Objective:To evaluate the quantity and function changes of dendritic cells ( DC) in peripheral blood of patients with repeated implantation failure ( RIF).Methods:30 patients with RIF and 15 normal controls were enrolled in this study,and the peripheral blood was collected during the mid-luteal phase.The percentage of DC subsets and the expression levels of functional molecules were assessed by flow cytometric analysis.Results:Compared with normal controls,the percentage of lin-HLA-DR+DC cells accounting for leukocytes in patients with RIF was not significantly different ( P>0.05).There were also no significant differences in the expression levels of co-stimulatory molecules ( CD80 and CD86) and immune tolerant molecules CD200 on DC cells surfaces between patients with RIF and normal controls ( all P>0.05).In addition,the percentage of CD11c+CD123-mDC accounting for DC cells was significantly increased in patients with RIF (P<0.05),however,the percentage of CD11c-CD123+pDC was similar (P>0.05). Conclusion:The percentage of mDC accounting for DC cells was significantly increased in patients with RIF, which may be one of factors affecting pregnancy outcomes.
4.Experiment on pruning of Cistanche deserticola inoculated in artificial Haloxylon ammodendron forest.

At present, the objective of cutting and pruning Cistanche deserticola is to harvest in successive years and enhance the harvesting yield and quality of C. deserticola in the process of the artificial cultivating C. deserticola. An experiment was conducted focusing on cutting and pruning C. deserticola in artificial forests of Haloxylon ammodendron drip-irrigated with saline water at the hinter-land of the Taklimakan desert, according to different growth stages and lengths. The results were following: (1) The effect of cutting on C. deserticola was similar to that of pruning, which resulted in three kinds of morphological types, not related to the bloom and size of C. deserticola. (2) The growth forms were diversified after pruning. Among them, there had sprouting new body, died or maintaining life with no sprouting, mildewed on its surface layer, etc. However, some of new bodies were sprouting from the lower part of the old body. The death rate of bloomed C. deserticola was higher than that of the underground, and the death rate of the 40 cm in stubble height for C. deserticola was higher than those with the stubble height of 20 cm and 5 cm. (3) Most of the diameter of living C. deserticola after pruning was increasing, but some of them changed little. (4) The mildew and rot of C. deserticola and the broken of the roots of the H. ammodendron and the fallen of the point of the inoculated when it was dug, which would cause the death of the C. deserticola. On the other, the yield-increasing effect and the economic benefit of the techniques of the pruning of Cistanche would need further research and evaluate. Therefore, the application of this technique needs to be cautious.

5.Clinical research of Modified Buzhong Yiqi Decoction to applicate in the perioperative period of intertrochanteric fracture in elderly patients with Qi deficiency and blood stasis type

AIM To observe the curative effects of Modified Buzhong Yiqi Decoction (Codonopsis Radix,Astragali Radix,Atractylodis macrocephalae Rhizoma,ect.) on perioperative period of intertrochanteric fractures in elderly patients with Qi deficiency and blood stasis type.METHODS One hundred and twenty cases of elderly intertrochanteric fractures were randomly divided into treatment group and control group,60 cases in each group.All patients underwent closed fracture reduction and proximal femoral nail anti-rotation (PFNA) fixation.During the perioperative period,the control group only received conventional treatment,while the treatment group received conventional treatment and Modified Buzhong Yiqi Decoction once a day.The levels of interleukin-6 (IL-6),interleukin-10 (IL-10) and tumor necrosis factor (TNF-α) of both groups in preoperative and in the 1st day,the 7th day,the 14th day of postoperative were observed and compared.The complications and the curative effects according to the Harris score also were observed and compared between the two groups.RESULTS In preoperative,there was no significant difference in levels of IL-6,IL-10 and TNF-α between the two groups (P > 0.05).The levels of IL-6 and TNF-α in the 1 st day of postoperative in the control group were significantly higher than those in the treatment group (P <0.05,P <0.01).And the levels of IL-10 were similar between the two groups (P >0.05).In addition,in the 7th day and the 14th day of postoperative,the levels of IL-6,IL-10 and TNF-α in both groups were all decreased,moreover,the descent degree in treatment group was more significant than those in the control group (P < 0.05,P < 0.01).The treatment group had the lower incidence of complications and the higher curative effects than those in the control group,both differences were statistically significant (P <0.05).CONCLUSION Modified Buzhong Yiqi Decoction applying to the perioperative period of femoral intertrochanteric fractures in elderly patients with Qi deficiency and blood stasis,can effectively reduce the perioperative inflammatory response,reduce the incidence of postoperative complications,and improve the curative effects.So it has a positive effect on the rehabilitation for the traumatic patients and is worthy of clinical promotion.

