1.Epidemiologicai analysis of plague in Qinghai province from 2001 to 2010

Objective To explore the plague epidemical trend of nearly a 10 years data in Qinghai province to provide basis for making the prevention and control measures. Method The regional distribution and time distribution of animal and human plague, monitoring and plague foci of survey data in Qinghai from 2001 to 2010 were analyzed with Excel software 2003. Results In Qinghai province, a total of 167 strains of Yersinia pestis were isolated from infected animals and insects in 10 years. Yersinia pestis was mainly distributed in Wulan,Delinha, Geermu, and Tianjun, along the Qinghai-Xizang railway. Human plague was occurred every year from 2001 to 2010 except 2002, 2007, 2008, and 2010. In the 10 years, there were 37 plague cases and 16 of these cases died, the mortality was 43.24%. The plague cases were mainly distributed in Nangqian, Qumalai, Chenduo,Zhiduo, Xinghai, Tongde, Tianjun, Wulan and Qilian. And these cases were found mostly in the period from May to October, especially in the period from August to October. Major clinical type of the plague cases was lung-type (62.16%,23/37). Conclusions The plague epidemic situation in Qinghai province is still severe, animal plague occurred year after year, and human plague outbreaks occasionally. Monitoring and early warning in the key areas should be strengthened, and the comprehensive measures of plague prevention and control should be carried out to reduce the incidence and prevalence of plague.
2.Reliability and validity of Burnout Questionnaire in occupational population.

OBJECTIVETo evaluate the reliability and validity of simple Burnout Questionnaire in occupational population.
METHODSA self-administered Burnout Questionnaire comprising 19 items was developed in light of Maslash Burnout Questionnaire and used for investigating 458 employees.
RESULTSCronbach's alpha between total items of Burnout Questionnaire and each factor fell in between 0.82 and 0.85 through the consistency test. All 19 items of Burnout Questionnaire were subjected to factor analysis, and three latent factors were identified, wherein 56.3% of total variance could be explained. According to the contents described in the items and Maslash burnout theory, they were emotional exhaustion, depersonalization, and personal accomplishment. According to the covariance and the variance analysis, there was significant difference in the integral of the type of work among the three subitems (P < 0.01); there was significant difference in the integral of depersonalization between two sexes (P < 0.01); There were significant difference in the integral of the personal achievement among different level of education (P < 0.01).
CONCLUSIONThe reliability and validity of Burnout Questionnaire is acceptable and can be used for assessing burnout in occupational population.

3.Hierarchical regression analysis for relationship between job stress and job burnout in Shanghai employees.

OBJECTIVETo identify related factors of job burnout in Shanghai employees.
METHODSFour hundred fifty-six employees in Shanghai were investigated in this study. Self-administered questionnaires were used to assess job burnout and job stress, based on Maslach Burnout Inventory and the Job Demand-Control model as well as Effort-Reward Imbalance Model. Hierarchical linear regression was employed to analyze the relationship of job burnout to personal characteristics and job stress.
RESULTSThe indexes of three dimensions of job burnout were emotional exhaustion 19.70 +/- 8.92, depersonalization 11.95 +/- 4.45 and reduced personal accomplishment 28.10 +/- 10.08. Job stress was found to be affected differently in three dimensions of job burnout. Job demand, effort and over-commitment had positive impact on emotional exhaustion. Job control had a negative association with emotional exhaustion. There were significant relationship between depersonalization and age, sex and education of employees. Job control, reward and over-commitment affected the index of depersonalization. Education level and social support increased personal accomplishment index.
CONCLUSIONIt is necessary to reduce job stress and care about personal characteristics in preventing job burnout.

4.Mechanical property analysis of PVA/HA composite hydrogel and its finite element simulation

Objective To study the effect from the content change in hydroxyapatite (HA) on mechanical properties of polyvinyl alcohol/hydroxyapatite(PVA/HA)composite hydrogel and its load bearing characteristics based on experiment and finite element simulation. Methods The compression and stress relaxation tests of PVA/HA composite hydrogel were performed on the UMT test machine. Through the simulation and experimental results, the load-bearing characteristics of PVA/HA composite hydrogel and the effect of HA on PVA/HA composite hydrogel performance were investigated. Results With HA content increasing, the compression modulus of PVA/HA composite hydrogel was first increased and then decreased, while its permeability coefficient was first decreased and then increased. When the HA content was 3%,the compression modulus of PVA/HA composite hy drogel reached maximum (1.25 MPa)and its permeability coefficient reached minimum(1.59×10-3 mm4•N-1•s-1). The fluid bearing proportion of PVA/HA composite hydrogel was first increased and then decreased with the loading time increasing, which presented a nonlinear change. The proportion of the fluid bearing was significantly increased after HA added into PVA / HA composite hydrogel. The stress relaxation rate was first increased and then decreased with HA content increasing. Under the same loaded displacement, PVA / HA composite hydrogel with 3% HA content could disperse more contact stress. Conclusions The internal bearing characteristics of PVA / HA composite hydrogel have a great impact on its mechanical properties. When the HA content is 3%, the PVA / HA composite hydrogel has the best mechanical properties, which are closer to those of the natural articular cartilage.
5.Scaling up the in-hospital hepatitis C virus care cascade in Taiwan

