1.Effects of preoperatively glutamine administration on rat lung injury induced by intestinal ischemia/reperfusion

Objective To evaluate effects of pretreatment with glutamine on lung injury induced by intestinal ischemia/reperfusion(II-R) in rats. Methods Glutamine or saline were injected through tail vein before the model of intestinal ischemia/reperfusion in rats were established.The gene expression of intercellular adhesion molecule-1(ICAM-1) and heat shock protein-70(HSP-70) were tested with RT-PCR methods.The levels of heat shock protein-70 in the lung were measured with Western Blotting.Myeloperoxidase and malondialdehyde and pathological changes were also measured. Results The gene expression of heat shock protein-70 was enhanced by pretreatment with glutamine,and the level of HSP-70 was parallelly increased.Nevertheless,MPO,MDA and the gene expression of ICAM-1 were inhibited. Conclusion Pretreatment with glutamine can lessen the lung injury induced by intestinal ischemia/reperfusion in rats,the induction of HSP-70 gene may be one of the potential mechanisms.
2.Construction and characterization of a new simian/human immunodeficiency viruses clone carrying an env gene derived from a CRF07_BC strain.

BACKGROUNDThe CRF07_BC recombinant strain has been one of the most predominantly circulated HIV-1 strains in China, it is therefore necessary and urgent to develop a relevant animal model to evaluate candidate vaccines targeting HIV-1 CRF07_BC. A highly replication-competent simian/human immunodeficiency viruses (SHIV) construct containing the Chinese CRF07_BC HIV-1 env gene with the ability to infect Chinese rhesus monkeys would serve as an important tool in the development of HIV vaccines. The aim of this study was to examine whether SHIV XJDC6431 with the env fragment from a Chinese HIV-1 isolate virus could infect the human and monkey peripheral blood mononuclear cell (PBMC), establish infection in Chinese rhesus macaque.
METHODSA SHIV strain was constructed by replacing the rev/env genes of SHIV KB9 with the corresponding fragment derived from the HIV-1 CRF07_BC strain. The infectious activity of the SHIV clones was determined in vitro in PBMCs from both non-human primate animals and humans. Finally, one Chinese rhesus macaques (Macaca mulatta) was infected with one SHIV via intravenous infusion.
RESULTSOne SHIV clone designated as SHIV XJDC6431, was generated that could infect macaque and human PBMC. The virus produced from this clone also efficiently infected the CCR5-expressing GHOST cell lines, indicating that it uses CCR5 as its coreceptor. Finally, the virus was intravenously inoculated into one Chinese rhesus macaque. Eventually, the animal became infected as shown by the occurrence of viremia within 3 of infection. The viral load reached 105 copies of viral RNA per ml of plasma during the acute phase of infection and lasted for 10 weeks post infection.
CONCLUSIONSWe conclude that SHIV XJDC6431 is an R5-tropic chimeric virus, which can establish infection not only in vitro but also in vivo in the Chinese rhesus macaque. Although the animal inoculated with SHIV XJDC6431 became infected without developing a pathologic phenotype, the virus efficiently replicated with a persistent level of viral load in the plasma. This suggested that the SHIV could be used as a tool to test candidate AIDS vaccines targeting the Chinese HIV-1 CRF_07BC recombinant strain.

3.Effect of the degree of labor pain at the time of intraspinal labor analgesia on the incidence of cesarean delivery

Objective:To assess the degree of labor pain at the time of intraspinal labor analgesia in women influencing the incidence of cesarean delivery.Methods:Five hundred and ninety patients were enrolled in the study and divided into 3 groups in the light of numeric rating scale (NRS).The woman was with singleton pregnancies, with cervical dilatation of less than 4 cm and vertex fetal presentations.Namely, L group:1-4;M group:5-7;H group:8-10, respectively.The EA procedure was performed at the lumbar spine between 2-3 or 3-4.We inserted a catheter into the epidural space started with 1.0% carbonated Lidocaine to make the volume 5 mL.After the block plane arising, injected a mixture of 0.1% Ropivacaine co-administered with 5 μg of Sufentanil to make the total volume 10 mL in all cases, and connected the patient controlled epidueal analgesia (PCEA) thirty minutes later.The CSEA procedure was started with an intrathecal standard mixture of 0.1% Ropivacaine co-administered with 3 μg of Sufentanil to make the total volume 3 mL in all cases, then connected the PCEA ninety minutes later.To observe and record the NRS score when analgesia was performed.The time of the first stage of labor, the time of the second stage of labor and the way of delivery were recorded in our study respectively.Results:There was no statistically significant differences among the three pain groups in the NRS score after 30 min, dilatation, duration of first stage of labor and duration of second stage of labor, respectively.There was no statistically significant differences among the three pain groups in the incidence of instrumental delivery and cesarean delivery.Conclusions:There is no effect of the degree of labor pain at the time of intraspinal labor analgesia in women on the incidence of cesarean delivery.
4.Clinicopathological significance of aberrant methylation of the fragile histidine triad gene in patients with hepatocellular carcinoma.