Objective To explore the expression level and influencing factors of CD4 +CD25 + regulatory cells(Treg)among male drug -abusers with syphilis.Methods 144 male drug -abusers with syphilis,41 male drug -abusers without syphilis,35 male non -drug abuser with syphilis and 35 male healthy volunteers were enrolled in this study.Expression levels of CD4 +CD25 +Treg of them were detected.A questionnaire interview was conducted among these participants, including drug abuse duration,drug types and abuse ways.Results The levels of Treg expression of drug -abusers with syphilis were significantly higher than that of drug -abusers without syphilis and non -drug abuser with syphilis (P <0.01 ).Despite of the stage of syphilis,types of drugs and abuse way,Treg expression levels with drug abuse duration of"above 1 -year"were significantly higher than those of "1 -year or below"(P <0.01 ).Treg expression levels were positively correlated to drug abuse duration (R2 =0.672,P <0.01)among patients.Among the participants,people both with traditional and new drugs were significantly higher than that with only traditional or new drug (P <0.01).In addition, people both with injecting and non -injecting abuse were significantly higher than that with one way of drug -abuse (P <0.01).Among those with drug abuse duration of "above 1 -year",Treg levels of drug -abusers with traditional drugs and injecting abuse way were remarkably higher than that with new drug and non -injecting abuse respectively (P <0.01). Conclusion Among male drug -abusers with syphilis,suppression of cellular immune function resulting from increased CD4 +CD25 +regulatory T cells is related to drug abuse duration,and is also influenced by drug types and abuse ways.
7.Effect of valproate combined with bone marrow mesenchymal stem cell transplantation on spinal cord injury in rats

Objective To investigate the effects of intraperitoneal injection of valproic acid (VPA) combined with transplantation of bone marrow mesenchymal stem cells (BMSCs) on neurological function recovery after spinal cord injury (SCI) in rats.Methods BMSCs from 3-4 weeks old healthy male Sprague-Dawley (SD) rats were cultured in vitro.The third generation BMSCs were detected by flow cytometry.Spinal cord injury model was made by modified Allen's technique.Sixty adult male SD rats were randomly divided into five groups(n =12 in each group):Sham operation group and model group(received the same dose of normal saline at the same time point),VPA group(VPA,one time per q12 h with 300 mg· kg-1 · d-1).BMSCs group (1 × 106 cell· mL-1BMSCs 1 mL injected into the spinal cord of injured segment)and combined group(VPA + BMSCs).The BBB scale at the 14 d after injury was evaluated.The expression of Caspase-3 protein was detected by immunohistochemistry.Then the neuronal apoptosis rate was observed by TdT-mediated dUTP nick-end labeling(TUNEL) staining.Results BBB Rating:On the 14 d after SCI,the rate in sham operation group,model group,VPA group,BMSCs group and combined group were 21,(4.51 ± 0.41),(7.25 ± 0.95),(7.32 ± 0.55),(9.76 ± 0.95)point.The BBB score of the three treatment groups was significantly higher than that of model group with significantly (P < 0.05).On the 14 d after SCI,the caspase-3 expression in sham operation group,model group,VPA group and BMSCs group and combined group were (1.64 ± 0.41) %,(26.22 ± 4.81) %,(21.21 ± 4.04) %,(21.47 ± 3.32) %,(16.15 ± 3.09) %.The expression of Caspase-3 in three treatment groups was significantly lower than that model group with significantly(P <0.05).While the expression of caspase-3 in combined group was significantly lower than that in VPA group and BMSCs group with significantly(P <0.05).On the 14 d after SCI,the apoptosis rate in sham operation group,model group,VPA group and BMSCs group and combined group were (4.18 ± 0.39)%,(40.51 ± 3.81)%,(19.95 ± 2.04)%,(20.87 ±2.32)%,(12.83 ± 1.09)%.Compared with model group,the apoptosis rate of the three treatment groups was lower with significantly(P < 0.05).The apoptosis rate of combined group was significantly lower than that of VPA group and BMSCs group with significantly (P < 0.05).Conclusion It is very significant effects of intraperitoneal injection of VPA combined with BMSCs on neurological function recovery after spinal cord injury in rats.The mechanism may be related to down-regulation of Caspase-3 expression,leading to inhibition of apoptosis.
8.Impact of depression on prognosis of patients with coronary heart disease undergoing revascularization

