1.Risk Factors for Infections in Patients with Cirrhosis: A Case-control Study

OBJECTIVE To identify the risk factors for infections in patients with cirrhosis.METHODS A case-(control) study was employed.A total of 286 patients hospitalized between 1999 and 2003 were included in.Patients who developed infection complication were defined as case,the remains were as control.The data of possible risk factors,including serum levels of albumin(Alb),prothrombin time(PT),total bilirubin(T-Bil),Child-Pugh score,ascite,esophageal variceal bleeding,total cholesterol(T-chol),other complication,the total white blood cell(wbc),surgery,and days of hospitalization,were retrospectively collected.According to multivariate Logistic regression model,the potential risk factors were assessed.RESULTS Infections developed in 127((44.4%))(patients) and 100(78%) of those developed infection during hospitalization.The mortality rate of case was(20.5%,) which was significant higher than that of control(3.1%,P0.08).Fifty five patients(30.7%) had spontaneous bacterial peritonitis(sbp).Bacterial culture of samples were prepared from 97 of 127 patients((76.4%),) and was positive in 62(63.9%) patients.From these culture-positive samples,35(56.4%)(samples) were Gram-negative bacterial strains,8 of 15 samples from patients developed sepsis were Gram-positive bacteria.On multivariate analysis,the Alb level and days of(hospitalization) were the independent risk factors for cirrhosis patients with infection complication.The odds ratio was 0.33(95% CI(0.32)-0.55),and 0.23(95% CI(0.14)-(0.38),) respectively.CONCLUSIONS Patients with advanced cirrhosis with low Alb level and long days of(hospitalization) should be carefully treated,and administration of broad spectrum antibiotic covering Gram-positive bacteria needs to be considered in treatment of sepsis.
2.Combined therapy of thymoma

Thymomas are most frequent primary tumors of anterior mediastinum.Surgery is the principal treatment method.Thymomas are sensitive to chemotherapy,and chemotherapy has showed a sig- nificant anti-tumor effect in locally advanced,recurrent or metastatic thymomas.Chemotherapy,radiother- apy or combined chemotherapy and radiotherapy before or after operation can offer patients excellent re- sponses,improve resectable rate.Some patients who are once inoperable obtain curative and palliative operation.Radiotherapy after operation can decrease residual tumor tissue,increase local control,de- crease recurrence,and prolong the survival.The multimodality therapy plays an important role in thy- moma treatment.

Objective To investigate the mechanisms of estrogen in the pathogenesis of systemic lupus erythematosus(SLE),we compared the effects of 17β-estradiol(E2)on bone marrow(BM)-derived dendritic cells (BMDCs)in(NZBxNZW)F1(NZB/w F1)female mice befbre and after the disease onset.Methods Immature,BMDCs were differentiated from BM monocytes cultured with rmGM-CSF,rmlL-4,E2 and estrogen receptor (ER)modulator-tamoxifen(TAM)for 7 days.Then immature DCs were stimulated by LPS for 24h to get mature BMDCs.Flow cytometry was utilized to detect the production of costimulatory molecule CD40 and intracellular IL-6,IL-10,IL-12 and TNFα of DCs.Mixed lymphocytes reaction was used to measure the stim-ulatory activity of DCs on spleen T lymphocytes.Results E2 mainly increased the expression of CD40 on im-mature BMDCs but reduced its expression on mature BMDCs.E2 enhanced the stimulatory activity of immature BMDCs,but weakened the activity of mature BMDCs.E2 decreased the production of cytokines of BMDCs in young mice,but increased them in old mice.Conclusion E2 modulates the functions of BMDCs of lupus model mice by binding with ER.The modulation varies depending on disease progression and cell maturation status.
4.The role of estrogen in the pathogenesis of lupus in NZB/w F1 mice by modulating the function of den dritic cells

Objective To investigate the effect of estrogen on dendritic cells (DCs) and its role in disease progression by comparing the effects of 17β-estradiol (E2) on spleen DCs in systemic lupus erythe matosus (SLE) murine modeI-(NZBxNZW) F1 (NZB/w F1 ) female mice before and after the disease onset.Methods Anti-CD11c antibody labeled magnetic bead was used to purify spleen DCs.Flow cytometry was utilized to detect the co-stimulatory molecules and intracellular cytokines of DCs.Mixed lymphocytes reaction (MLR) was used to measure the stimulatory activity of DCs to T iymphocytes.And semi-quantitative RT-PCR was employed to check the expression of estrogen receptor (ER).Results E2 could modulate the expression of surface molecule CD40,the production of cytokines IL-6,IL-10,IL-12 and TNFa,and the stimulatory a bility of spleen DCs in SLE model mice.Tamoxifen (TAM) could antagonize E2 effects and E2 could affect the estrogen receptor (ERα) level of DCs.These changes of DCs varied with age.Conclusion E2 may be in volved in the pathogenesis of SLE by modulating DCs by binding ERa.The effects of E2 vary in different stages of SLE progression.

