1.Symptomatic improvement of cervical spondylosis by sliding cupping and its relation to disease course and age

BACKGROUND: Sliding cupping has been used for treating cervical spondylosis, with the effects of dredging the meridian system, activating qi and removing blood stasis.OBJECTIVE: To probe into symptomatic improvement of cervical spondylosis with sliding cupping and the relationship between curative effect and course of disease as well as age.DESIGN: Self controlled observation.SETTING: Department of Physiotherapy, General Hospital of Air Force of Chinese PLA.PARTICIPANTS: Totally 100 out-patients with cervical spondylosis receiving treatment in the Department of Physiotherapy, General Hospital of Air Force of Chinese PLA were involved from December 2000 to December 2003. Among them, 39 cases were male and 61 cases were female; 9 cases were under 30 years old, 38 cases were 31 to 40 years old, 42 cases were 41 to 50 years old and 11 cases were above 50 years old. There were 36 cases with less-than-one-year course of disease, 49 with 1 to 5-year course and 15 with more-than-5-year course. Fifty-one cases were cervical type; 25 cases vertebral artery type; 24 cases nerve root type. X-ray examination revealed that there were 69 cases whose cervical vertebral physiological curvature became straight or reverse, or with certain degree of degenerative change. Vertebroarterial blood supply insufficiency was detected in 23 cases by rheoencephalogram.INTERVENTIONS: The patients were asked to be in prone sitting position,their nape and shoulder were fully exposed, and the negative pressure cupping was applied to patient' s nape and shoulder until petechiae appeared on the skin. The treatment was given once or twice a week. After one-month treatment, the curative effect was calculated according to subjective symptoms, signs, and the results of X-ray and rheoencephalogram examination before and after treatment. Symptom disappearance: pain was 0 grade; the cervical vertebral physiological curvature restored by 10°- 15° and rheoencephalogram showed that the contraction peak velocity of the vertebroarterial blood supply became normal. Marked improvement: the pain reduced by more than 2 grade, the cervical vertebral physiological curvature restored by 6°9° and rheoencephalogram showed that the vertebroarterial blood supply restored to 80% of the normal range. Improvement: the pain reduced by 1 grade, the cervical vertebral physiological curvature restored by 3°- 5° and rheoencephalogram showed that the vertebroarterial blood supply restored to 40% - 80% of the normal range. Inefficacy: the pain did not reduce, the cervical vertebral physiological curvature restored by less than 3° and rheoencephalogram showed that the vertebroarterial blood supply became less than 40%.RESULTS: According to the intention-to-treat analysis, 100 patients entered the result analysis. For symptom disappearance, marked improvement,improvement, inefficacy there were 27, 17, 5 and 2 cases respectively in cervical type; 8, 10,4 and 3 cases respectively in the vertebral artery type;4,9,7 and 4 cases respectively in the nerve root type. There were 19, 12, 3 and 2 cases respectively with less-than-one-year course of disease; 18, 19, 7 and 5 cases respectively with 1 to 5-year course; 2, 5, 6 and 2 cases respectively with more-than-5-year course. And there were 5, 3 and 1 cases respectively with age < 30 years; 18, 13, 4 and 3 cases respectively with age of 31 to 40 years; 15, 18, 5 and 4 cases respectively with age of 41 to 50years; 1, 2, 6 and 2 cases respectively with age > 50 years.CONCLUSION: The effect of sliding cupping was good for patients with cervical spondylosis of vertical type, next of vertebroarterial type and bad of nerve root type. The shorter the course of disease is, the younger the patient is, and the better the curative effect is.
2.The study of mRNA expression of adiponectin in omental adipose tissue of patients with type 2 diabetes

Objective To study the adiponectin mRNA expression in omental adipose tissue of type 2 diabetes. Methods RT-PCR method was used to examine the adiponectin mRNA expression. Fasting levels of BG,TG,TC,HDL-C,LDL-C of all the subjects were checked and their blood pressure,height,weight,waist circumference,hip circumference were measured to calculate BMI,WHR. Results The adiponectin mRNA expression in omental adipose tissue was decreased in diabetes group versus non-diabetes group (P<0.05),and negatively correlated with WC,TG,FBG,WHR(P<0.05). Conclusions Type 2 diabetic patients show lower expression of adiponectin mRNA in omental adipose tissue than that of non-diabetic control.WC,TG,FBG, WHR and course of disease are correlated with adiponectin mRNA expression and insulin resistance

