Objective To investigate the expression of basic fibroblast growth factor(bFGF) and proapoptotic gene Bax,Bcl-2 in amniochorion and their relationship with premature rupture of membranes(PROM).Methods Immunohistochemical stain was used to detect the expression of bFGF,Bax and Bcl-2 in amniochorion collected from 30 pregnant women with PROM and 30 normal pregnant women without PROM.Results The expression of bFGF and Bax were located in the plasma of all types of cells including trophoblast, epithelial, mesenchymal and fibroblast, but the expression of Bcl-2 was only observed in the chorionic syncytiotrophoblast;the positive average integral calculus oculus dehtor of bFGF in the PROM group was significantly lower than that of the normal group(0.051?0.012 vs 0.098?0.027,P0.05).Negative correlation was found between the bFGF and Bax expression in the PROM group(r=-0.616,P0.05). Conclusions The reduced expressions of bFGF and excessive apoptosis in the amnio-chorion play an important role in PROM.

Objective To evaluate the semen quality and sperm morphology in the patients with different grades of varicocele. Methods Semen from 121 patients with varicocele which were divided into three groups, gradel, grade Ⅱ and grade Ⅲ, were studied and those of 23 normal male were taken as the control. Semen analysis was performed with the methods described in the WHO manual and sperm morphology was evaluated by WHO cri-teria. Results A significant reduction of semen quality and sperm morphology and an increase of small oval head, tapering head and amorphous head sperm were found in patients with different grades of varicocele compa-ring with those of the control. There was no difference in routine analysis between different groups. A reducetion of normal morphology percentage in grade Ⅲ were found comparing with grade Ⅱ (P<0.01). An increase of a-morphous head sperm in grade Ⅲ was found comparing with that of sperm in grade Ⅱ (P <0.01). Canclusions The routine semen analysis can not distinguish seminal damage between different grades of varicocele, but the sperm morphology can reflect the sperm state. Therefore, the patients with varicocele should not only get routine semen analysis but also check the sperm morphology.

Objective To evaluate the efficacy and safety of tislelizumab combined with chemotherapy in the treatment of urothelial carcinoma in the real word. Methods We enrolled 32 patients with urothelial carcinoma who were treated with tislelizumab and chemotherapy (gemcitabine/cisplatin or paclitaxel). The incidence of treatment-related adverse reactions during treatment and the efficacy evaluation were statistically analyzed. Results All patients were divided into two groups: 15 patients in the tislelizumab combined with paclitaxel group and 17 patients in the tislelizumab combined with GC group. Among 24 efficacy-evaluable patients, the ORR was 54.2% and the DCR was 83.3%. The ORR were 50.0% and 58.3%, and the DCR were 75.0% and 91.7% in the tislelizumab combined with paclitaxel group and the tislelizumab combined with GC group respectively. Common treatment-related adverse reactions included anemia (56.3%), loss of appetite (53.1%) and skin pruritus (50.0%). The grade 3-4 treatment-related adverse events occurred in 21.8% of patients. Common immune-related adverse reactions included skin toxicity (53.1%) and immune colitis (9.4%). Conclusion Tislelizumab combined with chemotherapy on urothelial cancer has significant curative effect, safety and controllability, but attention should be paid to immune-related adverse reactions.

Objective To investigate the influence of the speed of body mass growth in gestational diabetes mellitus(GDM)patients on the growth and development of neonatal body. Methods All of the one hundred and thirty-three pregnant women with GDM diagnosed by OGTT and hospitalized in Suining Cental Hospital for delivery were involved in the study.heir fasting plasma glucose, total cholesterol and HbAlc during a week before delivery and the speed of body mass growth at 3 weeks before childbirth were recorded.According to the body length at 1 week after delivery and body quality standards, all infants were divided into the good group and the poor group. Then, independent samples t test analysis was applied to analyze the effect of the fasting plasma glucose, total cholesterol,HbAlc and the speed of body mass growth in mother on the growth and development of neonatal body in two groups. The influence of factors which has statistically significant on neonatal body was analyzed by Multiple factors Logistic regression analysis and the association between those factors and the growth and development of neonatal body was showed on scatter diagram.Meanwhile,ROC curve was used to evaluate The effect degree of various factors on the growth and development of newborns and their best diagnostic points were calculated. Results The fasting plasma glucose((7.44 ± 2.27)mmol/L),total cholesterol((6.05±1.57)mmol/L),HbAlc((5.18±0.53)%)and the speed of body mass growth at 1st week ((0.35±0.20)kg/w),2nd week((0.28±0.21)kg/w)and 3rd week((0.36±0.18)kg/w)before delivery in good group were lower than those in the poor group((8.61±3.25)mmol/L,(6.78±1.84)mmol/L,(5.62 ±0.76)%,(0.71± 0.16)kg/w,(0.84 ± 0.11)kg/w,(0.76 ± 0.13)kg/w))(t=2.222,2.413,5.575, 11.617,20.183,14.321,P<0.05).Multivariate logistic regression analysis showed that the content of HbAlc (OR=2.391),the speed of body mass growth at 2nd week before delivery(OR=3.045)and the speed of body mass growth at 1st week before delivery(OR=4.081)had significant effect on the growth and development of neonatal body and the speed of body mass growth at 1st week before delivery had the greatest influence(P<0.05); There were negative correlation among HbAlc,neonatal length and the increase in body mass(r=-0.490,-0.481,P<0.05); at 2nd week before delivery,HbAlc,neonatal length and the increase in body mass were negatively correlated(r=-0.353,-0.396,P,<0.05); at 1st week before delivery,HbAlc,neonatal length and the increase in body mass were negatively correlated(r=-0.495,-0.474,P,<0.05).At the same time,ROC analysis showed that the AUC of HbAlc was 0.566 while the speed of body mass growth at 1st week and 2nd week before delivery were 0.723 and 0.796,respectively and the AUC of the speed of body mass growth at 1st week before delivery was the highest. Conclusion The speed of body mass growth at 1st week before delivery can slow down the increase of neonatal length and body mass, and can be used as a therapeutic target for the clinical control of neonatal dysplasia in GDM patients.
