1.Doppler tissue imaging to assess right ventricular function in patients with chronic pulmonary hypertension

Objective To investigate the right ventricular function of patients with chronic pulmonary hypertension using Doppler tissue imaging(DTI)and to evaluate the relationship between DTI parameters and pulmonary artery systolic pressure(PASP).Methods Thirty-five patients with chronic pulmonary hypertension and thirty-five healthy subjects underwent Doppler tissue echocardiographic evaluation of right ventricular function.The tricuspid lateral annular systolic velocity(Sm),early(Em)and late(Am)peak diastolic velocities,isovolumic contraction time(IVCT)and isovolumic relaxation time(IVRT)were acquired from apical 4-chamber view.Results When compared with control group,Sm,Ea and Em/Am values were found to be lower and Am,IVRT values higher in patients with chronic pulmonary hypertension.When all patients were analyzed,there was a significant positive correlation between PASP and IVRT(r=0.78,P
2.Application of total hip replacement for intertrochanteric fractures following the failure of dynamic hip screw fixation in four elderly people

BACKGROUND:Many internal fixations,such as dynamic hip screw,Gamma screw,proximal femoral nail,angle steel plate,as well as locking proximac femoral plate,are utilized in treating intertrochanteric fractures,especially the dynamic hip screw However,the failure rate is gradually increased.OBJECTIVE:To explore the application and clinical efficacy of total hip replacement for the treatment of elderly intertrochanteric fracture fixation after failure of dynamic hip screw.METHODS:A total of four cases with intertrochanteric fractures were treated by total hip replacement after failure of dynamic hip screw fixation was selected.According to Evans typing,one case were type Ⅱ,two cases were type Ⅲ A,and one case was typeⅢB.Internal fixation displacement could be found at half to 1 year after dynamic hip screw fixation.Because of bone disunion,coxa adducta and pain,the patients could not walk.Sequentially,total hip replacement was performed with 45°abduct angle and10°-15°anteversion angle The clinical efficacy was evaluated by Harris scoring criteria.RESULTS AND CONCLUSION:All the cases were operated smoothly,with 1.5-2 hour operation duration and 400-600 mL blood loss.No case appeared allergic response to bone cement.By 3-12 months follow-up,4 successful operative cases do not appear prosthesis loosening,subsidence and are satisfied with well hip function.The average Harris score were 81 points.The results revealed that application of total hip replacement after the failure of DHS in elderly intertrochanteric fracture fixation,which shortened the time patients stay in bed to reduce complications and improved the hip joint function.
3.Effect of dark rearing on development of visual sense and proteomics of visual cortex in growing rat

Background Previous study on critical period plasticity in local cortical circuits primarily was only to test the role of some proteins in visual cortex on visual development based on the existed neural signals expression system,but whole containing proteins analysis in cortical circuits is lack.To perform a whole containing proteins analysis has an important significance for critical period plasticity study.Objective This study was to investigate the influence of dark rearing on visual sense and proteomics in visual cortex for growing rat.Methods Two SD female rats were fed in two cages together with 12 newborn rats on the same day respectively,and half number of newborn rats were exchanged each other from first day after delivering and marked by eartipping.The newborn rats in a cage were bred in the dark environment for 40 days,and newborn rats in other cage were bred in the nature environment as controls.The blink response of rats to nearby object was examined and compared between the two groups of rats.Then three rats from two cages were sacrificed respectively and bilateral primary visual cortex tissue was isolated.Proteomics in rat primary visual cortex was detected by two-dimensional electrophoresis and mass spectrometry and the result was checked from database.Then the survival rats in the dark environment returned to the nature environment for 10 days,and the blink response of rats to nearby object were compared with that of agematched rats in the natural environment.The use and care of experimental rats followed the instruction of Ethic Committee of Nankai University.Results The blink response of rats was (0.33 ± 0.35) times in the dark environment for 40-day group,and that in the natural environment for 40-day group was (6.42±0.68) times,with a significant difference between them (t =24.38,P<0.01).After returned to natural environment for 10 days,the blink response times of rats were less than those of the natural environment group ([5.00±1.22] times vs.[6.11±0.59]times),but this change was not statistically significant (t =2.09,P>0.05).Two-dimensional electrophoresis and mass spectrometry assay revealed that 36 different proteins in visual cortex were found in the dark-feed rats compared with the natural environment rats,including 26 loss proteins and 10 extra proteins.Among the different proteins,Eps 15 homology domain-containing protein-3 (EHD3),tubulin alpha-1A chain and 2 ',3 '-cyclic-nucleotide 3 ' phosphordiesterase were the known proteins.Conclusions Dark rearing cause reversible visual loss in critical period plasticity newborn rat,and the change of proteomics in visual cortex is probably an affecting factor.
4.Identification of plasmid-mediated AmpC ?-lactamase in Escherichia coli