4.Experience of preventing infection in atlanto-axial operations with transoropharyngeal approach: a report of 80 cases

Objective To summarize the experience of preventing infection in atlanto-axial operations via transoropharyngeal approach. Methods The methods and experiences of prevention of infection in 80 cases of atlanto-axial dislocation operated on via transoropharyngeal approach were retrospectively summarized and analyzed. Result There was no infection in all the 80 cases. Conclusion As long as preventive measures were perfectly taken, the post-operative infection could be effectively prevented and the transoropharyngeal operation would be safe.
5.Subtotal vertebrectomy of axis for spinal cord decompression by transoral anterior approach

Objective To evaluate the clinical results of subtotal vertebrectomy of the axis for spinal canal decompression by transoral anterior approach in the treatment of atlanto-axial dislocation with spinal cord compression. Method fifteen cases of atlanto-axial dislocation with spinal cord compression caused by congenital deformity or trauma were subjected to subtotal vertebrectomy of the axis via transoral anterior approach . Results After an average follow-up of 20 months, curative effect was evaluated according to Symon and Lavender. Vertebral canal vector diameters in MRI were measured. The total clinical effective rate was 100%, and the remarkable effective rate reached 60%. The average improvement rate of vertebral canal decompression was 79.8%. No sign of spinal cord and arteries injury and infection were observed. Conclusion Subtotal vertebrectomy of the axis via transoral anterior approach is proved to be feasible to treat cervical spinal cord compression on C 2 vertebra level.
6.Influence of Aspirin on growth and NOS expression in astrocytoma cell line

Objective:To investigate the effect of Aspirin on the proliferation and NOS expression in astrocytoma cell line,and probe into ins mechanism.Methods:The effect of Aspirin on the growth of astrocytoma cells evaluated by MTT assay;NOS protein levels were determined by immunohistochemistry,NO and CEA concentration in the medium were determined by Griess assay and lepton catch immuning method respectively.Results:Aspirin can inhibit the growth of astrocytoma cells,induce the expression of iNOS,increase the concentration of NO in the medium. The effect of these were in a concentration dependent pattern. Moreover,Aspirin can reduce the concentration of CEA in the medium.Conclusion:Aspirin inhibit the growth of astrocytoma cell line.UP regulated iNOS expression resulting a increase of NO concentration are ascribed to mechanism of antrproliferation activity of Aspirin. CEA is a good indicator in monitoring curative effect of astrocytoma.
7.Combined internal fixation with Magerl and Brooks Techniques for atlantoaxial instability

Objective To evaluate the clinical effect and application value of internal fixation of Magerl combined with Brooks technique for atlantoaxial instability. Methods A total of 18 patients with atlantoaxial instability, reducible atlantoaxial dislocation with reduction after traction and irreducible atlantoaxial dislocation with traction reduction after anterior laxation were treated with internal fixation of Magerl combined with Brooks technique using autologous bone grafts. Results The patients were followed up for an average 18 months (6-36 months). Screw was probably cut into the vertebral artery in one case. The guide needle was cut into the pharynx due to incorrectly penetrating the C 1 anterior arch of atlas for 3.5 cm in one case but no early complications such as spinal cord injury and local infection occurred. The spinal cord function improvement was marked in 3 cases, good in 9, mild in 4 but unchanged in 2. No case got worse. There were no loosening or break of the screw and cable. Conclusions Combined fixation of Magerl and Brooks techniques reaches three-point fixation and improves the reduction effect. It is necessary to use the combined fixation with best biomechanical stability for atlantoaxial dislocation patient with reduction or near reduction if the structure of the posterior arch of C 1,2 is intact.

