1.Clinical study of abdominl type of tumor cells to destroy the loss after neoadjuvant chemotherapy in treatment of epithelial ovarian cancer stage Ⅲ C

Objective To investigate the effect of abdominl type of tumor cells to destroy the loss after neoadjuvant chemotherapy in treatment of epithelial ovarian cancer stage Ⅲ C.Methods Eighty cases patients of stage ⅢC epithelial ovarian cancer who were treated in Traditional Chinese Medicine Hospital of Meizhou from June 2010 to June 2012 were enrolled in this study and all were consistent with the diagnostic criteria and no operation contraindications.They were randomly divided into two groups with 40 cases in each group according to the paired grouping method.The observation group was treated with abdominal type intermediate tumor cell reduction surgery after neoadjuvant chemotherapy,and the control group was treated with abdominal intermediate tumor cells.Two groups were treated by paclitaxel and carboplatin chemotherapy after the operation,the curative effect between the two groups was compared.Results Blood loss during operation of patients in observation group were lower than control group,the difference was significant((512.6±98.6) ml vs.(785.6±91.2) rnl,P<0.001).The operation time of patients in observation group was shorter than that of control group,the difference was significant((215.6±42.6) min vs.(285.6±43.9) min,P<0.01).The ascites of patients in observation group were less than that of control group,the difference was significant((689.5±215.6) ml vs.(1 265.9±325.6) ml,P<0.01).There was no significant difference in the length of stay between the two groups (P>0.05).There were 11 cases(27.5%) of complete remission,24 cases(60.0%) of partial remission,4 cases of stable (10%,4/40),1 case (2.5%,1/40) of progress in the observation group recently;while in control group,complete remission in 5 cases(12.5%),partial remission in 9 cases (22.5%),stable in 20 cases(50%,20/40),progress in 6 cases (15.0%,6/40),the differences between the two groups were statistical significance (P<0.001).The 3 year survival rate of observation group was significantly higher than that of control group (92.5% vs.72.5%,P =0.0394).Conclusion The effect of abdominl type of tumor cells to destroy the loss after neoadjuvant chemotherapy in treatment of epithelial ovarian cancer stage ⅢC is better,it has the important clinical value.
2.Effects of intranasal administration of heparin-binding epidermal growth factor-like growth factor on expressions of nestin and caspase-3 in the perihematomal tissue after intracerebral hemorrhage in rats

Objective To investigate the effects of intranasal administration of heparin-binding epidermal growth factor-like growth factor (HB-EGF) on expressions of nestin and caspase-3 in the perihematomal tissue after intracerebral hemorrhage in rats. Methods A total 36 adult male Sprague-Dawley rats were randomly divided into 3 groups: an HB-EGF group, a control group, and a sham operation group (n = 12 in each group). Then they were further divided into 4-, 7-, and 10-day subgroups (n = 4 in each subgroup). A model of intracerebral hemorrhage was induced by injecting type Ⅳ colagenase. At 1 to 3 days after modeling, the HB-EGF group was intranasaly administrated HB-EGF. The control group was administrated the same volume of saline. The Bederson score was conducted at the corresponding time points and the rats were sacrificed after the forelimb placing test. Immunohistochemistry was used to detect the expression of nestin and caspase-3 in the perihematomal tissue. Results The sham operation group did not have neurological deficits. The Bederson scores (P < 0. 05) and the result of forelimb placing test (al P < 0. 01) at day 4 and 7 in the HB-EGFgroup were better than those in the control group. There were no significant differences at day 10. At day 4, 7, and 10, the number of nestin positive cels in the perihematomal tissue in the HB-EGF group was significantly more than that in the control group (al P < 0. 05), and the number of caspase-3 positive cels was less than that in the control group (al P < 0. 01). Conclusions Intranasal administration of HB-EGF can improve the early neurological function, upreguate the nestin expression and downregulate the caspase-3 expression in the perihematomal tissue after intracerebral hemorrhage in rats, indicating that intranasal administration of HB-EGF may promote the proliferation of precursor cels and decrease cel apoptosis in the perihematomal tissue.
3.Clinical Study onTong-Bu San-ShengDecoction for Short-term Outcome and Progression-free Survival for Advanced Lung Squamous Carcinoma Patients Received Chemotherapy

