1.Protective effects of curcumin or dexamethasone on ischemia-reperfusion injury of rat lung grafts

Objective To investigate the curative effects of curcumin(CUR)or dexamethasone (DXM)on ischemia-reperfusion injury(IRI)of rat lung grafts.Methods Male SD rats were randomly divided 4 groups:CUR group(CUR was administered intraperitoneally to both donors and recipients at 3 h prior to operation);DXM group(DXM was administered intraperitoneally at 30 min prior to operation);vehicle group(Animals were injected with the DMSO to both donors and recipients at 3 h prior to operation);sham group(Time-matched control animals underwent the same surgery,except that no graft was implanted).Six animals were sacrificed at different reperfusion periods of 2 h and 24 h,respectively.Oxygenation indexes(PO_2/FiO_2),lung injury scores,wet/dry ratio(W/D)and myeloperoxidase(MPO)activity in the transplanted lung were measured.Malondialdehyde(MDA), total anti-oxidative capacity(TAOC),tumor necrosis factor(TNF)-?and interleukin(IL)-6 in the transplanted lung and serum were determined.Results The levels of LPV PO_2/FiO_2 were significant- ly higher in the CUR and DXM groups than in the vehicle groups both 2 and 24 h after reperfusion,re- spectively(P

The high molecular weight glutenin subunits (HMW-GS) are the main components of storage proteins of wheat,and play a critical role in determining the visco-elastic properties of gluten. There are both quantitative and qualitative effects of HMW-GS on the processing properties of wheat. Current knowledge of the molecular structures,compositions and properties of the gluten proteins of wheat is summarized in details,and the role of the HMW-GS in determining the quality of the grain for breadmaking and how their amount and composition can be manipulated leading to changes in dough mixing properties is also discussed systematically.
3.The effects of extracorporeal membrane oxygenation in severe cardiopaths with crisis

Objective To investigate the effects of extracorporeal membrane oxygenation (ECMO) in severe cardiopaths with crisis. Methods Fourteen cases of severe eardiopaths with crisis were treated by ECMO with V A- ECMO technique, whose pump was centrifugal pump and whose tubes was spread byheparin. The cases included 8 fulminating myocarditis (FM) cases with ventricular arrhythmias or/and acute heart failure or/and cardiac shock and 6 acute myocardial infarction cases with pump failure or cardiac shock,in whom 10 cases with cardiorespiratory resuscitation. Haemodynamics and blood gas analysis, and so on were measured before and after treatment. Results The support time of ECMO was 3-106 h, mean (32.4±27.6) h. After ECMO mean arterial blood pressure (MAP), left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDd), partial pressure of arterial oxygen, saturation of blood oxygen improved (P < 0.05 or < 0.01 ), negative value of base excess decreased significantly( P < 0.01 ). MAP had notchanged before and after stopping ECMO [ (80.02±10.20) wan Hg (1 mm Hg = 0.133 kPa) vs (76.34±9.15) mm Hg] (P > 0.05), however, LVEF and LVEDd improved continually (P <0.05). Conclusion ECMO can provide oxygen supply and stable circulation volume for severe cardiopaths with crisis to recover cardiorespiratory function or save valuable time to treat primary disease.

Objective To study the relationship among apoptosis,NF-KB activation and uPA expres- sion in human colon carcinoma cell line HCTll6 induced by 5-fluorouracil,and to observe the effect of in- hibiting activity of NF-KB by PDTC on apoptosis as well as expression of uPA.Methods Cell apoptosis was analysed by Annexin V-FITC.Fluctuation of NF-KB and uPA was detected by semi-quantitative immuno- histochemistry.Results 5-fluorouracil could induce apoptosis and activate NF-KB.PDTC could significantly increase the apoptosis and suppress the activation of NF-KB induced by 5-fluorouracil.There was a positive correlation between the changes of uPA and NF-KB.Conclusion 5-fluorouracil could induce apoptosis,ac- tivate NF-KB and up-regulate expression of uPA of HCT116 cells.The mechanism of enhanced apoptosis by PDTC may be related to suppressing activation of NF-?B and down-regulating expression of uPA.
5.Survey on Off-label Drug Use in Some Medical Institutions of Sichuan Province

