1.Efficacy and safety of drospirenone-ethinylestradiol on contraception in healthy Chinese women: a multicenter randomized controlled trial

Objective To evaluate the contraception efficacy, mode of bleeding, side effects and other positive effects of drospirenone-ethinylestradiol (Yasmin) in healthy Chinese women. Methods This was a multicenter, randomized, control study of 768 healthy Chinese women who consulted about contraception. The subjects were randomized into Yasmin group (30 μg ethinylestradiol plus 3 mg drospirenone, 573 cases) or desogestrel group (30 μg ethinylestradiol plus 150 μg desogestrel, 195 cases) with the ratio of 3: 1. Each individual was treated for 13 cycles. Further visits were required at cycle 4, cycle 7, cycle 10 and cycle 13 of treatment. Weight, height, body mass index were evaluated at each visit. The menstrual distress questionnaire (MDQ) was given to the women at baseline, visit 3 (cycle 7) and visit 5 (after cycle 13). Results The values of basal features were similar between two groups (P> 0.05). The Pearl index (method failure) of Yasmin was 0. 208/hundred women year which was lower than that of desogestrel (0. 601/hundred women year). The mode of bleeding was similar between two groups after trial without showing any significant difference. According to MDQ subscale, the improvement of water retention and increasing appetite during inter-menstrual period and water retention and general well-being during menstrual period in the Yasmin group ( -0. 297, -0. 057, 0. 033, 0. 150 respectively) was more obvious than that in the desogestrel group ( - 0. 108, 0. 023, 0. 231, - 0. 023 respectively) with a significant difference (P < 0. 05 ). Some other values which improved in beth two groups, especially the improvement of breast tenderness and pain and skin abnormality in Yasmin group (18.0%, 89/494; 12. 6%, 62/494) was more distinct than that in desogestrel group (11.3%, 19/168; 5.4%, 9/168). The mean weight increased in desogestrel group (0. 57 kg) while it decreased in Yasmin group ( -0. 28 kg) with a significant difference (P < 0. 01 ). Conclusions Both Yasmin and desogestrel have good efficacy on contraception and similar modes of menstrual bleeding. Yasmin is better than desogestrel in terms of weight control and premenstrual syndrome of oral contraceptive.
2.Evidence integration of the best sleep position nursing in patients with obstructive sleep apnea-hypopnea syndrome based on Johns Hopkins nursing evidence-based practice model

Objective To retrieve and integrate relevant evidence on best sleep position nursing in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) so as to provide scientific and effective references for clinical departments.Methods Firstly, we put forward questions aiming at sleep position of OSAHS patients with the Johns Hopkins nursing evidence-based practice model. And then, we retrieved related studies by computers in the round and carried out assessment of literature quality and level of evidence for various researches. Finally, we integrated the evidence and made recommendations.Results Combining the judgment of specialized persons, a total of 7 evidences on sleep position nursing of OSAHS patients were integrated, such as adapt-to-the-crowd and effects of position nursing, the best sleep position and method to maintain position.Conclusions The sleep position nursing is very important for OSAHS patients. In further clinical transformation and application, clinical departments should well judge on selection of patients, conditions of evidence application to make sensible decisions so as to improve the quality of nursing by improving patients' sleep hypopnea with the scientific nursing methods through applying the best evidence.
3.Effects of aerosol inhalation and airway dripping for airway humidification in patients with tracheotomy: a Meta-analysis

Objective:To evaluate the effect of aerosol inhalation and airway dripping for airway humidification of patients with tracheotomy, and explore airway humidification methods suitable for China's national conditions.Methods:The documents published from January 2010 to October 2020 in The Cochrane Library, PubMed, Embase, China National Knowledge Infrastructure (CNKI) , Wanfang Data and VIP were searched by computer, including randomized controlled trials (RCTs) on the application of aerosol inhalation and airway dripping as a method of airway humidification in patients with tracheotomy. Two researchers used the quality evaluation method of Cochrane Handbook (5.1.0.) to rigorously evaluate the quality of the documents and extract data, and conducted a meta-analysis of the documents that met the quality standards.Results:A total of12 RCTs were included, with a total of 1 010 patients. The results of meta-analysis showed that the sputum viscosity in the aerosol inhalation group was lower than that in the airway dripping group [ RR=0.29, 95% CI (0.21, 0.40) , P<0.01], the incidences of airway mucosal injury, irritating dry cough and lung infection also decreased [ RR=0.34, 95% CI (0.23, 0.51) , P<0.01; RR=0.24, 95% CI (0.16, 0.38) , P<0.01; RR=0.48, 95% CI (0.38, 0.61) , P<0.01], the airway humidification effect was better than that of the airway dripping group [ RR=0.1.34, 95% CI (1.23, 1.45) , P<0.01]. Conclusions:The application of aerosol inhalation to humidify the airway in patients with tracheotomy can significantly reduce the sputum viscosity, airway mucosal injury, irritating dry cough, and the lung infection, and achieve a good airway humidification effect.
4.Based on Pulse Diagnosis Device to Explore the Effect of Baduanjin on the Function of Viscera in Healthy Individuals