OBJECTIVETo investigate the aberrant methylation of fragile histidine triad (FHIT) gene and to explore possible relationship between the aberrant methylation of FHIT and clinicopathological features in hepatocellular carcinoma (HCC).
METHODSThe hypermethylation of FHIT was detected by the methylation specific PCR (MSP) method in 45 patients with HCC (tumoral and nontumoral tissue), 14 cases of normal livers and 4 HCC cell lines (SK-Hep-1, Hep-G2, Hep-3B and Huh7). The correlation of FHIT methylation and clinicopathological features was analyzed.
RESULTSThe frequencies of hypermethylation of FHIT in tumoral and nontumoral tissue, normal liver and cell lines were 71.1%, 64.4%, 14.3% and 75.0%, respectively. A significant relation between hypermethylation of FHIT and poor survival was present (P = 0.0430).
CONCLUSIONSHypermethylation of FHIT is a frequent and early event in HCC, it might relate to a poor prognosis for patients with HCC.

5.Effects of crystalloid and colloid coload on placental circulation of puerperae underwent spinal-epidural anesthesia

Objective:To compare the effects of crystalloid and colloid coload on placental circulation for puerperae underwent spinal‐epidural anesthesia and fetus .Methods : Totally 100 cases scheduled for planned cesarean delivery under spinal anesthesia were recruited for this study .American Society of Anesthesiologists (ASA) score was Ⅰ‐Ⅱ .Parturients were randomly selected into H group and R group ,with 50 cases in each group .H group was received 6% hydroxyethyl starch 130/0 .4 sodium chloride injection ,while the R group was applied with Lactated Ringer solution .Two groups were received speedy injection of HES or Lactated Ringer solution before spinal‐epidural anesthesia ,the speed with 0 .4 mL/(kg · min) until delivery . The dilatation stopped immediately after delivery , and puerperal received routine infusion according to haemodynamics .Before anesthesia ,5 min after anesthesia ,the systolic/diastolic ratios (S/D) ,pulsatility index (PI)and resistance index (RI)were detected by using color Doppler ultrasound .After delivery ,the umbilical artery blood and umbilical veins blood were analyzed ,and the Apgar scores were compared .Phenylephrine 0 .1 mg was applied for hypotension during surgery .Results:After spinal‐epidural anesthesia ,the umbilical artery PI and S/D in two groups were reduced .There was no statistical significance of RI between before anesthesia and after anesthesia or between two groups . The incidence of hypotension during surgery in H group was lower than that in R group (P=0 .03) .The phenylephrine requirements in R group was higher than that in H group (P=0 .001) .There was no statistical significance of blood gas analysis and Apgar score in two groups .Conclusions :The spinal‐epidural anesthesia can lower the resistance of umbilical artery blood .Comparing to Lactated Ringer′s solution ,colloid coload significantly increased placental perfusion ,and decrease the incidence rate of hypotension during surgery .
6.Preliminary experience of clinical applications of the 7th UICC-AJCC TNM staging system of esophageal carcinoma.

OBJECTIVETo compare the instructive value of the 6th and 7th editions of the UICC-AJCC staging system in prognosis of esophageal cancer (EC) patients.
METHODSThe staging and prognosis of 1397 esophageal carcinoma patients undergoing curative resection from Jan. 2003 to Dec. 2006 in our hospital were retrospectively reviewed and analyzed according to the 6th AJCC staging system and the 7th UICC-AJCC staging system.
RESULTSThe 5-year overall survival (OS) of EC patients with curative resection was 38.5% (481/1250 cases), with a follow-up rate of 89.5% (1250/1397 case). In overall terms, both the editions were statistically significant discriminators of OS (P < 0.05). The 5-year OS of stages I, II and III patients were 64.9%, 43.5%, 25.2% according to the 6th edition, and 63.5%, 44.5%, 23.5% according to the 7th edition, respectively. Distinct differences in survival were present among patients categorized as stage Ia and Ib according to the 7th edition (P < 0.05), with a 5-year OS of 80.0% and 58.3%, respectively. Similarly, according to the 7th edition, the 5-year overall survivals (OS) of the stages IIIa, IIIb and IIIc patients were 28.2%, 18.4% and 16.7%, respectively, showing that the prognoses were significantly different (P < 0.05). In addition, according to the 7th edition, the prognoses of patients in stages N0, N1, N2 and N3 were also significantly different (P < 0.01), and the 5-year OS were 50.0%, 31.5%, 18.7% and 16.7%, respectively.
CONCLUSIONSBoth the 6th and 7th editions of UICC-AJCC staging system are significant discriminators for survival of esophageal cancer patients. The 7th edition is proved to be more accurate in prognosis. The number of lymph node metastases is an important predictor of prognosis.