OBJECTIVE:To provide reference for formulating related provisions,guidelines and regulations for off-label drug use,standardizing medical behavior in medical health institutions. METHODS:According to the connotation of off-label drug use,Questionnaire for off-label drug use was developed for medical institutions,in which drug name,treatment disease,the type and use evidence ofoff-label drug usewere included. The questionnaires were delivered to pharmacists who participatedClinical phar-macist practice skills training in Sichuan province to collect the related information of off-label drug use for statistical analysis, and suggestions were put forward. RESULTS:Totally 150 questionnaires were sent out,124 were effectively received with effec-tively recovery of 82.7%;the surveyed pharmacists came from 22 medical institutions,including 18 tertiary hospitals and 4 second-ary hospitals;there were 128 information about off-label drug use,including 14 (10.9%) with incomplete reporting information. The another 114 information were major in“super indication”(61) and“super administration”(43),accounting for 53.5% and 37.7%,respectively,and there are 8 other drug overdose(7.0%),2 super object(1.8%). The relevant off-label drug use mainly included anti-infection drugs (34 information,29.8%),anti-tumor drugs (16 information,14.0%),TCM injections (8 informa-tion,7.0%),and etc.;the relative concentration of varieties were metronidazole,dexamethasone,gentamicin,methotrexate, azithromycin,irinotecan,vancomycin,nystatin,etc.. Some medical institutions were absence of effective regulatory measures. CONCLUSIONS:Off-label drug use is quite common in medical institutions of Sichuan province,some of them exist obvious med-ication risk. It is suggested that the state should be as soon as possible to develop related provisions,guidelines and regulations for off-label drug use,endowing legal force for off-label drug use;medical staff should avoid no evidence-based drug use,unreason-able or even unnecessary off-label drug use;pharmaceutical production enterprises should strengthen communication with medical institutions and pay attention to collect information related to the clinical medication,timely update the drug instructions to ensure the legitimate rights for patients and the interests for medical staff.
6.Risk factors affecting the prognosis of adult cardiogenic shock patients treated with extracnrporeal membrane oxygenation

Objective To find out risk factors affecting the prognosis of adult cardiogenic shock patients treated with extracorporeal membrane oxygenation.Methods From January 2003 to December 2010,patients with cardiogenic shock required veno-arterial ECMO after failure of conventional therapy and intra-aortic balloon pump counterpulsation therapy were retrospectively studied. Patients with severe traumatic brain injury,advanced malignancies and multiple organ failure were excluded.All patients were divided into survival group and death group.The risk factors were found out using one-way ANOVA and a multivariate logistic regression analysis was used to determine independent factors associated with survival.Results Thirty-one patients successfully weaned from ECMO. Twenty-two patients were successfully discharged.The average duration of ECMO was 41.56 ± 43.07 hours.Factors associated with failure of hospital discharge were age,pre-ECMO levels of ejection fraction,pre-ECMO levels of lactate,disseminated intravascular coagulation,renal failure and multiorgan failure (P < 0.05). Conclusions Irreversible heart failure and the complications are significantly correlated with survival,and the early use of ECMO for cardiogenic shock and recognize the factors are key to the success of ECMO treatment.
7.MRI demonstrates abnormalities of motor nerves and extraocular muscles in congenital fibrosis syndrome due to mutations in KIF21 A

AIM:To observe the structural basis of ocular motility abnormalities in patients with congenital fibrosis of the extraocular muscles type Ⅰ ( CFEOM Ⅰ) due to missense mutations in the developmental kinesin KIF21A using high - resolution magnetic resonance imaging ( MRI) .
METHODS: Totally 11 affected individuals reported KIF21A mutations were correlated with MRI studies demonstrating extraocular muscles ( EOMs ) size, location, contractility, and innervation. EOMs and the motor nerve in the orbits were imaged with T1 weighted in a triplanar scan using a dual-phased coils with 2. 0mm thick. Motor nerves were imaged at the brainstem using head coils and 3D-FIESTA with 0. 6-mm thick.
RESULTS: Patients with CFEOM Ⅰ exhibited different degrees of hypoplasia of oculomotor nerve, the abducens nerve and the trochlear nerve were also affected, of which 8 cases of orbital section could see the signal of abnormal nerve dominated by oculomotor nerve to lateral rectus. The both sides of six EOMS in all patients exhibited variable atrophy and abnormal bright internal signal on T1 imaging, particularly severe for the superior rectus and levator muscles.
CONCLUSION: High - resolution MRI can directly demonstrate pathology of motor nerves,affected EOMs, and ‘Pulley' hypoplasia caused by CFEOM Ⅰ due to mutations in KIF21A,and these findings suggest that the neuronal hypoplasia is the etiological factor of CFEOM.
8.Analysis of epidemiological features of endemic arsenism in Huhhot of Inner Mongolia in 2004-2006

