1.Analysis on hospitalization expenses of elderly inpatients covered by medical insurance in Beijing and the influential factors

Objective To analyze hospitalization expenses and influential factors of elderly inpatients with medical insurance,and to provide scientific evidences for curbing hospitalization expenses and adjusting medical insurance policy.Methods Based on hospitalization data derived from Beijing town during 2007-2011 from 65082 elderly urban inpatients under medical insurance,descriptive analysis and multiple stepwise linear regression methods were used to analyze the hospitalization expense and its influential factors.Results The last 5 year witnessed rising hospitalization expense and average hospitalization expense per admission of elderly patients with medical insurance; among the components of hospitalization expense,cardiovascular/cerebrovascular/tumor/respiratory system diseases cause the heaviest medical burden; days of stay,hospital level,out-of-pocket proportion are the most important influential factors on their hospitalization expense per time.Conclusion The hospitalization expense of elderly patients with medical insurance keeps growing.Therefore,shorter days of stay,rational referral of patients to appropriate hospitals,adjusting out-of-pocket proportion would be effective measures of curbing hospitalization expense.
2.Clinical Efficacy of Long-pulsed 1064 nm Laser Combined with Amorolfine Hydrochloride in the Treatment of Patients with Onychomycosis

Objective:To investigate the clinical efficacy of long-pulsed 1064nm laser combined with amorolfine hydrochloride in the treatment of patients with onychomycosis. Methods:Totally 106 cases of patients with onychomycosis were randomly divided into the observation group (n=53) and the control group(n=53) according to the random number table. The control group was given 5%amorolfine hydrochloride liniment, while the observation group was treated with long-pulsed 1064nm laser additionally. The course of treatment was 6 months. The clinical efficacy, length of deck in ward areas, length of deck in health zone, clearance of mycology and adverse reactions were compared between the groups. Results:The total effective rate of the observation group(90. 57%) was signifi-cantly higher than that of the control group(75. 47%, P<0. 05). After the treatment, the length of deck in ward areas in both groups were significantly decreased, while the length of deck in health zone were significantly increased (P<0. 05). The length of deck in ward areas in the observation group was significantly lower than that in the control group, while the length of deck in health zone was significantly higher than that in the control group(P<0. 05). The clearance rate of mycology in the observation group (86. 79%) was significantly higher than that in the control group (67. 92%, P<0. 05). There were no serious adverse reactions during the treatment course. Conclusion:Long-pulsed 1064nm laser combined with 5% amorolfine hydrochloride liniment in the treatment of patients with onychomycosis has remarkable, safe and reliable clinical effects, which shows important study value.
3.A childhood nasopharyngeal carcinoma with huge mass in skull base.

The patient chiefly complained about headache and facial pain with recurrent epistaxis during the past two months. The pain started from the teeth and intermittent pinching headache especially on the left side. She also had mild epistaxis which healed itself. Later on the epistaxis became severe which couldn't be easily cured. Her nasopharyngoscopy examination showed a mass in nasopharynx while cranial CT scan indicated a huge mass in nasopharynx with erosion of skull base. Enhanced CT scan showed significant thickness in nasopharynx, upwards which became a huge mass breaking through skull base especially on the left side. Adjacent parapharyngeal space and paranasal sinuses and skull base were widely involved with the enlargement of neck lymph nodes. The chronic mastoiditis, ethmoiditis and maxillary sinusitis were also found.

4.Adjustment of levothyroxine during pregnancy in women undergoing thyroid carcinoma surgery

Objective Hypothyroidism during pregnancy has been associated with impaired cognitive development and increased fetal mortality.In this retrospective study we attempt to identify the timing and adjustment of levothyroxine during pregnancy.Methods 20 women planning pregnancy after thyroidectomy because of thyroid cancer were observed before and throughout their pregnancies.Their thyroid function was measured before conception,and approximately every 4 weeks during pregnancy.The dose of levothyroxine was increased to maintain the thyrotropin concentration at preconception values throughout pregnancy.Results 21 pregnancies occurred in the 20 women and resulted in 20 full-term births.One woman had abortion.Their basal dose was all 100 μg.The dose increased by 12.5 μg a time to target the thyrotropin concentrations (0.1-0.25 mU/ml).The mean levothyroxine requirement increased 50 percent during the first half of pregnancy and plateaued by the 16th week.This increased dose was required until delivery.All the newborns and their mothers were healthy according to the follow-up.Conclusions We propose that women after thyroid cancer surgery should increase levothyroxine dose as soon as pregnancy is confirmed.Thereafter,serum thyrotropin level should be monitored and the levothyroxine dose adjusted accordingly.The adjustment dose is set at 12.5 μg and the maternal thyrotropin concentration is between 0.10 and 0.25 mU/ml,which guarantees healthy growth of fetus while suppresses thyroid tumor growth simultaneously.
5.Effects of intracorporeal anastamosis and extracorporeal anastamosis on abdominal infection associated with laparoscopic right hemicolectomy

