3.Basal cell carcinoma arising on two variants of epidermal nevus: A case series

Introduction:
Epidermal nevi are hamartomas of the epidermis and papillary dermis that are usually present during the first years of
life. Rarely, malignant transformations develop in association with epidermal nevi. Few cases have been reported worldwide, however the
lifetime risk and incidence are unknown.
Case Report:
This is a case series about basal cell carcinoma arising on epidermal nevus.
The first patient is a 42-year-old Filipino female, who presented with a verrucous plaque at birth on the left temple which then developed
multiple, discrete to confluent, grayish, papules and nodules on the surface. Histological examination revealed nevus sebaceus and basal
cell carcinoma, pigmented type.
The second patient is a 53-year-old Filipino male, who presented with a papillomatous plaque on the left temple since the first year of life
which then increased in size along with the presence of a solitary bluish-black macule noted by dermoscopic examination. Histologic examination showed verrucous epidermal nevus and basal cell carcinoma, pigmented type.
Conclusion
Two rare cases of basal cell carcinoma arising on epidermal nevus are reported. Despite the rarity of malignant transformation on epidermal nevus, any suspicious growth warrants a biopsy. Knowledge of these cases is important for probing suspicious growth
over an epidermal nevus that would prompt early treatment before these lesions progress in size making it harder to manage.

4.A Case of Melanoma in situ Arising in a Small Congenital Melanocytic Nevus.

It is uncertain whether small congenital melanocytic nevi is associated with an increased risk of melanoma. However, if a small congenital melanocytic nevus develops suspicious changes, becomes symptomatic, or is irregular, then an excisional biopsy may be justified. We report an interesting case of a 33-year-old man who was diagnosed with melanoma in situ, arising in a small congenital melanocytic nevus with a cyst. Histologic examination revealed that intradermal nests of nevus cells had surrounded the epidermal cyst. In the basal cell layer, and in the wall of the epidermal cyst, atypical melanocytes were observed.

Nevus with a cyst is characterized by a melanocytic nevus that is closely associated anatomically with an epidermal cyst. It is important to recognize this common benign condition because the growth of these lesions is sometimes misinterpreted by the patient and clinicians as presenting either a new melanoma or malignant transformation of a preexisting lesion. We report a case of intradermal melanocytic nevus associated with an epidermal cyst which developed in a 68-year-old man.

6.A Case of Tubular Apocrine Adenoma.

Tubular apocrine adenoma is a well-circumscribed, intradermal benign tumor, with tubular structures showing apocrine differentiation. Histologically, the characteristic features are various-sized dilated tubules with intraluminary papillary projection and apocrine decapitation secretion. It usually presents as a solitary lesion and is sometimes associated with nevus sebaceous and/or syringocystadenoma papilliferum. We have in report a case of tubular apocrine adenoma with epidermal cysts which developed in the right inguinal area of a 37-year-old man.

A nevus with cyst is defined as a single lesion of a melanocytic nevus, and this is commonly associated with an epidermal cyst. For the previously reported cases of a nevus with cyst, most of the melanocytic nevi were of the intradermal or compound type. There have been rare reported cases of dysplastic nevus, blue nevus, Spitz nevus, congenital melanocytic nevus and so forth. We report here on a rare case of congenital melanocytic nevus combined with an epidermal cyst in a 21-year-old woman.

8.Immunoreactivity of Epidermal Growth Factor Receptors in Melanocytic Lesions.

BACKGROUND: Epidermal growth factor receptors (EGFR) have been reported to be absent in melanomas. But recently, the presence of EGFR on melanocytic cells was reported to be a marker of malignant transformation. OBJECTIVE: Our purpose was to investigate the presence of EGFF in melanocytic lesions and to determine whether EGFR presence correlates with the potential or malignant transformation of melanocytic cells. METHODS: We performed the immunohistochemical studies to reveal immunoreactivity of EGFR in 7 compound nevi, 10 intradermal nevi, and four melanomas using the Vectastain ABC immunoperoxidase stain system. RESULTS: Although the intensity of staining was slightly variable, all melanocytic cell types in the studied lesions of compound nevi, intradermal nevi, and melanoms had immunoreactive EGFR. Intense staining far EGFR of all nucleated layers of keratinocytes overlying a melanocytic lesion was also seen. But in the melanoma cells, the staining intensity was modarately deereased. CONCLUSION: Although we found no correlation of EGFR with the potential for malignancy in melanocytic lesions, the high level of expression within nevocytes and lesional keratinocytes suggests EGFR or transforming growth factor a, by acting through the EGFR, plays a role in the pathogenesis, maintenance, or evolution or these lesions.

The combination of an intradermal nevus with epidermal cyst or other adnexal tumors has often been described since 1952. Most of these reports have involved the coexistence of common epidermal cysts and melanocytic nevi. Herein, we report a 21-year-old Korean male, who initially presented to our department with a rapidly enlarging preexisting nevus on his back. The mass was surgically excised and histopathologically showed intradermal nevus combined with epidermal cyst.

10.A Case of Molluscum Contagiosum Occuring in a Soft Fibroma.

Molluscum contagiosum (MC) is a skin infection caused by a poxvirus, which is easily transmitted by direct skin to skin contact. MC is rarely associated with other skin diseases, including epidermal cysts, melanocytic nevus, halo nevus, soft fibroma or condyloma. The occurrence of MC on a soft fibroma is so rare that only three cases have been described so far. Here, we report an interesting a case of a solitary MC associated with soft fibroma occurring on the right popliteal fossa of a 6-year-old girl.
