7.Tirofiban in percutaneous coronary intervention of patients with acute non-ST segment elevation myocardial infarct:safety and efficacy

Objective:To evaluate the efficacy and safety of tirofiban,a platelet glycoproteinⅡb/Ⅲa Inhibitor,in percutaneous coronary intervention(PCI)of patients with acute non-ST segment elevation myocardial infarct(NSTEMI).Methods:A total of 114 patients with acute NSTEMI were enrolled in the trial from Sep.2005 to Jan.2007;they were randomly divided into 2 groups:tirofiban group(n=57)and placebo group(n=57).Patients in tirofiban group were given tirofiban for 24 h after PCI.All patients were routinely given heparin,aspirin and clopidogrel before CPI.The composite occurrence of death,myocardial infarction(MI),need for target vessel revascularization(TVR)after PCI,and the adverse effects(hemorrhage and thrombocypenia)were compared between the 2 groups.Results:One(1.8%)patient had angina pectoris and the other(1.8%)developed subacute thrombus in control group within 24 h after PCl;there was no such event in the tirofiban group.Two(3.6%)patients developed angina pectoris and 2(3.6%) developed subacute thrombus within 30 days after PCI in control group;one patient(1.8%)in birofiban group developed angina pectoris and one patient in birofiban group developed subacute thrombus.Each group had one case(1.8%)of upper digestive tract bleeding during hospitalization.No intracranial hemorrhage,skin/ mucosa hemorrhage,thrombocytopenia,or-death occurred in the 2 groups.Intravenous tirofiban treatment reduced the composite occurrence of death of NSTEMI patients after PCI(P
8.Changes of Signal Transduction of Toll-Like Receptors in Children with Acute Idiopathic Thrombocytopenic Purpura

Objective To investigate the role of signal transduction of toll-like receptors(TLRs)in immunological pathogenesis in children with acute idiopathic thrombocytopenic purpura(ITP).Methods Thirty children with actue ITP and 30 age-matched healthy children were studied.Real-time fluorescent quantitative PCR was used to evaluate the levels of TLR 1-10 and signal transducing molecules,and cytokines associated with TLRs,such as IL-1?,granulocyte-macrophage colony-stimulating factor(GM-CSF),macrophage colony-stimulating factor(M-CSF)and IFN-?/? mRNA.Expressions of co-stimulatory molecules such as CD40,CD80 and CD86 in mo-nocyte/macrophage(MC)was analyzed by flow cytometry.Results Compared with healthy control group,the mRNA levels of TLR3,7,8 and 9 in ITP group were significantly up-regulated(Pa0.05).Transcription le-vels of MyD88-dependent and-independent pathway molecules such as MD-2,MyD88,IRAK-4,TRAF6,TAK1,TRIF,TRAM,TBK-1 and IFN-? were significantly up-regulated in acute ITP(Pa0.05).Conclusion Aberrant activation of toll-like receptors signaling may be one of the initiating factors of immune dysfunction in children with acute ITP.
9.Reduction and screw fixation of condyle sagittal fracture

Objective To investigate the technique and clinical effect in the surgical reduction and screw fixation of sagittal condylar fractures.Methods Open treatment was performed on 21 cases (23 condyles) with sagittal condylar fractures.The condylar fracture segment was reduced and fixed by screw (long screw or lag screw).The patients began the mandible movement exercise early after the operation,and follow-up was performed in the first half year after the operation.Results Faverable results were obtained in all cases.Slight malocclusion was observed in 3 cases accompanied with mandible fracture,but normal occlusion was almost reestablished after one week of intermaxillary traction.The CT examination indicated the ideal condition of reduction and fixation of these condylar fractures.During the follow-up,the mouth opening was normal,and mandible movement and function was satisfactory.Conclusions The technique of reduction and screw fixation in condyle sagittal fracture can obtain stable reduction and fixation.
10.Screening for Causative Genes Involved in Children with Minimal Change Nephritic Syndrome

Objective To screen for the causative genes involved in the occurrence and development of minimal changes nephritic syndrome(MCNS) and to furtherly assist the genetic diagnosis and treatment of MCNS.Methods Human genome U133 Array Set from Affymetrix Inc was used to evaluate gene expression patterns in peripheral blood mononuclear cells(PBMC) isolated from 7 children with primary MCNS and 7 age-matched health volunteers.Reverse transcription-polymerase chain reaction(RT-PCR) and real-time PCR were performed to identify the findings of gene chip.Results Of 33 000 genes detected,969 genes showed significant difference between children with(MCNS) and healthy volunteers;552 genes were up-regulated,while 417 genes down-regulated significantly.Findings from RT-PCR and real-time PCR were consistent with those of gene chip.Conclusions Gene chip of expression patterns is a powerful method to detect expression difference of genes correlated with MCNS.Occurrence and development of MCNS can be a complicated process that many correlative genes may participate in.
