1.Testicular torsion (report of 18 cases)

Objective To evaluate the diagnosis and tre atment of testicular torsion. Methods 18 cases of testic ular torsion were reviewed and reported. Results The tes ticles of 5 cases with short time or incomplete torsion were salvaged by operati ve detorsion.The testicles of 13 cases with testicular necrosis were removed (re moval rate 70%). Conclusions Testicular torsion has to b e diagnosed correctly on time and the detorsion has to be performed early.For di agnosis of testicular torsion color-Doppler ultrasonography image is a reliable method.Preventive orchiopexy has to be performed in order to avoid recurrence o r relapse.
2.Analysis of risk factors for emergence agitation in adults undergoing general anesthesia for nasal surgery.

OBJECTIVE:
To identify the incidence and the risk factors for emergence agitation (EA) in adults undergoing general anesthesia for nasal surgery.
METHOD:
We examined 674 patients aged ≥ 18 years who underwent general anesthesia for nasal surgery between February 2013 and February 2015. The patients were divided into control group (518 cases) and EA group (156 cases) by Sedation-agitation scale (SAS) method. Demographic and clinical variables were assessed and the data were analyzed by multiple logistic regression analysis.
RESULT:
The overall incidence of emergence agitation was 23. 15%. Significant difference was observed between EA and the control group in many aspects, such as sex, age, ASA classify, smoking history, history of cerebrovascular disease, preoperative anxiety, the use of midazolam, anesthesia means, postoperative pain, postoperative analgesia, presence of a tracheal tube, and presence of a urinary catheter. The results of multiple logistic regression analysis indicated that the occurring of EA was significantly correlated with younger age, male, preoperative anxiety, inhalation anesthesia, postoperative pain, presence of a tracheal tube, and presence of a urinary catheter, while seniors, with the use of midazolam, total intravenous anesthesia, analgesia and natural awakening were protective factors.
CONCLUSION
EA following general anesthesia is a common complication in patients with adult nasal surgery. To reduce the occurrence and consequences of agitation episodes, elimination of the associated risk factors is necessary, especially in patients with risk factors.

3.Investigation Analysis of Pregnancy Contraindication in Specifications of Chinese Patent Medicine in Shunyi Hospital of Beijing Chinese Medicine Hospital

Objective To provide references for clinical rational use of Chinese patent medicine. Methods Totally 387 specifications of Chinese patent medicine in our hospital were analyzed, and were judged by pregnancy contraindication according to China Pharmacopeia (2010 edition). Results The 387 Chinese patent medicines involved 32 prohibited pregnancy ingredients, 1 taboo ingredient. In 89 Chinese patent medicines contraind prohibited ingredients, 33 marked prohibited ingredients, 18 marked contraindicated ingredients, 14 marked cautious ingredients, and 24 marked without-contraindication ingredients. 68 Chinese patent medicines contained pregnancy contraindication labels but without prohibited ingredients. Conclusion A unified standard of pregnancy contraindication should be established;researches on pregnancy contraindication should be strengthened;medicine specifications should be standardized and perfected.

5.The clinical significance of monitoring of coagulation and fibrinolysis related indexes after hip replacement for preventing and diagnosing lower extremity deep venous thrombosis

Objective To evaluate the clinical significance of monitoring of D‐dimer ,fibrinogen and fibrin degradation products after hip replacement for diagnosing and preventing the lower extremity deep venous thrombosis .Methods A total of 66 cases of patients who carried out hip replacement from April 2013 to April 2015 in the department of orthopaedic in this hospital were se‐lected .Patients complicated with lower extremity deep venous thrombosis were enrolled into the observation group ,while patients without lower extremity deep venous thrombosis were enrolled into the control group .Levels of D‐dimer ,fibrinogen and fibrin deg‐radation products were strictly monitored after hip replacement and were comparatively analysed .Results There were no statisti‐cally significant differences in levels of fibrinogen and fibrin degradation products between the two groups (P>0 .05) ,while statisti‐cally significant difference was found in level of D‐dimer between the two groups(P<0 .05) .Conclusion The monitoring of D‐di‐mer ,fibrinogen and fibrin degradation products after hip replacement could have clinical significance for diagnosing and preventing lower extremity deep venous thrombosis .
6.Clinical observation on Yinhuang decoction and external application of Xilei decoction in treating vesicular stomatitis