Objective To explore the differences between baduanjin and ninth set of broadcast gymnastics in their own pulse parameters before and after intervention for 12 weeks,and to provide objective indicators for the function of Baduanjin from the perspective of pulse diagnosis.Methods 60 healthy subjects were recruited,and 50 subjects were finally included according to the dropout criteria.The subjects were divided into the experimental group and the control group by the random number table method,and different interventions were adopted.Then,the pulse diagnosis of the two groups of subjects was collected,and the impact of the interventions on the viscera was judged by analyzing the Pulse Graph Parameters and contribution.Results The characteristic parameters of the experimental group include 8 time domain parameters(left Chi,left Guan,right Cun,etc.)and 9 frequency domain parameters(left Guan,left Chi,right Guan,etc.),which are mainly distributed in the left Cun,left Guan,left Chi,and right Guan;the characteristic parameters of the control group included 6 time domain parameters(left Cun,left Guan,right Chi,etc.)and 11 frequency parameters(left Guan,left Chi,right Guan,etc.),which were mainly distributed in left Chi,right Chi,and left Guan.Conclusion Practicing Baduanjin can significantly affect pulse diagram,and the accuracy and specificity after intervention are higher than those of the control group.Baduanjin has a certain influence on the circulation of qi and blood in the human meridians,and its mechanism of action may be related to affecting the overall circulation of qi and blood.In the experimental group,Baduanjin had a significant effect on the heart,liver,gallbladder,and kidneys,while the control group had the most obvious changes in the kidneys.For the overall intervention and coordination of the viscera,the Baduanjin group had better effects than the ninth broadcast gymnastics group.
5.Compound Chaijin Jieyu Tablets ameliorating insomnia complicated with depression by improving synaptic plasticity via regulating orexin A, melatonin, and acetylcholine contents

Objective To investigate the efficacy and mechanism of action of Compound Chaijin Jieyu Tablets (复方柴金解郁片, CCJJYT) in rats with insomnia complicated with depression. Methods Seventy-two Sprague-Dawley rats were randomly assigned into eight groups: the control, chronic unpredictable mild stress (CUMS), sleep deprivation (SD), CUMS + SD, positive drug (venlafaxine hydrochloride + diazepam), CCJJYT high-dose (CCJJYT˗2×), medium-dose (CCJJYT˗1×), and low-dose (CCJJYT˗0.5×) groups, with nine rats in each group. Depression-like behavior was evaluated by body weight, food intake, and behavioral tests such as the sucrose preference test (SPT), open field test (OFT), forced swimming test (FST), and pentobarbital-induced sleep test (PST). Hematoxylin-eosin (HE) staining and Golgi-Cox staining were used to observe changes in pathological tissue and synaptic morphology, respectively. Enzyme-linked immunosorbent assay (ELISA) was used to detect the contents of orexin-A and acetylcholine. The expression levels of orexin receptor 1 (OXR1), melatonin receptor 1 (MT1A), melatonin receptor 2 (MT1B), acetylcholinesterase (AChE), and choline acetyltransferase (ChAT) were detected by immunohistochemistry and Western blot. Results In the present study, rats in the model group showed significant behavioral changes as well as a reduction in hippocampal dendritic branch length and synaptic number, along with increasing the content of orexin A and acetylcholine (P< 0.05), and altered expression levels of OX1R, MT1A, MT1B, ChAT, and AChE in the hippocampus and prefrontal cortex after modeling (P < 0.05). CCJJYT can improve depressive insomnia behavior and synaptic plasticity of rats (P < 0.05), which is similar to that of the positive drug group. It can also decrease the content of orexin A and acetylcholine, and reduce the expression levels of OXR1 and ChAT in hippocampus and prefrontal cortex (P < 0.05), and increase the expression levels of MT1A, MT1B, and AChE proteins (P < 0.05). Conclusion CCJJYT has good antidepressant and insomnia effects, probably through the regu-lation of orexin-A, melatonin, and acetylcholine content in hippocampus and prefrontal cortex of rats, improving synaptic plasticity and thus exerting antidepressant and insomnia effects.
