1.Effect of hydroxychloroquine on the expression of suppressors of cytokine signaling in MRL/lpr mice kidney

Objective To study the effect of hydroxychloroquine (HCQ) on the suppressors of cytokine signaling (SOCS) in the kidney of MRL/lpr mice and evaluate the protection of HCQ on lupus nephritis.Methods Twenty-six rats were randomly divided into 3 groups:the control group which was consisted of 6 mice; the MRL/lpr group which was consisted of 10 mice; and the HCQ group which was consisted of 10.24-hour urinary protein level and anti dsDNA levels were tested.The expressions of SOCS-1 and SOCS-3 protein were determined by western blot.Analysis of variance was used to determine statistically significant differences between the two groups.A P value less than 0.05 was considered to be statistically significant.Results Compared with the control group,the levels of 24-hour urinary protein [(2 356.6±509.3) mg vs (440.3±90.1)mg] and anti ds-DNA (128.7±32.3 vs 14.9±1.4) were significantly increased in the MRL/lpr group,the expression levels of SOCS-1 and SOCS-3 protein were increased.After treated with HCQ,the levels of 24-hour urinary protein [(70.8±11.1) mg vs (2 356.6±509.3) mg] and anti-dsDNA (111.8±330.1 vs 128.7±32.3)were significantly decreased,the expression level of SOCS-1 and SOCS-3 protein decreased significantly (P< 0.05).Conclusion HCQ can inhibit SOCS expression in the kidney of lupus mice,reduce proteinuria and delay progression of immune disorders and lupus nephritis.
2.Effect of caveolin-1 on renal injury and the expression of tight junction protein in MRL/lpr mice kidney

Objective To study the effect of caveolin-1 on renal injury and the expression of tight junction proteins in MRL/lpr mice kidney.Methods The mice were divided into 4 groups:5 mice in the normal control group (BALB/c mice);the MRL/lpr lupus mice (n=18) were randomly divided into the MRL/lpr group in which 6 mice were included;the negative control group in which 6 mice were included;the caveolin1 transfection group in which 6 mice were included.The changes of urine protein,the levels of urea (BUN) and creatinine (Cr) were detected.The expressions of claudin-5,occludin,ZO-1 and caveolin-1 protein were determined by western bloting.Analysis of variance was used to determine statistical significant differences between the two groups.A significance level of 0.05 was considered as signigicant.Results Compared with the control group,24 h urine protein [(2 894±437) mg,(412±72) mg],BUN [(8.7±1.5) mmol/L,(6.9±0.4) mmol/L],Cr [(106±22) μmol/L,(85±4) μmol/L] were significantly increased,level of caveolin-1 protein increased (265±17,61±6),the level of occludin (114±12,190±12),claudin-5 (60±5,80±6) and ZO-1 (98±11,206±15) protein decreased in the MRL/lpr group (P<0.05).After caveolin-1 transfection,the levels of urinary protein [(1 253±249) mg,(2 894±437) mg],BUN [(6.5±1.3) mmol/L,(8.7±1.5) mmol/L],Cr [(78±17) μmol/L,(106±22)μmol/L] were significantly decreased,and the levels of occludin (218±16,114±12),claudin-5 (87±6,60±5)ZO-1 (313±17,98±11) were increased (P<0.05).Conclusion The expression of caveolin-1 protein in the renal tissues of lupus nephritis increases.Caveolin-1 can reduce the expression of tight junction proteins and contribute to progres-sion of lupus nephritis.
3.Study of the Expression and Significance of P53 and nm23 in Colorectal Adenocarcinom.

Objective To study the expression and significance of P53 and nm23 in colorectal adenocarcinoma.Methods The expression of P53 and nm23 was examined by immunohistochemical technique in 83 cases of colorectal adenocarcinoma.Results In colorectal adenocarcinoma,the positive intensities of P53 and nm23 was associated with tumour depth,histological grade,lymph node metastasis diseases and distant metastasis disease(P0.05).Conclusion P53 and nm23 may play an important role in the development and metastasis of colorectal adenocarcinoma.It is an useful marker for predictinng the prognosis of colorectal adenocarcinoma.
5.Effects of nimodipine on reducing the complications after microvascular decompression in patients with hemifacial spasm

