1.Mycophenolate mofetil combined with low dose of hormone and lamivudine in the treatment of hepatitis B virus associated glomerulonephritis

Objective To assess the efficacy of mycophenolate mofetil (MMF) combined with low dose of hormone and lamivudine in the treatment of hepatitis B virus associated glomerulonephritis (HBV-GN).Methods The clinical data of 49 HBV-GN patients diagnosed by renal pathology was reviewed.They were treated with MMF( ≥ 12 years old,0.75 g,two times a day; < 12 years old,0.4 g/m2,two times a day),low dose of hormone [0.5 mg/(kg·d) ],lamivudine( ≥ 12 years old,100 mg/d; < 12 years old,3 mg/kg,two times a day).Results Among of 49 HBV-GN patients,clinical cure rate was 71.4%(35/49),the total effective rate was 81.6%(40/49),85.7%(42/49) patients' HBV DNA level decreased from 5.43 ×104 copies/ml to < 1000 copies/ml.The complete remission rate was 88.0% (22/25),the partial remission rate was 8.0% (2/25),the inefficiency was 4.0% (1/25) in membranous nephropathy (MN);the complete remission rate was 44.4% (4/9),the inefficiency was 55.6% (5/9) in mesangial proliferative glomerulone phritis (MsPGN) ; the complete remission rate was 70.0%(7/10); the partial remission rate was 30.0%(3/10)in membrane proliferative glomerulone phritis (MPGN) ;the complete remission rate was 40.0% (2/5),the inefficiency was 60.0%(3/5) in focal segmental glomerulosclerosts (FSGS).There was significant difference among the different pathological type (P<0.05).There were less adverse reactions.Conclusions MMF combined with low dose hormone and lamivudine is safe and effective in treating HBV-GN.The efficacy and pathological type has some relationship,MN has the best response,MPGN and MsPGN are second,FSGS is poor.
2.Influence of Laparoscopic Uterine Artery Ligation on Ovarian Function

Objective To evaluate the influence of laparoscopic uterine artery ligation on ovarian function. Methods In this retrospective study ,46 patients with laparoscopic myomectomy were selected and randomly divided into Ligation group and Non-Ligation group. The serum concentrations of follicular stimulating hormone (FSH), luteinizing hormone ( LH), estrogen ( E_2) were measured be-fore treatment and 1 ,3 ,6,12 months after treatment. Ovulating functions of ovary were monitored. All results were compared between two groups. Results All patients ovulated after 6 months. There were no significant differences between two groups in the levels of FSH, LH and E_2,.before and after treatment(P>0.05). Conclusions Laparoscopic uterine artery ligation do not affect ovarian function of pa-tients with uterine leiomyoma.
3.Association of Epstein-Barr virus with lymphoepithelioma-like carcinoma of the parotid gland

Purpose To investigate the relationship between Epstein-Barr virus (EBV) and lymphoepithelioma-like carcinoma (LELC) of the parotid gland and detect the gene expression products of EBV harbouring in LELC cells. Methods Thirty-two parotid LELCs were collected from the Departments of Pathology, Sun Yat-sen University of Medical Sciences, Guangzhou, China during the period of January 1986 and December 1995. All the 32 formalin-fixed paraffin-embedded blocks had been consecutively re-sectioned again. Immunohistochemical and in situ nucleic acid hybridization methods for detection of EBV gene encoded products were performed. Results (1) 32 LELCs were found out of 125 parotid gland carcinomas, the frequency was 25.6% (32/125). (2) All of the 32 specimens contained a variable number of EBNA-1 and EBERs positive neoplastic cells. (3) Twenty-seven out of 32 specimens (27/32, 84.4%) had a portion of carcinoma cells expressing LMP-1. (4) No ZEBRA positive cell could be found. (5) EA-D, VCA and MA positivity rates for these 32 parotid LELCs reached to 71.9%(23/32), 68.8%(22/32), and 12.5%(4/32), respectively. Conclusions (1) The parotid gland LELC is frequently to be seen in Guangzhou locale of China, where is a high-incidence area of nasopharyngeal carcinoma (NPC). The parotid gland LELC and NPC are co-prevalent in Guangzhou locale. (2) This disease is also consistently associated with EBV infection. (3) The EBV infection of the parotid gland LELCs is essentially the type of latency Ⅱ, expressing EBNA-1, EBERs and LMP-1. (4) The latent infected EBV harbouring in LELC cells could in part be switched over to lytic cycle, producing EA-D, VCA or/and MA.
4.Video-assisted thoracoscopic surgery in the treatment of esophageal leiomyoma: a report of 39 cases.

OBJECTIVETo investigate the application and efficacy of video-assisted thoracoscopic surgery in the treatment of esophageal leiomyoma.
METHODSClinical data of 39 patients with esophageal leiomyoma from December 2002 to November 2008 treated by video-assisted thoracoscopic surgery were reviewed retrospectively.
RESULTSVideo-assisted thoracoscopic leiomyoma enucleations were performed in 38 patients, and one patient was converted to thoracotomy to repair the esophageal mucosa because of mucosa rupture during the operation. Thirty-six patients were treated through the right chest and the other 3 cases through the left. The operation time ranged from 1 to 5 (1.2+/-0.6) h. The blood loss ranged from 30 to 100 (50+/-8.3) ml. The time of chest tube placement ranged from 1 to 4 (1.5+/-0.7) d. The drainage amount was 100 to 500 (200+/-101) ml. The hospital stay ranged from 3 to 10 (3.6+/-1.2) d. The maximal diameter of the tumor ranged from 0.8 to 6.0 (3.0+/-0.7) cm. There were no surgery-related complications or deaths except the case with esophageal mucosa rupture. There were no uncomfortable complains and tumor recurrence within the follow-up period of 1 to 72 months after operation.
CONCLUSIONVideo-assisted thoracoscopic leiomyoma enucleation is a safe and effective approach for esophageal leiomyoma.

