1.Changes of Vascular Endothelial Growth Factor in Serum and Urine in Children with Steroid-Sensitive Nephrotic Syndrome

Objective To investigate the changs of serum and urine vascular endothelial growth factor(VEGF) in children with steroid-sensitive nephrotic syndrome(SSNS) at their active and remission stage.Methods Serum and morning urine levels of VEGF were assayed by Liquichip in 30 patients with SSNS at the active and remission stage and 30 normal age-and sex-matched controls.Results Serum levels of VEGF in patients at the active stage [(186.62?106.21) ng/L] were significantly higher those that at the remission stage [(118.75?73.08) ng/L] and than those in control group [(108.64?54.75) ng/L](P0.05).Morning urine levels of VEGF in patients at the active stage [(201.66?100.46) ng/L] were significantly higher than those at the remission stage [(116.35?55.99) ng/L] and than those in control group [(99.94?42.07) ng/L](P0.05).Conclusions Serum and morning urine levels of VEGF are significantly elevated in patients with SSNS,however they are significantly lower after the treatment of steroid.There is no obvious difference between the patients at the remission stage and healthy controls.VEGF involves pathogenetically and physiologically in SSNS during its onset and growth.
2.Association of angiotensinogen gene polymorphisms with the risk of essential hypertension in the elderly

Objective To investigate the association of angiotensinogen(AGT)gene A-6G、T174M and G-217A polymorphisms with the risk of essential hypertension(EH)in the elderly of Han nationality.Methods Genotypes of AGT gene A-6G,T174M and G-217A polymorphisms in 177 aged EH patients and 86 sex and age-matched controls were analyzed with gene chip technology.Results The A-6G and T174M polymorphisms of AGT gene were significantly associated with EH.The numbers of the three genotypes of A-6G were 113,58 and 6 in the patient group and 70,15 and 1 in the control group(P= 0.014)and those of T174M were 94,77 and 6,60,25 and 1(P=0.031),respectively.G-217A polymorphism was not related to EH.Individuals carrying A-6G AA and T174M CC genotypes showed 57% and 56% lower risk of EH(OR=0.43;95%CI=0.23-0.82 and OR=0.44;95%CI=0.25-0.79, respectively).Conclusions The A-6G AA and the T174M CC genotype may be related with decreased risk of EH and G-217A polymorphism may have little role in the etiology of EH in Han nationality.
3.System thrombolysis combined with percutaneous catheter fragmentation and thrombectomy in acute massive pulmonary embolism

Objective To evaluate the safety and clinical efficacy of system thrombolysis combined with percutaneous catheter thrombus fragmentation and thrombectomy for acute massive pulmonary embolism. Methods Ninteen patients with acute massive pulmonary embolism were treated with IVC filter placement, percutaneous catheter thrombus fragmentation and system thrombolysis combined with anticoangulation using low-molecular-weight heparin.Four of 19 patients underwent adjuvant Stranb Rotarex catheter thrombectomy.Results Twenty-one procedures were performed in 19 patients.Improvement of pulmonary artery patency and initial relief of symptoms immediately occurred in 18 of 19 patients after interventional therapy.The oxygen saturation increased from 86% to 97%.Pulmonary artery pressure decreased from 33? 5mm Hg(1mm Hg=0.133kPa)to 25?5mmHg after interventional therapy(t=13.2,P
4.Influence of topical Iotemax on intraocular pressure following excimer laser corneal refractive surgery

