1.Diagnostic value of combined detection of NGAL,L-FABP and KIM-1 in urine for early acute kidney injury in children

Objective To investigate the diagnostic value of combined detection of NGAL,L-FABP and KIM-1 in urine for early acute kidney injury in children.Methods 128 cases sepsis patients in our hospital during ICU from September 2015 to September 2016 were enrolled in the study,according to the occurrence of acute renal injury(AIK)were divided into observation group and control group,the incidence of AKI in 26 ca-ses as the observation group,102 cases without AKI as control group;the content of urine collected from two groups of children,NGAL L-FABP,KIM-1.Results In AKI group 6 h NGAL,L-FABP,Kim-1 in urine in-creased significantly higher than that of the other time points and non AKI group level,significant difference was statistically significant;6h NGAL,L-FABP,Kim-1 in urine combined detection efficiency is highest,sen-sitivity,specificity and positive pre detection value and negative predictive value were measured 86.7%,97. 8%,78.3% a,98.7% a,was significantly higher than the diagnostic efficacy of urinary NGAL,L-FABP,Kim-1;6h,L-FABP,NGAL in the urine level of KIM-1 and 48h in patients with creatinine and GFR levels of correla-tion analysis found that in children with early urinary NGAL,L-FABP,KIM-1 and 48h in children with creati-nine and GFR levels were positive related.Conclusion The detection of NGAL,L-FABP and KIM-1 in urine is of great significance in the diagnosis of AKI in children,which is worthy of clinical application.
2.Detection of ZnT-8A, IA-2A, IAA, GADA and ICA and 3-year onset risk of direct relatives of patients with type 1 diabetes mellitus

Objective To investigate the clinical value of detection of ZnT-8A,IA-2A,IAA,GADA and ICA and 3-year onset risk in direct relatives of patients with type 1 diabetes mellitus (T1DM).Methods The positive rates of serum zinc transporter 8 antibody (ZnT-8A) in T1DM patients (n =80),their direct relatives (n =100) and normal subjects (n =100) were detected by radioimmunoprecipitation assay.The islet cell antibody (ICA),glutamic acid decarboxylase antibody (GADA),protein tyrosine phosphatase autoantibody (IA-2A) and insulin autoantibody (IAA) were detected by Western Blot.The combined detection efficacy of different antibodies was evaluated with ROC curve and the 3-year onset of direct relatives was analyzed.Results The antibodies detection rates of serum ZnT-8A,IA-2A,IAA,GADA and ICA were significantly higher in T1DM group than direct relatives group and healthy group (P < 0.05),and the detection rates of above antibodies were significantly higher in direct relatives group than the healthy group (P < 0.05).The highest total positive rate detected by the five antibodies was 91.25%,and the highest total positive rates detected by the combination of 2,3 and 4 antibodies were 83.75%,87.5% and 90.0%,respectively.The area under ROC (AUC) of the 5 antibodies was the largest but there was no significant difference in AUC among different combinations of antibodies.All positive rates detected by combined antibodies were significantly higher than that of detection of ZnT-8A antibody alone (P < 0.05).During 3-year follow-up,direct relatives with ZnT-8A and GADA were diagnosed with T1DM,and there were no symptoms in the other individuals.Conclusion The combined detection of 5 antibodies can improve the diagnosis rate of T1DM.The combined detection of Zn T-8A and GADA is of great value in timely and accurate classification of diabetes mellitus,and they also can be used as indicators for epidemiological screening of direct relatives of T1DM patients.

4.Chain mediating effect of rumination and avoidance coping between information anxiety and suicidal ideation in college students

Objective:To explore the mediating roles of rumination and avoidance coping between information anxiety and suicidal ideation.Methods:In March 2022, a total of 896 college students were surveyed by the information anxiety scale(IAS), ruminative responses scale(RRS), coping style questionnaire(CSQ)and self-rating idea of suicidal ideation scale(SIOSS). The ANOVA analysis, Pearson correlation analysis and mediating effect analysis were performed by the SPSS 22.0 software.Results:The detection rate of suicidal ideation among college students was 9.15%(82/896). The information anxiety score was(73.84±17.29), the rumination score was(47.73±12.16), the avoidance coping score was(3.76±2.52), and the suicidal ideation score was(5.41±4.09). Information anxiety was significantly positively correlated with rumination, avoidance coping and suicidal ideation( r=0.49, 0.36, 0.37, all P<0.05). Rumination was significantly positively correlated with avoidance coping and suicidal ideation( r=0.42, 0.59, both P<0.05). There was a significantly positive correlation between avoidance coping and suicidal ideation( r=0.45, P<0.05). Information anxiety affected suicidal ideation of college students through four paths.The direct effect value of information anxiety on suicidal ideation was 0.06, accounting for 16.67% of the total effect. The effect values of the separate mediating effect of rumination and avoid coping were 0.22 and 0.04, and accounting for 61.11% and 11.11% of the total effect respectively. The chain mediating effect value of rumination and avoid coping was 0.04, accounting for 11.11% of the total effect. Conclusion:Information anxiety can directly affect suicidal ideation of college students and indirectly affect suicidal ideation through rumination and avoidance coping.

Consumptive thirst is widely discussed in Huangdi Neijing and it is classified as a"strange disease"in Suwen·Strange Diseases Treatise,which reflects the intractable nature of consumptive thirst.This paper explores and analyzes consumptive thirst based on the core concept in Huangdi Neijing.First,this paper approaches the subject through the use of image thinking method from Huangdi Neijing,recognizing that consumptive thirst can lead to changes in the internal climate of the human body.Then,guided by the theories of essence and qi,yin and yang,and the five elements in Huangdi Neijing,it deconstructs and analyzes the causes of these changes and the laws of qi transformation.It points out that the changing climate of consumptive thirst is characterized by"heat symptoms".The main cause of"heat symptoms"is spleen deficiency and excessive dampness,and its qi transformation law is the heat transformation of Shaoyin.The intractable nature of consumptive thirst is mainly reflected in the uncontrolled"heat symptoms"caused by the imbalance of the five elements.Based on the understanding of the heat symptoms of consumptive thirst,this paper proposes a treatment strategy for preventing the disease by resolving dampness and regulating the spleen,harmonizing kidney qi to prevent progression,and balancing yin and yang to treat chronic and recalcitrant conditions.The aim is to provide a reference for optimizing the treatment of consumptive thirst.
