1. Effects of chitosan and PC-chitosan-coated membranes on growth and hemocompatibility of vascular endothelial cells

Objective: To investigate the effects of chitosan (CHI) and PC-chitosan (PC CHI) coated membranes on the proliferation, migration, and hemocompatibility of vascular endothelial cells. Methods: CHI and PC-CHI were separately sprayed on the bottom of culture dish evenly to form the polymer membrane; the cylinder was made of 316 L stainless steel flake. Pig iliac endothelial cells (PIEC) were cultured on the polymer membranes, the stainless steel bottom of the cylinder, and the untreated bottom of culture dish (as blank control). Then the cell morphology was observed by light microscope and scan electron microscope; cell proliferation was measured by Cell Counting Kit-8 (CCK 8); and cell migration was also assayed. Whole blood activated partial thromboplastin time (APTT), prothrombin time (PT), fibrinogen (FiB), and thrombin time (TT) were examined by clotting method. Results: PIEC grew well on the membranes of CHI and PC-CHI, with normal morphology. After 24 h culture, the proliferation rates of PIEC were 88. 8% and 77. 8% on CHI and PC-CHI membranes, respectively. The survived PIEC on CHI and PC-CHI membranes were significantly more than those on 316 L stainless steel flake(P<0.01), and those on CHI membrane were significantly more than those on PC-CHI membrane(P<0.01). After 72 h culture, the numbers of migration cells on CHI and PC-CHI membranes were significantly more than those on 316 L stainless steel flake(P<0.01), and those on PC-CHI membrane were significantly more than those on CHI membrane(P<0.01). APTT in PC-CHI, 316 L stainless steel flake, and blank control groups were significantly longer than that of CHI group(P<0.01, P<0.05), and FiB levels in the former 3 groups were higher than that of CHI group(P<0.01). Conclusion: Coating with CHI and PC-CHI can improve the proliferation and migration of vascular endothelial cells on the culture dishes, indicating an excellent biocompatibility of CHI and PC-CHI membranes. PCPCH membrane has more potent antithrombotic effect than CHI does.
2.The Application of Multi-slice Spiral CT with Four Step Observation Method Diagnosing Rib Fine Fracture

Objective To approach the value of combination mode of different CT post-processing technique in diagnosing rib fine fracture.Methods 98 patients suspected with rib fractures underwent multislice spiral CT(MSCT) scanning,and CT features of rib fractures were observed with the combination of three different images:(A)volume rendering(VR),curved planar reformation(CPR) and axial view;(B)maximum intensity projection(MIP),CPR and sxial view;and(C) VR+MIP,CPR and axial view.The results were respectively recorded.Results 265 rib fine fractures were found among 98 patients.The detecting rate of fine fractures with C group was higher than that with A group (χ~2=6.67,P<0.01) and B group (χ~2=6.75,P<0.01).Conclusion MSCT four step observation method can improve the detecting rate of rib fine fracture,that is of important clinical value.
3.Surgical treatment of unstable pelvic fractures combined with acetabular fractures.

OBJECTIVETo discuss the treatment strategy of acetabular fractures and unstable pelvic fracture of the hip and to evaluate its outcome.
METHODSRetrospective analysis of clinical data in 32 patients with unstable pelvic fracture and acetabular fractures from January 2007 to June 2013 were collected. There were 18 males and 14 females aged from 18 to 62 years old (means 38 years old). According to Tile classification of pelvic fracture, 11 cases were type B1, 8 were type B2.1, 7 were type B2.2, 3 were type C1.1, 2 were type C1.2, 1 was type C3. According to Judet-Letournel classification, anterior column fracture was in 1 case, transverse fracture in 8, transverse plus posterior wall fracture in 6, T-type fracture in 1, anterior column plus half transverse fractures in 5, double column fracture in 11. Other combined injuries were treated early, the surgical operation were performed after stable condition. The hip joint function and the fracture reduction were assessed during follow-up.
RESULTSThe operative time was from 1.8 to 6.5 hours (averaged 3 hours). Two fat patients' incision occurred in fat liquefaction and healed after dressing, no incision infection happened. Only 1 case was lost to follow-up, 31 patients were followed up with a mean time of 23 months (6 to 42 months). The healing time of pelvic fracture was from 8 to 18 weeks (averaged in 10.6 weeks). The hip function was evaluated according to the Matta and Tornetta standard postoperatively, the result was excellent in 15 cases, good in 14 cases, fair and poor in 1 case respectively. The Majeed score of the hip function was 83.65? 7.67, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The healing time of acetabular fractures was from 8 to 16 weeks (averaged in 10.2 weeks). The fracture reduction was assessed by Matta standard, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The heterotopic ossification was evaluated by Brooker standard, 4 cases were grade I, 1 case was grade II . There were no infection, nonunion and necrosis of the femoral head in all patients. The nerve damage symptoms in 5 patients disappeared during 4 to 6 months after operation.
CONCLUSIONPatients with unstable pelvic fractures and acetabulum fractures were in a critical condition early, using the concept of damage control to save lives in a timely manner. Grasp the operation time in the late treatment, acetabulum fractures reach anatomical reduction as far as possible, pelvic fractures are given priority to stable reconstruction. The operation order was fixed pelvic ring first, according to the condition to choose the appropriate surgical approach and fixed mode.

