Canalith repositioning maneuver is effective to treat benign paroxysmal positional vertigo(BPPV). This case showed complicated form of the BPPV such as changes of canalolithiasis to cupulolithiasis, involvement of one canal to two canals and from unilateral to bilateral involvement during the reposition maneuver. This patient was diagnosed as left lateral canalolithiasis at first. After left barbecue maneuver, the type was changed to the right posterior cupulolithiasis. Semont maneuver was performed and then the type of BPPV was changed to combined type with right posterior canalolithiasis and left lateral canalolithiasis. We performed left barbecue maneuver and right Epley maneuver. Then the type of BPPV was changed to left lateral cupulolithiasis. After Brandt-Daroff maneuver and left barbecue maneuver, nystagmus and dizziness disappeared finally.

OBJECTIVE: To show the value of rotating chair test (ROT) as an adjunct to the conventional electronystagmography (ENG) examination including caloric test for the diagnosis of vestibular lesions. BACKGROUND: Since caloric stimulatim is regarded as a very low frequency (0. 003Hz) test, it is desirable to test the function of horizontal semicircula canals by stimulating with faster frequencies. Baloh et al. (1984a) described that lesions ofdifferentiated by ROT. Leigh and Zee(1991) told that ROT can give more accurate and reproducible results than do caloric tests. METHOD: 30 patients who suffered from vertigo with either peripheral or central pathology were given computerized ENG and ROT. We discuss the representative examples of our initial experience with ROT. RESULTS: 1.In eight patients with known peripheral pathology, low-frequency phase deficits, asymmetry toward the lesim side and VOR suppression by gazefixation were seen during ROT. With bilateral peripheral vetibular disease, the ROT could identify remaining vestibular function at higher frequencies. 2. Seven patients with lesions of central vestibular structure showed abnormalities of saccades and pursuit during ENG and failure of VOR suppression by gaze fixation during ROT. 3. Out of 15 patients whose lesions could not be localized distinctly after caloric test, four patients were suggested to have central pathology. CONCLUSION: ROT can be a useful method to study the vestibular abnonnahties and an adjunct to the conventional ENG test.

Vertigo*

The initial severe and disabling symptoms of vestibular dysfunction diminish gradually by a process of compensation. Although the process is complex and not fully understood, knowledge of the extent of compensation makes physicians to identify the causes and to determine treatment choices of the dysfunction. In this report we present 2 common cases of peripheral vestibulopathy that result from different disease processes, and review the physiologic and functional changes as the compensation proceeds that can be evaluated by vestibular function tests including electronystagmography(ENG), rotation chair, and dynamic posturography. These tests are valuable tools in the assessment of the degree of compensation as well as the side and site of lesions in vestibulopathy.

BACKGROUND AND OBJECTIVES: It is known that various inflammatory mediators released from the eosinophils and mast cells play important roles in the pathogenesis of nasal polyp. Among those mediators, the arachidonic acid has particular importance as a precursor of other mediators. By assaying the tissue concentration of the6-keto-PGF(1alpha), leukotrienes (LTs), and hydroxyeicosatetraenoic acids (HETE) in the nasal polyp, we aimed to investigate the role of arachidonic acid metabolite in the pathogenesis of antrochoanal polyp and nasal polyp associated with chronic paranasal sinusitis. MATERIALS AND METHODS:Three turbinate tissues taken during the septoplasty were served as the control. The experimental group consisted of 3 antrochoanal polyps and 7 inflammatory polyps. The tissue level of the 6-keto-PGF(1alpha), LTC(4), LTD(4), LTE(4), 15-HETE, and 12-HETE were measured using high performance liquid chromatography. RESULTS: The level of 6-keto-PGF(1alpha), LTC4, 15-HETE, 12-HETE were significantly lower in antrochoanal polyp than in the control turbinate. In the inflammatory polyp, the levels of 6-keto-PGF(1alpha) and LTC(4) were lower than the control. However, in the inflammatory polyp, LTD(4) and LTE(4) were detectable, which were not detected in the control turbinate and antrochoanal polyp. CONCLUSION: The results of this study indicate that the decreased arachidonic acid metabolism may underlie the pathogenesis of the antrochoanal polyp. However, in the pathogenesis of inflammatory polyp, the increased production of LTD(4) and LTE(4) may have an important role.