Background Excimer laser corneal refractive surgery is widely used because it reduces the dependency of refractive error eye to glasses.However,the adverse effects of anti-inflammatory drugs is a problem after operation.Lotemax eyedrops is a kind of steroidal anti-inflammatory drug,with a good effect on the operative eye.But its influence on intraocular pressure(IOP) after long-term topical administration is less studied.Objective This study was to investigate the influence of lotemax eyedrops on IOP after long-term topical administration in myopic eyes received excimer laser corneal refractive surgery.Methods An one-month retrospective case serial study was adopted.The case history data of 1552 eyes from 1552 Patients who received excimer laser corneal refractive surgery were collected in Tangshan Eye Hospital from 2011 January to 2012 January,including 321 eyes with laser in situ keratomileusis(LASIK),608 eyes with sub-Bowman keratomileusis (SBK) and 623 eyes with femtosecond laser for flap excimer laser in situ keratomileusis(FS-LASIK).Lotemax eyedrops was topically administered after operation 4 times per day initial and then decreased 1 time weekly for 1 month.IOP was measured in before,1 week,2 weeks,3 weeks,4 weeks after operation with non-contact tonometer.The increasing range of IOP was divided into <5 mmHg,5-15 mmHg and > 15 mmHg after operation.Optical head parameters were measured by Heidelberg retina tomography when IOP increasing after operation.Results Within 1 month,elevated IOP was found in 47 of 1552 eyes after administration of lotemax eyedrops,including 3.12% (10/321) in the LASIK group,3.29% (20/608) in the SBK group and 2.73% (17/623) in the FS-LASIK group,without significant difference among them(x2 =1.95,P>0.05).The IOP elevated 1 week after operation and remained normal after that till drug withdrawal in all eyes.There were no significant differences in the disk area (1.65 ±0.44) mm2 vs.(1.66 ±0.44) mm2,disk volume (0.40 ±0.09)mm3 vs.(0.39±0.08) mm3 and mean nerve fiber layer thickness (0.28 ± 0.08) mm vs.(0.29 ± 0.07) mm in IOP elevation duration compared with 1 month after IOP recovery(t =0.34,0.81,0.48,P>0.05).Conclusions Topical administration of 0.5% lotemax eyedrops after excimer laser corneal refractive surgery leads to reversible high IOP.It is very important to monitor the IOP during use of the drug.
5.Common clinical factor analysis of heart rate variability in patients with cerebral infarction

Objective To explore the characteristics of heart rate variability (HRV) and its impact factors in patients with cerebral infarction and provide evidence for treatment of cerebral infarction. Methods The clinical data of 190 patients with cerebral infarction were analyzed. These patients admitted to our hospital from May 2007 to June 2009 and CT or MRI confirmed their diagnoses. Fifty healthy subjects in the same period were chosen as control group. The 24 h HRV analysis technology was employed to compare the activity impairment of their cardiac autonomic nervous. According to age, gender, type of infarction, infarct location, severity and prognosis, patients with cerebral infarction were divided into several subgroups; the impact of these factors on patients with HRV was analyzed. Results HRV indexes in patients with cerebral infarction were lower than those in the control group (P<0.05). Patients that were older than or equal to 60-year-old showed lower HRV indexes as compared with the patients that were younger than 60-year-old (P<0.05). Except for the temporal frequency, the other HRV indexes satisfied the following conditions: lower HRV indexes in female were showed as compared with those in male (P<0.05); the HRV indexes in patients with lacunar infarction were significantly higher than those in patients with atherosclerotic cerebral infarction or cerebral embolism (P<0.05); patients with right insula infarction showed significantly lower time domain HRV indexes than patients with other position infarction (P<0.05); HRV indexes were low in patients with low GCS scores. The lowest HRV indexes were noted in patients with the incidence of death, followed by patients with improvement and cured patients. Conclusion Autonomic nervous system in patients with cerebral infarction is overbalanced. Early dynamic monitoring of HRV can determine the extent of brain injury and the evolution of the disease, contributing to early identification of high risk patients and guiding the clinical treatment.
6.Changes of Cytokines Measurement in Serum and Cerebrospinal Fluid in Asphyxia Ne onates and Its Relationship with Prognostic