5.Ellagic acid-induced hypercoagulable state in animals: a potentially useful animal hypercoagulable model for evaluation of anticoagulants.

OBJECTIVETo establish and evaluate a hypercoagulable animal model for the assessment of anticoagulants.
METHODSForty mice, thirty-two rats, and twenty-four rabbits were randomly and equally divided into control group (saline) and three ellagic acid (EA)-treated groups (low, middle, and high doses). In the mice, bleeding time (BT) was estimated with tail transaction, and clotting time (CT) with template method. Prothrombin time (PT) and the activated partial thromboplastin time (APTT) in rats and rabbits were measured by means of Quick's one-stage assay and modified APTT assay respectively. In addition, thrombin activity was estimated in rats with PT assay using a hemagglutination analyzer. The circulating platelet aggregates were detected in rabbits through platelet counting and presented as the circulating platelet aggregate ratio (CPAR).
RESULTSEA shortened BT and CT in mice, PT and APTT in rats, and increased thrombin activity and CPAR, all in a dose-dependent manner. EA also brought reduction of PT and APTT in rabbits in dose- and time-dependent manners.
CONCLUSIONEA could induce hypercoagulable state through activating coagulation system and platelets in mice, rats, and rabbits.

6.The preliminary research of DWIBS in lymph node metastasis of rectal cancer

Objective To investigate the application value of diffusion-weighted imaging with background suppression (DWIBS) in lymph node metastasis of rectal cancer.Methods 30 patients with rectal cancer diagnosed by colonoscopy underwent routine MRI and DWIBS examinations.Metastatic and non-metastatic lymph nodes were confirmed by postoperative pathology.Apparent diffu-sion coefficient (ADC)values of the solid part of metastatic and non-metastatic lymph nodes were measured and compared.ROC curve analysis was used to define the threshold between the metastatic and non-metastatic lymph nodes.Results In all 73 lymph nodes,there were 52 metastatic lymph nodes and the other 21 were non-metastatic.ADC values of the solid part of metastatic and non-metastatic lymph nodes were (0.881 ± 0.094)× 10 -3 mm2/s and (1.072 ± 0.108)×10 -3 mm2/s respectively,the difference was statistically significant (P <0.05).With the threshold ADC value of 0.957×10 -3 mm2/s,the sensitivity and specificity for dif-ferentiating metastatic from non-metastatic lymph nodes were 90.5% and 80.8% respectively.The diagnostic value was excellent (Az value = 0.91 9,P <0.05).Conclusion ADC values may be used to differentiate lymph node metastasis of rectal cancer,and DWIBS has higher application value in assessing lymph node metastasis of rectal cancer.
7.Evaluation of Clinical Detection Methods for Detection of Helicobacter Pylori Infection in Children

Objective To explore the optimal methods to detect helicobacter pylori(Hp)antigen in children.Methods 13C labeled urea breath test(13C-UBT)was performed on 937 children of alimentary department from Sep.2000 to Feb.2006.Gastric mucosa biopsy of 96 children was detected.Hp stool antigen(HpSA)status of the 557 children were evaluated.Both 13C-UBT and HpSA were assayed in 105 children from Apr.2003 to Apr.2004.Method of 13C-UBT was taken as the golden standard on diagnosis of Hp infection.Results Forty-one point seven percent children was positive for 13C-UBT.Forty point six percent children was positive for gastric mucosa biopsy.Thirty-eight point two percent was positive for HpSA.The difference was not significant.2.Among the 105 children performed by both 13C-UBT and HpSA,41.9% was positive for 13C-UBT and 39.0% for HpSA.Taking 13C-UBT as the golden standard,sensitivity of HpSA to diagnose Hp infection was 91.8%,and its specificity was 81.8%.Consistency Kappa coefficient was 0.743(P=0),which denoted that there was no significant difference on the positive detection rate between 13C-UBT and HpSA(P=0.388).Conclusions 13C-UBT and HpSA as non-invasive technique is effective to detect the Hp antigen.Compared to 13C-UBT,as a convenient,noninvasive,economical method,HpSA detection is much more acceptable to children and their patients.
8.Optical coherence tomography changes on age - related macular degeneration patients after photodynamic therapy

