1.Efficacy of precise hyperthermic intraperitoneal perfusion chemotherapy in the treatment of advanced pancreatic cancer

Objective To investigate the efficacy of precise hyperthermic intraperitoneal perfusion chemotherapy (HIPEC) in the treatment of advanced pancreatic cancer.Methods Thirty-six patients with advanced pancreatic cancer were analyzed retrospectively.Eighteen patients who received HIPEC combined with radiotherapy and chemotherapy were assigned as the treatment group and the other 18 patients who received chemotherapy and radiotherapy were assigned as the control group.Recent curative efficacy,Karnofsky Performance Status (KPS) score,postoperative complications and survivals between the two groups were analyzed,respectively.Results Significant differences were found between two groups in total short-term effective rate (P < 0.05).The total short-term effective rate of treatment group and control group were 66.67% (12/18) and 27.78% (5/18),respectively.The increment of KPS score of treatment group was significantly higher than that of control group (P < 0.05).There was no significant difference between two groups in postoperative complications (P > 0.05).The median overall survival time (OS) of treatment group was 11 months (7 ~ 31 months),and the median OS of control group was 7 months (4 ~ 18 months).The survival of the treatment group was longer than the control group (P < 0.05).Conclusions HIPEC treatment improved significantly the survival and life quality of advanced pancreatic cancer patients.With acceptable morbidity and mortality rates,HIPEC regime was an effective treatment modality for patients with advanced pancreatic cancer.
2.Analysis on Effect of Free-frying Granules of Traditional Chinese Medicine on Treatment of Chronic Prostatitis

This study was aimed to evaluate the efficacy and safety of free-frying granules of traditional Chinese medicine(TCM) on treatment of chronic prostatitis. A total of 120 chronic prostatitis patients that were diagnosed with the dampness-heat and blood-stasis syndrome and kidney-deficiency syndrome were divided into the TCM group and western medicine group by random digit table. Category II and ⅢA chronic prostatitis were treated with oral administration of Levofloxacin Mesylate Tablets (0.2 g, bid) and the category ⅢB chronic prostatitis was treated with oral administration of Terazosin Hydrochloride Tablets (2 mg, qd). In the TCM group, the dampness-heat and blood-stasis syndrome was treated with Tongluo Qingjie formula ( which was self-made formula containing Rhizoma Dioscoreae Hypoglaucae, Herba Patriniae, Semen Plantaginis, and etc.) and the kidney-deficiency syndrome was treated with Jiawei Wuzhi Yanzong pills (which containing Semen Cuscutae, Chinese Wolfberry, Radix Rehmanniae Preparata, Fructus Rubi and Schisandra chinensis, and etc.). Four weeks was one course of treatment. The results showed that the total effective rate of TCM group was better than that of western medicine group (P< 0.05). Effect of TCM group was obviously better than that of western medicine group in the improvement of prostatitis NIH-CPSI score, the quality of life and symptoms of the patients. TCM may increase lecithin corpuscle in prostatic fluid of ⅢA chronic prostatitis (P< 0.05). Meanwhile, no obvious toxiferous and side reactions were found. It was concluded that the free-frying granules of TCM have good efficacy and safety in the treatment of chronic prostatitis.
3.Flow cytometry analysis of unusual peripheral monocyte population

