1.Therapeutic Effect of milrinone in Treatment of Refractory Heart Failure Patients

Objective:To evaluate the effect of milrinone in treating refractory heart failure.Methods:5mg of milrinone in 250ml of 5% Glucose or 0.9% NaCl Solution were given daily by intravenous drip to 56 cases of refractory heart failure for a period of consecutive 7 days.The symptoms and signs were observed before and after the treatment,heart rate,blood pressure and heart function examined by Doppler echocardiography.Results:a.The total effective rate was 85.7%.b.The left ventricular ejection fraction (LVEF) and fraction shortening (FS) were increased significantly (P<0.01) after treatment.The rate of filling velocity in early diastole (E) over that in late diastole (A) was increased significantly (P<0.05).Conclusion:Milrinone has good effect for treating refractory heart failure.It improves not only the cardiac systolic function but also the diastolic function.
2.Adherence activity of periodontal pathogens and cariogenic bacteria to hydroxyapatite disks

Pg.The adherence activity of periodontal pathogens to HA was enhanced by Ss and Av.When cultivated with Sm,the adherence activity of Pg was reduced.Periodontal pathogens had no effect on the adherence activity of Sm and Av,and Ss excepted. Conclusion The adherence activity of periodontal pathogens is lower than that of cariogenic bacteria.Periodontal pathogens can utilize early-attached bacteria to become those predominant bacteria in periodontal ecosystem.
3.The report of success sequential therapy for ectopic pregnancy with fetal cardiac activity

Objective To investigate the effects and safety of sequential treatments with methotrexate and mifepristone for ectopic pregnancy with fetal cardiac activity.Methods 4 cases of ectopic pregnancy with fetal car- diac activity in our hospital were given by sequential therapy with methotrexate and mifepristone.Serum?-HCG,liv- er function and renal function,blood routine and gastrointestinal response were observed.Results 4 cases of ectopic pregnancy with fetal cardiac activity with 1~4 periods of sequential treatments were cured.Except light gastroin- testinal response,and one had slight rise of serum ALT level and AST level,no one had rnyelosuppression and heavy hepatic injury.Conclusion The sequential therapy with methotrexate and mifepristone is an effective and safe method for the treatment to ectopic pregnancy with fetal cardiac activity.
4.Experimental Study on Effects of Qingxiang Concentrated Pill on Mammary Gland Hyperplasia in Rats

Objective To observe the therapeutic effect of Qingxiang Concentrated Pill (QCP) on mammary gland hyperplasia (MGH) in rats. Methods Injection of estradiol were given to establish rat models of MGH and QCP was given QCP at the same time. Pathologic changes of mammary gland in rats were observed under light microscope. Changes of breast diameter, mammary gland volume and weight were measured; serum sex hormones levels, SOD activity and MDA content were also estimated. Results QCP could decrease the increased breast diameter, mammary gland volume and weight, reduce the numbers of mammary gland lobules and mammary acini and the diameter of acinar cavities. It could also decrease eatradiol level and MDA content in serum, inhibit the decrease of the coefficient of thymus and increase the serum progesterone level. Conclusion QCP can regulate sex hormone levels, inhibit lipid peroxidation and relieve the pathologic changes of mammary gland in MGH rats.
5.Targeted monitoring on ventilator-associated events

Objective To monitor ventilator-associated event (VAE) for the first time in an intensive care unit (ICU) in China,understand the applicability,incidence,and clinical significance of VAE in China.Methods Targeted monitoring on VAE was performed among patients ≥18 years and with mechanical ventilation (MV)>2 days in the ICU of a hospital between January 2014 and September 2015,incidence of VAE was calculated,and patients were grouped according to whether or not they had VAE,prognostic factors were analyzed statistically.Results A total of 1 004 patients were monitored,the total hospital stay was 13 795 days in patients who used ventilator,307 (30.58%) cases of VAE occurred,incidence of VAE per 1 000 ventilator-days was 22.25.Univariate analysis showed that patients with VAE had longer length of ICU stay and MV,and higher mortality rate than patients without VAE when they moved out of ICU (all P<0.05).Multivariate logistic regression analysis showed that VAE was independent risk factor for length of ICU stay,duration of MV,as well as mortality when patients moved out of ICU(all P<0.05).Conclusion Judgment of VAE is based on MV parameters,it is more objective and accurate.There is a high incidence of VAE among ICU patients,it may lead to poor clinical outcomes,and has good values for the targeted monitoring on ICU patients in large comprehensive hospitals of China.
6.Association of Epstein-Barr virus with lymphoepithelioma-like carcinoma of the parotid gland

