1.Dorsal foot flap transplantation for skin and soft tissue defects of the hand:survival rate and success analysis

BACKGROUND:Skin and soft tissue defects of hands are easy to appear due to different factors, such as trauma. Flap transplantation is a clinical treatment for soft tissue defects of hands. Dorsal foot flap, which is thicker with constant vessel traveling, is very suitable for repair of hand soft tissue defects. OBJECTIVE: To explore the success rate and survival rate of dorsal foot flap transplantation in the repair of hand skin and soft tissue defects. METHODS: A retrospective analysis was performed on the clinical data of 86 patients with skin and soft tissue defect of hands. These patients were respectively given conventional skin wound skin grafting (control group) and dorsal foot flap transplantation (observational group). Hand appearance, infection rate, perception recovery and complications were observed during the postoperative folow-up. RESULTS AND CONCLUSION:Skin flaps al survived after grafting, and the texture of flaps was good. Patients were satisfied with hand shape and the recovery of hand function. After flap transplantation, one patient had local skin necrosis at the donor region, and the scar healed after dressing; another patient had venous crisis, and was given vascular re-anastomosis. In the observation group, the postoperative appearance was better than that of the control group, the infection rate was lower than that of the control group, the perception recovery was better, and there were fewer complications (P < 0.05). These findings suggest that the dorsal foot flap transplantation is better to repair skin and soft tissue defects in the hand.
2.Construction of obese mouse models with high fat diet feeding:relationship between nutritional factor and metabolic syndrome

BACKGROUND:Metabolic syndrome greatly harms the human body, and is affected by many factors. Through constructing diet-induced animal models, we can better analyze the relationship between nutritional factor and metabolic syndrome, and provide reliable references for the clinical treatment of this disorder. OBJECTIVE:To construct obese mouse models with high-fat diet feeding and discuss the relationship between nutritional factor and metabolic syndrome. METHODS:Thirty mice were selected and randomly divided into model group (n=20) and control group (n=10), and were fed with high-fat and normal animal feeds for 10 consecutive weeks. RESULTS AND CONCLUSION:Compared with the control group, after 1 week of feeding with high-fat animal feeds, body weight of mice in the model group raised, and differences gradualy increased with the feeding time increased. After 8 weeks of feeding, body mass index of mice in the model group significantly raised (P < 0.05). After 4 weeks of feeding, fasting venous blood glucose level of mice in the model group significantly raised, and showed a gradual rise trend with feeding time. After 5 weeks of feeding, fasting insulin level of mice in the model group also began to rise. The oral glucose tolerance test showed that mice in the model group showed a gradual downward trend of glucose tolerance with feeding time. After 8 weeks of feeding, serum levels of total cholesterol and high density lipoprotein cholesterol in the model group significantly raised (P < 0.05). After 10 weeks of feeding, serum levels of triacylglycerol, total cholesterol, low density lipoprotein cholesterol and high density lipoprotein cholesterol in the model group raised (P< 0.05). The results demonstrate that obese mouse models were successfuly constructed with high-fat diet feeding, which can simulate the natural progression of metabolic syndrome in human, moreover, the nutritional factor is closely related to metabolic syndrome.
3.Effects of KIM-1 on high glucose induced the expression of MCP-1 and FN in rat tubular epithelial cells

Objective To evaluate the effects of KIM-1 on high glucose induced the expression of MCP-1 and FN in rat tubular epithelial cells and to explore the possible mechanisms of KIM-1 involved in renal interstitial fibrosis of DN.Methods The rat renal tubular epithelial cells (NRK52E) were cultured in vitro and divided into five groups:Normal control group (D-glucose 5.6 mmol/L),Hypertonic group (D-glucose 5.6 mmol/L + D-mannitol 24.4 mmol/L),High glucose group (Dglucose 30 rmmol/L),Control siRNA group,KIM-1 siRNA group.ELISA assay was used to assess the levels of MCP-1 and FN in the cells supernatant; Western blotting was used to detect the protein expression of KIM-1; RT-PCR was used to detect mRNA expression of KIM-1,MCP-1 and FN.Results Compared with the control group,the protein and mRNA expression of KIM-1 in the high glucose group were increased at 12 h (P < 0.05),and reached the peak at 48 h (P < 0.05); the protein and mRNA expression of MCP-1 and FN in high glucose group were increased at 24 h significantly (P < 0.05),and peaked at 48 h (P < 0.05).Compared with the high glucose group,the protein and mRNA expressions of MCP-1 and FN in KIM-1 siRNA group were decreased (P<0.05).Conclusions Down-regulating the expression of KIM-1 can significantly inhibit the expression of MCP-1 and FN,which suggests that KIM-1 may be involved in renal interstitial fibrosis of DN by regulating expression of MCP-1 and FN.
4.Influence of primary resection and KRAS gene mutation in prognosis of mild symptomatic patients with stage Ⅳ colorectal cancer