Objective To study the dynamic changes of cytokines including tumor necrosis factor-alpha (TNF-?),interleukin-6 (IL-6),IL-8 in serum and cerebrospinal fluid (CSF) of asphyxia neonates,and to analyze the relationship between cytokines levels and severity of brain damage and neurological outcome. Methods The concentrations of TNF-?,IL-6,IL-8 in serum and CSF were measured by radioimmunoassay in 63 asphyxia neonates. Neurological development was evaluated at 12 months by children′s developmental scale of china.Results The serum concentrations of TNF-?, IL-6,IL-8 were significantly higher in asphyxiated neonates than those in the controls,and they were correlated with the degree of encephalopathy. The level of serum TNF-? was hig-(hest) at the first day and IL-6 was highest at the third day. There was no marked dynamic changes within 5 days in serum IL-8 level. The concentrations of TNF-?,IL-8 in CSF were higher at the first and the third day.The dynamic changes of IL-6 in CSF were similar in serum and they were positively correlated. The serum concentration of IL-6 in severe brain injury group was much higher than those of normal and mild group.The CSF concentration of IL-6 in severe brain injury group was much higher than that of normal group. The CSF concentration of IL-8 in severe brain injury group was much higher than those of the normal and mild group. Conclusions The concentrations of TNF-?,IL-6 and IL-8 are increased both in serum and CSF in asphyxiated neonates which are correlated with severity of hypoxic-ischemic encephalopathy. Cytokine-mediated inflammatory reactions may participate in the mechanism of hypoxic-ischemic brain injury after asphyxiaion.The concentration of IL-6 in serum and IL-6, IL-8 in CSF are correlated with the neurological outcome.
7.Significance of aquaporin-1 and aquaporin-3 expression in colorectal carcinoma.

OBJECTIVETo investigate the significance of aquaporin-1(AQP-1) and aquaporin-3(AQP-3) in the development of colorectal carcinoma.
METHODSThe expression of AQP-1 and AQP-3 was investigated using immunohistochemical staining with Streptavidin Peroxidase in tissues from colorectal adenoma (CRA, n=25), colorectal cancer (CRC, n=50), and adjacent mucosa (CRT, n=50).
RESULTSThe positive rate of AQP-1 was 64%(32/50) in CRC, significantly higher than that in CRT (38%, 19/50) and CRA(32%, 8/25)(P<0.05). The expression of AQP-1 was associated with depth of invasion and lymph node metastasis in CRC patients(P<0.05). The positive rate of AQP-3 was 56% in CRT, 44% in CRA, and 52% in CRC. There were no significant differences (P>0.05). The expression of AQP-3 was associated with age, tumor diameter, and depth of invasion (P<0.05). No significant correlation between the expression of AQP-1 and AQP-3 in CRC was shown by Spearman correlation analysis(P>0.05).
CONCLUSIONSAQP-1 expression is increased in CRC while the expression of AQP-3 is not. There is no correlation between the expression of AQP-1 and AQP-3 in CRC.

8.Effects of acute cold exposure on pulmonary proinflammatory cytokine of rat.

OBJECTIVETo study the effects of acute cold exposure on the inflammation and pathologic injuries in pulmonary of rats, and explore the mechanism induced by cold stress.
METHODSForty male Wistar rats were randomly divided into five groups(n = 8): control group (23 ± 2) °C 2.5 h, -25°C 0.5 h group, -25°C 1 h group, -25°C 2 h group and -25°C 2.5 h group. Rats were exposed to cold at -25°C and no wind by keeping them in a low temperature chamber except control group. Rectal temperatures of the rats were measured before and after cold exposure. The morphological changes of pulmonary were observed by the optics microscope. The levels of tumer necrosis factor-α(TNF- α), interleukin-6 (IL-6) and interleukin-β (IL-1β) in lung tissue homogenate were measured by ELISA.
RESULTSCompared to the control group, body core temperatures of the -25°C 1 h group, -25°C2 h group and -25°C 2.5 h group were decreased significantly, and the D-values of rectal temperature were increased before and after cold exposure (P < 0.05). The infiltration of inflammatory cells and alveolar edema fluid appeared in the lung tissue of the -25°C 2.5 h group. The concentrations of tumor necrosis factor-α (TNF α), interleukin-6 (IL-6) and inter- leukin-1β (IL-1β) in lung tissue homogenate were increased significantly in -25°C l h group, -25°C 2 h group and -25C° 2.5 h group (P < 0. 05).
CONCLUSIONThe infiltration of inflammatory cells and the increase in proinflammatory cytokine from pulmonary may lead to the lung tissue injury after acute cold exposure.

