1.Clinical and molecular-genetic features and prognosis of mitochondrial encephalopathy in children

Objective To explore the clinical and molecular-genetic features and prognosis of mitochondrial encephalopathy in children. Methods A total of 11 patients diagnosed with mitochondrial encephalopathy were collected from 2008 to 2013 in Chinese PLA General Hospital, Department of Pediatrics. The clinical manifestations, laboratory tests, muscle pathology and mitochondrial gene sequencing in patients were analyzed and followed-up. Results The onset age of the patients ranged from 6 months to 12 years with the duration of disease from 2 months to 3 years. Of the 11 patients, 6 cases were diagnosed with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes (MELAS), 5 cases with mitochondrial disorder induced by mitochondrial respiratory chain enzyme deifciency. Main manifestations included convulsions, vomiting, headache, mental retardation, paralysis, etc. 10 patients had elevated lactate, and 7 patients had elevated pyruvate;6 cases showed increased slow wave in EEG background;Cranial MRI showed the damage involved in bilateral basal ganglia in 2 cases, temporal and occipital lobes in 3 cases, , multiple lesions in 2 cases, frontal and parietal occipital lobes in one patient and hypothalamus in one patient;3 patients were underwent MRA examination, 2 cases were normal, the other showed less shunts in contralateral left artery in the brain;3 cases who underwent MRS showed lactate peak. 2 cases of children underwent skeletal muscle biopsy, and one case showed abnormal mitochondria accumulation. Mitochondrial respiratory chain complex (I~V) defects test showed complex IV defects in 2 cases, complex V defects in 1 case, and combined defects of complex I+III in 2 case. 5 cases were found with mitochondrial DNA mutations in different sites of white blood cells including T8993G, T8993C, A3243G and 11,777 mutations. Conclusions Clinical manifestation of mitochondrial encephalopathy in children is strongly heterogeneous. Laboratory tests, brain imaging, gene mutation and respiratory chain enzyme examination can help to make early diagnosis and treatment. These diseases have poor prognosis.
2.The effectiveness of Entecavir on serum TGF-β1 in patients with decompensated cirrhosis

Objective To study the the effectiveness of Entecavir on serum transforming growth factor-β1 (TGF-β1) in patients with decompensated cirrhosis .Methods Eighty patients with decompensated cirrhosis were randomly divided into two groups ,the control group(n=40 cases) and the observation group (n=40 cases).The pa-tients in the control group were treated through the conventional treatmeat ,while the patients in the observation group were treated through the conventional treatment plus entecavir .They were treated for 24 weeks.Liver function,HBV DNA and serum TGF-β1 were detected.Results ALT,Tbil and ALB were improved after treatment in both groups (t=6.724,13.306,3.064,4.172,2.924,3.172,all P<0.05).They were more significantly improved in the obser-vation group than that in the control group (t=2.469,2.160,3.064,all P<0.05).HBV-DNA negative rate in the observation group was higher these in the control group (χ2 =4.82,P<0.05).Serum TGF-β1 in both groups were decreased after treatment(t=6.842,8.062,all P<0.01).There was a significant difference between two groups (t=3.178,P<0.05).Conclusion Entecavir can decrease serum TGF-β1 in patients with decompensated cirrhosis .
3.Multivariate Logistic regression analysis of the compliance in elderly diabetes and countermeasures

Objective To analyze the causes of poor compliance of elderly patients with diabetes mellitus , and put forward corresponding countermeasures to control blood glucose level and delay the occurrence of complications .Methods A total of 113 elderly patients with type 2 diabetes mellitus aged 60 years and over were recruited in endocrinology department of our hospital.The self-made questionnaire was used to investigate the compliance and the factors that affected the compliance.Results There were 48 patients (42.48%) with good compliance and 65 patients (57.52%) with poor compliance.Univariate analysis showed that the level of education, monthly income, drug types, understanding of diabetes-related knowledge and whether the elderly living alone affected compliance ( P<0.05 ) .Multivariate Logistic regression analysis showed that patients with low education level, poor monthly income, medication types and living alone were the main factors that influence the compliance of elderly patients with diabetes mellitus (P<0.05).And the level of education, monthly income level and the compliance of diabetic medication was positively correlated, medication types and living alone and diabetes compliance were negatively correlated (P<0.05).Conclusion The low level of education, poor monthly income, medication types and living alone are the main factors that affect the compliance of elderly patients with diabetes.The social and family support can effectively improve the compliance of elder patients with diabetes.
4.Establishment of HSP90 overexpressing cell line and effects of HSP90 overexpressing on cell stress responses

AIM: To establish a HSP90 highly expressing cell line and study the effect of high level of HSP90 on cell stress response. METHODS: The recombined plasimid pSmycHSP, which contains the full length DNA coding for human HSP90?, was introduced into mouse fibroblast cell line NIH-3T3 by electroporation after being subcloned, purified and identified by limited enzyme digestion. Screened by G418, the positive clones were selected and identified by immunofluorescence, immunocytochemistry and Western-blotting. NIH-3T3 cells transfected with empty plasmid served as control, hyperthermia(44 ℃, 20 min, 40 min)was used to simulate oxidative stress. The activity of lactate dehydrogenase (LDH) in supernatant and damage of DNA were detected by automatic biochemistry analyzer and flow cytometer separately to analyze the effect of high-level HSP90 on cell membrane and DNA injuries under stress condition. RESULTS: The rising level of HSP90 was shown by immunofluorescence, immunocytochemistry and Western-blotting in HSP90 overexpressing cell line. There was no difference in the leakage of LDH between HSP90 overexpressing cell line and control, but the damage of DNA was more severe at 44 ℃for 20 min in HSP90 overexpressing cell line than control. Compared with control, the above indices were relieved at 44 ℃ for 40 min in HSP90 overexpressing cell line. CONCLUSION: The NIH-3T3 derived cell line, which stably expressed high level of HSP90, was established. The effect of high-level HSP90 on cells is complex at different intensity of stress, and the protection may be shown at more severe stress circumstance.