Objective:To investigate the relationship between primary resection and KRAS gene mutation in the mild symptomatic patients with stage Ⅳ colorectal cancer,and to clarify its significance of prognosis.Methods:The clinical data of 46 mild symptomatic patients with stage Ⅳ colorectal cancer 2010 to December 2010 were collected.All the patients received primary resection.The KRAS gene mutation in the patients was detected by direct sequencing and the patients were followed up for 5 years.The influence of primary resection and KRAS gene mutation in prognosis of the patients with stage Ⅳ colorectal cancer was analyzed, and the clinical pathological features which might influence the prognosis were analyzed by survival analysis.Results:In 46 patients with colorectal cancer, KRAS gene mutation was found in 20 cases, the mutation rate was 43.4%, and most mutation was found at Codon 12. The KRAS mutation had relationship with the tumor site and multiple metastasis (P<0.05).The Kaplan-Meier survival curve and univariate analysis results showed that the median survival time of the patients with wild type of KRAS gene was 58.4 months, the median survival time of the patients with mutant type of KRAS gene was 42.2 months, and there was no significant difference (P>0.05).The median survival time of right colon cancer patients was 34.2 months, the median survival time of left colon cancer patients was 58.3 months, and there was sigificant difference (P<0.05).The cancer metastases including liver, lung and multiple metastasis were closely related to the poor prognosis of the colorectal cancer patients(P<0.05).The median survival time of the patients with stage Ⅳ colorectal cancer was 39.6 months after operation.Conclusion:After primary resection of the mild symptomatic patients with stage Ⅳ colorectal cancer,the median survival time of the patients with colorectal cancer in left colon site and right colon site were prolonged.Right colon cancer has more poorer prognosis than left colon cancer.KRAS gene mutation is associated with the tumor site and the multiple metastasis.The location of metastasis affect the prognosis.
6.Effect of KRAS gene mutation and clinical factors on postoperative prognosis of rectal cancer patients

Objective To investigate the effect of KRAS gene mutation and clinical factors on postopera-tive prognosis of rectal cancer patients and to explore their value in prognosis. Methods A total of 130 cases of rectal cancer patients from January to December 2010 were collected in the study. The tumor tissues sample was used to detect the KRAS gene mutation and 5-year follow-up was conducted. The correlation between KRAS gene mutation and clinical pathological features was analyzed.The clinic pathological factors that may affect the progno-sis were analyzed by survival analysis. Results Forty-five patients had mutations in No.2 expressed region of KRAS,with a mutation rate of 34.6%.KRAS gene mutation and stronger positive expression of EGFR(P<0.05), and multiple metastasis of tumor(P<0.05)were strongly coupled.The average survival of patients with wild-type KRAS gene was 57.5 months and that of patients with KRAS gene mutation 58.9 months but no significant differ-ence was observed(P>0.05).The TNM by high staging,multiple metastasis,lung metastasis and liver metasta-sis of cancer cells was closely related with poor postoperative prognosis of patients(P<0.05).The average surviv-al of postoperative patients in stage Ⅳ was 49months. Conclusions KRAS gene mutation in patients with rectal cancer after surgery is related with stronger positive expression of EGFR and multiple metastasis of cancer.TNM by high staging and metastatic sites affects the prognosis. The survival of rectal cancer after surgery in patients with stage Ⅳ are prolonged but the relation between KRAS genovariation and patients′ postoperative prognosis can not be determined.
7.A retrospective study on 1 152 patients with common bile duct stones

Objective To analyze the incidence of choledocholithiasis in patients who underwent cholecystectomy in our hospital,and to determine the incidences of recurrent stones and choledocholithiasis combined with cholangitis after treatment for choledocholithiasis.Methods A retrospective study was conducted on 1 152 patients with common bile duct stones treated from January 2013 to July 2017 in our hospital.Results Of the 1152 patients with common bile duct stones,572 patients had a history of cholecystectomy,accounting for 49.65% of all patients.402 patients had choledocholoithiasis combined with cholecystolithiasis (34.9% of all patients),278 patients had choledocholithiasis without cholecystolithiasis (24.1%).Choledocholithiasis could occur after cholecystectomy.The time of onset of choledocholithiasis was 5 ~ 10 years after operation in 49.1% of patients,and 200 of these patients with choledocholithiasis were treated with lithotripsy.There were 110 patients who underwent choledocholithotomy and cholecystectomy,and 90 patients who underwent ERCP and stone removal.There was no significant difference between the 2 groups (P > 0.05).Choledocholithiasis complicated with cholangitis developed in 511 patients (44.3% of all patients).These included 437 patients with acute mild cholangitis (85.5%) and acute obstructive suppurative cholangitis (14.5%).Recurrent choledocholithiasis combined with acute cholangitis after choledocholithotomy developed in 51 patients,and recurrent choledocholithiasis combined with cholangifts developed after ERCP in 50 patients.There was no significant difference between the two groups (P >0.05).Conclusions Choledocholithiasis after cholecystectomy accounted for a high proportion of patients.Recurrent choledocholithiasis could occur after choledocholithotomy.Common bile duct stones were susceptible to develop cholangitis.Thus,common bile duct stones should be treated as soon as possible.
8.Treatment of Allergic Rhinitis with Traditional Chinese Medicine: A Review

