1.Interventional therapy of postpartum hemorrhage: report of 19 cases
Chinese Journal of General Practitioners 2008;7(1):52-53
This investigation was to evaluate the implication of interventional therapy in postpartum hemorrhage at emergency settings. Due to the failure of conservative therapy, nineteen postpartum hemorrhage cases received digital subtraction angiography(DSA)followed by internal iliac or uterine artery embolization by using gelatin sponge. As a result, all of 19 patients showed good response to the treatment, and no recurrence Was found after 6 months' follow-up. Our findings suggested that artery embolization is a safe, effective and micro-invasive treatment strategy for intractable postpartum hemorrhage, which could reduce mortality and avoid hysterectomy as well.

miRNA is a kind of endogenous non-coding short RNA. Mature miRNA was formed through the process of shearing and transporting after genetic transcription. miRNA exhibits many important biological functions through regulating expression and translation of target mRNAs. Different miRNAs may act as oncogenes or antioncogenes, and have tissue specificity. The progress of the tumorigenesis is usually accompanied by expression-profile changes of miRNAs. MiRNA regulates many tumor biological behaviors such as differentiation, proliferation, apoptosis, invasion, metastasis, and drug resistance of tumors. Furthermore, some miRNAs have clinical significance in predicting prognosis of tumor patients.

Objective To investigate the features of transcatheter selective hepatic arteriography and efficacy of embolization in hepatic hemangioma. Methods Twenty-six patients of hepatic hemangioma underwent transcatheter selective hepatic arteriography and embolization with Lipiodol-ultrafluida as well as Pingyangmycin, and were followed-up for 3 - 12 months after treatment. Results The popcorn-like tumor stain sign and early fill-in and persistant staining sign of hepatic arterial angiography were observed in all patients before embolization. After one time of embolization, the tumor size decreased more than 25% in 22 patients (84.62%), less than 25% in 4 patients. No severe complication occurred. Conclusion Transcatheter selective hepatic arteriography and embolization is a favorable method of diagnosis and treatment for hepatic hemangioma.

OBJECTIVE: To introduce the research progression of fMRI in brain functional localization in China or abroad in the recent 15 years, so as to further comprehend the effect and value of fMRI.DATA SOURCES: A computer-based online search of Medline database was undertaken to identify English articles about fMRI published from 1990 to 2005 with the keywords "functional magnetic resonance imaging (fMRI), brain".STUDY SELECTION: The articles about experimental studies of fMRI articles irrelative to motor function, sensory function, hearing function, visuEnglish title.DATA EXTRACTION: Totally 6 144 relevant articles were collected, 18 articles were involved according to the inclusive criteria, and the others were excluded. cles about sensory function, including 1 about simple tactile stimulation,ry function, including 2 about simple visual stimulation, and 1 about binoction, including about hearing stimulation by various methods, and 1 about comparison between normal and abnormal subjects by the same stimulation.language function, 1 about visual language function, and 1 about listening basic principle.CONCLUSION: fMRI was more accurate, convenient and direct than other methods. It can not only show localization, size and dimension of the activated areas in different cerebral regions, but also show accurate anatomical position. Many aspects still need to be consummated, for example, exploration of the stimulating method, design of advanced hardware, post processing of the image and the effect of visualization. With the development of hardware, software and the endless endeavor of scientists, fMRI dooms to play an important role in illustrating the function of human brain.

Objective To investigate if glucose-insulin-potassium (GIK)would relieve the liver injury induced by endotoxemia in rats. Methods Sixty SD male rats, weight 200-250g,were randomly divided into three groups (n = 20):control group (group C),lipopolysaccharide group (group LPS,LPS 8 mg/kg)and Glucose-insulin-potassium group(group GIK,8 mg/kg LPS+GIK 4 ml·kg-1 ·h-1 ).All the rats were injected with 20 mg/kg ketamine intraperitonealy before trial. Erythrocin was daubed on the wound to avoid infection. The rats of group LPS and group GIK were injected LPS 8 mg/kg intraperitoneal, then, rats in group LPS and group GIK received saline(4 ml·kg-1 ·h-1 )or GIK(Glucose 200 g/L,Insulin 60 IU/L,KCL 60 mmol/L)infusion continuously. Liver and serum samples were collected on before injection,3 days after injection and 5 days after in-jection.Serum concentrations of ALT and AST were measured.TNF-αlpha of liver homogenate was detected by ELISA.The severity of liver damage was assessed by an approprite histopathological sco-ring system and apoptosis of parenchymal cells were assessed by TUNEL immunofluorescence assay. Results Compared with group control, the level of serum ALT and AST in group LPS and group GIK were significantly higher at 3 days after injection. The level of hepatic TNF-α,the hepatic damage score and the index of hepatic apoptosis in group LPS and group GIK were significantly higher on 3 days after injection and 5 days after injection.(P<0.05).Compared with group LPS,the level of hepatic TNF-αand the hepatocyte apoptosis rates decreased significantly in group GIK on 3 days after injection. The level of serum ALT and AST,hepatic TNF-α,the hepatic damage score and the hepatocyte apoptosis rates decreased significantly in group GIK at 5 days after injection(P <0.05).Conclusion Intraperitoneal injection of endotoxin can cause liver injury in rats, resulting in the liver hepatdysfunction and hepatocyte damage.GIK has protective effects on LPS induced liver injury in rats.

Objective To assess the radiographic and clinical outcomes of opening osteotomy supplemented with autologous anterior strut grafting and internal fixation through posterior-lateral approach in the treatment of unstable thoracolumbar fractures. MethodsFrom January 2008 through March 2010,a consecutive series of 13 thoracolumbar fractures were managed with opening osteotomy supplemented with autologous anterior strut grafting and internal fixation through posterior-lateral approach. They were 8 males and 5 females, aged from 20 to 58 years(median age,37.2 years). The fractures happened at T11 in 2 cases,at T12 in 3 cases,at L1 in 6 cases and at L2 in 2 cases. The autologous ilium was used for all the anterior strut grafts. The time from injury to surgery ranged from 21 days to 2 years (average,8 months). The mean heights of anterior and posterior borders of the vertebral body, the cobb angles and treatment-related complications were compared between preoperation and postoperation and the patients' satisfaction was assessed with the visual analogue scale(VAS).Statistical analyses were performed with the paried-sample t test. Results The patients were followed for 12 to 36 months (average,17 months). The latest follow-up radiographs were available for all patients. The mean heights of anterior and posterior borders of the vertebral body restored respectively from preoperative 21.02 ± 3.02 cm and 31.34 ± 5.18 cm to postoperative 31.88 ±3.46 cm and 33.75 ± 3.66 cm.The mean cobb angle was corrected from preoperative 28.46° ± 5.81° to postoperative 4.62° ± 3.48°.The mean VAS score decreased from preoperative 7.63 ± 0.72 to 1.39 ± 1.02 at the final follow-up.There were significant differences between preoperation and postoperation in the radiographic and clinical outcomes ( P < 0.05).No mortality or major complications occurred involving large blood vessels or abdominal viscera or internal organs. Conclusion The opening osteotomy supplemented with autologous anterior strut grafting and internal fixation through posterior-lateral approach is safe and effective for old unstable thoracolumbar fractures.
