1.Ethical Reflections on Hospital Administration

Based on their multiple years of personal experiences in hospital administration,authors of this article point out that medical service,humanistic service,and emotional service should be the core aspects in the current hospital competition.Medical professionalism embodied in the three levels of consciousness,behaviors,and regulations should be the main subject in the research on the role of medical ethics in hospital administration and medical treatment behaviors.It is an urgent ethical requirement and the chief task for hospital administrators to achieve humanistic medical service through humanistic administration of hospitals.
2.CD151 promotes proliferation and migration of PC3 cells via the formation of CD151-integrin α3/α6 complex.

Over-expression of CD151 was found to be associated with metastasis and poor prognosis of prostatic carcinoma. This study was designed to examine the mechanism by which CD151 promotes the proliferation and migration of prostatic cancer cells. The pAAV-CD151, pAAV-GFP and pAAV-CD151-AAA mutant plasmids were constructed and used to transiently transfect PC3 cells (a prostatic carcinoma 3 cell line) by the mediation of Fugene HD. Then, the cells were assigned to control group, pAAV-GFP group, pAAV-CD151 group, and pAAV-CD151-AAA group respectively. Cell proliferation was evaluated by using the 3-[4,5-dimet-hylthiazol-2-yl]-2,5, diphenyltetrazolium bromide (MTT) method. Cell migration assay was performed by using Boyden chambers. The formation of CD151-integrin α3/α6 complex was determined by the method of co-immunoprecipitation. The protein expression levels of CD151 and extracellular signal-regulated kinase (ERK) were measured by Western blotting. The results showed that transfection of pAAV-CD151 or pAAV-CD151-AAA mutant increased the expression of CD151 protein in PC3 cells. Co-immunoprecipitation showed that more CD151-integrin α3/α6 complex was formed in the pAAV-CD151 group than in the control group, the pAAV-GFP group and the pAAV-CD151-AAA mutant group. Furthermore, the proliferative and migrating capacity of PC3 cells was substantially increased in the pAAV-CD151 group but inhibited in the pAAV-CD151-AAA mutant group. CD151 transfection increased the expression of phospho-ERK. Taken together, it was concluded that CD151 promotes the proliferation and migration of PC3 cells through the formation of CD151-integrin complex and the activation of phosphorylated ERK.
3.Effect of sodium ferulate on transdifferentiation and extracellular matrix of rat renal tubular epithelial cells induced by transforming growth factor beta 1

BACKGROUND: Renal tubulointerstitial fibrosis is mainly featured as the accumulation of extracellular matrix (ECM) in renal interstitium. The tubular epithelial-myofibroblast transdifferentiation (TEMT) is important to the pathogenesy of renal tubulointerstitial fibrosis. OBJECTIVE: To examine the effects of sodium ferulate (SF) on TEMT, and ECM main components such as collagen Ⅰ, collagen Ⅲ and fibronectin, in rat renal tubular epithelial cellsinduced by transforming growth factor-beta 1 (TGF- β1)- DESIGN: Randomized and controlled experimental study based on cells. SETTING: Department of Kidney in West China Hospital of Sichuan University. MATERIALS: Rat renal tubular epithelial cells (NRK-52E) originated from American Type Culture Collection (ATCC), were offered by the laboratory of Department of Nephrology in Australian Monash Medical Center. Cell strain used in this study was cultured at the 36th passage. SF white crystal with water solubility and more than 98.0% purify, was from Chengdu Hengda Pharmaceutical Co., Ltd. Different concentrations of SF (125,250, 500μreel/L) were designed in this study. Rabbit anti-rat α-smooth muscle actin (α -SMA) was produced by Wuhan Boster Company. Enzyme-linked immunosorbent assay (ELISA) kit was the produced of Shanghai Senxiong Science and Technology Co.,Ltd. Human recombinant TGF- β1 was produced by R&D Company. DNA Engine OpticonTM real-time fluorescence quantitative polymerase chain reaction apparatus was the product of MJ Research Company. METHODS: Rat renal tubular epithelial cells (NRK-52E) cultured in vitro were divided into five groups. Control group was added with serum-contained DMEM; TGF-β1-induced group was added with TGF-β1 at final concentration of 5 ng/L; SF at different concentrations groups were added with 125, 250, 500 μ mol/L SF and TGF- β1 at final concentration of 5 ng/L,respectively. MAIN OUTCOME MEASURES: The contrast phase microscope, real-time fluorescence quantitative polymerase chain reaction and ELISA method were used to detect TEMT of NRK52E cells induced by TGF-β1 and levels of collagen Ⅰ, collagen Ⅲ and fibronectin in the supernatant. RESULTS: Morphology of NRK52E cells: Compared with control group, TGF-β1 could induce the transdifferentiation of NRK52E cells, showing fibroblast-like in morphology after 3 days, which were previously the typical road stone-like epithelial cells. In three different concentration SF groups, the morphologic transformation stimulated by TGF-β1 could be partly ameliorated in a dose-dependent manner. Expression of α-SMA mRNA: Compared with control group, 5 ng/L TGF- β1 enhanced expression of α-SMA at 6 hours, and reached a peak at 72 hours; SF depressed the expression in a dose-dependent manner at 72 hours (P < 0.05). Changes of ECM: After induced by 5 ng/L TGF- β1 for 72 hours, the levels of collagen Ⅰ, collagen Ⅲ and fibronectin in the supernatant increased significantly (P < 0.05), whereas SF decreased these levels in a dose-dependent manner (P < 0.05). CONCLUSION: TGF- β1 induces the TEMT, and promotes the secretion of collagen Ⅰ, collagen Ⅲ and fibronectin. SF can inhibit TGF- β1-induced TEMT In a dose-dependent manner.
4.Overexpression of anchor attachment protein in colorectal carcinoma