Purpose To investigate the relationship between Epstein-Barr virus (EBV) and lymphoepithelioma-like carcinoma (LELC) of the parotid gland and detect the gene expression products of EBV harbouring in LELC cells. Methods Thirty-two parotid LELCs were collected from the Departments of Pathology, Sun Yat-sen University of Medical Sciences, Guangzhou, China during the period of January 1986 and December 1995. All the 32 formalin-fixed paraffin-embedded blocks had been consecutively re-sectioned again. Immunohistochemical and in situ nucleic acid hybridization methods for detection of EBV gene encoded products were performed. Results (1) 32 LELCs were found out of 125 parotid gland carcinomas, the frequency was 25.6% (32/125). (2) All of the 32 specimens contained a variable number of EBNA-1 and EBERs positive neoplastic cells. (3) Twenty-seven out of 32 specimens (27/32, 84.4%) had a portion of carcinoma cells expressing LMP-1. (4) No ZEBRA positive cell could be found. (5) EA-D, VCA and MA positivity rates for these 32 parotid LELCs reached to 71.9%(23/32), 68.8%(22/32), and 12.5%(4/32), respectively. Conclusions (1) The parotid gland LELC is frequently to be seen in Guangzhou locale of China, where is a high-incidence area of nasopharyngeal carcinoma (NPC). The parotid gland LELC and NPC are co-prevalent in Guangzhou locale. (2) This disease is also consistently associated with EBV infection. (3) The EBV infection of the parotid gland LELCs is essentially the type of latency Ⅱ, expressing EBNA-1, EBERs and LMP-1. (4) The latent infected EBV harbouring in LELC cells could in part be switched over to lytic cycle, producing EA-D, VCA or/and MA.
7.Combination chemotherapy of gemcitabine and cisplatin by double way plus implantation of radioactive seed 125I in treating stage Ⅲ non-small cell lung cancer

Objective To assess the therapeutic effect of combination chemotherapy of gemcitabine and cisplatin by double way plus implantation of radioactive seed 125I implantation in treating stage Ⅲ non-small cell lung cancer. Methods Sixty cases with stage Ⅲ non-small cell lung cancer were randomly divided into two groups with random number table. In group A (in interventional treatment group, n = 30),the gemcitabine 1000 mg/m2 and one third of the cisplatin 100 mg/m2 was given using seldinger technique for transcatheter bronchial arterial infusion chemotherapy on day 1. Two-thirds of the cisplatin 100 mg/m2 was infused in veins on day 2 and 3. The gemcitabine 1000 mg/m2 was infused in veins on day 8, 21 days for a period. In group B (interventional - 125I groups), the method of combination chemotherapy of gemcitabine and cisplatin was the same as in Group A. After ten days of arterial perfusion, 125I seeds were implantated, 21 days for a period. All patients received at least 2 cycles. The imaging evaluation of patients after treatment standards included complete remission (CR), partial remission (PR), stable (SD),progressive disease (PD), effective rate (CR + PR)/30 and clinical benefit rate (CR + PR + SD)/30.Non-parametric rank sum test was used to compare short-term effect of the two groups treatment of two cycles.x2 test was used to compare year survival, Kaplan-Meier method was used to calculate median survival,log-rank test method was used to difference between the groups. Results In group A, there were 17 PR,9SD and 4 PD. The overall response rate was 56. 7% (17/30) and clinical beneficial rate was 86. 7% (26/30). In Group B, there were 2 CR, 21 PR, 7 SD. The overall response rate was 76.7% (23/30) and clinical beneficial rate was 100% (30/30). There was significant difference between the two groups (P =0. 036). In group A, the 1 year survival rate was 46. 7% (14/30) and the 2 year survival rate was 36. 7%(11/30), median survival time (MST) was 10 months . In group B, the 1 year survival rate was 76. 7%(23/30) and the 2 year survival rate was 63. 3% (19/30) , median survival time (MST) was 27 months.There was a significant difference between two group in 1 year survival rate (P = 0. 017), 2 year survival rate (P = 0. 039) and median survival time (P = 0. 006). Conclusion The treatment effects of Ⅲ stage non-small cell lung cancer by gemcitabine and cisplatin combination chemotherapy with double way plus radioactive seed 125I implantation was better than gemcitabine and cisplatin combination chemotherapy with double way.
8.Preliminary study of the transcatheter arterial chemoembolization in combination of percutaneous injection of chemoembolization agent intra-portal vein tumor thrombosis in treatment of primary hepatic carcinoma accompanied by portal vein tumor thrombosis

