2.Strategy of Service and Maintenance of Large-scale Medical Equipment

With wide range applications of the ultra-large-scale integrated circuits, multilayer circuit board and components surface welding technology, clinical medical engineers have been confronted with challenges in the service and maintenance of largescale medical equipments and the traditional model has faced the great challenges too. The clinical medical engineers should be actively get involved in operation, maintenance and service of large-scale medical equipment, in the same time; it's necessary for buying suitable service supply, protecting the smooth function of large- scale equipment and meets the clinical needs.
3.DETERMINATION OF VITAMIN E IN PEANUT OIL AND SOME FOODS BY FLUOROMETER AND TLC SCANNER

Vitamine E in peanut oil was determined by means of f Juorometric method (Ex = 285nm, Em=324nm) and TLC scanning method (? = 292nm). Both methods include saponification of the oil, isolation o'f the unsaponifiable matters and the respective final measurements. The result of the former method indicates the total vitamine E in terms of ?-vitamine E and the latter method may indicate their ?,?,? and 6 analogues separately. These two methods are simple, time-saving and reliable. The recovery of VE and the standard deviation of replication of fluorometric method and TLC scanning method are 99.3%, ? 0.587mg/100g sample and 103.04%, ?0.959mg/100g sample respectively. Both methods are also applicable to other foods such as fortified cakes, confectionaries, infant foods etc., but the oil in these samples must be extracted before saponification.
4.The value of computer aided detection in digital chest radiograph on the diagnosis of pulmonary nodules

Objective:To evaluate the computer aided detection system on the diagnosis of pulmonary nodules. Methods: 120 cases of examined patients with solitary lung nodules up to 9-30 mm in diameter were evaluated. All nodules had been verified by means of biopsy. 120 cases of healthy patients were selected as control group on the basis of confirmation on the chest CT. All chest radiograph in the two group were obtained with a digital radiography system. Five experienced chest radiologists and five residents detected the chest radiograph with or without CAD output images. The scales of performance were evaluated with receiver operating characteristic curve(ROC curve). As the development of clinical medicine has depended on modern medical equipment increasingly,clinical medical engineers have become the bridges between modern clinical medicine and engineering.They keep the normal running of hospitals and promote the hospital modernization.The equipment service & maintenance isn't the only responsibility of medical engineers.In fact,engineers should be responsible for all medical activities relevant to engineering equipment.The main tasks of engineers should shift from the equipment service & maintenance to the application safety and quality control in the whole process of the equipment usage,which include equipment application safety and risk control,failure service and preventive maintenance,quality control and management,measurement and monitor of adverse event,etc.
6.Development of multi-body-position tripod of X-ray apparatus for field battle

Objective To develop new-type multi-body-position tripod of X-ray appratus for field battle.Methods One screw was added to the bottom of axis stayer of the original tripod for the inversion of fixed bulb so that the bulb could vertically project.Three support feet were porlonged at the same time.Results The X-ray appratus of the new-type multi-body-position tripod could project for patients in satnding or lying position and the range of focus adjustmet was enlarged,which could meet nomal requirements.The new-added crew was firm and its support quality was greatly over the quality of X-ray appratus.Compared with the original one,the barycenter of the appratus was lower,and the hemline length was longer.It remaind portable for the overall quality and length didn't change apparantly.Conclusion The new-type multi-body-position tripod of X-ray appratus for field battle meets the practical field requirments.
7.CT,MRI and pathological analysis of peripheral primitive neuroectodermal tumors

Objective To analyze the CT and MRI appearances and to improve the knowledge of peripheral primitive neuroectodermal tumors(pPNETs).Methods CT or MRI were performed in 19 patients of pPNETs,which were confirmed by pathology.Results All the tumors were with unclear margin.Three cases occurred in intracalvarium,7 in the extremities,2 in the chest,4 in the abdomen,and 3 inside the spine.CT appearance of the tumors arising from soft tissue showed large,ill-defined,non-calcified mass and heterogeneous appearance with hypodense cystic areas.The tumors demonstrated heterogeneous contrast enhancement.The pPNETs arising from bone demonstrated extensive lytic lesion with large soft tissue mass,one case with newly-born bone and demonstrated heterogeneous contrast enhancement.The tumors demonstrated homogeneous intensity as muscle on SE T1WI and heterogeneous hyperintense signal on T2WI.Immunohistochemically,tumor cells showed positive for CD99,Syn,GFAP and negative for LCA.Conclusion The pPNETs show no characteristic manifestations on CT and MRI.However,CT and MRI can show the intra-tumor structures and the extent of the tumor very well,which is helpful in differentiating diagnosis,predicting resectability,detecting distant metastases and evaluating the response to treatment.
8.Investigation of aquaporin-1 expression and diffusion weighted imaging with multiple b values in adrenal gland in a rat model of diabetes mellitus

