6.A case of acne keloidalis nuchae treated by surgical excision.

A 43-year-old man presented with well-defined purple to skin-colored 2X1cm sized plague and various sized papules on the posterior neck. Histopathological findings were follicular and perifollicular inflammation with plasma cells, lymphocytes and neutrophils and dermal fibrosis shown as hypertrophic scar tissue. The patient was diagnosed as acne keloidalis nuchae and treated by surgical excision. We report a case of acne keloidalis nuchae showing typical clinical and histopathological findings.

7.Notch Intracellular Domain Expression in Various Skin Fibroproliferative Diseases.

BACKGROUND: The effects of the Notch signaling pathway in fibroproliferative skin diseases have not been fully elucidated. OBJECTIVE: The aim of this study was to investigate the expression of activated Notch signaling molecules in various skin fibroproliferative diseases. METHODS: Immunohistochemical analysis of Notch intracellular domain (NICD) expression in keloid, hypertrophic scar, morphea, dermatofibroma, and normal control skin specimens was performed, and the clinical characteristics of patients with various skin fibroproliferative diseases were analyzed. RESULTS: NICD was highly expressed in fibroblasts of keloids and moderately to highly expressed in hypertrophic scars and dermatofibromas, whereas low or no expression was detected in the fibroblasts of normal skin specimens and morpheas. NICD was constitutively expressed in keratinocytes, endothelial cells, and immune cells in normal skin specimens. CONCLUSION: NICD was significantly expressed in human fibroproliferative skin disorders, especially keloids, suggesting that an activated Notch signaling pathway is involved in the pathogenesis of skin fibrosis.

8.Idiopathic Retroperitoneal Fibrosis Treated with High Dose Steroids.

Idiopathic retroperitoneal fibrosis is proliferation of fibrous tissue with inflammatory process due to unknown etiology, that may involve the ureters and other retroperitoneal structures. The majority of reported cases have been treated surgically by ureterolysis or intraperitonealization of the ureters. Recently, successful management of retroperitoneal fibrosis by administration of steroids also has been reported. We report two cases of patients who underwent successful treatment of an idiopathic retroperitoneal fibrosis with high dose steroids therapy.

10.Laparoscopic examination and treatment of abdominal cocoon with infertility: a report of 6 cases.

OBJECTIVETo assess the abdominal cocoon complicated with infertility using laparoscope and its clinical management.
METHODSSix cases of abdominal cocoon treated in our hospital from January 1998 to December 2002 were retrospectively reviewed.
RESULTSOf the 6 patients all with primary and tubal infertility, 3 received routine closed laparoscope operation, and other 3 failed for laparoscopy and were transferred to laparotomy. During the surgery it was found that the abdomen and pelvis were filled with multiple layer fibrous tissue which enveloped the bowel and reproductive organs, making exploration nearly impossible. It was difficult to ablate the envelope.
CONCLUSIONAbdominal cocoon can be diagnosed in those primary and tubal infertile patients when the charge is hindered in the process of laparoscope. The optimal treatment of the infertility for those patients is in vitro fertilization-embryo transfer.