This study was aimed to investigate the efficacy ofTong-Bu San-Sheng(TBSS) Decoction to reduce the toxicity and side effects of chemotherapy, as well as prolong progression-free survival (PFS) for advanced lung squamous carcinoma patients who received chemotherapy. A total of 83 lung squamous carcinoma cases were divided into two groups by patients’ wishes. The control group contained 41 cases were treated by the chemotherapy of gemcitabine plus cisplatin (GP). The trial group contained 42 cases were treated by chemotherapy plus Chinese herbal medicine TBSS decoction. The toxicity and side effects of chemotherapy, as well as short-term outcome were evaluated. PFS of patient was recorded. The results showed that there were no differences on granulocytopenia (P = 0.115) or short-term outcome (P = 0.081) for patients of both groups after chemotherapy. The percentages of nausea, vomiting and thrombocytopenia in the trial group were lower than that in the control group (P = 0.037,P = 0.040). The PFS of patients in the trail group were prolonged compared to patients in the control group (4.31 ± 0.24 VS 3.78 ± 0.16 month;P = 0.043). It was concluded that Chinese herbal medicine TBSS decoction cannot reduce granulocytopenia caused by chemotherapy, or improve the tumor response rate (RR) of short-term outcome. However, it can prolong PFS, relieve nausea, vomiting and thrombocytopenia during chemotherapy.
4.The clinical significance of plasma complement factor H in the prognosis of patients with peritoneal dialysis

Objective To explore the levels of plasma complement factor H(CFH)in peritoneal dialysis(PD)patients and its judging value for prognosis.Methods Eighty-two PD patients followed up by the Department of Nephrology,The Second Affiliated Hospital of Naval Medical University were included in the study.Patients were divided by the CFH levels based on immunoassay turbidimetric detection into low H factor group(<320 μg/mL,n=41)and high H factor group(>320 μg/mL,n=41),while 82 healthy individuals matched by gender and age were selected as the healthy control group.The correlation between plasma CFH and laboratory results,complications,and prognosis in PD patients was analyzed over a 3-year follow-up.Results There were no differences between the two groups in terms of gender,age,dialysis duration,urine output,and dialysis adequacy,whereas the plasma albumin,total protein and cholesterol were significantly lower in the low CFH group compared with the patients in the high CFH group(P<0.05).During the 3-year follow-up,the incidence of peritonitis was significantly higher in the low CFH group than in the high CFH group(P<0.05).Multivariate regression analysis further confirmed that low CFH level was an independent risk factor for the development of PD associated peritonitis(OR=4.24,95%CI 1.18-15.33).Conclusions Reduced levels of plasma CFH in PD patients might suggest the increased risk of hypoalbuminemia and PD-associated peritonitis.

Objective To study the relationship between socioeconomic status (SES) and esophageal squamous cell carcinoma(ESCC) in Taixing city,Jiangsu province.Methods A populationbased case-control study was carried out to collect data including demography,socioeconomic status indicators and possible risk factors.Unconditional logistic regression was used to compare different SES indicators and composite wealth scores constructed between cases and controls,using the principal component analysis methodology.Results Factors as:having received high school or higher education (OR=0.66,95%CI:0.46-0.96),living space over 67 m2 per-capita (OR=0.71,95%CI:0.54-0.94),drinking tap water longer than 5 years (OR=0.76,95% CI:0.59-0.98) and wealth score >0.93 (OR=0.63,95%CI:0.48-0.83) were associated with increased risk of ESCC.Compared to those worked in farming,forestry,animal husbandry,fishery and water conservancy,the individuals who worked in business-service industry were at lower risk of ESCC (OR =0.65,95%CI:0.43-0.97).Conclusion An inverse association of low SES and ESCC were found in Taixing people that called for further explanation.
6.Antibiotic-loaded bone cement enhances ability of tibial cortex transverse transport for treating infected wounds

BACKGROUND:Diabetic foot patients with wound infections constitute a large patient population,and there is currently no satisfactory treatment approach. OBJECTIVE:To investigate the clinical efficacy of a modified tibial cortex transverse transport combined with antibiotic-loaded bone cement for treating refractory diabetic foot ulcers. METHODS:A total of 46 diabetic foot ulcers patients,27 males and 19 females,with an average age of 64.37 years,were selected from Beijing Chaoyang Hospital,Capital Medical University and Beijing Chaoyang Integrative Medicine Rescue and First Aid Hospital from January 2020 to January 2023.All of them underwent the modified tibial cortex transverse transport combined with antibiotic-loaded bone cement treatment.Ankle-brachial index,WIFi(Wound/Ischemia/Foot infection)classification,pain visual analog scale score,and ulcer area were recorded before and 3 months after surgery. RESULTS AND CONCLUSION:(1)The mean ulcer healing time for the 46 patients was(58.07±24.82)days.At 3 months postoperatively,there were significant improvements in ankle-brachial index,pain visual analog scale score,ulcer area,and WIFi classification in 46 patients,as compared to the preoperative values,with statistically significant differences(P<0.05).Two patients experienced pin-tract infections,without infection or ulcer recurrence during the follow-up period.(2)These findings indicate that the modified tibial cortex transverse transport combined with antibiotic-loaded bone cement effectively alleviates patients'pain,improves lower limb circulation,controls infections,and promotes ulcer healing.