BACKGROUND: Bone marrow mesenchymal stem cells (MSCs) have been widely accepted by medical investigators due to their advantages including easy obtaining, minimal invasion, with infinite proliferation and multi-differential potential, and without immunological rejection in the autologous transplantation. OBJECTIVE: The goal of this study is to isolate and purify rat bone marrow MSCs in vitro, so as to observe the effects of different concentrations of serum containing kidney-tonifying and blood-activating Chinese herbs on the in vitro proliferation of rat bone marrow MSCs.DESIGN: A randomized controlled animal experiment.SETTING: Hip Center, First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine.MATERIALS: Forty healthy male SD rats of SPF grade, weighing 170 to 180 g, were provided by the Laboratory Animal Center, Guangzhou University of Traditional Chinese Medicine. The protocol was performed in accordance with ethics guidelines for the use and care of animals. The involved rats were divided into 4 groups by random digit table with 10 rats in each: normal control group, high-, middle-, and low-concentration groups. METHODS: This study was performed at the Laboratory Animal Center, Guangzhou University of Traditional Chinese Medicine between January and March 2005. Rat bone marrow MSCs were isolated and purified by Percoll density gradient centrifugation, and cultured in vitro to establish rat bone marrow MSCs culture system. Rats in the high-, middle-, and low-concentration groups were intragastrically administrated with 4.4, 2.2 and 1.1 g/kg serum containing kidney-tonifying and blood-activating Chinese herbs, which equaled to 20, 10 and 5 times of adult dosage, respectively. Rats in the normal control group were intragastrically administrated with purified water for 1 week. One hour after the last administration, 6 mL blood was taken from abdominal aorta of each rat under the aseptic condition. Then, it was centrifuged at 2 000 r/min for 15 minutes, and meanwhile serum was collected. 10% rat serum containing kidney-tonifying and blood-activating Chinese herbs was added to the medium in the high-, middle-, and low-concentration groups, while 10% fetal bovine serum was added in the normal control group. MAIN OUTCOME MEASURES: ① MSCs growth status; ② MSCs morphology was observed by HE staining and Giemsa's staining; ③ MSCs antigen expression was detected by an immunocytochemical method; ④ Effects of different concentrations of serum containing kidney-tonifying and blood-activating Chinese herbs on MSCs growth.RESULTS: ①The primarily cultured bone marrow MSCs began to adhere to the wall 24 hours later and 80% of them reached the confluence 7 days later. ② MSCs took appearance in long shuttle shape or polygon. These cells were little. Nuclei were located in the middle part of cells or a little deviation. The ratio of nucleus to cytoplasm was a little high. ③CD44 expression was found in the cytoplasm of mononuclear cells, and colored blue. Partial MSCs expressed c-Kit. Their cytomorphology and phenotypic expression have the characteristics of MSCs. ④Three days after serum containing kidney-tonifying and blood-activating Chinese herbal medicine being added to high-, middle-, and low-concentration groups, the number of bone marrow MSCs was dose-dependently increased as compared with that in the normal control group. CONCLUSION: Serum containing kidney-tonifying and blood-activating Chinese herbs promotes the in vitro proliferation of bone marrow MSCs.
10.The clinical study of CD64 in infected children treated in ICU of Shanghai Children Hospital

Objective To evaluate the values of CD64 expression in diagnosis of infected patients referred to intensive care unit.Method Sixty febrile children referred to the hospital intensive care unit from 2009.11 to 2010.03 were enrolled for a retrospective study.Fever was defined as a body temperature reaching 38℃ or higher with specifically bacterial infection or highly suspected with bacterial infection or viral infection.There were 28 patients with bacterial infection and 32 with viral infection.The non-infectious diseases such as juvenile rheumatoid arthritis and Kawasaki disease were excluded.The controls were 50 healthy children asking for physical examination.On admission,CD64 were measured by using flow cytometry,and blood routine examination,ESR,PCT,blood cultures and sputum cultures were simultaneously detected in all febrile patients.Data were statistically analyzed by using SAS 16.0 software.Data are given as means±SE.Categorical variables were analyzed using X2 test and continuous variables were compared by applying paired 1-tailed t test,Significance level was set at less than 0.05.Results of them,57.1%bacterial infection patients and 71.9%viral infection patients contracted pneumonia.CD64 in bacterial infection patients、viral infection patients and the subjects of control group were(12.6±9.7),(5.4±2.42)and (2.9±0.77),respectively.The CD64 in the bacterial infection patients were significantly higher than those in the virus infection patients(F=11.002,P=0.004).Conclusions CD64 in infected children referred to a hospital intensive care unit can be clearly distinguished between bacterial infections and viral infections, providing an important guidance and a flexible strategy for clinical treatment and determine the timing of withdrawal.