Background/Aims:
Obstacles exist in facilitating hepatitis C virus (HCV) care cascade. To increase timely and accurate diagnosis, disease awareness and accessibility, in-hospital HCV reflex testing followed by automatic appointments and a late call-back strategy (R.N.A. model) was applied. We aimed to compare the HCV treatment rate of patients treated with this strategy compared to those without.
Methods:
One hundred and twenty-five anti-HCV seropositive patients who adopted the R.N.A. model in 2020 and another 1,396 controls treated in 2019 were enrolled to compare the gaps in accurate HCV RNA diagnosis to final treatment allocation.
Results:
The HCV RNA testing rate was significantly higher in patients who received reflex testing than in those without reflex testing (100% vs. 84.8%, P<0.001). When patients were stratified according to the referring outpatient department, a significant improvement in the HCV RNA testing rate was particularly noted in patients from non-hepatology departments (100% vs. 23.3%, P<0.001). The treatment rate in HCV RNA seropositive patients was 83% (83/100) after the adoption of the R.N.A. model, among whom 96.1% and 73.9% of patients were from the hepatology and non-hepatology departments, respectively. Compared to subjects without R.N.A. model application, a significant improvement in the treatment rate was observed for patients from non-hepatology departments (73.9% vs. 27.8%, P=0.001). The application of the R.N.A. model significantly increased the in-hospital HCV treatment uptake from 6.4% to 73.9% for patients from non-hepatology departments (P<0.001).
Conclusions
The care cascade increased the treatment uptake and set up a model for enhancing in-hospital HCV elimination.

Background/Aims:
Direct‐acting antivirals (DAAs) have been approved for hepatitis C virus (HCV) treatment in patients with end-stage renal disease (ESRD) on hemodialysis. Nevertheless, the complicated comedications and their potential drug-drug interactions (DDIs) with DAAs might limit clinical practice in this special population.
Methods:
The number, class, and characteristics of comedications and their potential DDIs with five DAA regimens were analyzed among HCV-viremic patients from 23 hemodialysis centers in Taiwan.
Results:
Of 2,015 hemodialysis patients screened in 2019, 169 patients seropositive for HCV RNA were enrolled (mean age, 65.6 years; median duration of hemodialysis, 5.8 years). All patients received at least one comedication (median number, 6; mean class number, 3.4). The most common comedication classes were ESRD-associated medications (94.1%), cardiovascular drugs (69.8%) and antidiabetic drugs (43.2%). ESRD-associated medications were excluded from DDI analysis. Sofosbuvir/velpatasvir/voxilaprevir had the highest frequency of potential contraindicated DDIs (red, 5.6%), followed by glecaprevir/pibrentasvir (4.0%), sofosbuvir/ledipasvir (1.3%), sofosbuvir/velpatasvir (1.3%), and elbasvir/grazoprevir (0.3%). For potentially significant DDIs (orange, requiring close monitoring or dose adjustments), sofosbuvir/velpatasvir/voxilaprevir had the highest frequency (19.9%), followed by sofosbuvir/ledipasvir (18.2%), glecaprevir/pibrentasvir (12.6%), sofosbuvir/velpatasvir (12.6%), and elbasvir/grazoprevir (7.3%). Overall, lipid-lowering agents were the most common comedication class with red-category DDIs to all DAA regimens (n=62), followed by cardiovascular agents (n=15), and central nervous system agents (n=10).
Conclusions
HCV-viremic patients on hemodialysis had a very high prevalence of comedications with a broad spectrum, which had varied DDIs with currently available DAA regimens. Elbasvir/grazoprevir had the fewest potential DDIs, and sofosbuvir/velpatasvir/voxilaprevir had the most potential DDIs.
7.Mechanical bearing characteristics and finite element analysis on bovine knee articular cartilage