Objective To investigate whether nimodipine could reduce complications after microvascular decompression in patients with hemifacial spasm. Methods 220 patients with hemifacial spasm , admitted into our hospital from May 2011 to Oct. 2013 , were all treated by microvascular decompression. They were divided randomly into two groups. Patients in the treatment group were treated with nimotop 50 mg/d after the operation for the first 3 days, then was orally taken 30 mg/d for a week. Patients in the control group were treated with the same after the operation without nimodipine. The postoperative complications including hearing deficit and peripheral facial paralysis were closely observed within three months after operation. Results The total effective rate was 100% in both groups. The incidence rate of peripheral facial paralysis and hearing deficit in treatment group was significant lower than that of the control group (P < 0.05). Conclusion Nimodipine has significant effect on reducing the incidence rate of peripheral facial paralysis and hearing deficit after microvascular decompression in patients with hemifacial spasm.
6.Correlation studies on serum interleukin-33 with rheumatoid arthritis associated interstitial lung disease

Objective To study the association between serum interleukin-33 (IL-33) level and human stromelysin-2 (ST2) level in patients with rheumatoid arthritis (RA) associated with interstitial lung disease (RA-ILD) and its correlation with lung function and other laboratory parameters.Methods Two hundred and forty-five newly diagnosed RA patients during March 2012 to March 2013 in the in-patient and out-patient clinic of the First Affiliated Hospital of Liaoning Medical College were enrolled into this study.Patients were further divided into RA group (n=187) and RA-ILD group (n=58).Sixty subjects who came to the hospital for routine health check-up was composed of the normal control group.The clinical data of the two groups and controls were collected and their serum IL-33 and ST2 concentrations were measured.The t test was used to compare the difference between the two groups.Multiple variance analysis was used to com-pare the difference between groups.Pearson's correlation analysis was applied to explore the relationship between IL-33 concentrations and related variables.Results ① This study showed that the prevalence of RA associated interstitial lung disease was 23.7%(58/245).② The concentration of IL-33 [(746±43) pg/ml] and ST2 [(3413±169)pg/ml] of the RA-ILD group was significantly higher than that of the RA group [(433±42) pg/ml,(1500±147) pg/ml] (P<0.01).③The vital capacity (VC%),forced vital capac-ity (FVC%),maximal midexpiratary flow curve (MMF%) and carbon monoxide diffusing capacity (DLCO) of the RA-ILD group were significantly lower than those of the RA group.④ The serum level of IL-33 was negatively correlated with that of RF and ACPA (IL-33 and RF,r=0.82,P<0.01; IL-33 and ACPA,r=0.55,P<0.01).Serum level of IL-33 was negaitively correlated with DLCO (r=-0.80,P<0.01).Conclusion IL-33 participates in the pathogenesis of RA; and may be involved in the pathogenesis of RA-ILD.

Objective To investigate the effect of siRNA targeted against Bcl-xl on cell proliferation of rheumatoid arthritis (RA) and the effect on expres-ion of apoptosis relevant factors Bcl-xl,Bax,Caspase-3.Methods Human RA synovial cells were cultured and passed by tissue block collagenase digestion method.The siRNA plasmid targeting Bcl-xl gene was constructed by Bcl-xl cDNA sequence provided by gene banks,while single missense sequences were used as negative controls.LipofectamineTM 2000 was used to transfect synovial cells.The effect of Bcl-xl silencing on proliferation of synovial cells was evaluated by MTT at 24,48,72 hours after transfection.The expressions of Bcl-xl,Bax,Caspase-3 protein,synovial apoptosisrelated factors were determined by Western blotting after transfected at different time points.The average of multiple-sample average was analyzed by single-factor x2 test or LSD-t and Tamhane's T2 test was used for twotwo comparison.Results MTT result s showed that RNA interference that specifically silent Bcl-xl could obviously suppress the proliferation of sliding film cells,which was most eveident at 48 hours.This inhibition was gradually weakened with prolonged time,but when compared was the control group,differences was significant (P<0.05).Western blotting results displayed that when compared to the control group,Bcl-xl protein expression obviously declined after transfection P<0.01,which was the least at 48 h time point.Bax,Caspase3 protein expression were obviously increased when compared to the coutrol group.Conclusion The expression of Bcl-xl,a RA synovial cell anti-apoptotic factor,is significantly reduced by specific RNA interference silencing Bcl-xl,which may play an important role in inhibiting the excessive proliferation of synovial cells.