AIM: To better reveal the changing process of macula area after photodynamic therapy (PDT), the changes of tomographic imaging on macular fovea optical coherence tomography ( OCT ) scan at different follow- up time points after treating age-related macular degeneration ( AMD) by PDT were investigated.
METHODS: This cohort study included 30 eyes of 26 patients diagnosed as AMD. The dosage of verteporfin was calculated according to manufacturer's instruction. All participants got FFA, ICG, OCT scan and best corrected visual acuity before and followed up for 2wk, 1, 2, 3mo after PDT. The standard five lines combined with 6 lines OCT scan covered key parts of lesion and the scan locations before and after were kept the same. The bilaminar foveal thickness (BFT), outer high reflectivity band thickness ( OHRBT ) , and the total area of intraretinal fluid ( IRF ) and subretinal fluid ( SRF ) were measured at different time points. The relationship between the changes and follow up time was analyzed. Statistical analysis of the data was performed using SPSS for windows version 13. 0.
RESULTS: There months after treatment, an improvement of 2 lines or better on the Snellen chart was achieved on 22 eyes, visual acuity of 3 eyes without significant change, 3 eyes got decreased, 2 cases lost follow up. The average macular thickness of improved 22 eyes before treatment was 722. 5±55. 6μm, 2wk after treatment, 708. 3±45. 3μm, 1mo, 584. 4±49. 3μm, 2mo, 430. 7±50. 2μm and 180. 6 ±36. 3μm at 3mo. The OHRBT before and after treatment respectively were 302. 3 ±50-2μm, 277. 5±42. 3μm, 202. 7±40. 1μm, 180. 6±35. 7μm, 100. 8±22. 9μm. The total area of both IRF and SRF was estimated as 0. 34±0. 12mm2 , 0. 25±0. 07mm2 , 0. 10±0-05mm2 , 0. 08±0. 04mm2 , 0. 05±0. 01mm2 .
CONCLUSION:SRF and retinal edema of patients with AMD were absorbed 1mo after PDT, and the BFT, OHRBT were significantly tend to be thin.
9.The application of MR imaging in differentiating benign and malignant lymph node and assessing therapeutic effect

Objective To approach the application value of diffusion weighted imaging with background suppression (DWIBS) combined with routine MR sequence in differentiating benign and malignant lymph nodes,and assessing therapeutic effect of lympho-genic tumors.Methods 48 patients with cervical lymph node enlargement who were confirmed by pathology and follow up under-went DWIBS and routine MRI examination,malignant lymph nodes were 83,benign lymph nodes were 79.16 patients with malig-nant lymph nodes were rechecked after radiotherapy and chemotherapy,Apparent diffusion coefficient (ADC)values of the solid part for lymph nodes were compared.Results More lymph nodes can be detected in DWIBS than conventional sequence.ADC values of the solid part for malignant lymph node(0.898±0.111)×10-3 mm2/s were lower than that of benign lymph node(1.043±0.106)× 10-3 mm2/s,there was significant difference between them (P<0.05).ADC values of all malignant lymph nodes after treatment (1.205±0.121)×10-3 mm2/s were significantly higher than that of pretherapy (0.883±0.090)×10-3 mm2/s (P<0.05).Conclu-sion DWIBS could more sensitively detect lymph node than conventional MR sequence.ADC value could provide some reference values for differentiating benign and malignant lymph nodes and assessing therapeutic effect.
10.Correlation of computed tomography perfusion and Cyfra21-1 in head and neck squamous carcinoma

Objective To estimate the value of computed tomography perfusion for clinical stage and approach the correlation of perfusion parameters and Cyfra21-1.Methods 63 patients with head and neck squamous carcinoma were confirmed by pathology and follow up underwent CT perfusion,which were divided into three groups by international clinical staging criteria(stageⅠ,stageⅡand stageⅢ-Ⅳ).BF,BV,MTT,TTP and Cyfra21-1 were recorded and compared with correlation in different clinical staging.Results There was no significant difference of Cyfra21-1 between stageⅠand stageⅡ(Z =1.439,P =0.1 62).There was significant differ-ence of Cyfra21-1 between stageⅠand stageⅢ-Ⅳ(Z =3.356,P =0.000),stageⅡand stageⅢ-Ⅳ(Z =4.959,P =0.000).There was significant difference of BF and BV between stageⅠand stageⅡ,stageⅠand stageⅢ-Ⅳ(P <0.05),of MTT and TTP between stageⅠand stageⅢ-Ⅳ,stageⅡand stageⅢ-Ⅳ(P <0.05).There was no significant difference of BF and BV between stageⅡand stageⅢ-Ⅳ(P >0.05),of MTT and TTP between stageⅠand stageⅡ(P >0.05).Cyfra21-1 and perfusion parameters in all groups have relationship(r=0.76,0.76,-0.82,-0.82,P <0.05).Conclusion The statistically significant of positive correlation be-tween Cyfra21-1 and perfusion parameters in head and neck squamous carcinoma suggests that CT perfusion could play a complemen-tary role in clinical assessment.