Objective To analyze the relationship between compression deformation and mechanical bearing characteristics of articular cartilage. Methods Indentation test was used to measure the compression displacement of bovine knee articular cartilage with different indenter diameters and under different loads. The fluid flow and bearing characteristics of the articular cartilage were simulated by the finite element model. Results The maximum relative error between simulated maximum compression displacement and experimental result was 1.73%. The elastic modulus and permeability coefficient of cartilage increased with indenter diameter increasing under the same load, while decreased with indentation load increasing at the same indenter diameter. Fluid flow was mainly in the internal cartilage when the load was exerted on cartilage. With the load sustaining, fluid flow was gradually moved to outside of the cartilage. Pore pressure, axial stress and radial stress on the cartilage surface then presented a nonlinear change due to the fluid flow. Conclusions Fluid flow, pore pressure and stress distribution on the cartilage surface have a great impact on its bearing characteristics, which varies greatly with different indenters and under different loads.

Objective Throush identify biochemical characteristics and virulence factors of 2 strains suspected Yersinia pestis(Y.pestis)isolated from the dead Marmota himalayana(M.himalayana)to confirm the nature epidemic focus in Dege County,Sichuan Province.Methods Y.pestis was analyzed by specific staining and shape,culturing characteristics,splitting-test by bacteriophage,test of biochemical characteristics and glycolysis ability,virulence factors,virulence,nutritional requirement,plasmid,genetic test and genetic type. Results The tested strains were Gram staining bacilus.The main biochemical characteristics were Arabinose(+)、 Rhamnose(-),Maltose(+),Melibiose(-),Glycerol(+),Denitrification(+).The virulence factors with FI+.VW+, Pgm+,Pst I+;and with the common 6.0×106,45.0×106,65.0×106 plasmids,also with the virulence-relative plasmid gene.Both their absolutely lethal dose(LD100)in mice were 50 bacteria.The nutritional requirement appeared which were depended on Phenylalanine and Methionine.With the Genomovar 5 genotype characteristics of M.himalayana plague foci of Qinghai-Tibet plateau.The difference between tested strains and Yersinia pseudotubercuosis on the 3 different culture medium was obvious.The tested strains had a Y.pestis' specific 3a fragment,Pst I and FI-Ag,at 22 ℃,the strains could be split by bacteriophage completely.Conclusions According to the diagnostic criteria of plague in China,the 2 suspected strains isolated from Dege County,Sichuan Province ale confirmed as Y.pestis.both with powerful virulenceand with the characteristics of the Y.pestis of M.himahtyana in Qinghai-Tibet plateau plague natural focus.

OBJECTIVETo study the pathogenic ecology characteristics of plague in Qinghai plateau.
METHODSApplied molecular biology techniques, conventional technologies and geographic information system (GIS) to study phenotypic traits, plasmid spectrum, genotype, infected host and media spectrum etc.of 952 Yersinia pestis strains in Qinghai plateau plague foci, which were separated from different host and media in different regions during 1954 to 2012.
RESULTSThe ecotypes of these strains were Qingzang plateau (91.49%, 871/952),Qilian mountain (6.41%, 61/952) and Microtus fuscus (1.26%, 12/952).83.6% (796/952) of these strains contained all the 4 virulence factors (Fr1, Pesticin1,Virulence antigen, and Pigmentation), 93.26% (367/392) were velogenic strains confirmed by virulence test.725 Yersinia pestis strains were separated from Qinghai plateau plague foci carried 9 kinds of plasmid, among which 713 strains from Marmot himalayan plague foci carried 9 kinds of plasmid, the Mr were 6×10(6), 7×10(6), 23×10(6), 27×10(6), 30×10(6), 45×10(6), 52×10(6), 65×10(6) and 92×10(6) respectively. 12 Yersinia pestis strains were separated from Microtus fuscus plague foci carried only 3 kinds of plasmid, the Mr were 6×10(6), 45×10(6), 65×10(6). Meanwhile, the strains carrying large plasmid (52×10(6), 65×10(6) and 92×10(6)) were only distributed in particular geographical location, which had the category property. The research also confirmed that 841 Yersinia pestis strains from two kinds of plague foci in Qinghai plateau had 11 genomovars. The strains of Marmot himalayan plague foci were given priority to genomovar 5 and 8, amounted to 611 strains, genomovar 8 accounted for 56.00% (471/841), genomovar 5 accounted for 23.07% (194/841). Besides, 3 new genomovars, including new 1(62 strains), new 2(52 strains), new 3(48 strains) were newly founded, and 12 strains of Microtus fuscus plague foci were genomovar 14.
CONCLUSIONThe main host and media of Qinghai plateau plague foci directly affected the spatial distribution regularities of plague epidemic and the pathogens characteristics, meanwhile the polymorphism of plague ecological geographic landscape leds to the complexity of Yersinia pestis' genotype.