Objective:To compare the effect of intracorporeal anastamosis and extracorporeal anastamosis on abdominal infection associated with laparoscopic right hemicolectomy.Methods:The clinical data of 210 patients with colon cancer who underwent laparoscopic right hemicolectomy in Dalian Third Peoples′s Hospital, Liaoning Province from January 2015 to December 2019 were analyzed retrospectively.Among them, 79 patients underwent intracorporeal anastamosis (intracorporeal anastamosis group) and 131 patients underwent extracorporeal anastamosis (extracorporeal anastamosis group). The perioperative indexes and postoperative abdominal infection were compared between the two groups.Results:In intracorporeal anastamosis group, the intraoperative bleeding was (45.2±4.2) mL, the operative time was (161.3±22.4) min, the number of lymph node dissection was (30.8±9.6), the postoperative exhaust time was (3.3±1.2) d, and the postoperative hospital stay was (7.6±0.5) d. In extracorporeal anastamosis group was (42.1±5.0) mL, (167.3±26.7) min, (32.9±8.6), (3.4±1.0) d and (7.5±0.6) d, respectively, there were no significant difference between the two groups (t value were 0.417, 0.207, 0.829, 0.338 and 0.293, respectively; P value were 0.699, 0.845, 0.231, 0.734 and 0.802, respectively). In intracorporeal anastamosis group, the incidence of abdominal infection (with anastomotic fistula)was 13.9%(11/79), the incidence of abdominal infection (without anastomotic fistula)was 10.1%(8/79), and in extracorporeal anastamosis group was 1.5%(2/131)and 0.8%(1/131), the differences were statistically significant (χ 2=12.805, 10.238; P=0.003, 0.008). In intracorporeal anastamosis group, the incidence of respiratory system infection was 1.3%(1/79), the incidence of urinary system infection was 2.5%(2/79), the incidence of surgical incision infection was 1.3%(1/79). In extracorporeal anastamosis group was 3.1%(4/131), 0.8%(1/131) and 3.1%(4/131), respectively.There were no significant difference between the two groups (χ 2 value were 0.662, 0.420 and 0.662, respectively; P value were 0.364, 0.587 and 0.364, respectively). Conclusion:Laparoscopic right hemicolectomy with intracorporeal anastamosis and extracorporeal anastamosis have the same surgical effect, but intracorporeal anastamosis may increase the risk of postoperative abdominal infection.
6.A cross-sectional investigation on health-related knowledge and skills of patients in Chongqing City and its influencing factors

Objective To investigate the status on health‐related knowledge and skills among the patients in the Chongqing area and to provide basis for developing public health‐related knowledge and skills .Methods Seven hundred and sixty‐eight patients from 8 general hospitals were sampled ,using the stratified random sampling method .A cross‐sectional investigation on health literacy was conducted with questionnaire“ health awareness survey of patients”,and SAS8 .0 was used to statistically analyze the data .Results The awareness rate of health literacy in Chongqing was 4 .82% .As for the 3 health literacy related aspects including basic health concept and knowledge ,health lif‐estyle and behaviors ,and the health related skills ,the rates were 16 .28% ,2 .21% and 38 .41% respectively .Besides ,the rate of the 5 as‐pects including health conception ,the prevention of infection ,the prevention of chronic diseases ,the safety and the first aid ,the basic medical treatment of analysis were 10 .68% ,20 .31% ,8 .59% ,23 .57% ,10 .68% respectively .There noticed significant differences in different re‐gions ,education ,profession ,the condition of relatives engaged in medical ,medical insurance category(P<0 .05) .Profession and the condition of relatives engaged in medical were the main influencing factors on the level of health‐related knowledge and skills among the patients(P<0 .05) .Conclusion The prevalence of basic health‐related knowledge and skills is low in Chongqing city and we should make comprehensive interventions to improve health liter of patients .
7.The analysis and thoughts on traditional Chinese medicine international cooperation projects of China academy of Chinese medical sciences

8.Analysis and study on current status of European students in traditional Chinese medicine colleges and universities

The paper analyzed the scale, educational background, degree, and financial source of the European students in Chinese TCM colleges and universities by using excel 2013. The results showed that there were 19 TCM colleges and universities recruiting European students, and the number of campus students, enrollment students, graduation students were 409, 257 and 280 in 2017 respectively. The campus students in 2017 was highest since five years ago, but was less than the Afro-Asia states. Training was main form of studying abroad. Most campus students were at their own expenses, or received the help from Chinese government. The education for European students in Chinese TCM colleges and universities has made some achievement, but there were still some questions: the European students was not too much, the education's attraction was not strong, and the financial help from international organization and foreign government was scarce.
9.The current condition of traditional medicine in the United Republic of Tanzania and coping strategies on China-Tanzania traditional Chinese medicine cooperation

This paper summarizes and analyzes the current conditions of Tanzania traditional medicine practitioners, organization structure, herbal medicine resources, and the exported pharmaceutic products and related companies. The result shows that Tanzania government gradually pays the attention to the development of its own traditional medicine industry. Due to the current TCM cooperation dilemma with Tanzania, coping strategies should be adopted to provide certain references for the next stage of China-Tanzania TCM cooperation, such as adjusting the professional fields of dispatched experts, adjusting cooperative fields, including the export and investigation of TCM companies to Tanzania.
10.Current situation and development suggestions of traditional Chinese medicine in Germany

The paper introduces the current situation of traditional Chinese medicine (TCM) in Germany from the aspects of legislation and management, clinical treatment, education, scientific research, and medicinal plant resources. Based on this, this paper concludes that the major problems of TCM development in Germany are lack of effective legal protection, most TCM treatment therapiess are not covered by German health insurance, and the clinical level of TCM practioners is relatively low. Under the guildance of the Belt and Road Initiative, China should formulate relevant policies to promote international TCM cooperation between China and Germany, help Chinese institutions to build bridges with German institutions, cultivate German TCM talents, strengthen TCM cooperation on science and technology, and encourage Chinese enterprises to go global, therefore promoting the gradual development of TCM in Germany.