7.The effect of As2O3 on induction of apoptosis and inhibition of telomerase activity in colon cancer LS-174T cells.

OBJECTIVETo study the impact of arsenic trioxide (As2O3) on human colorectal carcinoma LS-174T cells and their activity of telomerase.
METHODSLS-174T cells and xenograft model of nude mice were treated with As2O3. The inhibitory effect of As2O3 on survival of LS-174T cells was determined by MTT assay. Apoptosis was determined by electron microscopy and fluorescence microscopy. Cell cycle was assessed by flow cytometry. Telomerase activity in LS-174T cells was determined by PCR-ELISA kit.
RESULTSWith the increasing concentration of As2O3, the ratio of living cells to dead cells decreased significantly, and the IC50 value was 5.23 micromol/L. Apoptosis curve appeared after 24 h and cells turned to apoptosis in a time-dependent manner. As2O3 inhibited the telomerase activity in cell extraction, obviously in a concentration-dependent and time-dependent manner. Inhibitiory effect of As2O3 on xenograft model of nude mice was observed by tumor volume and weight measurement, showing a significant difference between As2O3 and control groups (P < 0.05).
CONCLUSIONBoth the experiments in vitro and in vivo showed an inhibitory effect of As2O3 on colonrectal cancer S-174T cell growth, probably by induction of apoptosis and inhibition of telomerase activity.

8.Comparison of the short-term outcomes of surgical treatment for non-small cell lung cancer via video assisted thoracoscopic surgery and open thoracotomy.

OBJECTIVETo compare the short-term outcomes of surgical treatment for non-small cell lung cancer (NSCLC) by video-assisted thoracoscopic surgery (VATS) and open thoracotomy (OT).
METHODSData of 737 consecutive NSCLC patients who underwent surgical treatment for non-small cell lung cancer by video-assisted thoracoscopic surgery and 630 patients who underwent pulmonary resection via open thoracotomy (as controls) in Cancer Institute & Hospital, Chinese Academy of Medical Sciences between January 2009 and August 2011 were retrospectively reviewed. The risk factors after lobectomy were also analyzed.
RESULTSIn the 506 NSCLC patients who received VATS lobectomy, postoperative complications occurred in 13 patients (2.6%) and one patient died of acute respiratory distress syndrome (0.2%). In the 521 patients who received open thoracotomy (OT) lobectomy, postoperative complications occurred in 21 patients (4.0%) and one patient died of pulmonary infection (0.2%). There was no significant difference in the morbidity rate (P > 0.05) and mortality rate (P > 0.05) between the VATS group and OT group. In the 190 patients who received VATS wedge resections, postoperative complications occurred in 3 patients (1.6%). One hundred and nine patients received OT wedge resections. Postoperative complications occurred in 4 patients (3.7%). There were no significant differences for morbidity rate (P = 0.262) between these two groups, and there was no perioperative death in these two groups. Univariate and multivariate analyses demonstrated that age (OR = 1.047, 95%CI: 1.004 - 1.091), history of smoking (OR = 6.374, 95%CI: 2.588 - 15.695) and operation time (OR = 1.418, 95%CI: 1.075 - 1.871) were independent risk factors of postoperative complications.
CONCLUSIONSTo compare with the NSCLC patients who should undergo lobectomy or wedge resection via open thoracotomy, a similar short-term outcome can be achieved via VATS approach.

9.Effect of Xinling Wan in treatment of stable angina pectoris: a randomized, double-blinded, placebo parallel-controlled, multicenter trial.

To evaluate the effectiveness and safety of Xinling Wan on patients with stable angina pectoris, a randomized, double-blinded, placebo parallel-controlled, multicenter clinical trial was conducted. A total of 232 subjects were enrolled and randomly divided into experiment group and placebo group. The experiment group was treated with Xinling Wan (two pills each time, three times daily) for 4 weeks, and the placebo group was treated with placebo. The effectiveness evaluation showed that Xinling Wan could significantly increase the total duration of treadmill exercise among patients with stable angina pectoris. FAS analysis showed that the difference value of the total exercise duration was between experiment group (72.11±139.32) s and placebo group (31.25±108.32) s. Xinling Wan could remarkably increase the total effective rate of angina pectoris symptom score, and the analysis showed that the total effective rate was 78.95% in experiment group and 42.61% in placebo group. The reduction of nitroglycerin dose was (2.45±2.41) tablets in experiment group and (0.50±2.24) tablets in placebo group on the basis of FAS analysis. The decrease of symptom integral was (4.68±3.49) in experiment group and (3.19±3.31) in placebo group based on FAS analysis. Besides, Xinling Wan could decrease the weekly attack time and the duration of angina pectoris. PPS analysis results were similar to those of FAS analysis. In conclusion, Xinling Wan has an obvious therapeutic effect in treating stable angina pectoris, with a good safety and a low incidence of adverse event and adverse reaction in experiment group.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
