1.Plasma Levels of Dehydroepiandrosterone Sulfate ( DHEA - S ) and Total Testosterone in the Patients with Female androgenetic Alopecia.

BACKGROUND: It has been known that androgenetic alopecia in both male and female is due to the role of the androgen hormone in the hair follicle, but the level of the androgen hormone in the plasma is controversial in relation with the development of the alopecia. OBJECTIVE: We have tried the clarify if there is any or no difference in the plasma levels of the hormones between the normal female and the patient with alopecia, because the range of the plasma DHEA-S and total testosterone in the normal female is very wide. METHODS: Plasma levels if the hormones were examined in 22 patients with female androgenetic alopecia and 20 normal females. RESULTS: 1. In DHEA-S, 21(95.5%) aut of 22 female androgenetic alopecia patients were in the normal range while 20(100%) of the 20 normal females were all within the normal range. However the mean value of the hormone(88.89+101.41 g/dl) in the patient group was significantly higher than that(82.18+44.03 g/dl) of the normal group(p<0.05). 2. In total testosterone, 17(94.4%) out of the 18 patents were in the normal range while 20 (100%) of 20 normal females were all within the normal range. However the mean value of the hormone(0.22+0.20ng/ml) in the patient group was significantly higher than that(0.08+0.09ng/ml) of the normal group(p<0.05). 3. As for distribution of DIEA S according to the age, the value of the patient group was higher than that of the normal group in all age groups studied. In the distribution of total testosterone according to age, there was no difference between the normal and the patient in the twenties. After the twenties, the testosterone levels were higher in the patients than the normal females. CONCLUSION: There were significant differences in mean values of plasma DHEA-S and total testosterone between the patients and the normal females although individuals in both groups were largely wit,hin normal range of the hormones.

2.A case of atypical fibroxanthoma.

A 43-year-old male patient showed well defined, irregular, dark eythematous, thick plaque and tumor with ulceration on the right chest, right axilla, pubis and scrotum, Histopathological findings rervealed atypical lymphocytes showing epidermotrophism and heavy infiltrations in the deep dermis and subcutaneous tissue. The infiltrative cells were positively stained with leukocyte common antigen and T-cell UCHL-1 antigen, but not with L26 antigen. This clinicohisto-pathological features were consstent with dermblade type of mycosis fungoides. The authors report clinicopathological findings of the case with its thrapeutic responses to systemic chemotherapy and radiation treatment.

3.A case of hibernoma arising in the scalp.

Hibernoma is a rare subcutaneous neoplasm resembling the brown fat of the hibernating animal. It is usually found in the interscapular, cervical or axillary regions. We report a case of this rare tumor which developed on the occeipital scalp of a 42-year-old woman.

4.Comparison of Sodium Nitroprusside and Esmolol Induced Hypotension for Total Hip Arthroplasty.

BACKGROUND: Esmolol is a short acting sympathetic beta receptor antagonist, and it was successfully applied to induced hypotension. Esmolol lowers blood pressure by decreasing cardiac output, and does not cause vasodilation. This property of esmolol may help to decrease bleeding during induced hypotension. In this study, we tried to elucidate the effect of esmolol on induced hypotension for total hip arthroplasty. METHOD: Twenty patients receiving total hip arthroplasty were randomly divided to two groups. Esmolol group (10 patients) received esmolol as a hypotensive agent, and sodium nitroprusside (SNP) group (10 patients) received SNP as a hypotensive agent. We measured arterial blood gas analysis, vital sign, amounts of bleeding, amounts of transfusion and administered fluid, and various laboratory findings. RESULTS: Induced hypotension was successfully performed in either esmolol and SNP group. Heart rate increased by SNP, and decreased by esmolol. There were no statistically significant differences between the two groups in amounts of bleeding, amounts of transfusion or administered fluid, and laboratory findings. Arterial oxygen tension was relatively constant in esmolol group, but decreased in SNP group. CONCLUSION: Esmolol can be used as a single hypotensive agent during induced hypotension without significant side effects during total hip arthroplasty.

5.Transforming of the drug resistance plasmid from Staphylococcus aureus into Escherichia coli.