Objective To evaluate the effects ofYinhuangdecoction and external application ofXilei decoction in treating vesicular stomatitis.Methods From January 2012 to October 2013, 126 enrolled cases were randomly divided into a treatment group and a control group with 63 in each. The treatment group, which was treated with acycloguanosine tablets as the control group did, was additionally given orallyYinhuang decoction and external application ofXilei decoction. The symptoms, clinical effects comparison were observed before and after the treatment.Results After 7-days treatment, the cure rate of the treatment group was 95.2%(60/63), and the control group was 73.0%(46/63). Their difference was conspicuous(χ2=17.407,P<0.05). The treatment group attained better curative effects(P<0.05) and got shorter symptomatic relief time of the duration of fever, time of tain disappeared, time of ulcer dissipated and cure time(The T-value is respectively 9.590, 6.983, 5.864, 5.814,P<0.01).ConclusionsYinhuang decoction and external application ofXilei decoction is effective for treating vesicular stomatitis.
7.Application value of high-sensitivity troponin T in acute coronary syndrome

Objective To investigate the application value of high-sensitivity troponin T (hs-cTnT)in acute coronary syndrome (ACS).Methods Serum hs-cTnT,CK and CK-MB were detected 198 patients with chest pain including 105 cases of acute myocar-dial infarction(AMI)and 93 cases of unstable angina(UA),and 83 cases of healthy physical examination as control.Results The positive rates of hs-cTnT,CK and CK-MB in the patients with diagnosed AMI were 88.6%,48.6% and 71.4% respectively,which were significantly higher than those in the patients with UA and the healthy controls(P <0.05 ).The positive rate of hs-cTnT in early AMI patients was higher than that of CK and CK-MB.Conclusion Compared with CK and CK-MB in myocardial Zymogram, hs-cTnT has very high sensitivity and specificity for diagnosing myocardial injury,could reduce the misdiagnosis and missed diagno-sis of ACS and is a marker for diagnosing the myocardial injury in early ACS.
8.Application of aCGH technology in diagnosing small supernumerary marker chromosome

Objective To explore the clinical application of array comparative genomic hybridization (aCGH) and karyotype a-nalysis in the prenatal evaluation of fetal with small supernumerary marker chromosome (sSMC) .Methods One case was indenti-fied with de novo small supernumerary marker chromosome .G-banding analysis indicated that the fetus had a karyotype of 47 , XY ,+Mar .aCGH was used to define the precise location and size of de novo chromosome .Results aCGH revealed that there was 2 .03 Mb duplication from 15q11 .1-q11 .2 in the fetus .aCGH revealed the presence of small supernumerary marker chromosome . Conclusion The technologies of aCGH can be used for identifying the origin of small supernumerary marker chromosome and defi-ning the loci of the chromosome .Combined with the karyotype analysis ,it can be applied to genetics analysis and prenatal diagnosis .
9.Comparison of the efficacy of amisulpride and risperidone in the treatment of patients with first -episode schizophrenia

Objective To compare and analyze the efficacy and safety of amisulpride and risperidone in the treatment of patients with first -episode schizophrenia(FES).Methods 80 patients with FES were randomly divided into the observation group (n =40,orally given amisulpride)and the control group (n =40,orally given risperidone) according to random number table.The total effective rate,positive and negative symptoms scale (PANSS),adverse reaction rate of two groups were compared.Results 1 week after treatment,the negative symptoms score of the observation group was significantly lower than that of the control group [(20.89 ±3.07 )points vs.(18.15 ± 3.64)points,t =3.639,P =0.001].8 weeks after treatment,the PANSS scores and factor score of the two groups had no significant differences(all P >0.05).The total effective rate and adverse reaction rate of the observation group were 90.00%,17.50% respectively,which of the control group were 82.50%,27.50% respectively,there were no statistically significant differences (χ2 =0.949,P =0.330;χ2 =1.147,P =0.284).Conclusion Amisulpride and risperidone has similar efficacy and safety in the treatment of patients with FES,but amisulpride has better early effect in improving negative symptoms.
10.A Nosocomial Infection Point-Prevalence Survey:Results and Analysis

OBJECTIVE To estimate the current situation about nosocomial infections(NI),and find possible reasons to improve measures.METHODS A point-prevalence survey of NI in all inpatients was made on June 11,2007.RESULTS Of the 755 patients investigated,the prevalence rate of NI was 4.77%(36 cases).In all of the NI occurred,the common sites of NI were respiratory tract(69.30%),superficial surgical incision(13.80%),and skin soft tissue(8.33%).The usage rate of antibiotics was 57.40%,The single usage rate of antibiotics was 60.70%,double antibiotics usage 38.30%,and triple antibiotics usage 0.90%.CONCLUSIONS The departments of nephrology and neurosurgery are with high NI rate.Strengthening the management of severe patient and rational using antibiotics can help to reduce prevalence rate of NI.