BACKGROUND AND OBJECTIVES: The purpose of this study was to test whether platelet activating factor (PAF) impairs the mucociliary clearance function of the eustachian tube (ET)in dose dependent manner and PAF-antagonist can prevent the impairment of mucociliary function of the ET induced by PAF. RESULTS: Coomassie Brilliant Blue (CBB) dye transport time (DTT) in normal guinea pigs was 69 seconds. The dye transport time after the applications of normal saline, PAF 1, and 2 ug/ml of PAF into the bullae were 66, 74, and 157 seconds, respectively. The time was over 15 minutes when 4, 8, and 16 ug/ml of PAF were applied. The DTT were 62 seconds when the animals were pretreated with PAF-antagonist (WEB 2170). There were significant delay in the DTTs after treatment with 2, 4, 8, and 16 ug/ml of PAF. Histopathological examination of ETs from groups with significant delay in DTTs showed intact cilia, mucus plugs, increased inflammatory cells, and exfoliation of cells. CONCLUSION: This study demonstrated that PAF impaired mucociliary clearance function of the eustachian tube in dosedependent manner. This impairment of mucociliary clearance function was prevented by pre-treatment with PAF-antagonist. The findings of this study suggest that PAF plays an important role in the pathogenesis of otitis media by impairing eustachian tube clearance function.

BACKGROUND AND OBJECTIVES: There are caloric test, SCEP, DP in Step velocity and VOR asymmetry in SHA test as parameters for evaluation in unilateral peripheral vestibulopathy in parameter of vestibular function test. The aim of this study is to assess the correlation between these parameters and intensity of spontaneous nystagmus in vestibular neuritis. MATERIALS AND METHOD: We evaluated 75 patients who were diagnosed as vestibular neuritis with 3 degree spontaneous nystagmus at vestibular function test lab from January 2000 to July 2005 in Dankook university hospital. The correlations between direction and intensity of spontaneous nystagmus and monothermal caloric test, SCEP and DP in step velocity, VOR asymmetry (0.01, 0.04, 0.16 Hz) in SHA test were analysed. And correlations according to sex, age were also analysed. RESULTS: The correlation coefficient between intensity of spontaneous nystagmus and monothermal caloric test was 0.60. The correlation coefficient between intensity of spontaneous nystagmus and SCEP was 0.35. The correlation coefficient between intensity of spontaneous nystagmus and Tc DP was 0.36 and it showed no significant correlation. The correlation coefficient between intensity of spontaneous nystagmus and VOR asymmetry (0.01, 0.04, 0.16) was 0.57, 0.46, 0.51, respectively and there was higher correlation in man, age under 60. CONCLUSION: Intensity of spontaneous nystagmus showed close relations to monothermal caloric test, VOR asymmetry of SHA, Tc DP, SCEP DP in sequence. In addition, young male patients tends to be closely related to intensity of spontaneous nystagmus.

Photodynamic therapy (PDT) is a treatment modality by highly reactive oxygen intermediates generated through the interaction of light with a photosensitizer. We treated actinic keratosis occurred on the face of a 65-year-old woman with topical PDT using 5-aminolevulinic acid (ALA) (MEDAC, Hamburg, Germany) and a 632nm diode laser (Creamoptec, Germany). 20% ALA was applied topically to the lesion sites which were then occluded for 6 hours. Light was administered at a wavelength of 632nm. The light intensity was 100mW/cm2 and the light dose was 100J/cm2. Although the diode laser was expensive, it was easy to handle the laser and the clinical results were excellent with respect to initial complete response and cosmetic outcome. There was only slight pain and a burning sensation during treatment.