Objective To investigate the expression of anchor attachment protein(AAP)in colorectal carcinoma tissues and serum level of AAP in colorectal carcinoma patients and to identify the relationship among AAP expression,lymph node and liver metastases.Methods Immunohistochemistry was used to detect AAP expression,and ELISA was used to test AAP level in peripheral vein blood in 83 patients with colorectal carcinoma.Results The expression rate in normal colorectal mucosa,primary cancer,lymph nodes and liver metastases were 20.5%、53.0%、69.8% and 80.0%,respectively.The positive rate of AAP in primary cancer,lymph nodes and in liver metastasesfoci was higher than that in normal mucosa(P

6.Effect of Jiuqiang Naoliqing on the TXA_2 and PGI_2 level in spontaneous hypertension rat plasma

ObjectiveTo investigate the effect of Jiuqiang Naoliqing(JNQ) on the TXA2 and PGI2 level in spontaneous hypertension rat (SHR) plasma.MethodsThe plasma was separated after the SHR and Wistar rats were treated with JNQ at the dose of 0.133g/kg,0.265g/kg,0.530g/kg and 1% carboxymethyl cellulose respectively for 5 weeks. The level of TXB2 and 6 keto PGF1α ,stable metabolin of TXA2 and PGI 2,in SHR plasma was tested by radioimmunoassay.ResultsThe level of TXB2 and the ratio of TXB2/6 keto PGF1α (T/P) in SHR plasma increased significantly(P<0.01),and there was no significant difference in the concentration of 6 keto PGF1α between Wistar rats and SHR plasma(P>0.05). JNQ could increase the generation of 6 keto PGF1α and decrease the level of TXB2 and T/P in SHR plasma after treated with different dosages for 5 weeks.ConclusionJNQ may improve the balance between TXA2 and PGI2 in SHR plasma.
8.Thin barium meal examination of papilla function after endoscopic sphincterotomy

Objective To assess short term results of papilla functional status after endoscopic sphincterotomy (EST) with thin-barium meal examination.Methods From August, 2001 to December, 2003, eighty-nine patients were included for endoscopic sphincterotomy. Size of EST was (0.5~1.5) cm. Patients were prospectively followed on the short term-period (7 days, 6 months and 1 year) by clinical and thin-barium(100/100 V/W) meal examination which would be observed biliary gas and barium reflux from duodenal papilla.Results The patient number of gas reflux shows: 19 of 89 cases(21.3%) in one week, 5 of 36 cases(13.9%) in six months, 13 of 23 cases (13.0%)in one year; barium reflux with thin-barium meal examination shows: 11 of 89 cases(12.4%) in one week, 3 of 36 cases(8.3%) in six months, 2 of 23 cases(8.7%) in one year. In the size of EST more than 1.1 cm, these were nine patients (47.4%) with gas reflux, and seven patients (36.8%) with barium reflux, and five patients with gas-barium mix reflux. 6 month and 1 year after sphincterotomy, includes EST size 1.2 cm,2 cases and 1.5 cm,3 cases.Conclusion Thin-barium meal examination of papilla function after endoscopic sphincterotomy is an efficient procedure. Incidence rate of gas reflux and thin barium reflux were closely related to the size of EST.

Objective To explore the clinical application of ultrasonography on tethered spinal cord syndrome (TCS).Methods Ultrasonic feature of 45 patients with TCS were analyzed retrospectively.
