1.Roles of Percutaneous Coronary Intervention in Improving Left Ventricular Function, Reducing Incidence of Cardiovascular Events of Patients With Ischemic Cardiomyopathy

Objective To investigate the roles of percutaneous coronary intervention in improving left ventricular function, reducing incidence of cardiovascular events of patients with ischemic cardiomyopathy. Methods Ninety cases of ischemic cardiomyopathy patients from August 2012 to August 2015 were randomly selected, these patients were divided into two groups, the study group (n=45) and control group (n=45) according to the random number table. The control group of patients were given routine treatment, while the study group of patients were given conventional treatment combined with percutaneous coronary intervention. Results The LVEDV, LVEF of the study group were significantly higher (P<0.05), the incidence of acute myocardial infarction onset, hospitalization rates again, mortality rate 8.89% (4/45), 13.33% (6/45), 2.22% (1/45) were significantly lower than the control group 24.44% (11/45), 40.00% (18/45), 17.78% (8/45) (P<0.05) . Conclusion Percutaneous coronary intervention can improve left ventricular function and reduce the incidence of cardiovascular events of patients with ischemic cardiomyopathy.
2.Clinical Significance of Combined Detection of Serum Retinal-binding Protein,Cystatin C and β2-micoglobuimin in Diagnosis of Early Renal Injures of Gestational Diabetes Mellitus

Objective To study the diagnosis value of serum retinol binding protein (RBP),cystatin C(Cys C) and β2 microglobulin (β2-M) in early renal injures of gcstational diabetes mellitus (GDM) patients.Methods 85 case of GDM pregnant women admitted to Shiyan Maternal and Child Health-Care Hospital from Jan.2009 to Jan 2015 were chosen as research objects,and were divided into simple diabetes group (35 cases),micro proteinuria group (30 cases) and massive proteinuria group (20 cases) according to the urinary albumin excretion rate (UAER),while 30 cases of healthy pregnant women were recruited randomly during the same period as control group.The 24h urine protein,serum RBP and renal function indicators [blood urea nitrogen (BUN),creatinine (Ser),Cys C and β2 M],the positive rates of RBP,Cys C,β2-M and combined detection of RBP,Cys C,β2-M of the four groups were compared.Results The 24h urine protein in simple diabetes group,micro proteinuria group and massive proteinuria group were significantly higher than that in the control group (t=3.91~ 16.33,all P<0.01),the difference between the 3 groups were statistically significant (t=6.78~ 16.94,all P<0.01).The levels of BUN,Scr,Cys C,β2-M and RBP in micro proteinuria group and massive proteinuria group were significantly higher than those in control group and simple diabetic group (t=3.68 ~ 18.54,all P<0.01),there were significant difference in above indexes between micro proteinurine group and massive proteinuria group (t=4.70~ 10.87,all P<0.01).The positive rates of RBP,Cys C,β2-M and combined detection of RBP,Cys C and β2-M in micro proteinuria group and massive proteinuria group were significantly higher than those in control group and simple diabetic group (x2 =20.27~38.57,all P<0.01).There was no significant difference in the positive rates between micro proteinuria group and massive proteinuria group (x2 =0.62~0.93,all P>0.05).The positive rate of combined detection of the three indicators was higher than that of the single detection in the same group (x2=3.97~6.65,P<0.05 or P<0.01).Conclusion The detection of serum Cys C,RBP and β2-M has a high clinical value in the diagnosis of early renal damage in patients with GDM.The positive rate of combined detection of 3 indexes was higher than that of single index.

4.Integrin α 4 Positive Subpopulation in Adipose Derived Stem Cells Effectively Reduces Infarct Size through Enhanced Engraftment into Myocardial Infarction

The efficacy of adipose-derived stem cells (ASCs) on myocardial infarction is limited due to poor survival and engraftment. Integrin-mediated cell adhesion is a prerequisite for its survival and homing. ASCs expressed insufficient integrin α 4 , limiting their homing capacity. This study aims to characterize integrin α 4+ ASC subpopulation and investigate their therapeutic efficacy in myocardial infarction. We used fluorescence-activated cell sorting to harvest integrin α 4+ ASCs subpopulation, which were characterized in vitro and transplanted into myocardial infarction model. Positron emission tomography imaging were performed to measure infarction size. Cardiac cine magnetic resonance imaging was used to evaluate heart contractile function. Compared with the unfractionated ASCs, integrin α 4+ ASCs subpopulation secreted a higher level of angiogenic growth factors, migrated more rapidly, and exhibited a stronger anti-apoptotic capacity. Vascular cell adhesion molecule-1 was obviously up-regulated at 3 days after myocardial infarction, which interacted with integrin α 4 receptor on the surface of ASCs to enhance the survival and adhesion. Thus, we implanted unfractionated ASCs or integrin α 4 Integrin α 4+ ASCs subpopulation exhibited more robust engraftment into the infarcted myocardium. Integrin α 4 ASCs subpopulation more effectively decreased infarct size and strengthen cardiac function recovery than did the unfractionated ASCs. Integrin α 4 ocardial damage in animal model. Mechanistically, their more robust engraftment into the infarct area, higher migratory capacity and their increased release of paracrine factors contribute to enhanced tissue repair.
5.Time to intubation with McGrath ™ videolaryngoscope versus direct laryngoscope in powered air-purifying respirator: a randomised controlled trial.

