Academic Journal of Second Military Medical University 1982;0(01):-
This article reviews the epidemiology, etiology, classification and hearing reconstruction of the microtia. The operative timing, the materials of the auricle stents, and the criterion for the successful reconstruction are also discussed. It is emphasized that the operative techniques, the prevention and treatment of the complications are very important in the auricle reconstruction using MEDPOR framework.

Objective To study the methodology of the ear reconstruction using Medpor framework. Methods Using Medpor material as ear framework, 112 cases of ear reconstruction were performed using the expanded skin flap on the mastoid region(method Ⅰ) and the temporal fascia flap adding skin graft ( method Ⅱ) to cover ear framework. Twenty-three cases adopted method Ⅰand 89 cases adopted methodⅡ. Results One hundred and nine cases of the reconstructed ear were successful with a good appearance and 3 cases were failure. The exposure of Medpor framework occurred in 17 cases in this series. Among them the method Ⅰ occurred in 8 cases (34 %) and the method Ⅱ was used in 9 cases (10%). The exposure of Medpor framework in 14 cases were repaired using local skin flap or local fascia flap and skin graft. In 3 cases Medpor framework had to be removed due to the sever exposure. Conclusion It seems that Medpor framework is a good alternative for the ear reconstruction.

Objective To investigate the effect of integrin expression on procollagen synthesis of fibroblasts in wound healing. Methods Antisense oligonucleotides were used to intercept the expression of integrin ? 5 or ? 1 subunit on fibroblasts respectively in wound healing, then the changes of procollagen mRNA were detected by RT-PCR. Results The antisense oligoncleotdes could specifically inhibit the expression of integrin subunit. Decreased expression of integrin?5 or ?1 subunit on fibroblasts significantly lowered mRNA of procollagen ? 1(Ⅰ) and ? 1(Ⅲ). Conclusion Reduction of integrin expression on fibroblasts in wound may inhibit procollagen synthesis in the level of transcription. These results provide the experimental gists on studying of scar reduction in wound healing.

ObjectiveTo study the effects of vitamins on human pr eadipocyte proliferation and differentiation. MethodsVit amins (A, B, C, D, E and K families) were added to the culture media of human p readipocytes, then the proliferation of cells, the expression of GPDH and lipid droplet accumulation in the cytoplasm were recorded. ResultsVitamin A inhibited human preadipocyte proliferation and differentiation. Vi tamin C enhanced differentiation and proliferation. Vitamin B 6 stimulated diff erentiation. vitamin D 3 inhibited differentiation. Vitamin E had a strong inhi bitory effect on human preadipocyte differentiation. Vitamin K 1 had an unexpec tedly great stimulatory effect on human preadipocyte differentiation. Vitamin K 3 inhibited both human preadipocyte proliferation and differentiation. Other v itamins tested showed no noticeable effects. ConclusionT hese data are advisory to our balanced daily intake of vitamins.

Nicotinic acetylcholine receptors concentrate on the postsynaptic membrane in the neuromuscular junction and participate in the neuromuscular transmission, which is of great importance to normal muscle function. The nicotinic acetylcholine receptor is a kind of pentamer. There are 2 types of nicotinic acetylcholine receptor due to the different subunit:fetal type ?-AChR (? 2???) and adult type ?-AChR(? 2???). This article reviews the difference of 2 types of nAChR in expression period, distribution, regulation mechanism and physiological significance.

Objective: To design the ribozymes to cleave human TIMP 1 mRNA, and embed them into U 6snRNA to make them stable. Methods: Ribozymes were designed according to the “hammerhead structure” described by Symons.Computer was used to analyze the possible cleavage sites. Results: Three ribozymes targeting the nt123, nt299 and nt353 on TIMP 1 mRNA were designed. Embedding ribozyme in U 6snRNA had little effect on its binding with the substrate. Conclusion: Computer assisted design is indispensable in studying ribozyme. Embedding ribozymes in U 6snRNA may be a good way to solve the problems existing in ribozyme study. [

Objective:To study the neural and vascular distribution in the triceps surae of rabbits for reconstructing the muscles of motor function. Methods: Triceps surae on one side was stained with the standard method of Sihler’s nerve staining, and the blood vessels on the other side were injected with a mixture of 30% barium sulfate and gelatin, and then they were X-photographed. The resulting pictures were compared to study the intramuscular nerve and blood vessel distribution. Results: Tricep surae on the first side became transparent or semi-transparent, and its shape kept intact after being stained. The intramuscular nerve branches were clearly visualized after being stained. The muscle was divided into 3 neuromuscular compartments and Lateral gastrocnemius (LG) was subdivided into 3 subunits. The intramuscular vascular configuration on the other side was also clear on soft X-ray films. The distribution maps of the nerves and blood vessels were grossly consistent. Conclusion: Sihler’s nerve staining is able to show the original 3-dimensional picture of the intramuscular nerve branches in the triceps surae of rabbits and can be used to observe the relation between the blood vessels and nerves in the muscle in combination with intramuscular angiography. According to the neurovascular distribution, tricep surae of rabbits can be divided into different subunits (compartments) as independent function units. The design and application of the subunit as a compartment can meet the need of muscular function after transplantation.

Objective To explore the methodology of skin flap muscle flap (double flaps) and and effect of the double flaps on eyelid bag blepharoplasty. Methods Through lower eyelid incision, skin flap and muscle flap were formed, orbicularis muscle and orbital septum separated between the skin and flaps, direct access to the inferior orbital rim depression.Arcuate edge of the orbicularis muscle was fully stripped and released, especially the orbicularis muscle at the inferior orbital rim periosteal surface. The orbital fat was released, sutured below the inferior orbital rim periosteal surface. The orbicularis muscle flap was designed, lift and fixed to enhance midface to achieve the rejuvenation effect in the lower eyelid region. Results The 36 patients treated had no hematoma and flinch in the lower eyelid.Within 3 to 18 months follow-up,there were no ectropion, and facial nerve damage and other complications. Conclusions Double-flap is a feasible method as it can effectively enhance midface rejuvenation and correct eyelid bags deformity.

Objective To study the methods and clinical effects of joint expansion of double fascia flaps wrapping the Medpor to perform full-ear reconstruction on congenital microtia deformity. Methods 60 patients with congenital microtia deformity were selected.including 22 left ears and 38 right ears.Majority had loss of ear auricle or only partial earlobe.Phase Ⅰ procedures included:temporal-parietal branch was used for the vascular pedicle, and mastoid area behind the ear was moved and expanded for fascia flap.Phase Ⅱ included:the joint expansion of double fascia flap wrapped Medpor to do full-ear reconstruction. Results Flaps survived and no complications for all 60 patients.Reconstructed ears had lifelike appearance and clear anatomical structures.Auriculocephalic angulars positions and sizes were consistent with the healthy ears.None of them had exposed prosthesis and scar contraction.Three-dimensional shapes were good. Conclusions Joint expansion of double fascia flap wrapping Medpor could avoid the chest wall deformity and exposed prosthesis. The fine structure of the reconstructed ear is satisfactory.
