1.The application strategy for mammotome in breast diseases

Objective To explore the indication,operative methods,operation skill and prevention of complications of mammotome(MMT) in breast diseases.Methods From Aug.2009 to Aug.2013,3676 breast lesions in 1356 patients received MMT micro-invasive operation by decussation process with different incision and needle tracks according to the location of lesions.Experience of the indication,operation technique and prevention of complications were summed up.Results 775 lesions were in left breast and 639 in right breast.652 patients had single lesion and 865 patients had multiple lesions.The pathology results:320 cysts (8.71%,320/3676) were adenosis of breast with mammary duct ectasia or with metaplasia apocrine.36 solid and cystic tumors (0.98%,36/3676)were intraductal papillomas.3310 solid tumors (90.04%)included 13 breast cancers (0.39%,13/3310),2246 fibroadenomas(67.85%,2246/3310) and 1051 adenosis(31.75%,1051/3310).10nodules of calcification were adenosis of breast.13 patients with breast cancer received modified radical mastectomy.The postoperative results:skin bruises in 184 cases(5%),hematoma in 36 cases(0.98%).No postoperative residual was found.Conclusion According to the strict application of MMT minimally invasive surgery and by decussation process based on the appropriate incision and needle track,MMT minimally invasive surgery can achieve safe,minimally invasive,good-looking and completely removal effect.
2.Effects of L-Arginine on Bcl-2,Bax in Placenta and Blood Flow of Belly Stalk of Fetal Growth Restriction

NMDA (N-methyl-D-aspartate) receptors are a subtype of glutamate receptor, and play an important role in both synaptic transmission and synaptic plasticity. When excessively excited, NMDA receptors can cause cell death in many neuropathological scenarios such as OGD (oxygen and glucose deprivation) and brain injury. However, recent studies have revealed that under the physical and some pathological conditions, NMDA receptor could promote neuronal survival and protect neurons from damage.

4.Prevalence of mental disorders in the male juvenile detention centers of Hunan and Sichuan

To investigate the prevalence of mental health problems in male violent offenders of Hunan and Sichuan Provinces; and to compare the types and severity of problems between the violent and nonviolent offenders.Methods:Ninety-one violent juvenile offenders and 64 nonviolent juvenile offenders in the juvenile detention centers of Hunan,and 81 violent juvenile offenders in the juvenile detention centers of Sichuan; 39 high school students from a middle school of Hunan Province and 49 from a middle school of Sichuan Province were investigated using the Investigation Screening Inventory for Child Mental Disorder and the Kiddy Schedule for Affective Diseases and Schizophrenia (K-SADS-PL),and classified according to the Diagnostic and Statistical Manual of Mental Disorders.Results:In the Hunan violent group,86.6% met the criterion of conduct disorder (CD),17.6% of substance abuse,9.9% of substance dependence,while in the Hunan nonviolent group,75.0% met the criterion of CD,and 11.7% of substance abuse; these were all significantly higher than those of the control group (P<0.05).In the Sichuan violent group,17.3% met the criterion of attention deficit/hyperactivity disorder (ADHD),18.5% of oppositional defiant disorder (ODD),69.1%of CD,and 22.2% of substance abuse; these were all significantly higher than those of the Sichuan control group.Moreover,63.7% of Hunan-violent,55.6% of Sichuan-violent,and 45.0% of Hunannonviolent offenders had a previous offence records.Conclusion:Mental and behavioral disorders among delinquent youth is becoming a serious problem,and there is an urgent need to develop and implement effective assessment and treatment approaches for juvenile offenders with the aim of reducing offence and recidivism in this population.
5.Early diagnosis of lower extremity deep venous thrombosis and therapeutic effect of anticoagulation therapy with Roberts' age adjusted warfarin loading protocol in patients with acute ischemic stroke