Objective To investigate the character of aquaporin-1(AQP-1)expression in adrenal gland in diabetes mellitus and evaluate adrenal gland damage and function alterations by DWI with multiple b values. Methods Twenty male Sprague-Dawley rats were randomly selected by computer and randomized into 2 groups:untreated controls(n=10)and diabetes(DM)(n=10). Rats in diabetes group were fed with high-sucrose and high-fat diet, controls were fed with common diet. After fed with high-sucrose and high-fat diet for 4 weeks, rats in diabetes group were injected with streptozotocin(STZ). Forty days after diabetes induction with streptozotocin(STZ), MR imaging was performed in a 7.0 T scanner. Venous blood from the tails was collected before MRI scan to measure blood glucose, blood glucose more than 16.7 mmol/L wasregarded as diabetic status. All the rats underwent DWI with 18 b values(0 to 4500 s/mm2). Maps of pure diffusion coefficients(D), pseudo-diffusion coefficients(D*)and ultra-high ADC(ADCuh)were acquired. Rats were sacrificed after MRI scan for adrenal gland histopathology, AQP-1 immunohistochemistry analysis and AQP-1 optical density(OD)measurements. Student t test was used to compare the difference of D*, D, ADCuh and OD of AQP-1 between two groups. Results Eight diabetic animals developed hyperglycemia(two rats died during the modeling process). MRI scan was performed in all of the 18 rats. Signal intensity of D*map, D map and ADCuh map decreased gradually. ADCuh increased significantly in DM animals(0.24 ± 0.06) × 10-3mm2/s compared with control animals(0.18 ± 0.07) × 10-3 mm2/s(P<0.05), whereas there was no significant difference found between the two groups in their respective D*and D values(P>0.05). There was a noticeable increase in the AQP-1 labeling in the adrenal cell membrane and cytoplasm in DM animals compared with control animals. DM rats showed an increased OD value of AQP-1 in adrenal gland compared with the control animals(P<0.05). Conclusions We found significantly higher AQP-1 expression in adrenal gland in DM animals compared with controls. Ultra-high b-Values DWI may work as a useful way for noninvasive evaluation the change of adrenal function in DM.
9.Clincal study of treatment for refractory diabetic wound

Objective To explore the repair method for refractory diabetic wound. Methods A total of 206 patients with refractory diabetic foot ulcers were treated with proper surgical treatments.Results Of all, 106 patients were treated by skin flap (51.5 % ), with one stage wound healing rate of 85.8%; 122 patients were repaired with split-thickness skin graft ( 59.2% ), with survival rate of the graft for 79.5%. Simple toe amputation was made in 34 patients (46 toes). The high level amputation was performed in 56 patients (27.2%). Of all, 132 patients were followed up for 6-18 months, which showed that ulcer recurred in 12 patients (9.1%). Conclusion Timely and effective treatment as well as flap and skin graft repair could reduce high level amputation rate of diabetic foot ulcer and promote the quality of life.
10.Effect of highly selective nerve injury on skin wound healing in rats

ObjectiveTo establish a rat model of pure sensory nerve injury or pure motor nerve injury combined with skin defect to examine the effect of highly selective nerve injury on skin wound healing.MethodsA total of 90 male Sprague-Dawley rats were randomly and equally divided into three different groups including Group A (posterior rhizotomy + cutaneous excision wounding group),Group B (anterior rhizotomy + cutaneous excision wounding group) and Group C ( sham operation + cutaneous excision wounding group).The wound healing rate was detected at days 2,7,14 and 21 after injury.The CGRP mRNA expression was determined by reverse transcription-polymerase chain reaction (RT-PCR) at days 1,3,7 and 14 after injury.The Bcl-2 protein expression was determined by immunohistochemistry at days 3,7,14 and 21 after injury.ResultsThe wound healing rate in the Groups A and C was significantly higher than that in the Group B at day 2 ( P < 0.05 ),with no statistical difference between Group A and Group C (P > 0.05 ).The wound healing rate in the Group C was higher than that in the Groups A and B at days 7 and 14 (P <0.05),with no statistical difference between Group A and Group B (P >0.05).The wound healing rate in the Groups B and C was higher than that in the Group A at day 21 ( P < O.05 ),with no statistical difference between Group B and Group C ( P > 0.05 ).The CGRP mRNA was expressed in three groups and the expression was up-regulated after wound.The CGRP mRNA expression in the Groups B and C was significantly higher than that in the Group A at days 1,3 and 7,with no statistical difference between Group B and Group C ( P > 0.05 ).The CGRP mRNA expression was distributed as Group C > Group B > Group A at day 7 ( P < 0.05 ) and Group B > Group A > Group C at day 14 (P < 0.05 ).Immunohistochemistry indicated that there was no statistical difference in Bcl-2 protein expression at day 3 after injury in all groups; that the Bcl-2 protien expression in the Group C was significantly higher than that in the Groups A and B at day 7 ( P < 0.05 ),with no statistical difference between Group A and Group B ( P > 0.05 ) ; that the Bcl-2 protien expression was distributed as Group C > Group B > Group A at day 14 ; that the Bclo2 protien expression in the Group B was higher than that in the Groups A and C at day 21 (P <0.05),with no statistical difference between Group A and Group C (P > 0.05).ConclusionsIn wound healing process,the sensory nerve plays a more important role than the motor nerve.Denervation of the nerves ( especially the sensory nerves) is not conducive to wound healing,and complete innervation is necessary for normal wound healing.