9.Effect of Supportan on nutritional status and immune function of late-staged gastric cancer patients undergoing chemotherapy.

OBJECTIVETo evaluate the effect of Supportan, an enteral nutrition (EN) specific for tumor patients, on the nutritional status and immune function of late-staged gastric cancer patients undergoing chemotherapy.
METHODSSixty-six late-staged gastric cancer patients undergoing chemotherapy were randomly divided into EN group (n=33) and control group (n=33). During chemotherapy, the patients in EN group received Supportan and the patients in the control group received basic diet. On the 14th day before chemotherapy and after chemotherapy, nutritional status and cell immune indicators were evaluated.
RESULTSAs for nutrition indicators, there were no significant differences in EN group before and after chemotherapy (P > 0.05). Total protein, hemoglobin, prealbumin and transferrin significantly decreased after chemotherapy compared with those before chemotherapy in the control group (P< 0.01). The levels of CD4(+), CD8(+) T cells and CD4/CD8 were significantly increased, and NK cells, serum levels of IL-1, IL-6 were significantly decreased after chemotherapy in EN group (P< 0.01). The levels of IL-6 and TNF-alpha were significantly higher after chemotherapy than those before chemotherapy in the control group(P< 0.01). Curative effects of immune nutrition in EN group were superior to that in the control group, however, the differences were not statistically significant. The incidences of nausea, vomiting and marrow inhibition in Supportan group was lower compared with those in the control group, but with no significant difference.
CONCLUSIONSupportan can prevent malnutrition of the late-staged gastric cancer patients undergoing chemotherapy, and improve immune function and alleviate adverse effects of chemotherapy.

10.Comparison of radiography,CT and MR imaging in detection of arthropathies in patients with hemophilia

Objective To compare MR,CT,and radiography in the detection of arthropathies in patients with hemophilia.Methods Forty-one symptomic joint images in the 14 patients with hemophilia, aged from 11 to 24 years,were used in this study.Each joint had the examinations of radiography,CT and MR within one day.The severity of each joint was staged using conventional radiographic classification. Severe HA patients with stage 5 were excluded from the study.Imaging findings of soft tissue swelling, osteoporosis,epiphyseal overgrowth,joint erosion,cyst,joint space narrowing,bone marrow,joint effusion, hemorrhage,synovial hypertrophy,widened intercondylar notch as well as anterior and posterior crueiate ligaments(only for knee joint)were used for the all imaging comparison.Results The 41 symptomatic joints in 14 patients with hemophilia were classified by radiographic criteria into stage 0(n=5),stage 1(n=7),stage 2(n=6),stage 3(n=8)and stage 4(n=15).Soft tissue swelling or joint effusion was observed in 33 joints by radiographs,in 34 joints by both CT and MR.Joint erosions were demonstrated in 34 joints by MR,in 33 joints by CT and 20 joints by radiographs.Joint cysts were shown in 21 joints by MR,in 18 joints by CT and 9 joints by radiographs.Significant differences in detection of erosion and cyst were found between radiography with either CT(P0.05).MR showed improvement for detecting nlore loci of both erosion and cyst than CT and radiography,and also CT showed the improvement than radiography.Bone marrow edema 14 joints, hemon'hage in 34 joints and synovial hypertrophy in 27 joints were revealed on MR images.Conclusion MRI is superior to CT and conventional radiography in detecting the abnormal changes and should be considered as the first choice among the imaging modafities in evaluating hemophilic arthropathies.