Objective To explore the early diagnosis of lower extremity deep venous thrombosis (LDVT)and evaluate the therapeutic effect of anticoagulant therapy in hospitalized patients with acute ischemic stroke. Methods According to Wells model for suspecting lower extremity deep venous thrombosis,patients with suspected LDVT were confirmed by compression ultrasonography. If the patients diagnosed with LDVT had no contraindications to anticoagulant therapy,they were treated with low molecular weight heparin(LMWH)subcutaneous injection and oral warfarin at the same time.The dosage of oral warfarin was determined by Roberts'age adjusted warfarin loading protocol.LMWH was stopped when the patients'international normalized ratio(INR)was 2.0~3.0 for two consecutive days. Results From January 2003 to August 2007,2067 cases with acute ischemic stroke were admitted to the department of neurology in Huizhou Municipal Central Hospital including 18 cases with LDVT and the incidence was 0.9%.The patients with LDVT all had paralytic extremities including 13 left legs and 5 right legs with deep vein thrombosis.All the 18 cases were treated by anticoagulant including 17 cases with oral warfarin treatment for 3 months.Symptoms in all LDVT patients were eliminated.12 cases had been observed for one year and 5 cases for three months after they stopped taking warfatin.There were no patients with pulmonary thromboembolism and LDVT recurrence. Conclusions By using Wells model for suspecting LDVT,patients with acute ischemic stroke-complicated LDVT can be timely diagnosed.The goal of prompt and enough anticoagulant can be achieved according to Roberts'age adjusted warfarin loading protocol.Because of racial difference,population difference and other unknown factors,the incidence of acute ischemic stroke patients with complicated LDVT is much lower in Huizhou.It suggests that it should be unnecessary to use LMWH in patients with acute ischemic stroke to prevent LDVT in Huizhou.
6.Impact of chemotherapy compliance on the therapeutic efficacy of patients with locally advanced nasopharyngeal carcinoma

Objective To evaluate the impact of chemotherapy compliance on the therapeutic efficacy of induction chemotherapy plus concurrent chemoradiotherapy versus induction chemotherapy plus radiotherapy alone for patients with locally advanced nasopharyngeal carcinoma (NPC). Methods Based on intention to treat analysis (ITT) for 400 patients, 314 patients were analyzed by per protocol (PP) analysis. The patients were divided into induction chemotherapy plus concurrent chemoradiotherapy group (IC/CCRT, 127 patients) or induction chemotherapy plus radiotherapy group (IC/RT, 187 patients). The patients who completed 2 cycles of induction chemotherapy and at least 2 cycles of concurrent chemotherapy in the IC/CCRT group and the patients who completed 2 cycles of induction chemotherapy in the IC/RT group were analyzed. Radiotherapy was given by two-dimensional technique with γ-ray, X-ray and electron beams. The chemotherapy regimen was FUDR plus carboplatin for induction chemotherapy and carboplatin alone for concurrent chemotherapy. Results The follow-up rate was 96.2%. 295 patients were followed to at 3 years. Based on PP analysis, Grade 3/4 toxicity was found in 23.6% of the patients in IC/CCRT group and 13.4% in the IC/RT group (χ~2 =5,50,P=0.019). No grade 4 toxicity was found in the IC/RT group. The median follow-up time was 3.9 years, and no significant difference was found between the two groups in 3-year overall survival (78.1% : 84.6% ;χ~2 = 0. 61, P =0. 435), disease-free survival (74.3 % : 70.1% ;χ~2= 0. 12, P= 0.731), Iocoregional relapse-free survival (89.7% : 89.5% ; χ~2= 0. 10, P= 0.748), or distant metastasis-free survival (78.9%:76.5% ;χ~2=0.05,P=0.825). Conclusions With more severe toxicities, the IC/CCRT regimen does not improve the overall survival in locally advanced NPC patients compared with the IC/RT regimen.

