1.Feasibility of E-cervix cervical elasticity analysis in evaluating normal cervical function in non pregnant women

Objective:To explore the feasibility and influencing factors of E-cervix cervical elasticity analysis technology in analyzing normal cervical function during non pregnancy.Methods:213 women who underwent vaginal ultrasound examination in the Ultrasound Department of Foshan Maternal and Child Health Hospital from May 2019 to November 2019 were selected as the research objects. Taking the median sagittal section of the cervix as the initial section, the E-cervix technology software package was started to automatically obtain the elastic contrast index (ECI), hardness ratio (HR), cervical strain rate (IOS), cervical strain rate (EOS), cervical strain ratio (IOS/EOS) and cervical length (CL). The relationship among age, menstrual cycle, BMI index, birth history, delivery mode and elastic parameters were compared.Results:There was no correlation between the elastic parameters and age, and there was no significant difference among different age groups ( P>0.05); there was no significant difference in the elastic parameters of cervical tissue in menstrual period, proliferative period and secretory period ( P>0.05); there was no significant difference in the elastic parameters of underweight, normal and overweight ( P>0.05); CL was positively correlated with body mass index (BMI) ( r=0.225, P<0.05), there was no correlation between other parameters and BMI ( P>0.05); there was no significant difference between the elastic parameters of patients with and without birth history ( P>0.05); the CL of women with cesarean section [(34.22±4.96)mm] was higher than that of women with natural birth [(29.03±4.14)mm] ( P<0.05), and the other parameters had no statistical significance ( P>0.05). Conclusions:The elastic parameters of cervix obtained by E-cervix technique are not affected by age, BMI, menstrual period, reproductive history and delivery mode, and can be used for quantitative evaluation of cervical function.
2.Application of arterialised Flow-through venous flap with palmaris longus tendon in repair of dorsal digit composite tissue defect

Objective:To investigate the effect of arterialised Flow-through venous flap with palmaris longus tendon in repairing dorsal digit composite tissue defect.Methods:From March, 2010 to December, 2018, 23 cases (23 digits) of dorsal digit composite tissue defect were repaired with arterialised Flow-through venous flap with palmaris longus tendon. There were 17 males and 6 females aged between 23 to 53 (average, 37.2) years old. Causes of injury: 15 cases of strangulation, 5 of electric plane and 3 of thermal crush. Number of injured digit were 1 thumb, 11 index fingers, 9 middle fingers and 2 ring fingers. All of the injured digits had skin and extensor tendon defects with an area of 2.0 cm×1.8 cm-4.2 cm×2.6 cm, and the length of extensor tendon defect was 1.6-2.6 cm. One case had terminal and central tendon insertion defects and 5 suffered with terminal tendon insertion defect. Three cases were repaired by emergency surgery, and 20 were repaired by sub-emergency surgery. All donor sites were directly sutured. The shape, colour, texture, sensation, recovery of digit function and donor site appearance were followed-up at outpatient clinic.Results:The patients were followed-up for 8 to 23 (average, 11) months. The flap was soft, with mild pigmentation and recovery of protective sensation. The range of motion of the proximal and distal interphalangeal joints was 145°-170° (average,162.6°). According to Strickland hand function evaluation method, the results were excellent.Conclusion:The arterialised Flow-through venous flap with palmaris longus tendon is an ideal method in repair of the dorsal digit composite tissue defect.
3.The effects of intrinsic hand muscles training after digital flexor tendon repair: a clinical study

Objective To observe the effects of training on intrinsic hand muscles after digital flexor tendonrepair. Methods A total of 60 patients with digital flexor tendon repair were randomly assigned into an experimentgroup (n = 30) and a control group (n = 30). Patients in the experiment group received intrinsic hand muscles train-ing, in addition to the passive mobilization of the flexor tendons and active mobilization of the extensor tendons admin-istered to those in the control group. Minnesota manual dexterity test and Purdue pegboard test were adopted to as-sessment the outcome after 3 months of training. Results After 3 months of training, there were significant differ-ences between the two groups in terms of the results of Minnesota manual dexterity test and Purdue pegboard test( P < 0.01 ). Conclusion Intrinsic hand muscles training after digital flexor tendon repair can improve fine move-ment and coordination of hand.
4.Effect of neuromuscular electrical stimulation on hyolaryngeal structural movement in post-stroke dysphagia