Objective To investigate the impact of depression on clinical outcome of patients undergoing revascularization.Methods Self-rating depression scale (SDS) assessment was made before and after coronary artery bypass grafting (CABG,n =345 ) and percutaneous coronary intervention( PCI,n =308 )procedure.Patients were divided into depression and non-depression group.All patients were followed up for 12 months after procedure for the occurrence of rehospitalization and major adverse cardiovascular events (MACE) including all-cause mortality,nonfatal myocardial infarction or target lesion revascularization.Results Depression was present in 40.9% ( n =141 ) of patients after CABG,which was significantly higher than before procedure (24.3%,P < 0.01 ).The MACE rate was significantly higher in patients with post-procedure depression[ 8.5% ( 12/141 ) ] than in patients without depression [ 2.9% (6/204),P <0.05 ] and the incidences of target lesion revascularization and rehospitalization were also significantly higher in depression patients than in non-depression patients during the 12 months follow-up ( all P < 0.05 ).Depression was present in 36.4% (n =112) of patients after PCI,which was significantly higher than that before procedure (28.6%,P <0.05).The MACE rate [8.0% (9/112) vs.2.0% (4/196)]and rehospitalization rate [ 12.5% ( 14/112 ) vs.4.6% ( 9/196 ) ] were significantly higher in depression patients than in patients without depression during the 12 months follow-up ( P < 0.05 ).There was no significant difference on SDS score between the PCI and CABG before the procedure.However,after the procedure,the SDS score for patients undergoing CABG was significantly higher than in patients undergoing PCI (48.9 ± 9.8 vs.45.7 ± 10.5 P =0.01 ).The level of serum IL-6 was significantly higher in depression patients than in patients without depression ( P < 0.05 ).Conclusion Prevalence of depression is high in patients treated with revascularization procedures and is linked with poor post-procedure prognosis.
9.Risk Assessment of Echocardiography Parameter Scoring System for 1 Year Re-admission in Patients With Left Ventricular Systolic Dysfunction