Objective To analyze a population of cells on the right lower lateral of monocyte population in forward scatter/side scatter(FSC/SSC)(X-axis/Y-axis)scatterplot of peripheral blood leucocyte by flow cytometry(FCM)and its influencing factors.Methods The type of cells were identified based on cluster of differentiation antigen(CD)by FCM.The impact of temperature,hemolysin concentration,and incubation time was evaluated.Blood lipid tests were performed to observe the relation between them by statistical methods.Results (1) Phenotypo of this population of cells on the right lower lateral of monocytes in FSc/SSC scatterplot is CD+45 CD+13 CD+14 CD3- CD-19 ,which was the same as monocyte cells:(2)The monocytes in FSC/SSC scatterplot shifted to left side after using haemolysin;(3)The monocytes showed less resistance to antihemolysin in 37℃ than that in 220C:There were more monocytes shifting to left side with the increase of haemolysis time:(4)The swarming ratio of monocytes in patients (31.5%,40/126) Was higher than it in normal controls (5.1%,5/98)(x2=22.74,P<0.01);(5)The levels of total serum cholesterol(TC),triglyeride(TG),low density lipoprotein cholesterol(LDL-C), apoprotein B100(Apo B100) in patients with swarming monocytes were lower than that in the patients without swarming monocytes,(P<0.05).There was no statistical significance between the two groups with respect to levels of total bilirubin(TBIL),albumin(Alb),hish density lipoprotein cholesterol(HDL-C),apoprotein A I(Apo A I),lipoprotein(Lpa).Conclusions Peripheral blood monocytes can be divided in two groups in FSC/SSC scatterplot when analyzed with FCM.The presence of this population of cell Was related to resistance to hemolysin.It can be influenced by haemolysis time and incubation temperature.Therefore,the effect of swarming monocytes and abnormal cell membrane should be taken into consideration when the markers and function of monocytes are detected by FCM.
4.The primary clinic application of hyperthermic intraperitoneal chemotherapy (HIPEC) guided by B ultrasound in the treatment of malignant ascites

Objective To investigate the measurement , feasibility and clinic effect of hyperthermic intraperitoneal chemotherapy (HIPEC) guided by B ultrasound in the treatment of malignant ascites from peritoneal carcinomatosis. Methods From July 2011 to June 2013, B ultrasound-guided approach was used to perform HIPEC on 36 patients affected by malignant ascites secondary to peritoneal carcinomatosis. Every patient underwent HIPEC for three times , by way of continuous circulatory perfusion into peritoneal cavity with saline at 400 ~ 600 mL/min and intraperitoneal perfusion with 5-FU mitomycin-C and cisplatin for 90 minutes with an inflow temperature of (43 ± 0.2)℃. These patients were followed up for a long term. Results Intraoperative course was uneventful in all cases. Complete clinical regression of ascites and related symptoms was achieved in all the 26 patients, partial regression achieved in 8 patients, and no curative effect achieved in 2 cases. The acquired total clinic effectiveness was 94.44%. No postoperative deaths and complication related to the procedure occurred in this study. The KPS grades of patients rose (P < 0.001), the level of tumor markers decreased, including CA199 (P < 0.001), CEA (P < 0.001), CA125 (P = 0.003). Conclusion HIPEC guided by B ultrasound appears to be a safe, feasible and effective procedure for the treatment of debilitating malignant ascites from unresectable peritoneal carcinomatosis , which would have a clinic good perspective in future.
5.Effects of serum cystatin C level on the occurrence and its long-term prognosis of contrast agent-induced acute kidney injury after coronary intervention in elderly patients

Objective:To investigate the effect of serum cystatin C level on the occurrence and its long-term prognosis of contrast agent-induced acute kidney injury(CI-AKI)after percutaneous coronary intervention(PCI)in elderly patients.Methods:A total of 848 elderly patients(≥60 years)undergoing PCI in our department between Mar 2015 and Dec 2017 were enrolled in a prospective cohort.The CI-AKI was defined as the increase of serum creatinine ≥44.2 μmol/L within 48-72 h after using iodine contrast agent or more than 25 % higher than base level within 48-72 h after PCI.A receiver operating characteristic curve was used to analyze the optimal cut-off value of Cystatin C for predicting CI-AKI after PCI.Patients were divided into 2 groups based on the optimal cut-off value of Cystatin C: the high Cystatin C group(Cystatin C ≥1.3 mg/L, n=178)and the control group(Cystatin C<1.3 mg/L, n=670). The differences in the incidence of CI-AKI after PCI and major adverse cardiac events(MACE)at 1 year follow-up were compared between the two groups.The Cox regression model was further used to analyze the predictors of the long-term prognosis after PCI.Results:Of 848 patients receiving PCI, the incidence of CI-AKI was 9.4%.The incidence of MACE at 1 year after PCI was higher in the high Cystatin C group than in the control group(15.7% vs.9.3%, χ2=6.524, P=0.011). Cox regression analysis confirmed that the high baseline level of Cystatin C was the most independent predictive factor for MACE at 1 year of follow-up( HR=16.244, P<0.001). Conclusions:The high baseline level of Cystatin C(≥1.3 mg/L)is an independent risk factor for CI-AKI and is also the most important predictor for the occurrence of long-term MACE in elderly patients undergoing PCI.
6.Effect of Wuwei Fuzheng Yijing Decoction on Sperm DNA Integrity in Male Infertility Patients