Objective To explore the effect of neuromuscular electrical stimulation(NMES) on hyolaryngeal structural movement in post-stroke dysphagia.Methods Thirty patients with post-stroke dysphagia were randomly divided into 3 groups (n =10) namely group A,group B and group C.On the basis of routine swallowing training,medication and rehabiliation given to group C,group A was provided with VitalStim therapy on both suprahyoid and infrahyoid muscles,while group B was only on suprahyoid muscles lasting 2 weeks.Before and after treatment,the Video Fluoroscopy Swallowing Study (VFSS) was carried out and the upward and forward displacement distances of the hyoid and thyroid cartilage when swallowing pap were measured.Moreover,water swallow test,dysphagia severity scale and DOSS scores were used to assess the swallowing function.Results After 2 weeks' treatment,the water drinking test score,dysphagia severity scale and DOSS scores increased significantly in all groups than before treatment.The water drinking test score of group A (2.40 ± 1.26) and group B (2.10 ± 0.99),dysphagia severity scale of group A (5.30 ± 1.89) and group B(5.20 ± 1.69),as well as the DOSS level of group A (5.20 ± 1.40) and group B (5.10 ± 1.45) were significantly better than the control group.However,no significant difference was observed in all measurements between group A and group B.The group B revealed a significantly increase in anterior hyoid excursion distance of (12.15 ± 7.59) mm,much bigger than that before treatment and that of group A and B after treatment.Conclusion NMES on the basis of traditional swallowing training can improve the swallowing function of post-stroke dysphagia patients,and NMES on suprahyoid muscles can further induce an increase in anterior hyoid excursion.
5.Comparison of adverse effect between Cinepazide Maleate and low Molecular Dextran injection after digital replantation surgery

Objective Comparing the clinical effect between Cinepazide Maleate and low Molecular weight Dextran,to the one which is more effective,safer and less unhealthy to patients after digital replantation.Methods From April 2010 to April 2012 in our hospital during single finger replantation patients,the observation group 120 cases of postoperative to Cinepazide Maleate injection 320mg/d intravenous drip,the control group of 120 cases treated with low Molecular Dextran 500ml/d IVGTT.Results The effective rate of observe group and control group was 96.67% and 92.50%,respectively.The difference between the two groups was too small to have statistically significant difference (P > 0.05),but the adverse effect of treatment group was only 5.8%,much lower than 14.17% in control group however with the statistically significant difference (P <0.05).Conclusion Cinepazide Maleate Injection and low Molecular weight Dextran both have apparent effect in patients after digital replantation surgery.Both of them significantly improved microcirculation,enhanced the survival rate of replantation.However Cinepazide Maleate injection has fewer adverse effects,which is one of idea auxiliary medicine and deserved more extensive clinic application in currently microsurgery.
6.Evaluation of different administration speed on Polyethylene glycol electrolyte solution for colonoscopy preparation

Objective To evaluate the cleanliness and tolerance of different administration speed on polyethylene glycol electrolyte solution (PEG-ES) for colonoscopy preparation. Methods 97 consecutive asymptomatic individuals underwent colonoscopy and therapy were enrolled and randomly assigned into 2 groups. Subjects in group A drank 2000 ml PEG-ES (1000 ml every 10 ~ 20 min rapidly) within 1 h before colonoscopy;Subjects in group B drank 2000 ml PEG-ES (250 ml every 10 min) within 2 h before colonoscopy. The total time of drinking PEG-ES, the first defecation time and total numbers of defecation after drinking PEG-ES, score and degree of Boston bowel preparation scale (BBPS) and PEG-ES related adverse effects of the two groups were assessed and compared. Results There were no significant differences in gender, age and cecal insertion rate between group A and group B (P > 0.05). The total time of drinking PEG-ES, the first defecation time in group A were significant faster than those in group B, while total numbers of defecation was significantly more than that in group B (P < 0.05). No patients in the two groups complained PEG-ES related bellyache and vomiting, a little subjects in group B complained PEG-ES related nausea and bloating (P < 0.05), but the incidence were both less than 10.00 %. Accepting rate of colonoscopy preparation in group A was lower than that in group B, but both more than 90.00%, while accepting rate of re-colonoscopy than that in group B. BBPS score of the right half colon, transverse colon, total colon were significantly higher in group A than that in group B respectively (P < 0.05), while that of the left colon no significant differences between the two groups (P > 0.05). Conclusions Both regimens met the requirement of conventional colonoscopy and therapy, while rapid drinking PEG-ES within 1 h provides more better colonic cleansing quality for colonoscopy preparation.
7.Effects of neuromuscular electrical stimulation on swallowing and the velocity of hyolaryngeal excursion in post-stroke dysphagia