This article reviews the clinical studies about the treatment of allergic rhinitis (AR) with traditional Chinese medicine (TCM) in recent years. AR is a common and frequently occurring disease in the department of otolaryngology. The common manifestations of AR include nasal congestion, sneezing, runny nose, and nasal itching. AR, belonging to the category of rhinitis, is a dominant disease in the TCM treatment of otorhinolaryngologic diseases and head and neck diseases. The ancient literature has laid a solid theoretical basis on the etiology and pathogenesis of AR. According to the ancient literature, the theoretical basis, and their own experience, modern doctors classified the causes of AR into the disorders in Zang-fu organs and six meridians. Most of the explanations focus on the disorders in Zang-fu organs, especially the lung, spleen, and kidney. The clinical studies in this field mainly involve TCM treatment alone, integrated TCM and Western medicine treatment, and TCM external treatment. These therapies have good efficacy in the clinical treatment of AR. Among them, TCM treatment alone has significant advantages in alleviating the symptoms and nasal signs, declining the scores of related scales, and reducing the eosinophil count (EOS) of nasal secretions, with definite long-term efficacy. Integrated TCM and Western medicine treatment can complement with each other. The TCM external treatment methods include acupuncture, moxibustion, acupoint catgut embedding, acupoint application, and nasal irrigation with TCM, which are safe, simple, acceptable by patients and have good therapeutic effect. Finally, the clinical research status of TCM treatment of AR was summarized, and suggestions were put forward from three aspects: standardizing the clinical research protocol of TCM, encouraging pure TCM research, and exploring the mechanism of TCM treatment on the basis of frontier research achievements. This review aims to provide higher-level evidence for subsequent clinical research and promote the research on dominant AR diseases.
9.Treatment of Aural Vertigo Ménière's Disease with Traditional Chinese Medicine: A Review

Ménière's disease (MD) is an inner ear disease characterized by vertigo, tinnitus, hearing loss, and ear stuffiness. Modern therapies such as drugs, surgery, and vestibular function rehabilitation have limited effects in relieving the symptoms and reducing the recurrence. Traditional Chinese medicine (TCM) can alleviate the symptoms of MD with simple operation and mild adverse reactions while emphasizing psychological adjustment. The TCM treatment of MD is individualized depending on different stages and pathogenic factors. The internal treatment mainly targets phlegm, dampness, water, wind, fire, deficiency, and blood stasis. External interventions include acupuncture and moxibustion. This paper reviewed the published articles about the treatment of MD with TCM. In recent five years, the published studies were mainly clinical trials and experience discussion (or case reports), and few reports of fundamental research were published. In these studies, the Western medicine diagnosis of MD mostly refers to the Diagnostic Basis and Efficacy Evaluation of Ménière's Disease (Guiyang, 2006) and the Guidelines for Diagnosis and Treatment of Ménière's Disease (2017), while the TCM diagnosis mostly refers to the Criteria of Diagnosis and Therapeutic Effect of Diseases and Syndromes in Traditional Chinese Medicine issued by the National Administration of TCM in 1994. The efficacy was mostly evaluated based on clinical efficacy, scales, syndrome scores, pure tone audiometry, etc., while caboratory indexes were rarely used. The available clinical studies about the treatment of MD with TCM generally have low quality of evidence and single intervention means. In the future, the research on the treatment of MD with TCM can be improved by standardizing the research program, improving the quality of evidence, exploring more intervention methods, and strengthening basic research.

Since December 2019, the corona virus disease 2019 (COVID-19) caused by the 2019 novel coronavirus (2019-nCoV) has been reported in Wuhan, Hubei Province. Almost 70% of patients susceptible to 2019-nCoV are over age of 50 years, with extremely large proportion of critical illness and death of the elderly patients. Meanwhile, the elderly patients are at high risk of osteoporotic fractures especially osteoporotic vertebral compression fractures (OVCF). During the prevention and control of COVID-19 epidemic, orthopedists are confronted with the following difficulties including how to screen and protect OVCF patients, how to accurately diagnose and assess the condition of OVCF patients with suspected or confirmed COVID-19, and how to develop reasonable treatment plans and comprehensive protective measures in emergency and outpatient clinics. In order to standardize the diagnosis and treatment of patients with OVCF diagnosed with COVID-19, the authors jointly develop this expert consensus. The consensus systematically recommends the standardized emergency and outpatient screening and confirmation procedures for OVCF patients with suspected or confirmed COVID-19 and protective measures for emergency and outpatient clinics. Moreover, the consensus describes the grading and classification of OVCF patients diagnosed with COVID-19 according to the severity of illness and recommends different treatment plans and corresponding protective measures based on the different types and epidemic prevention and control requirements.