ObjectiveTo observe the effect of Wuwei Fuzheng Yijing decoction on sperm DNA integrity in male infertility patients. MethodOne hundred male infertility patients with sperm DNA damage (spleen and kidney Qi deficiency and blood stasis) that met the inclusion criteria were randomly divided into an observation group (Wuwei Fuzheng Yijing decoction) and a control group (L-carnitine oral solution), with 50 cases in each group. After 12 weeks of treatment, the DNA fragmentation index (DFI) and traditional Chinese medicine (TCM) syndrome score, progressive sperm motility (PR), sperm survival rate, and sperm concentration of the two groups before and after treatment were observed and compared. ResultThe total effective rate of the observation group was 89.80% (44/49) higher than 62.50% (30/48) of the control group (P<0.05). Compared with the conditions before treatment, the DFI and TCM syndrome score of the two groups were reduced after treatment, while the PR, sperm survival rate, and sperm concentration were increased (P<0.05), and the observation group was superior to the control group in terms of the above indexes (P<0.05). ConclusionWuwei Fuzheng Yijing decoction obviously repairs sperm DNA of male infertility patients and improves sperm quality, with high safety.
7.Clinical Efficacy of Fuzheng Huaji Longbi Decoction in Treating Benign Prostatic Hyperplasia with Syndrome of Healthy Qi Deficiency and Blood Stasis

ObjectiveTo explore the clinical efficacy and safety of Fuzheng Huaji Longbi decoction in treating benign prostatic hyperplasia (BPH) in the patients with the syndrome of healthy Qi deficiency and blood stasis. MethodA total of 94 BPH patients were randomized into control and observation groups, with 47 patients in each group. The control group was treated with doxazosin mesylate sustained-release tablets, and the observation group with Fuzheng Huaji Longbi decoction on the basis of the therapy in the control group. After eight weeks, the international prostate symptom score (IPSS), quality of life (QOL) score, residual urine volume (RUV), maximum urinary flow rate (Qmax), TCM syndrome score, TCM symptom score, electrocardiogram, and liver and kidney function were determined to evaluate the clinical efficacy and safety of the two groups. ResultAfter 8 weeks of treatment, the total response rate in the control group was 63.64% (28/44), which was lower than that (84.44%, 38/45) in the observation group (χ2=5.026, P<0.05). The clinical efficacy in the observation group was higher than that in the control group (Z=-2.17, P=0.030). The treatment in both groups decreased the IPSS, QOL score, RUV, and TCM syndrome scores and increased the Qmax (P<0.05). Moreover, the observation group had lower IPSS, QOL score, RUV, and TCM syndrome score (P<0.05) and higher Qmax than the control group after treatment (P<0.05). The treatment in the observation group decreased all the TCM symptom scores (P<0.05), while that in the control group only decreased the frequency of urination at night and the scores of dysuria, weak urine stream, and post-urinary drainage (P<0.05). After treatment, the observation group had lower frequency of urination at night and lower scores of mental fatigue, cold limbs, lower abdominal discomfort, and loose stool than the control group (P<0.05). No adverse events associated with the administration of Fuzheng Huaji Longbi decoction were observed during the treatment period. ConclusionFuzheng Huaji Longbi decoction is effective in treating BPH in the patients with the syndrome of healthy qi deficiency and blood stasis. It can relieve the clinical symptoms and improve the quality of life, being a safe and reliable choice for clinical application.