Epiretinal membrane (ERM) is a relatively common macular disease that forms along the surface of the internal limiting membrane (ILM) of the retina to some reason.The pathogenesis is not clear.Microincision vitrectomy surgery has been confirmed as a minimally invasive and very safe modality of treatment.Deciding when to perform a vitrectomy can be difficult.There are many factors which can affect the postoperative visual acuity,such as age,the thickness of macular,integrity of photoreceptor inner segment/outer segment (IS/OS) junction.
9.Cost-utility Analysis of Alendronate Sodium Preventing Osteoporotic Fractures in Postmenopausal Women

OBJECTIVE:To evaluate the economics of alendronate sodium preventing osteoporotic fractures in postmenopausal women,and to provide reference for osteoporotic and osteopenia patients to select economical and effective therapy plan. METH-ODS:Using Markov economic model,TreeAge Pro 2011 software was used for cost-utility analysis using RCTs literature data of basic medication(calcium and vitamin D3)and basic medication combined with alendronate sodium as baseline data. Using quality adjusted life years(QALYs)and incremental cost utility ratio(ICUR)as economics evaluation indexes,the stability of established model was investigate by single factor sensitivity analysis. RESULTS:QALYs of the basic treatment group was 0.704 years,and QALYs of drug combination group was only 0.708 years. However,ICUR of two groups reached 714252.44. For the late period of hip fracture and other fractures,drug combination group was in high cost and low benefit,obviously had no economic advan-tage. For vertebral fractures,the cumulative probability of fracture in drug combination group was significantly lower than basic treatment group,and ICUR of the two groups was only 13902.17,with economic advantage. For hip fracture,the cumulative prob-ability of fracture in basic treatment group was the same as drug combination group,and ICUR was 19109.00,with economic ad-vantage. For wrist fractures,curative effect and economy needed further study. CONCLUSIONS:For the low-risk population with osteoporotic fractures,calcium and vitamin D3 are more economical in preventing osteoporosis fractures. For the high-risk popula-tion with vertebral and hip fractures,alendronate sodium combined with basic treatment is effective and economical.
10.Expression of human epidermal growth factor receptor 2 in pancreatic ductal adenocarcinoma and its clinical significance

Objective:To investigate the expression of human epidermal growth factor receptor 2 (HER2) in pancreatic ductal adenocarcinoma(PDAC) and its relationship with the prognosis of patients with PDAC.Methods:From January 2001 to December 2012, 109 paraffin embedded PDAC tissue samples and 27 normal pancreatic tissue samples were collected from the Department of Pathology, Huadong Hospital Affiliated to Fudan University. The expression of HER2 protein in pancreatic tissue was detected by immunohistochemical Envision two-step method. HER2 expression was evaluated according to Hercept test, and its relationship with clinicopathological features and survival time was analyzed.Results:The expression of HER2 protein was negative (-) in 29.4% of PDAC tissues, weakly positive (+ ) in 35.8%, positive (+ + ) in 25.7% and strongly positive (+ + + ) in 9.2%, respectively, and the overexpression rate (+ + , + + + ) was 34.9%; the negative (-) and weakly positive (+ ) expression of HER2 protein in normal pancreatic tissues accounted for 88.9% and 11.1% respectively. There was no expression with positive (+ + ) or strongly positive (+ + + ), therefore, the overexpression rate was 0. The overexpression rate of HER2 protein in PDAC and normal pancreatic tissues was significantly different ( P=0.000). The expression of HER2 protein was significantly correlated with age, and the expression of HER2 protein in patients with PDAC over 65 years old was significantly higher than that in patients with PDAC under 65 years old ( P=0.043), but not with gender, tumor location, tumor grade, T stage, N stage and nerve invasion (all P>0.05). Univariate Cox proportional hazards analysis showed that HER2 expression was associated with postoperative survival time of patients with PDAC ( P=0.032). Multivariate Cox proportional hazards analysis showed that HER2 expression was an independent prognostic factor for survival of patients with PDAC ( P=0.040). The median survival period of patients with HER2 expression + + + was significantly longer than that of patients with HER2 expression -~+ + (128.4 months vs 21.5 months), and the difference was statistically significant ( P=0.038). Conclusions:The overexpression of HER2 in PDAC tissue was related to the age of patients. The survival time of patients with HER2 strongly positive PDAC was significantly longer. HER2 can be considered as an index to evaluate the biological behavior and prognosis of PDAC.