Objective To assess the effectiveness of enhanced tibial transverse transport(TTT)in con-junction with vacuum-assisted closure(VAC)therapy for managing recalcitrant diabetic foot ulcers.Methods A retrospective analysis was conducted on data from diabetic foot patients with Wagner grade≥2 who were treated at Beijing Chaoyang Hospital and Beijing Chaoyang Hospital of Integrated Traditional Chinese and Western Medicine between July 2020 and December 2022.The patients were categorized into three groups based on their treatment regimen:VSD treatment(VSD group),modified TTT treatment(TTT group),and combined application of TTT and VSD(combined group).A one-year follow-up was performed to assess general data,ulcer area before and three months after surgery,ankle brachial index,visual analog pain score,as well as adverse events within one year post-surgery among the three groups.Results The VSD group consisted of 43 patients,while the TTT group consisted of 43 patients,and the combined group consisted of 42 patients.There were no statistically significant differences in baseline characteristics among the three groups(P>0.05).Patients in the VSD group had longer ulcer healing time,higher pain scores,lower ankle brachial index(P<0.05),larger ulcer area(P=0.029),and higher one-year ulcer recurrence rate compared to those in the TTT group.On the other hand,patients in the combined group had shorter ulcer healing time compared to those in the TTT group(P=0.046).However,there were no significant differences observed between these two groups regarding ulcer area(P=0.362),pain scores(P=0.932),ankle brachial index(P=0.671),and one-year ulcer recurrence rate(P=0.710).Conclusions The efficacy of modified TTT surpasses that of VSD in promoting ulcer healing,alleviating pain,and enhancing lower limb circulation.Furthermore,the combination of VSD with modified TTT demonstrates a potential to further expedite wound healing time.
8.Expression of PD-L1 and tumor infiltrating lymphocyte markers in uterine cervical carcinoma

Objective:To investigate the expression of PD-L1, CD4, CD8 and CXCL-13 in cervical carcinoma, and their clinicopathological significance was analyzed.Methods:A total of 77 patients with cervical carcinoma in the Seventh Medical Center, PLA General Hospital from January 2019 to December 2021 were included. All patients received radical surgical resection in the Seventh Medical Center of Chinese PLA General Hospital. The expression of PD-L1, CD4, CD8 and CXCL-13 was detected by immunohistochemical (IHC) method. The correlation between IHC markers and patients′ clinicopathological parameters was analyzed.Results:There were 59 cases of squamous cell carcinoma and 18 cases of adenocarcinoma (ranging from 29 to 69 years) with an average of (49.4±9.8) years. PD-L1 was expressed in different degrees in cervical squamous cell carcinoma and adenocarcinoma (χ2=4.975, P=0.026); CD4 +, CD8 +and CXCL-13 +tumor infiltrating lymphocytes (TIL) were observed in the carcinoma cell nests and peritumoral stroma. PD-L1 expression in cervical carcinoma was moderately correlated with the number of CD4 +TIL in the carcinoma nests, and the number of CD8 +, CXCL-13 +TIL infiltration in the carcinoma nests and stroma, but not to the patient′s age, histologic differentiation, presence or absence of vascular invasion, presence or absence of lymph node metastasis and FIGO stage ( P>0.05). Conclusions:The high expression of PD-L1 in cervical carcinoma tissues is closely related to the number of TIL in the carcinoma nests and peritumor stroma, suggesting that they may have important reference value for predicting the response to immunotherapy in patients with cervical carcinoma.