Objective To investigate genotypes and antibiotic resistance of Escherichia coli producing plasmid-mediated AmpC ?-lactamase in Anhui province.Methods A total of 407 clinical isolates of nonrepeated Escherichia coli were collected from different cities in Anhui province.AmpC ?-lactamase producing isolates were identified by cefoxitin three-dimensional test and antibiotic susceptibility was identified by agar dilution test.Plasmid extraction,PCR amplication of corresponding group was performed, followed by sequencing.Results The positive rate of cefoxitin three-dimensional test was 8.1% (33/407), and the prevalence of plasmid-mediated AmpC ?-lactamase was 3.0% (12/407).bla_(CMY-2) gene,bla_(DHA-1) gene and bla_(ACT-2) gene were identified by PCR amplification and confirmed by sequencing in 5 strains,4 strains and 2 strains,respectively.A new CMY genotype was identified,with its sequence revealed 97% identity to the deduced amino acid sequence with previous CMY-2.This is also the first report on ACT-2 genotype in China.The susceptibility test showed that all strains were resistant to cephamycins and piperacillin,and susceptible to imipenem.Two strains of Escherichia coli producing DHA-1 were resistant to fourth-generation cephalosporin.Conclusions CMY-2,DHA-1 and ACT-2 are the most common genotypes in plasmid-mediated AmpC ?-lactamase produced by clinical Escherichia coli isolates in Anhui province. Carbapenems could be the first choice for the treatment of infection caused by AmpC ?-lactamase producers.

Objective To demonstrate the diffusion tensor imaging(DTI)characteristics of multiple sclerosis(MS)plaques,periplaque white matter regions and normal appearing white matter (NAWM)regions in patients with MS,and to evaluate the clinical values of DTI and three-dimensional brain fiber tracking for the diagnosis of MS.Methods Conventional MRI and DTI were performed in 32 patients with MS and 32 age-matched control subjects.Fractional anisotropy(FA)and apparent diffusion coefficient(ADC)maps were generated and coregistered with T_2-weighted MR images,FA and ADC values were calculated in regions of interest in plaques,periplaque white matter regions,NAWM regions and white matter regions in control subjects.And three-dimensional brain fiber tracking maps were generated by using the DTI.Results TheADCwas(1.233?0.119)?10~(-3)mm~2/s in MS plaques,(0.973?0.098)?10~(-3) mm~2/s in periplaque white matter regions,(0.748?0.089)?10~(-3)mm~2/s in NAWM,and(0.620? 0.094)?10~(-3)mm~2/s in control subjects.The FA was 0.225?0.052 in MS plaques,0.311?0.050 in perip]aque white matter regions,0.421?0.070 in NAWM,and 0.476?0.069 in control subjects. In order to summarize and perfect the basic skill practicing method in acupuncture teaching and form a characteristic teaching, the research on skill training and technique improving of acupuncture and tuina major was carried out. The undergraduate students in acupuncture and tuina major from College of Acupuncture-Moxibustion and Tuina were randomly divided into an experimental group and a control group. Students from both groups received the basic courses of acupuncture-moxibustion and tuina as requested in the syllabus and the special teaching was added to the experimental group, namely "three practice" (strength practice, coordination practice, targeted practice of acupuncture basic skills) and "three enlightenment" (technique enlightenment, mind and qi enlightenment, efficacy enlightenment). During and after the courses, the teaching effectiveness was evaluated on the basis of needling manipulation. The peacetime score in the experimental group was superior to that in the control group (38.03 +/- 1.14 vs 33.25 +/- 1.31, P < 0.05), the total score in the experimental group was superior to that in the control group (84.03 +/- 6.30 vs 78.05 +/- 6.55, P < 0.05). The special teaching method of "three practice" and "three enlightenment" can improve the effect of skill training teaching method. It highlights the unique feature of central plains by integrating the essence of Shaolin internal qigong and Chen-style Taiji, it can also provide new ideas for the practical teaching of TCM professional courses and the construction of characteristic professionals.