7.Analysis of human brucellosis monitoring results in Baotou city of Inner Mongolia in 2010

ObjectiveTo find out the prevalence of brucellosis in Baotou,and to provide a scientific basis for integrated control of brucellosis.MethodIn 2010,high-risk people 7 to 70 years of age engaged in livestock farming,grazing,slaughtering,processing and selling were investigated in 9 districts (banners,counties)of Baotou city.In accordance with the principle of informed consent,blood samples were collected,serum titer was tested using the rose bengal agglutination test and tube agglutination test(SAT),and serum titer of 1:100(+ +) was considered as positive brucellosis,and results were evaluated based on brucellosis monitoring standards(GB 16885-1997).ResultsA total of 5832 copy blood samples were collected,and 250 cases were tested positive,the positive rate was 4.25%.The number of new cases was 164,accounting for 2.81% (164/5832).Age was mainly concentrated in the 40 to 60 years,accounting for 66.0%(165/250).Brucellosis infection in the livestock industry accounted for 80.87% (4757/5882).From the regional distribution,DAMAOQI had the most detected cases,and the detection rate was 12.10% (73/603).ConclusionsContinue to increase the brucellosis prevention knowledge propaganda,carry out brucellosis surveillance of high-risk people,quarantine,and strengthen immunity is the only measure of effective control of brucellosis epidemic.
8.Analysis of monitoring results of drinking water-borne endemic arsenic poisoning in Tuyouqi, Baotou of Inner Mongolia in 2010

Objective To understand the arsenic level in drinking water and the current situation in prevention and control of endemic arsenic poisoning in Tuyouqi,Baotou of Inner Mongolia.Methods Through door-to-door survey,we selected patients with arsenic poisoning in order to monitor the arsenic poisoning condition of seven villages(Gangfangying,Laohuyao,Shilaozhang,Youfangying,Wanglaosi,Dajing and Xiaojing) in Tuyouqi,Baotou in 2010,and the disease was diagnosed based on Standard of Diagnosis for Endemic Arsenism(WS/T 211-2001).We also investigated the operation of Salaqi Water Company,Maodai Water Company and the Laohuyao water improvement project.We collected two samples per place in both low water period and high water period to measure water arsenic with silver diethyldithiocarbamate spectrophotometry(WS/T 28-1996).Results Three water improving projects were operating in normal conditions.Water arsenic in Salaqi Water Company and Maodai Water Company was level less than 0.01 mg/L,which met the National Safety Standards of drinking water with arsenic.Water arsenic in Laohuyao improvement project was 0.053 mg/L,which was higher than the drinking water safety standards.The seven villages in Tuyouqi had improved their water conditions.Among them,the arsenic level in Laohuyao was 0.052 mg/L,and the remaining six villages were < 0.05 mg/L.There were 6 suspected cases,14 mild cases,4 moderate cases and 10 severe cases of the 2095 people monitored and no new cases found.The age of patients with arsenic poisoning was in the range of 40 to 70.Conclusions The monitoring results show that drinking water arsenic level in some villages in Tuyouqi is still excessive,but no new arsenic poisoning patients in arsenic poisoning areas are found.
9.Development of targeted therapies in small cell lung cancer

Small cell lung cancer(SCLC) is a type of neuroendcorine cancer with high growth fraction, early metastatic spread and poor prognosis, accounting for approximately 15% of all newly diagnosed lung cancer cases.Patients with SCLC are generally treated with platinum-based chemotherapy in combination with radiotherapy.Despite good responses to chemotherapy in the early stage of treatment, patients develop drug resistance and recurrence soon.There has been limited success with the targeted approaches in clinical trials completed in the past several years.Novel targeted and more effective treatment strategies for SCLC are in urgent need With increasing translational research and a better understanding of the molecular basis of small cell lung cancer, a number of new targeted drugs such as kinase inhibitors, angiogenesis inhibitors, apoptosis inducers, proteasome inhibitors, epigenetic regulators, immune checkpoint inhibitors have been developed and investigated in various preclinical studies.Some of them have entered clinical trials.At the same time, a number of novel treatment strategies such as immunotherapy and combination treatment receive attention.This review summarizes potentially molecular targeted therapies that have been developed and employed recently, and ongoing and future clinical trials in attempt to improve patient outcomes in SCLC, and meanwhile to invite future potential SCLC new treatment strategies.
10.Reconstruction of thumb opposing function by transfering the extensor carpi ulnaris and the extensor pollicis longus muscle tendons

Objective To evaluate results of reconstructing thumb opposing function by transfering the extensor carpi ulnaris and the extensor pollicis longus muscle tendons. Methods Forty-nine patients with dysfunction of thumb opposing were admitted to this study. Twenty-nine had median nerve injury in the wrist, twenty complicated ulnar nerve injury at the same time. Thumb opposing function of all patients was reconstructed by transfering the extensor carpi ulnaris and the extensor pollicis longus muscle tendons. Results All patients with median nerve injury got satisfactory results. 80% of patients complicated ulnar nerve injury got fine therapeutic effect. Conclusion It is a convenient and efficient procedure that reconstructing thumb opposition function by transfering the extensor carpi ulnaris and the extensor pollicis longus muscle tendons.