Objective To observe the effect of neuromuscular electrical stimulation (NMES) at different intensities on functional swallowing and the velocity of hyolaryngeal excursion in post-stroke dysphagia.Methods Thirty stroke survivors with dysphagia were randomly divided into a control group,an NMES group and an intensive NMES group,each of 10.All 3 groups received conventional swallowing training,while the patients in the NMES group also received NMES at 30 ~ 80 Hz in an intensity of ≤ 25 mA once a day and those in the intensive NMES group received it twice a day.Before,as well as 2 and 4 weeks after the treatment,video fluoroscopy when swallowing pap was used to determine the superior and anterior excursion distances of the hyoid and larynx and the excursion's duration and the corresponding velocity.A water drinking test (WDT) was also administered,and dysphagia severity scale (DSS) and penetration-aspiration scale (PAS) ratings were assessed.Results After the treatment,significant improvement was observed in the average WDT,DSS and PAS scores of all three groups compared to before the treatment.The average WDT score of the intensive NMES group was significantly better than that of the control group after 2 weeks of treatment.After 2 and 4 weeks of treatment,the average DSS of the intensive NMES group was significantly better than that of the control group,while the intensive NMES group's average PAS score was significantly better than those of both of the other groups.The average anterior hyoid excursion velocity of the intensive NMES group was significantly faster than those of the other two groups after both 2 and 4 weeks of treatment.Pearson correlation analysis indicated that the PAS score was significantly correlated with the anterior hyoid excursion velocity.Conclusion Two NMES sessions a day are superior to only one session in improving functional swallowing after stroke.It better promotes quick movement of the body parts involved.
8.Effects of Allicin on myocardial fibrosis and TLR4/NF-κB pathway

Objective:To observe the effect of Allicin in cardial fibroblasts (CFs) proliferation and Collagen secretion,and to explore its role on TLR4/NF-κB signal pathway.Methods:CFs of neonatal Wistar rats were isolated and cultured ,then was stimulated with AngⅡ.CFs proliferation was measured by thiazolyl blue ( MTT) assay.The expression of collagenⅠ,collagenⅢ was measured by ELISA.mRNA expression of TLR4 and NF-κB were detected by reverse transcription-polymerase chain reaction ,protein expression of TLR4 and NF-κB were detected with Western blot.Results: Allicin could reduced MTT value of cardial fibroblasts ( P<0.01 ) , and inhibited expression of collagenⅠ,collagenⅢ(P<0.01),which in a dose-dependent manner.Allicin could reduced mRNA expression of TLR4 and NF-κB and protein expression of TLR 4 and NF-κB in CF induced by Ang Ⅱ ( all P<0.01 ) .Conclusion: Allicin can inhibit Myocardial fibrosis ,which mechanism is possible by inhibiting TLR 4/NF-κB signal pathway.

Diagnosis of acute heart ischemia induced by coronary artery spasm(CAS)is not clear yet.We have systematically studied the histopathological,enzyme-histochemical and ultrastructural of the autolytic and the ischemic changes ofthe raf's heart induced by the injection of the pitutrin into the rat's sublingualvein.The PTAH stain demonstrated that some irregular transverse bands hadappeared in the muscular fibers.The adenosine triphosphatase activity in smallarteries was decreased.The author suggests that the results are helpful fordignosis of acute heart ischemia caused by CAS.
10.Clinical evaluation of endoscopic submucosal dissection with improved snare for colorectal polyps and submucosal tumors

Objective To evaluate the therapeutic effectiveness and safety of endoscopic submucosal dissection (ESD) with improved snare for colorectal polyps and submucosal tumors.Methods The clinical data such as clinical pathological characteristics, en bloc resection rate, curative resection rate, complications and follow-up results of 20 patients with colorectal polyps or submucosal tumors, who were treated by ESD, were retrospectively analyzed.Results 20 patients (total 24 lesions) with colorectal polyps or submucosal tumors were performed ESD operation successfully, in which 16 cases (total 20 lesions) were treated by ESD with improved snare (the trial group), 4 cases were treated by ESD with HOOK knife (the control group).The surgical success rate was 100.0% (24/24), the en blot resection rate was 100.0% (24/24). The mean polyps diameter in the trial group was (2.4 ± 0.9) cm (1.2~4.0 cm), while that in the control group was (2.2 ± 0.6) cm (1.5~3.0 cm). The mean procedure time in the trial group was (83.3 ± 23.9) min (45.0~120.0 min), while the control group was (66.2 ± 15.4) min (45.0~80.0 min), compared with the control group, the trial group need more times to complete the operation. The postoperative pathology as follows: 15 cases were adenoma (including 3 cases of adenoma with partial gland high-grade intraepithelial neoplasia), 1 case was hyperplastic polyp, 1 case was high-grade intraepithelial neoplasia, 2 cases were neuroendocrine tumor (total 4 lesions) 1 case was stromal tumor. Except for 1 case of stromal tumor from the muscularis propria was taken full thickness resection, no patient appeared complications such as hemorrhage, perforation, infection. No patient need intraoperative and postoperative surgical treatment.Conclusion Improved snare can be used to endoscopic submucosal dissection for large colorectal polyps and submucosal tumors safely and effectively, expanded the use and indications of snare in endoscopic resection, but need more times to completed the operation.