BACKGROUND:
The therapeutic potential of exosomes from human umbilical cord mesenchymal stem cells (HUMSCsExo) for delivering specific circular RNAs (circRNAs) in treating premature ovarian failure (POF) is not well understood.This study aimed to explore the efficacy of HUMSCs-Exo in delivering hsa_circ_0002021 for POF treatment, focusing on its effects on granulosa cell (GC) senescence and ovarian function.
METHODS:
Bioinformatic analysis was conducted on circRNA profiles using the GSE97193 dataset from GEO, targeting granulosa cells from varied age groups. To simulate granulosa cell senescence, KGN cells were treated with cyclophosphamide (CTX). HUMSCs were transfected with pcDNA 3.1 vectors to overexpress hsa_circ_0002021, and the HUMSCsExo secreted were isolated. These exosomes were characterized by transmission electron microscopy (TEM) and Western blotting to confirm exosomal markers CD9 and CD63. Co-culture of these exosomes with CTX-treated KGN cells was performed to assess b-galactosidase activity, oxidative stress markers, ROS levels, and apoptosis via flow cytometry.Interaction between hsa_circ_0002021, microRNA-125a-5p (miR-125a-5p), and cyclin-dependent kinase 6 (CDK6) was investigated using dual-luciferase assays and RNA immunoprecipitation (RIP). A POF mouse model was induced with CTX, treated with HUMSCs-Exo, and analyzed histologically and via immunofluorescence staining. Gene expression was quantified using RT-qPCR and Western blot.
RESULTS:
hsa_circ_0002021 was under expressed in both in vivo and in vitro POF models and was effectively delivered by HUMSCs-Exo to KGN cells, showing a capability to reduce GC senescence. Overexpression of hsa_circ_0002021 in HUMSCs-Exo significantly enhanced these anti-senescence effects. This circRNA acts as a competitive adsorbent of miR-125a-5p, regulating CDK6 expression, which is crucial in modulating cell cycle and apoptosis. Enhanced expression of hsa_circ_0002021 in HUMSCs-Exo ameliorated GC senescence in vitro and improved ovarian function in POF models by modulating oxidative stress and cellular senescence markers.
CONCLUSION
This study confirms that hsa_circ_0002021, when delivered through HUMSCs-Exo, can significantly mitigate GC senescence and restore ovarian function in POF models. These findings provide new insights into the molecular mechanisms of POF and highlight the therapeutic potential of circRNA-enriched exosomes in treating ovarian aging and dysfunction.
10.Comprehensive analysis of unplanned abdominal ? pelvic reoperations in Peking University Cancer Hospital and Institute from 2008 to 2018

Objective To summarize the characteristics of abdominal?pelvic unplanned reoperation (URO) in a cancer hospital. Methods Retrospectively descriptive cohort research was adopted. The classification of diseases was based on ICD?10, and surgical classification was based on ICD?9?CM?3. Medical record summary database from 2008 to 2018 in Beijing Cancer Hospital was collected, and all URO information of abdominal?pelvic surgery was retrieve. The time of URO, disease type, surgery type and cause were statistically analyzed. Distribution of main disease incidence and constituent ratio, and the application of major surgery and surgery type composition ratio were analyzed as well. Results From 2008 to 2018, a total of 46854 cases underwent abdominal?pelvic surgery (including gastrointestinal, hepatic?biliary?pancreatic, gynecological, urological, or esophageal cancer surgery) and 713 patients received URO (1.52%), including 486 males and 227 females (2.14: 1.00) with a mean age of (58.1 ± 12.2) years. A total of 246 patients (34.50%) had comorbidity, and with comorbidity: without comorbidity was 1.13: 1.00. The hospital stay was (44.5 ± 43.0) days, and the total cost was (178000 ± 112000) yuan. There were 22 deaths (3.09%). The median interval between URO and the first operation was 8 (0 to 131) days. The highest rate of URO was 2.45% (89/3629) in 2012, while the lowest was 0.95% (53/5596) in 2015. The top 3 major cancer types of URO included colorectal cancer (222 cases, 31.14%), gastric cancer (166 cases, 23.28%) and esophageal cancer (45 cases, 6.31%). The cancer types with the highest URO rate included pancreatic cancer (3.97%, 30/756), gastric cancer (1.81%, 166/9171) and colorectal cancer (1.80%, 222/12333). The top 3 surgical procedures resulting in URO were rectal resection (109 cases, 15.29%), total gastrectomy (79 cases, 11.08%), and total pancreatectomy (64 cases, 8.98%). Focusing on 497 URO cases from 2012 to 2018, 465 cases (93.56%) followed relatively difficult grade III and IV surgery, while only 32 cases (6.44%) followed grade I and II surgery. The top 5 main causes of URO were bleeding (225 cases, 31.56%), anastomotic leak (225 cases, 31.56%), infection (89 cases, 12.48%), intestinal obstruction (53 cases, 7.43%), and wound dehiscence (35 cases, 4.91%), adding up to a total of 87.94% of all URO. Conclusion This study summarizes the epidemiology of abdominal?pelvic URO in our cancer institution, providing the solid baseline for future investigation of URO and the subsequent formulation of corresponding prevention and intervention measures.