Objective To assess the therapeutic outcomes of transcatheter arterial chemoembolization combined with percutaneous injection of chemoembolization agent intra-portal vein tumor thrombosis for primary hepatic carcinoma accompanied by portal vein tumor thrombus. Methods Thirty patients with primary hepatic carcinoma accompanied by portal vein tumor thrombosis of type Ⅱ and type Ⅲ were randomly divided into two groups. The Child-Pugh ratings (class A and B) of group A and B were 9 vs 9 (class A) and 5 vs 7 (class B) respectively (χ~2 = 0.201, P > 0.05). The constitution of Type Ⅱ and type Ⅲ portal vein tumor thrombus in group A and B were 8 vs 9 and 6 vs 7 respectively (χ~2 =0.002, P>0.05). The median values of ALT, TBIL, ALB and AFP in group A and B were 58.7U/L vs 70.5 U/L (W=191.5, P>0.05), 21.4 μmol/L vs 21.7μmol/L (W=203, P>0.05), 35.3 g/L vs 37.5 g/L (W = 214, P > 0.05) and 680 μg/L vs 873 μg/L (W = 179. 00, P > 0.05) respectively. Group A was treated with transcatheter arterial chemoembolization (TACE) using emulsion made up of adriamycin, cisplatin, mitomycin and ultraliquidlipiodol plus percutaneous injection of chemoembolization agent intra-portal vein tumor thrombosis using emulsion consisted of cisplatin and ultraliquidlipiadol, while group B was treated with TACE only as a control group. Survival analyses were performed via the Kaplan-Meier test in SPSS11.5 with the log-rank tests with an threshold of 0.05. Results The 3, 6 and 12 months survival cases of group A and B were 11 vs 10, 10 vs 3, and 7 vs 0 respectively. The median survival time of group A and group B were 14.0 months and 4.0 months respectively. The difference of the two groups was significantly (χ~2 =11.728, P<0.01). There was no severe side-effect related to therapy in both groups. Conclusion Comparing with the control group, TACE combined with percutaneous injection of chemoembolization agent intra-portal vein tumor thrombosis could significantly prolong the median survival time of patient with primary hepatic carcinoma accompanied by type Ⅱ and type Ⅲ portal vein tumor thrombosis.
9.Investigation on Indoor Air Biological Pollution of House Which Patients With Perennial Allergic Rhinitis Lived in

Objective To investigate the house air pollution and its influence on the patients with perennial allergic rhinitis. Methods The method of case control study was used in 2001-2002 23 patients with perennial allergic rhinitis selected as the case and 23 normal as the control house dust mites in particular D.pteronyssinusDp and D.farinaeDf fungus bacteria was detected. Results The allergic skin test of dust and acarid were both positive among the patients with perennial allergic rhinitis. The colony of fungi in the house of patients was 917?668 cfu/m3 that of bacteria was 2 679?1 723 cfu/m3 Dp was 0.72 ?0.29100 IU/ml and Df was 0.61?0.34100 IU/ml in houses-dust compared with the control no significant deferences had been seen. Conclusion The patients with perennial allergic rhinitis are susceptible to the allergens in house dust mites.
10.DNA repair gene XRCC3 Thr241Met polymorphism and susceptibility to cardia and non-cardia gastric cancer: a case-control study.

OBJECTIVETo study the association between genetic polymorphisms of XRCC3 Thr241Met gene and susceptibility to gastric cardia and/non-cardia gastric cancer, and to investigate the combined effect between genes and surrounding environment.
METHODSA case-control study with respective control group was conducted. Genotypes were investigated by PCR-RFLP. Unconditional logistic regression models were used to estimate odd ratios (ORs) and their 95% confidence intervals(95% CI).
RESULTSThe frequency of XRCC3 CC, CT and TT genotypes were 43.2%, 46.5% and 10.3%, respectively in cardia cancer cases and 53.2%, 40.9% and 5.8% respectively in non-cardia gastric cancer cases while 59.6%, 35.1%, 5.3%, respectively in control group. Variated genotypes (CT and TT) increased the risk of cardia cancer after adjusting for potential confounders (OR = 1.76, 95% CI: 1.07-2.90). On cardia cancer risks, there seemed combined effects between variated genotype and high rate of alcohol drinking, low intake of fresh vegetables and having chronic gastritis. Combined effects between variated genotype and smoking, having chronic gastritis were observed in non-gastric cancer group.
CONCLUSIONXRCC3 variated genotype was one of the risk factors of cardia cancer while different risks of factors might exsit between cardia and non-cardia gastric cancer.
