1.Advancement in the hip arthroscopy surgery

In the past two decades,with the development of electron optics,computer and imaging,hip arthroscopy surgery has become the best effective method in the diagnosis and treatment of the hip discord,as a method integrates less invasive and invisible characteristics.The indication of hip arthroscopy gradually expands and has become the gold standard in the diagnosis and therapy of hip discord.Recent research shows hip arthroscopy can provide early diagnosis and treatment hip unexplained pain,such as femoroacetabular impingement(FAI),labral tears,chondral lesions,furthmore hip arthroscopy shows good prospects in the treatment avascular necrosis of femoral head,synovial abnormalities,post hip arthroplasty discomfort.
2.Ho∶YAG Laser Operations Under Arthroscope for Meniscal Injuries

Objective To evaluate the outcomes of meniscal injures treated by Ho∶YAG laser using arthroscopy.MethodsA total of 31 patients with meniscal injury were treated with holmium∶YAG laser by arthroscopy from January 2006 to March 2008 in our hospital.Among the cases,11 had flap tear,8 had horizontal tear,6 showed bucket-handle tear,and 6 were found with longitudinal tear.The pre-and post-operative Lysholm score,and compilations in the patients were recorded.Results The operation was completed successfully in all the cases with a mean operation time of 44 min(rang,22-55 min).No injuries to the vessels and nerves occurred in the surgery.The patients got out of bed in 1 to 4 days after the surgery(mean,2.1 days).All the surgical wounds achieved primary healing.Follow-up was carried out in the patients for 9 to 28 months(mean,14.5 months).The patients showed significantly lower Lysholm score at 6 months during the follow-up than that determined before the operation(92?6 vs 57?13,t=13.61,P=0.00).Among the cases,21 showed excellent outcomes,7 were good,and 3 were improved.The rate of excellent and good results was 90.3%(28/31).Conclusion Ho∶YAG laser is a safe,simple,and effective tool in cutting menisci under a arthroscope.
3.By means of key discipline construction improve medical quality

Objective In order to improve the medical quality of our hospital,we adopted the method to strengthen the construction of key disciplines.Methods Xi'an Honghui hospital were retrospectively analyzed from 2008 to 2008 in order to improve medical quality,to take in the discipline construction of main methods and results,and tries to summarize.Results The clinical medical treatment,teaching,scientific research and human resources allocation,etc.Work with key subject as the center,to other disciplines has played a significant radiation and driving effects,medical quality and level are improved significantly.Conclusions The key subject is the inner motive power of sustainable development of the hospital,is one of the important elements to build hospital brand,discipline construction is the key to promote the development of the hospital,is the core and soul of hospital management.
4.The effect of pulsed electromagnetic fields on the proliferation of immunomagnetically separated human pre-cartilaginous stem cells

Objective To study the biological effects of pulsed electromagnetic fields (PEMFs) on the pro-liferation of immunomagnetically separated human precartilaginous stem cells (PSCs) in vitro. Methods The cells from an aborted fetus's metaphysis were digested using collagenase. The PSCs were isolated by magnetic cell sorting (MACS), then subcultured and amplified. Flow cytometry, immunohistochemistry, immunofluorescence and RT-PCR analysis were performed to identify the purified PSCs. The PSCs were stimulated by PEMFs at 50 Hz frequency and 1 mT intensity. Cell proliferation was measured at different time points using methyl thiazolyl tetrazolium ( MTT), and the cell growth curve was plotted. Flow cytometry was applied to measure the cell cycle and apoptosis. Results The PSCs were successfully cultured. There was fibroblast growth factor receptor-3 (FGFR-3) on their sur-face. Cell proliferation was promoted after 4 and 6 days of PEMF stimulation. The percentage of cells at the S phase was higher than in a control group. Early, late and total rates of apoptosis in the experimental group decreased signifi-cantly. Conclusion PEMFs can enhance the proliferation and inhibit the apoptosis of human PSCs, and it is possi-ble to cultivate the high density human PSCs in vitro.
5.Clinical analysis of nosocomial infections in hospitalized cancer patients and effective precautionary measures

Objective:The present study aimed to conduct a clinical analysis of nosocomial infections in hospitalized cancer pa-tients and propose effective precautionary measures. Methods:We retrospectively analyzed 56,430 cases of discharges from the Cancer Institute and Hospital of Tianjin Medical University between January and December 2012. Results:Among 825 cases, the rate of noso-comial infections was 1.46%;46.42%of the patients aged>60 years. The highest infection rate was observed in the Medical Depart-ment of Stomach Cancer. The highest composing rate was recorded in the Department of Hepatobiliary Cancer. The major pathogenic bacteria were Gram-negative bacilli;fungal infections were relatively rare. The most vulnerable body parts were the abdomen and diges-tive tract, which were mainly infected by pathogenic Escherichia coli. Conclusion:Healthcare workers should consider the importance of influencing factors and take precautionary measures to reduce the rate of nosocomial infections in cancer patients.
6.The Impact of Autocapture Function on the Service life of Cardiac Pacemaker