5.Combination of tolterodine and tamsulosin for benign prostatic hyperplasia.

OBJECTIVEOveractive bladder may coexist with bladder outlet obstruction induced by benign prostatic hyperplasia (BPH). This study aimed to evaluate the efficacy of the combined use of tolterodine and tamsulosin in the treatment of BPH accompanied by overactive bladder.
METHODSWe selected 53 cases of clinically diagnosed BPH, and randomly assigned them to a tamsulosin group (n = 25) to receive 0.2 mg of tamsulosin once a day and a tamsulosin + tolterodine group (n = 28) to be treated with 0.2 mg of tamsulosin once a day plus 2 mg of tolterodine twice a day, both for 12 weeks. Before and after the treatment, we obtained the International Prostate Symptoms Score (IPSS), quality of life (QOL) score and Qmax, and recorded the adverse events.
RESULTSAll the patients accomplished the 12-week treatment. The tamsulosin group showed a significant decrease in IPSS and QOL from 21.50 +/- 5.42 and 4.58 +/- 0.94 before the treatment to 14.80 +/- 4.21 and 2.78 +/- 0.91 after it (P < 0.05), but a significant increase in Qmax from (12.20 +/- 6.60) ml/s to (16.40 +/- 5.13) ml/s (P < 0.05). In the tamsulosin + tolterodine group, IPSS and QOL were decreased from 20.90 +/- 5.15 and 4.61 +/- 0.86 at the baseline to 14.90 +/- 5.32 and 2.12 +/- 0.87 after the medication (P < 0.05), Qmax was increased from (13.30 +/- 7.80) ml/s to (16.70 +/- 6.32) ml/s (P < 0.05), and the score on the urinary storage phase symptoms was reduced from 10.12 +/- 3.10 to 4.77 +/- 0.75 (P < 0.05).
CONCLUSIONTamsulosin could quickly relieve BPH-induced lower urinary tract symptoms (LUTS) , while the combined use of tolterodine and tamsulosin could even better alleviate the LUTS and improve the QOL of BPH patients.

6.Posttranscriptional induction of p21Waf1 mediated by ectopic p16INK4 in human diploid fibroblast.

BACKGROUNDBoth p16(INK4) and p21(Waf1) are tumor suppressors with similar biological functions in the regulation of cellular senescence. Previous reports showed that p16(INK4) could be activated by p21(Waf1) through transcriptional factor Sp1 in HeLa cells. This study was undertaken to determine the effects of p16(INK4) on the expression and functions of p21(Waf1).
METHODSHuman diploid fibroblast 2BS cells were stably transfected with sense (2BS/p16(INK4)), antisense p16(INK4) (2BS/asp16(INK4)) or empty vector (2BS/neo). Then they were assayed by reverse-transcription polymerase chain reaction (RT-PCR), fluorescence activated cell sorting (FACS) and Western blot.
RESULTS2BS/p16(INK4) cells exhibited cell cycle arrest in both G1 and G2/M phases. Endogenous p21(Waf1) protein levels increased twofold in the 2BS/p16(INK4) cells, but not decreased in the 2BS/asp16(INK4) cells. p21(Waf1) mRNA levels were not affected in neither 2BS/p16(INK4) nor 2BS/asp16(INK4) cells.
CONCLUSIONp16(INK4) may play an important role in the regulation of cellular senescence by modulating the p21(Waf1) protein level via the posttranscriptional mechanism.

9.Morphological observation and changes of hydroxyproline content in hypertrophic scar of rabbits

Objective To establish animal model for hypertrophic scar and study the characters of its morphology and collagen metabolism. Methods A total of 64 round wounds (diameter of 6 mm each) with total skin loss were made on the ventral side of rabbit ear using a trephine. Morphology and collagen metabolism of scar wounds were studied at 14,21,35,70 and 98 days after operation, respectively. Results There were 76% elevated scars developed (45/59 wounds) on the ventral side of rabbit ear at 21 days and 46% elevated scars disappeared (11/24) at 98 days after operation. There were numerous fibroblast proliferation and whorl-arranged collagen fibers at 21 and 35 days. The number of fibroblast decreased, but irregular-arranged fibers still presented in the elevated scars at 70 and 98 days after operation. Hydroxyproline content in elevated scars at 21 days was higher than that in normal skin (P<0.05), and at 35 days was 3 times as that in normal skin and at 98 days was also markedly higher than that in normal skin (P<0.05). Conclusion Excessive deposition of collagen is a characteristic of hypertrophic scar in rabbits. The conversion of normal scarring to hypertrophic scarring in rabbits occurs at 14~21 days after operation. Both development and regression of hypertrophic scar in rabbit are quicker than that in human.