Objective: To establish an echocardiography parameter scoring system for assessing the risk of 1 year re-admission in patients with left ventricular systolic dysfunction (LVSD). Methods: A total of 412 chronic LVSD patients treated in our hospital from 2007-01 to 2016-01 were studied and the end point event was 1 year re-admission. The data included in 280 patients from 2007-01 to 2014-12 for establishing the scoring system and 132 patients from 2015-01 to 2016-01 for verifying the system. Based on 7 echocardiography parameters, the patients were divided into 7 sets of groups: ① Left ventricular diameter (LVD): Group0, n=290 and Group1, n=122;② Mitrial regurgitation (MR): Group0, n=203, Group1, n=138 and Group2, n=71; ③ Tricuspid regurgitation (TR): Group0, n=302, Group1, n=90 and Group2, n=20; ④ LVEF: Group0, n=272 and Group1, n=140; ⑤ Pulmonary artery systolic pressure: Group0, n=282 and Group1, n=130; ⑥ Hydropericardium: Group0, n=347 and Group1, n=65; ⑦ Hydrothorax:Group 0, n=261, Group1, n=86 and Group2, n=65. The parameters were identified by COX regression analysis, weighted value of scoring system was calculate by hazard ratio (HR), predictive value for1 year re-admission was assess by ROC curve and finally, scoring integration was verified by validation data group. Results: The integration score was calculated as follows: LVD>60mm=1 point; TR: Group1=1 point and Group2=3 points; MR: Group1=2 points and Group2=4 points; Hydrothorax: Group1=2 points and Group2=3 points;Hydropericardium=1 point. COX regression analysis indicated that for 1 year re-admission: HR=1.552 in Group1 vs Group0, HR=3.374 in Group2 vs Group0 and HR=4.562 in Group3 vs Group0, all P<0.05. The AUC of ROC for establishing the data was 70.0% (95% CI 0.640-0.761) and for verifying the data was 70.4% (95% CI 0.616-0.792); the best integration score was 4 points. Conclusion: Echocardiography parameter scoring system may better predict the risk of 1 year re-admission in LVSD patients which is superior to single echocardiography parameter.
10.Influence of CETP gene -629C/A polymorphism on the efficacy of atorvastatin treatment and clinical outcome.

OBJECTIVETo investigate cholesteryl ester transfer protein (CETP) gene polymorphism -629C/A among Han Chinese patients with coronary heart disease (CHD) in Tianjin region, and to assess the influence of genetic factors on therapeutic effect of atorvastatin and clinical outcome in order to provide a pharmacogenomic basis for personalized treatment.
METHODSFrom October 2010 to July 2011, 232 patients with angiographically confirmed CHD were recruited. Polymorphism of position -629 of CETP gene promoter was determined with polymerase chain reaction - restricted fragment length polymorphism (PCR-RFLP) method. Serum level of CETP was determined with enzyme-linked immunosorbent assay (ELISA). Lipid level in all patients was determined at baseline and after 12 months of treatment with 20 mg/d atorvastatin. Clinical follow-up was carried out for more than a year (12-23 months). Major adverse cardiac events including death, non-fatal infarction, revascularization and stroke (MACE) were recorded. A Kaplan-Meier log-rank test was used to compare MACE-free survival for individuals with various genotypes.
RESULTSThe frequency of -629A allele was 0.408. Compared with CC or CA genotypes, individuals with AA genotype had lower CETP levels and higher high-density lipoprotein cholesterol (HDL-C) levels, albeit without statistical significance (F = 0.893, P = 0.411 and F = 1.279, P = 0.282, respectively). There also appeared to be a negative correlation between serum HDL-C and CETP levels, though no statistical significance was detected (r = -0.151, P = 0.081). After 12 months atorvastatin therapy, individuals with CC genotype had greater reduction of low-density lipoprotein cholesterol (LDL-C), reduced LP(a) and elevated HDL-C compared with CA or AA genotypes. LDL-C level has decreased by 35.41% in CC homozygotes, 18.84% in CA heterozygotes and 8.15% in AA homozygotes (P = 0.001). HDL-C level has increased by 14.37% in CC homozygotes, 10.48% in CA heterozygotes and 6.64% in AA homozygotes, respectively. However, above changes did not reach statistical significance (P = 0.470). The incidence of MACE after a mean follow-up of (18.66 ± 5.99) months was 7.76%, which included 2 (0.86%) deaths, 5 (2.16%) non-fatal infarctions, 9 (3.88%) revascularizations and 2 (0.86%) strokes. The cumulative MACE-free survival rates were 92.4%, 85.3% and 65.0% for CC, CA and AA genotypes, respectively (Log-rank P = 0.444).
CONCLUSIONOur results suggested that AA variant for the -629A allele of CETP gene had higher HDL-C levels and reduced CETP levels, though patients with CC genotype appeared to have better benefited from statin therapy with reduction in LDL-C and LP(a) levels. Long-term clinical prognosis was however not affected by the 3 genotypes.