INTRODUCTION:
During the coronavirus disease 2019 (COVID-19) pandemic, multiple guidelines have recommended videolaryngoscope (VL) for tracheal intubation. However, there is no evidence that VL reduces time to tracheal intubation, and this is important for COVID-19 patients with respiratory failure.
METHODS:
To simulate intubation of COVID-19 patients, we randomly assigned 28 elective surgical patients to be intubated with either McGrath™ MAC VL or direct laryngoscope (DL) by specialist anaesthetists who donned 3M™ Jupiter™ powered air-purifying respirators (PAPR) and N95 masks. The primary outcome was time to intubation.
RESULTS:
The median time to intubation was 61 s (interquartile range [IQR] 37-63 s) and 41.5 s (IQR 37-56 s) in the VL and DL groups, respectively ( P = 0.35). The closest mean distance between the anaesthetist and patient during intubation was 21.6 ± 4.8 cm and 17.6 ± 5.3 cm in the VL and DL groups, respectively ( P = 0.045). There were no significant differences in the median intubation difficulty scale scores, proportion of successful intubations at the first laryngoscopic attempt and proportion of intubations requiring adjuncts. All the patients underwent successful intubation with no adverse event.
CONCLUSION
There was no significant difference in the time to intubation of elective surgical patients with either McGrath™ VL or DL by specialist anaesthetists who donned PAPR and N95 masks. The distance between the anaesthetist and patient was significantly greater with VL. When resources are limited or disrupted during a pandemic, DL could be a viable alternative to VL for specialist anaesthetists.

6.Efficacy and Safety of Xianling Gubao Capsule Combined with Glucosamine in the Treatment of Knee Osteoar- thritis:A Meta-analysis

OBJECTIVE:To systematically evaluate the efficacy and safety of Xianling gubao capsule combined with glucosamine in the treatment of knee osteoarthritis (KOA),and to provide evidence-based reference for clinical treatment of KOA. METHODS:Retrieved from Cochrane library ,PubMed,Embase,CNKI,Wanfang database ,CBM and VIP during the inception to Nov. 2020,randomized controlled trials (RCTs)about glucosamine (control group )combined with Xianling gubao capsule (trial group)in the treatment of KOA were collected. After data extraction ,Cochrane 5.1.0 system evaluator manual was used to evaluate the quality of the included clinical studies met inclusion criteria ,and Rev Man 5.4 software was used for Meta-analysis. RESULTS : A total of 11 RCTs with 970 patients were included in this study. Results of Meta-analysis showed that cure rate [OR =2.41,95%CI (1.48,3.93),P<0.001],marked effective rate [OR =1.95,95%CI(1.49,2.56),P<0.001],total effective rate [OR =4.00,95%CI (2.57,6.24),P<0.001],visual analogue scale (VAS)score [MD =-2.34,95%CI(-2.51,-2.17),P<0.001],knee function score [MD =31.32,95%CI(27.89,34.75),P<0.001],Japanese Orthopaedic Association (JOA)low back pain score [MD = 12.22,95%CI(9.68,14.76),P<0.001] and pain relief time [MD =-1.55,95%CI(-1.84,-1.25),P<0.001] of trial group were all significantly better than those of control group. The incidence of ADR in trial group was significantly lower than control group [OR =0.04,95%CI(0.02,0.12),P<0.001]. CONCLUSIONS :Xianing gubao capsule combined with glucosamine in the treatment of KOA is significantly better in cure rate ,marked effective rate ,total effective rate ,VAS,score,knee function score,JOA low back pain score and pain relief time ,and could significantly reduce the incidence of ADR.

Pulmonary hypertension (PH) is an extremely malignant pulmonary vascular disease of unknown etiology. ADAR1 is an RNA editing enzyme that converts adenosine in RNA to inosine, thereby affecting RNA expression. However, the role of ADAR1 in PH development remains unclear. In the present study, we investigated the biological role and molecular mechanism of ADAR1 in PH pulmonary vascular remodeling. Overexpression of ADAR1 aggravated PH progression and promoted the proliferation of pulmonary artery smooth muscle cells (PASMCs). Conversely, inhibition of ADAR1 produced opposite effects. High-throughput whole transcriptome sequencing showed that ADAR1 was an important regulator of circRNAs in PH. CircCDK17 level was significantly lowered in the serum of PH patients. The effects of ADAR1 on cell cycle progression and proliferation were mediated by circCDK17. ADAR1 affects the stability of circCDK17 by mediating A-to-I modification at the A5 and A293 sites of circCDK17 to prevent it from m1A modification. We demonstrate for the first time that ADAR1 contributes to the PH development, at least partially, through m1A modification of circCDK17 and the subsequent PASMCs proliferation. Our study provides a novel therapeutic strategy for treatment of PH and the evidence for circCDK17 as a potential novel marker for the diagnosis of this disease.