Objective This study aimed to estimate the impact of the threshold of Autocapture algorithm on the pacemaker's service life.Methods Seventy-four patients implanted with VVI pacemaker were retrospectively evaluated.Among them,48 were implanted with pacemaker of autocapture function.Diagnostic data were retrieved from device memory.Pacemaker's service life was estimated according to the working flow and voltage.Results (1) The average working voltage of the control group and the observation group was (2.8 ± 0.4) V and (1.1 ± 0.4)V respectively.The difference was statistically significant;(2) The battery life in the observation group was (12.59 ± 0.55) a,significantly longer than that in the control group (6.74 ± 1.12) a,with an 86.8% increase of the device's estimated service life (P<0.05).Conclusion The Autocapture function results in a significant service life of cardiac pacemaker and represents valuable clinical technology.
7.Literature and bioinformatic analysis of dysregulated genes in lung tissues of hyperoxic lung injury

Objective To explore the pathogenesis mechanism of hyperoxic lung injury and the effective means for its clinical treatment,the difference of the gene expressions between lung tissues of hyperoxic lung injury and normal lung was compared.Methods The differentially expressed genes between lung tissues of hyperoxic lung injury and normal lung were obtained from PubMed.The dysregulated genes in lung tissues of hyperoxic lung injury were analyzed by a series of bioinformatics methods,including pathways,gene ontology and functional annotation clustering analysis.Results 467 lines of differentially expressed genes were found and genes more than 2-fold regulated were 189.We sought the mapping of genes in the KEGG databases through functional annotation tools,and we discovered there were 5 lines of pathways with difference having outstangding statistical significance through metabolic pathways enrichment degree analysis.It reflected the pathways were closely related to hyperoxic lung injury (the 2-fold upregulated genes were 14,the 2-fold down-regulated genes were 6).GO analysis revealed that these genes were involved in hematopietic cell lineage,axon guidance,adherens junction,T cell receptor signaling pathway and focal adhesion.Conclusions Therefore,it is believed that the above-mentioned 20 lines of gnes are the major ones for the hyperoxic lung injury and the research on them will provide effective means for revealing the molecular mechanism of hyperoxic lung injury and identifying the targeted therapy.
8.Package and identification of replication deficient recombinant adenovirus expression vector of channelrhodopsin-2

Background Channelrhodopsin-2 (ChR2)is a cation channel isolated from the eyespot of Chlamydomonas algae and has been used to control neuron activity.The light stimulation is a more precise fashion whether space or time than that of electrical,magnetic and ultrasound stimulation. Objective This study was to construct a replication deficient recombinant adenovirus cxpression vector of ChR2 and to determine its function.Methods Human embryo kidney 293 (HEK293) cell line was cultured and passaged in DF12 medium containing 10% fetal bovine serum(FBS).The ChR2 gene was cloned at the downstream of cytomegalovirus(CMV)promoter of the adenoviral shuttle plasmid pSB291 in sense direction,and the resultant recombinant plasmid pSB291-hChR2- GFP was transfected into HEK293 cell together with plasmid pBHG lox ( deltaE1,3 ) containing adenoviral genome,then small amounts replication deficient recombinant adenovirus expression vector of ChR2 (Ad-ChR2) was obtained.Through amplification gradient centrifugation and dialysis,pure Ad-ChR2 was obtained.Visual cortex cells derived from 4 1-day-old clean Long Evans rats were primary cultured with serum-free culture media and infected by AdChR2.When expressing green fluorescencc,those cells received the stimulated of blue light with 460 nm.Patch clamp technique was applied to record an action potential. Results After purification and concentration,the titer of AdhCHR2 reached 7.9×1010 PFU/ml.Twenty-four hours after transfect of Ad-ChR2,HEK293 cell membrane showed the green fluorescence for the recombinant plasmid with green fluorescence protein under the inversed fluorescence microscope.The HEK293 cells change their shape from flat to round 13 days after transfected.The primary cultured visual cortex cells exhibited the green fluorescence 3-5 days after infected by Ad-ChR2.The action potentials evoked by blue light stimulation were recorded with patch clamp on those cells expressing green fluorescence. Conclusions Ad-ChR2 expressing vector is constructed successfully in this study.It is verified that Ad-ChR2 expressing vector can infect visual cortex cells with visual function.This result is very important for visual plasticity study.
9.The effect of oxidized low-density lipoprotein on the maturation and its immune function of monocyte-derived dendritic cells.

OBJECTIVETo observe the effects of ox-LDL on monocyte-derived dendritic cells.
METHODSDCs were derived from healthy donors and divided into four groups according to the method of stimulation. The cells of blank group, negative group, experimental group and positive group which were treated with PBS, LDL, ox-LDL, TNF-alpha, individually. ox-LDL was added during the late stage of monocyte differentiation. Flow cytometry was used to analyze the cell surface markers and the endocytoses of DCs. (3)H-TdR incorporation was used to measure the proliferation of syngeneic and allogeneic T cells. ELISA assay was used to measure IL-12, MCP-1and MIP1 in cultured medium. Western blot analysis was used to evaluate the content of IkappaBalpha and NF-kappaB of DCs.
RESULTSAddition of ox-LDL during the late stage of monocytes differentiation can upregulate the cell surface markers including CD40 (22.3% vs. 45.6%) and CD86 (25.9% vs. 82.4%), increase the secretion of IL-12 (31.43 pg/ml vs. 126.73 pg/ml) and MCP-1 (59.6 ng/ml vs. 116.3 ng/ml), reduce DCs uptake capacity (46.8% vs. 10.7%), enhance allogeneic T cells proliferation (SI: 4.5 vs. 5.7), promote IkappaBalpha degradation and upregulate the expression of NF-kappaB in DCs.
CONCLUSIONox-LDL can promote the maturation of PBMCs-derived DCs by promoting IkappaBalpha degradation.