OBJECTIVETo discuss the possible mechanism of drug resistance transmission between Staphylococcus and Escherichia coli.
METHODSThe chloramphenicol resistance plasmid of Staphylococcus aureus was extracted to transform the sensitive Escherichia coli, and the drug-resistant Escherichia coli were screened by drug sensitivity test.
RESULTSThe drug-resistant Escherichia coli were successfully obtained.
CONCLUSIONStaphylococcus may have a natural shuttle plasmid of drug resistance, which can transform Escherichia coli under specific conditions.

8.Dietary assessment according to frequency of food consumed away from home among children and adolescents: Based on the 2010~2012 Korea National Health and Nutrition Examination Survey.

PURPOSE: The aim of this study was to investigate the dietary quality of food consumed away from home among Korean children and adolescents. METHODS: Data were obtained from the 2010~2012 KNHANES (Korea National Health and Nutrition Examination Survey) and included 3,583 subjects aged 7 to 18 years old. The frequency of food consumed away from home was dichotomized into more than once or less than once per day. RESULTS: Thirty percent of subjects were shown to consume food away from home more than once per day, and older children from higher income households showed a higher frequency of food consumed away from home compared to other children. The percentage of children and adolescents cited as regularly skipping breakfast was 19%. Individuals who ate out more than once per day showed higher consumption of energy and sodium than those who ate out less than once per day. CONCLUSION: Excessive intakes of energy and sodium are associated with obesity, high blood pressure, diabetes, and obesity-related diseases. Thus, there is a need for nutritional intervention and educational efforts to improve child nutrition and prevent obesity.

9.Pseudolipomatosis of the Gastrointestinal Mucosa: Report of 6 cases with analysis of possible factors involved during endoscopic procedure.

Pseudolipomatosis of the colonic mucosa has been recognized as a lesion featured with aggregations of gaseous spaces in the lamina propria, but its pathogenesis remains still unclear. This paper describes 6 cases of pseudolipomatosis occurring in the mucosa of stomach and large intestine, and the possible factors involved in gastrointestinal endoscopic procedure to produce gaseous entrapment in the lamina propria were analysed. All cases received either gastroscopy or colonoscpy before endoscopic biopsy. Mucosal tissues from both stomach and recto-sigmoid colon revealed multiple aggregations of small air-spaces resembling fatty infiltration in the lamina propria. Rarely were similar infiltrations within the muscularis mucosae and adjacent lymphoid follicles. Evidence for pneumatosis cystoides intestinalis or ulcerative colonic lesion was not associated, although one showed a small gastric ulcer nearby. Repeated inflations and deflations of the stomach or colon during the endoscopic procedure with miner mechanical trauma by instrument to the mucosa seem to contribute to its pathogenesis.
10.Comparative Evaluation of the Treatment of Herpes Zoster with and without Sympathetic Block.

BACKGROUND: Prevention of postherpetic neuralgia as well as management of the symptoms of the acute stage is an important therapeutic goal in the treatment of herpes zoster. Corticosteroid, acyclovir and sympathetic block have all been advocated as effective therapeutic tools for the treatment of the disease. OBJECTIVE: This study was conducted to evaluate the necessity of sympathetic block for prevention of postherpetic neuralgia in acute herpes zoster. METHODS: The author observed the duration and degree of pain and the incidence of postherpetic neuralgia in 54 patients with herpes zoster treated with three different therapeutic measures. Group A was treated with triamcinolone acetonide 80mg three times, acyclovir and sympathetic blockade, group B with triamcinolone acetonide 80mg three times and acyclovir, and group C with triamcinolone acetonide 40mg three times and acyclovir. RESULTS: 1. Addition of sympathetic blockade to corticosteroid and acyclovir therapy did not seem to reduce the duration of pain and incidence of postherpetic neuralgia. 2. The Incidence of postherpetic neuralgia was higher in the elderly, especially with underlying systemic disease including diabetes melitus, hypertension, etc. Al] the therapeutic measures could not completely prevent the development of postherpetic neuralgia in those patients. 3. Regarding corticosteroid dosage, daily triamcinolone acetonide 80mg for three consecutive days showed no better effect than daily 40mg for consecutive days. CONCLUSION: When herpes zoster was treated with proper dosages of corticosteroid and acyclovir in its acute stage, sympathetic block showed no additional beneficial effect.
