1.Cyclooxygenase-2 expression in clear cell renal cell carcinoma

Introduction: Cyclooxygenase-2 (COX-2) promotes carcinogenesis by inducing proliferation and angiogenesis while decreasing apoptosis and immunosuppressive activity. It is overexpressed in many malignancies including renal cell carcinoma (RCC). The aim of this study was to investigate COX-2 expression in clear cell RCC and its association with tumour grades and demographic parameters. Materials and Methods: Thirty-six clear cell RCC cases were selected. There were 21 (58.3%) men and 15 (41.7%) women with median age of 56.6 years (range: 16-74 years). Chinese constituted 16 (44.4%) of the cases; Malays 14 (38.9%) cases and Indian 6 (16.7%) cases. There were 6 (16.7%) grade 1, 20 (55.6%) grade 2, 10 (27.8%) grade 3 and none was grade 4. The paraffin embedded tissues were cut at 4 μm thick and stained with COX-2 monoclonal antibody. Results: Eighteen (50%) of the RCC cases were immunopositive, of which all showed strong positivity. The immunopositive cases showed cytoplasmic membrane positivity. Conclusion: There was no significant association between COX-2 expression with grade, age, sex and ethnicity (p=0.457, p=0.054, p=0.389 and p=0.568 respectively). Strong positivity of COX-2 suggest that COX-2 may play a role in cell proliferation and in carcinogenesis.

2.Study on recurrent characteristics and metastases of squamous cell carcinoma of skin

109 records and 109 histopathologic samples of squamous epithelioma patients at the Hospital K during 1992-1996 period were studied. By histopathologic classification, the malignity was divided into I, II, III, IV grade. Results showed that epithelioma was common in the elderly, principally in open area of the skin such as in head, neck and limbs (93.6%). The most of squamous epitheliomas were at the grade I, II, III. Among squamous epithelioma cases, 18% had post operative recurrent, 13.8% had gland metastasis, 55% recurred just in post operative first year, and 70% had 1 time of recurrent. >5 cm large tumors and the tumors of grade III, IV, high rate of recurrent was noted

Basal cell carcinoma (BCC) of the scrotum is uncommon and its pathogenesis is
not well understood. The clinical behaviour of scrotal BCC is thought to be more
aggressive and has a higher metastatic potential than BCC of other regions. The
mainstay of treatment for localized scrotal BCC is wide local excision, while
metastatic disease may need systemic chemotherapy for palliative control. A rare
clinical scenario of scrotal BCC presenting concurrently with another metastatic
cancer has never been reported. The present case illustrates the diagnostic challenge
and management dilemma due to simultaneous presentation of scrotal BCC and
metastatic squamous cell carcinoma (SCC). A 70-year-old man complained of a
non-healing scrotal ulcer while he was being investigated for metastatic squamous
cell carcinoma (SCC) of the external auditory meatus. The scrotal lesion was
initially thought to be metastatic SCC. It was later confirmed to be BCC with
biopsy and histopathological examination. He underwent surgical resection of the
scrotal BCC for local control followed by palliative chemotherapy with cisplatin
and radiotherapy for lymph node metastases. He remained well and did not have
any local recurrence following 6 months after palliative treatment. Surgery coupled
with palliative chemoradiation can offer good quality of life for patients with scrotal
BCC and concurrent metastatic SCC.

Neuroendocrine carcinoma (NEC) of the endometrium is a rare, aggressive subtype of endometrial cancer. We report a 61-year-old female with a history of breast cancer, s/p modified radical mastectomy, chemotherapy, radiotherapy and hormonal (tamoxifen) therapy, who presented with post-menopausal bleeding. Patient underwent TAH-BSO with lymph node dissection, and was diagnosed with a mixed small cell neuroendocrine carcinoma (SCNEC) and large cell neuroendocrine carcinoma (LCNEC), confirmed by positive immunohistochemical staining for neuroendocrine markers. No other lesions were identified on PET-CT, making a primary endometrial NEC the most likely diagnosis. We review the clinical and pathologic characteristics of endometrial neuroendocrine carcinomas.