8.Structure and Anti-HIV Activity of Betulinic Acid Analogues

Firstly discovered in 1980s,human immunodeficiency virus (HIV) continues to affect more and more people.However,there is no effective drug available for the therapy of HIV infection.Betulinic acid existing in various medicinal herbs and fruits exhibits multiple biological effects,especially its outstanding anti-HIV activity,which has drawn the attentions of many pharmacists.Among the derivatives of betulinic acid,some compounds exhibited inhibitory activities at the nanomolar concentration,and have entered phase Ⅱ clinical trials.This paper summarizes the current investigations on the anti-HIV activity of betulinic acid analogues,and provides valuable data for subsequent researches.
9.Application of DNA Microarray and Sanger Sequencing to the Genetic Diagnosis of Nonsyndromic Hearing Loss

Objective To study genotypes in nonsyndromic hearing loss (NSHL ) patients from Guangxi Zhuang Autonomous Region hearing speech rehabilitation center using DNA microarray in combination with Sanger sequencing .Methods Deaf patients received routine physical and otorhinolaryngoloical examinations as well as pure tone autiometry .Brainstem auditory evoked potential test was performed in uncooperative children .Blood samples were obtained from a total of 136 patients ,male 81 ,female 55 ,age from one year five month to seventeen ,having nonsyndromic hearing loss .Genomic DNA was extracted and then 9 hot mutation spots in 4 susceptibility genes were detected by DNA microarray .GJB2 and SLC26A was further detected by Sanger sequencing in the patients with negative results and heterozygotes .Results Among the 136 patients with nonsyndromic hearing loss ,20 cases were positive for GJB2 gene ,SLC26A4 gene or mitochondrial 12SrRNA gene mutations .There were 14 .71% (20/136)patients were positive for hot mutation spots in the deafness related genes ,25% (34/136)patients carried muta‐tions of deafness related genes using DNA microarray in combination with Sanger sequencing .Six SLC26A4 rare mutations (c .259G> T ,c .754C> T ,c .1229C> T ,c .1548_1549insC ,c .1705+5A>G and c .2086C> T) were de‐tected by Sanger sequencing .c .235delC was the most common mutation in GJB2 gene .c .919-2A>G ,c .754C> T and c .1229C> T were the common mutations in SLC26A4 gene .The mutation rate of GJB2 and SLC26A4 was 38 . 24% .and 58 .82% ,respectively .Conclusion Prevalent deafness-associated gene mutations in the nine loci studied were less frequently detected in nonsyndromic hearing loss patients from Guangxi Zhuang Autonomous Region hear‐ing speech rehabilitation center .It can improve the detection rate of deafness gene mutations by using gene microar‐ray in combination with Sanger sequencing .GJB2 and SLC26A4 are the common causative genes .
10.Analysis of articles published in Chinese Journal of Burns winning high citation rate during 2000-2009.

OBJECTIVETo analyze the inherent quality of articles published in our journal enjoying high citation rate, and to explore strategies on improving impact of papers.
METHODSScientific papers published in Chinese Journal of Burns from February 2000 to December 2009, with citation rate equal to or higher than 20 times were collected for classification according to their publication year, publication form, subject distribution, regional and institutional distribution, frequency of authors appeared in those published papers, frequency of winning prizes, and sources of fund (national, ministerial, or provincial). Data were processed by Microsoft Excel software.
RESULTSAltogether 64 scientific papers with high citation rate were published from 2000 to 2006. Original articles and expert forum accounted for 55 (86.0%). Twenty-one articles of clinical study were cited frequently, among them one was cited for 79 times. Articles dealing with subjects with popular interest or cutting-edge problems were cited frequently. Most articles winning high citation rate were originated from institutions located in Chongqing, Beijing, Shanghai, and Xi'an, etc. Those scientific papers which were instructed by specialists, with high level foundation and won prizes were cited with high frequency. The top 20 articles were mainly cited by excellent doctoral dissertations and master theses originated from 11 institutions for higher education, and source journals of Chinese Scientific and Technical Papers and Citations Database.
CONCLUSIONSAuthors should emphasize subject planning in order to compose papers with high quality. The editorial board should make arrangements with influential specialists with related skills for their contributions based on subjects of popular interest concerning the cutting-edge problems of the specific specialty, and pay close attention to papers on clinical study and those with funding from high levels, to improve impact of the articles.