8.Effects of Taijiquan on Motor Function and Depression in Patients with Post-stroke Depression

Objective To explore the effect of Taijiquan on motor function and depression in patients with post-stroke depression. Meth-ods From January to October, 2015, 60 stroke patients with depression were randomly divided into control group (n=30) and Taijiquan group (n=30). Both groups accepted routine rehabilitation, while Taijiquan group accepted another training of six actions from simplified Taijiquan. They were assessed with Fugl-Meyer Assessment (FMA), Barthel Index (BI) and Hamilton Depression Rating Scale (HAMD) be-fore, and four and eight weeks after treatment. Results The scores of FMA and BI increased more in Taijiquan group than in the control group after treatment (t>2.351, P<0.05), while the score of HAMD decreased more (t>2.169, P<0.05). Conclusion Taijiquan can relieve the depression and improve the motor and activities of daily living in patients with post-stroke depression.
9.Management of benign anastomotic stenosis after rectal cancer surgery

Objective:To discuss the occurrence, treatment and prevention of benign anastomotic stenosis after radical resection for rectal cancer.Methods:The clinical data of 63 patients with benign anastomotic stenosis from Jan. 2016 to Dec.2020 at Department of Gastrointestinal Surgery, Renji Hospital Shanghai Jiaotong University School of Medicine, were retrospectively analyzed, including general conditions, intraoperative conditions, postoperative complications, anastomotic stenosis, treatment process and outcome. The relationship between stenosis type and treatment and outcome were analyzed.The measurement data obeying normal distribution was expressed by ( Mean± SD), and the t test was used comparison between groups. The chi-square test was used comparison between enumeration data. Results:Of all the 63 patients, 22 (34.9%) cases presented with membranous stenosis, 30 (47.6%) cases with tubular stenosis, and 11 (17.5%) cases with diffused stenosis. Three of the 9 patients with high stenosis underwent balloon dilatation through endoscopy, 3 were placed with self-expandable metal stent and the rest 3 patients underwent resection and reconstruction of the anastomosis. All the 54 patients with low stenosis underwent digital anal expansion, and finally the effective rate was 53.7% (29/54). Endoscopic balloon dilatation was successfully performed in 8 cases, including 4 cases were placed metal stent throngh endoscopy. Eight patients underwent trans-anal stricturotomy. In 5 patients with low diffused stenosis, either ileostomy was preserved or permanent colostomy was performed due to failure to treatment. There were more male patients, protective ileostomy, anastomotic leakage and low stenosis in patients failed to treatment than in the cured patients ( P>0.05). However, all the 5 patients who failed to treatment were suffered from diffused stenosis, and the difference was statistically significant compared with those who were cured ( P<0.05). Conclusions:Postoperative anastomotic stricture after anterior rectectomy requires different treatment strategies according to the location and types of stricture. Endoscopic balloon dilatation is preferred for high stenosis, and metal stents can be placed optionally. Digital anal expansion is preferred for low anastomotic stenosis, and endoscopic or minimally invasive transanal surgery is feasible if digital anal expansion fails.
10.Effects of Sheng-wu Capsule on learning-memory ability and hippocampal histology in aged rats

ObjectiveTo investigate the effects of Chinese compound Sheng-wu Capsule on learning-memory ability and histology including the neurons, astrocytes and expression of nerve growth factor(NGF) and its receptor (TrkA) in hippocampus CA1 of aged rats. MethodsSheng-wu Capsule consists of 6 kinds of Chinese medicinal herbs such as polygonum multiflorum,ginseng, and Rhizoma Acori Tatarinowii.The water-soluble component of polygonum multiflorum,2,3,5,4'- tetrahydroxystibane-2-O-β-D-glucoside was the quality standard of Sheng-wu Capsule.Aged Wistar rats(22 months old) were orally administrated with Sheng-wu Capsule(1.8g/kg and 0.9g/kg) for 2 months. Positive control drug was Piracetam(0.56g/kg). The learning and memory ability was tested by passageway water maze; the number and morphology of neurons was detected by HE staining; the proliferation of astrocytes was detected by immuno-histochemistry for glial fibrillary acidic protein(GFAP); the expression of neurotrophic substance was detected by immuno-histochemistry for NGF and TrkA receptor by ABC method. Image analysis to determine average cell number, size(area) and staining density in hippocampal CA1 region was achieved by VISILOG 5.0 image pattern analyser.ResultsBoth NGF and TrkA-immunoreactive neurons in aged rats significantly decreased as well as cell number, size and staining density in CA1 region of hippocampus(P<0.01). Aged rats showed damage of neurons and increase of GFAP positive cells in the same area,model rats indicated learning-memory deficit too. In Sheng-wu Capsule administrated groups,the significant little decrease in cell number, size and staining density of NGF and TrkA-immunoreactive neurons in CA1 region of hippocampus was observed(P<0.01-0.05). The loss of Neurons and proliferation of astrocytes were inhibited and learning-memory ability was also improved in Sheng-wu groups.Conclusions Chinese compound Sheng-wu Capsule significantly promotes the expression of NGF and TrkA in aged rats and it relieves the damage of hippocampus.This may be the mechanism by which Sheng-wu Capsule improves the learning and memory ability. Enhancing endogenous neurotrophic substance plays an important role in the treatment of Alzheimer's disease.