Objective To study the epidemiological features of the endemic arsenism in Hohhot of Inner Mongolia in 2004-2006 in order to get scientific evidence for the control and prevention. Methods The historical data in Huhhot revealed that 51 arsenic villages in 3 counties(Tumotezuo,Tuoketuo and Helingeer) were investigated. When arsenic content in drinking water was 0.05-0.15 mg/L,the situation of endemic arsenism was investigated in the households having high arsenic contents of water. When arsenic content in drinking water was greater than 0.15 mg/L,the survey was conducted to investigate arsenic patients with census. Census rate was not lower than 90%. Patients of endemic amenism were diagnosed by The Standard of Diagnosis for Endemic Arsenism (WS/T 211-2001). Results A total 28 083 people were examined,3978 were determined to be patients with arsenism,including 1722 being suspicious, 1897 mild,329 secondary and 30 severe. The total incidence of arsenism was 14.17% (3978/28 083),while the incidence of suspicious,mild,secondary and severe disease was 6.13% (1722/28 083) ,6.75% (1897/28 083),1.17% (329/28 083) and 0.11% (30/28 083),respectively. Suspicious and mild patients were most frequently seen. The incidence of dyspigmentation,pigmentation,ketatoses,both dyspigmentation and pigmentation and all of skin disorders was 8.65% (2428/28 083),3.19% (895/28 083),6.48% (1821/28 083),2.13%(598/28 083) and 0.94%(265/28 083),respectively. The most seen skin disorders were dyspigmentation and ketatoses. The disease mostly occurred in those aged over 40 years,the incidence was 89.52% (3561/3978). The incidence of male [15.08%(2209/14 644)]was more than that of female[13.16%(1769/13 439)].Conclusions Current condition of arsenism in Hohhot is mild,arsenic disease surveillance should keep going. Prevention and control needs to be reinforced in the future.
9.Effect of ERK1/2 Phosphorylation on Proliferation of Human Aortic Vascular Smooth Muscle Cells stimulated by Advanced Glycation End Products

Objective To investigate the effect of ERK1/2 phosphorylation on the proliferation of human aorta vascular smooth muscle cells (HAVSMCs) stimulated by advanced glycation end products (AGEs) Methods CCK8 was used to test the effect of AGEs with different concentration on the proliferation of HAVSMCs, and the effect of PD98059, a specific inhibitor of ERK1/2, on HAVSMCs proliferation stimulated by AGEs was also detected. Flow Cytometer (FCM) was used to detect the cell cycle transformation induced by AGEs. Western Blot was used to detect the expression of relative proteins. Results 10 mg/L AGEs observably facilitated the proliferation and the DNA synthesis of HAVSMCs and PD98059 (40 umol/L) markedly inhibited the proliferation and cell cycle evolution of HAVSMCs induced by AGEs. Furthermore, ERK1/2 phosphorylation, and PCNA were regulated by AGEs and thus it showed time and dose dependent. Conclusion AGEs participates in the proliferation of HAVSMCs by activating ERK1/2 signal path.
10.Clinical Value of ITMAIS in the EPLAD Assessment for Profound Hearing Impaired Children with Hearing Aids

Objective To explore the clinical value of ITMAIS in the early prelingual auditory development (EPLAD)assessment for bilateral profound hearing impaired children with hearing aids.Methods Profound sensorineural hearing impaired children fitted with hearing aids during October 2008 and July 2011 in our center were recruited in this retrospective study, and they would be tested with the ITMAIS at baseline, 3 months, and 6 months after hearing aids fitting.A total of 51 children were recruited in the study, and divided into 2 age groups according to the age at fitting, 1 yr group (28 children) and 2 yrs group (23 children).Results The EPLAD of children in both groups improved significantly in the first half of year after fitting (P<0.05).The mean EPLAD levels of children in both groups were close to or achieved the mean level of severe sensorineural hearing impaired children without intervention when they had had the hearing aids for 6 months.Conclusion Hearing aids could help the children with profound sensorineural hearing loss to improve the abilities of EPLAD.ITMAIS is a simple, an effective tool for assessing the early outcome of children with hearing aids, especially for the children who could not cooperate with the behavioral hearing test.