6.Comparison on Contents among Extracts from Loquat Leaf and Gruffs Using Alcohol as Solvent

OBJECTIVE:To further exploit the active components from Loquat leaf and its gruffs.METHODS:The chief components in loquat leaf and its gruffs were extracted by ultrasound extraction and Soxhlet extraction,respectively,the sepa?ration of ursolic acid and other constituents was performed by HPLC,and the relative amounts of which were determined and compared.RESULTS:Soxhlet extraction is superior to ultrasound extraction in the extraction of loquat leaf and8components in gruffs.Under the same extraction method,the content of ursolic acid in loquat leaves is equivalent to that of gruffs.Besides,the contents of6components in gruffs were above those in loquat leaves.CONCLUSION:The essential component-triterpenic acid in loquat leaves suffered no loss after extraction with water,the active components in gruffs can be further exploited.
7.Effect of Yangyinqingfei granula on immune function in mycoplasma pneumonia complicated with Epstein-Barr virus of children

Objective To investigate the effect of Yangyinqingfei granula on immune function in the treatment of mycoplasma pneumonia complicated with Epstein-Barr virus of children.Methods 108 mycoplasma pneumonia complicated with Epstein-Barr virus of children from our hospital were selected and randomly divided into two groups.The control group of 54 cases were treated by simple western medicine treatment and the experimental group of 54 cases were treated on the basis of the control group with Yangyinqingfei granula.The improvement of clinical symptoms, inflammation indexes and the levels of T lymphocyte subsets, clinical efficacy and complications were compared.Results The clinical efficacy in experimental group was 90.74%,which was significant higher than that in control group 75.93%(P <0.05).Compared with the control group,the remission duration of fever, cough and rales,the disappearance time of lung X-ray,the length of stay were shorter(P<0.05), the CRP,IL-6,TNF-αand IL-8 levels were lower ( P <0.05 ) , the CD3 +, CD4 +, CD4 +/CD8 + and NK levels were higher, the CD8 +level was lower ( P <0.05 ) . Conclusion Yangyinqingfei granula could reduce the inflammatory factors and improve immune function in the treatment of mycoplasma pneumonia complicated with Epstein Barr virus infection pneumonia in children.

The mammalian target of rapamycin (mTOR) disorders have a close relationship with autoimmune disease,tuberous sclerosis (TfSC),cancer,obesity and senescence.mTOR inhibitors are wildly used in pediatric renal transplantation and TSC.Growth is an important indicator of children health and safety evaluation of pediatric drugs must include impact on growth.No evidence showed mTOR inhibitors had side effect on growth in children.
9.Mycoplasma Culture and Analysis of Susceptibility Test

OBJECTIVE To compare the detection rates of Ureaplasma urealyticum(Uu),by 4 kinds rits and find out the best one.METHODS Using four kinds of mycoplasma detection kits,namely are French mycoplasmaIST2,Zhuhai Livzon′s ICS,Dark horse′s IES and Shanghai Its Prachanda,under the same conditions,the mycoplasma of the Clinical specimens for 260 cases were tested.RESULTS The detection rate of mycoplasma(Uu) for 48 hours by four kinds of brands kits about mycoplasma liquid culture was as follows 45.4% for French MycoplasmaIST2,36.2% for Zhuhai Livzon′s ICS,37.7% for Dark horse′s IES and 36.8% for Shanghai Its prachanda.The results of susceptibility test: the reagent of IST2 had.the lowest resistance to josamycin,doxycycline,clarithromycin,ofloxacin and azithromycin,and had the better results than the other three kinds of reagents.Mycoplasma reagents form different companies had significant differences in resistance rates to five kinds of antibiotics.CONCLUSIONS The results suggest that when we cultivate clinical mycoplasma should be careful to select of the mycoplasma liquid culture identification kit.
10.Nosocomial Pneumonia and Its Clinical Analysis

OBJECTIVE To investigate the clinical and etiology characteristics of nosocomial pneumonia. METHODS Patients with nosocomial pneumonia from Jan 2003 to Nov 2003 were chosen to be analyzed. RESULTS There were 241 patients with nosocomial pneumonia,the incidence rate of the nosocomial pneumonia in nosocomial infection patients was 41.77%,and the incidence rate of males was much higher than females(1.87∶1),and it was 43.15% in old person among patients with nosocomial pneumonia which was much higher than others,58.72% patients began to have the symptoms in the first 20 days from the day they accessed the hospital,the incidence rate of patients who had been treated by one antibiotic or two antibiotics combination,Immunosuppressors or glucocorticoids drugs were all higher than 50%,the incidence rate of patients who treated by radiotherapy,chemotherapy,thorax-abdomen operations,tracheotomy and ventilator were also quite high,the main pathogens were Gram-negative bacilli(63.95%). CONCLUSIONS We should make heavier prevention and treatment measures in front of target population and their high risk factors and the related rings,so that we can get the real effects.
