in our wards with the selected approaches. Surgical approaches including serial excision or one-time radical excision, free skin graft, and expanded flap were adopted in accordance with the individual size and location of the facial nevi.
RESULTSAll cases experienced complete excision and had satisfactory cosmetic appearance in the end.
CONCLUSIONThe flexible surgical measures help to minimize the risk of malignant transformation and achieve good cosmetic results.

7.Content comparison of four constituents in aqueous extract of three Psoraleae Fructus processed products

AIM To compare the effects of sealed moistening with brine,simple stir-frying and stir-frying with brine on the contents of psoralen,psoralen,psoralen and psoralen in Psoraleae Fructus aqueous extract.METHODS The HPLC analysis of aqueous extract was performed on a 30 ℃ thermostatic Hibar C1s column (250 mm × 4.6 mm,5 μm),with the mobile phase comprising of 0.1% formic acid-methanol flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelength was set at 246 nm.RESULTS Compared with the raw product,the contents of glycosides and total components in the product processed with sealed moistening with brine were significantly decreased.Stir-frying with brine could significantly promote the dissolution of glycosides (psoralen and psoralen),but had no significant effect on that of aglycones (psoralen and isopsoralen).Simple stirfrying markedly increased the contents of various constituents.CONCLUSION Both simple stir-frying and stir-frying with brine can significantly increase the total content of four constituents in Psoraleae Fructus aqueous extract.
8.Epidemiologic characteristics of influenza in Shanghai from 2013 to 2015

Objective To study the epidemiologic characters of influenza in Shanghai from 2013 to 2015 . Methods Data of outpatient visits for influenza-like illness ( ILI ) were collected through influenza surveillance hospitals .Pathogenic surveillance to part of ILI samples was conducted . Results From 2013 to 2015, the ILI consultation rate ranged from 0.68%~3.73%, with an average rate of 1 .46%.In total 51 445 samples were collected , and 12673 samples were positive , with the positive rate of 24 .63%.The ratio of the patient visited for ILI aged from 25 to 60 was higher than other groups . Conclusion The peak of epidemic of influenza in Shanghai was in winter and summer .The ratio of the patients visited for ILI aged over 60 increased with years .More measures should be taken to protect the elder people .
9.Evaluation of brain dysfunction in chronic anoxic encephalopathy with evoked potentials.

AIMTo explore the relationship between evoked potentials (EPs) and chronic anoxic brain damage by chronic intermittent hypoxia (CIH), and provide theory evidence for diagnosis and treatment of anoxic encephalopathy.
METHODSBAEP and SLSEP were recorded in rat model with CIH (hypoxia group) and rat with normoxia (normal group). Morris water maze was used to observe learning and memory ability. Immunohistochemical method was used to investigate the expression levels of caspase-3 in brain tissue.
RESULTSThe peak latency (PL) of wave I, III, V and the interpeak latency (IPL) of wave III - V, I - V in BAEP in hypoxia group were much longer than that of in normal group (P < 0.05). The PL of wave N1, P1 of SEP in hypoxia group were much longer than that of in normal group (P < 0.05). In the water mase test, the escape latency (EL) of hypoxia group was much longer than normal group (P < 0.01). The number of caspase-3 positive cells in hypoxia group was much larger than that of in normal group (P < 0.05). There was a positive correlation among BAEP, SLSEP, the number of caspase-3 positive neuron and EL of water mase.
CONCLUSIONThe alteration of BAEP and SLSEP has an apparent correlation with chronic anoxic brain damage. This provides theory evidence for diagnosis and treatment of anoxic encephalopathy.

10.Application of bone marrow biopsy imprint in evaluating cellularity.

OBJECTIVETo study the value of bone marrow biopsy imprint in evaluating cellularity.
METHODSThe bone marrow tissues were obtained by trephine biopsy from 272 patients, and then put on the slides to make the imprints. The imprints was stained by Wright-Giemsa method, and the bone marrow smears and imprints were examined simultaneously according to the bone marrow cellularity criteria.
RESULTIn bone marrow cellularity, four grades (distinct decrease, extreme decrease, distinct increase, and extreme increase) were significantly higher in bone marrow imprints than those in bone marrow smears (P <0.05), but there was no significantly differences between bone marrow imprints and sections (P >0.05). Using bone marrow sections as standard, in cellularily decreasing samples, the consistent rate of bone marrow imprints and smears were both high (84.4% and 97.9%), in the group of the normal and increased cellularity, the consistent rate of the bone marrow imprints (84.4% and 97.7%) was significantly higher than that in smears (60% and 64%, P<0.01). The sensitivity, specificity, Youden index, positive predictive value and positive likelihood rate of bone marrow imprints were all higher than those of the smears. Using the bone marrow sections as gold standard, in 124 cases with decreased cellularity in smears, the positive diagnosis rate for aplastic anemia and dyshaematopoiesis based on bone marrow imprints was 37.1% with a false positive rate of 7.3% which was lower than that of the bone marrow smears (false positive rate of 29.8%, P<0.01).
CONCLUSIONTo evaluate bone marrow cellularity, bone marrow imprint is better than bone marrow smear. The combination of the two examinations can make the diagnosis more convenient and quicker.
