1.Transforminal lumbar interbody fusion technique in treatment of upper lumbar intervertebral disc herniation

[Objective] To investigate the surgical method of transforminal lumbar interbody fusion(TLIF)technique in treatment of upper lumbar intervertebral disc herniation.[Methods]From January 2002 to December 2006,a retrospective analysis on TLIF surgery treatment of 45 patients with upper lumbar intervertebral disc herniation were carried out and they acquired 12~24 months(mean,15 months)follow up.There were 29 males,and 16 females aging from 30~55 years(mean,39.2 years).Eleven intervertebral spaces were T12-L1 hernination,25 intervertebral spaces were L1、2 hernination,and 18 intervertebral spaces were L2、3 hernination of the space.Nine were double disc herniation.All cases were treated with TLIF technique.A retrospective analysis on surgical therapeutic effect was made.[Results]Five patients developed complications after operation and recovered by symptomatic treatment.Patients moved around in 2~3 d after surgery.Oswestry score improved from(52.32 ?9.17)to(20.33 ?5.72)in an average 15-months follow-up.X-ray showed that all cases had bone graft fusion and no adjacent segment degeneration occurred.[Conclusion]TLIF technique is an effective method in treatment of upper lumbar intervertebral disc herniation.It can offer satisfactory clinical effects.
2.Liner exchange into a well-fixed acetabular shell in revision total hip arthroplasty

Objective To investigate clinical outcomes and complications of isolated polyethylene liner exchange for revision total hip arthroplasty.Methods From April 1995 to December 2007,80 patients (93 hips) underwent revision total hip arthroplasty during which only polyethylene liner was exchanged with reservation of acetabular cup.There were 41 males and 39 females,aged from 27 to 82 years (average,53.3 years).The duration from the primary THA to the revision surgery ranged from 0.3 to 18.4 years (average,10.9 years).The reasons for liner exchange included:polyethylene wear with osteolysis (78 hips),polyethylene wear without osteolysis (5 hips),polyethylene wear with stem loosening (4 hips),recurrent dislocation (3hips),infection (1 hip),periprosthetic fracture (1 hip) and liner dislodgement (1 hip).Forty-seven liners were fixed into the old cup using cement,and 46 were fixed with the original locking mechanism.Sixty cross-linked polyethylene liners and 33 conventional polyethylene liners were used.Results All patients were followed up for 5 to 15 years (average,7 years).The average Harris hip score improved from preoperative 86.0±16.9 to 89.4±11.6 at final follow-up.Complications included dislocation (10 hips),infection (2 hips),periprosthetic fracture (1 hip) and liner dislodgement (1 hip).Ten hips underwent rerevision due to different reasons:cup exchange (5 hips),conventional polyethylene wear (2 hips),infection (2 hips) and liner dislodgement (1 hip).Using component loosening as the end point,the 10-year survival rate was 100% in the cement fixation group and 84.8% in the original locking group.Using rerevision as the end point,the 10-year survival rate was 90.4% in the cement fixation group and 65.0% in the original locking group.Conclusion Liner exchange either with cement or original locking mechanism is a safe and successful method.Highly cross-linked polyethylene has a higher wear resistance,which can reduce incidence of osteolysis and improve survival rate of prosthesis.

ThO Lysholm scorO of thO obsOrvation group was (94.80 ± 4.18)points, which was signifi-cantly highOr than (91.02 ± 1.96)points of thO control group (t=2.674,P<0.05).Lachman tOst and KT-1000 wOrO improvOd significantly comparOd with thosO bOforO opOration ( all P <0.01), whilO thOrO was no statistically significant diffOrOncO bOtwOOn thO two groups(all P>0.05),but thO pivot-tOst of thO obsOrvation group was bOttOr than that of thO control group, thO diffOrOncO was statistically significant ( P < 0. 05 ). ThO ACL anglO of thO obsOrvation group was (51.52 ± 5.18)°, which was significantly lowOr than (55.86 ± 2.45)° of thO control group ( P<0.05). According to modifiOd Lysholm scorOs classification, in thO obsOrvation group, 24 casOs wOrO OxcOllOnt, 2 casOs wOrO good,1 casO was fair, and thO OxcOllOnt and good ratO was 96.3%. In control group,18 casOs wOrO OxcOllOnt,5 casOs wOrO good,2 casOs wOrO fair, and thO OxcOllOnt and good ratO was 92.0%. ThO diffOrOncO in thO OxcOllOnt and good ratO bOtwOOn thO two groups was statistically significant(P<0.05).Conclusion DirOct insOrtion anatomical rOconstruction is a safO and OffOctivO tOchniquO for rOconstruction of ACL. It can rOstorO thO stability of ACL and rotation of knOO joint.
4.Clinical effects of arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligamennt

Objective To explore the operating methods and clinical effects of reconstruction of anterior cruciate ligament with LARS artifical ligament. Methods From October 2012 to June 2015,the clinical data of twenty-four patients with arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligamennt in Capital Medical University Beijing Friendship Hospital Pinggu Hospital were analyzed retrospectively. All patients underwent anatomical and isometric reconstruction of anterior cruciate ligament to drill femoral and tibial tunnel. The recovery of knee joint function was evaluated by Lysholm scoring system,and the clinical effect was observed. Results All 24 patients were available for follow-up with an average of ( 22. 5 ± 1. 5) months. No wound infection occurred during the period. Joint instability and weak leg symptoms and knee pain were obviously improved,the scope of joint activities was increasing. The Lysholm scores improved from a preoperative score of (40.20±1.26) points to postoperative (92.20±2.18)points,and the difference between the two groups was statistically significant (t=2. 156,P<0. 05). According to the modified Lysholm scoring standard,among 24 cases,15 cases were excellent,7 cases were good,2 cases were fair,and the excellent and good rate was 91. 6%(22/24). Conclusion The reconstruction of the anterior cruciate ligament with LARS artificial ligament has the advantages of small trauma, definite curative effect and less complications. It can achieve the purpose of anatomic reconstruction of the anterior cruciate ligament and can effectively restore the stability of the knee joint.
5.Clinical efficacy of Qingpeng ointment in treatment of acute herpes zoster neuralgia

Objective:To evaluate the efficacy of Qingpeng ointment in the treatment of early herpes zoster neuralgia.Methods:From January to June 2019, sixty patients with herpes zoster (34 males and 26 females, aged 12 to 48 years, onset time of 1 to 7 days) were randomly divided into experimental group and control group. Both groups were given the same antiviral and neurotrophic therapy after admission. The experimental group was treated with Qingpeng ointment for external use in addition. The numerical rating scale (NRS), sleep score (AIS) and adverse reactions of the two groups were evaluated at pre-treatment, 3, 5, 7, 15 and 30 days after treatment.Results:Compared with pre-treatment, the NRS and AIS scores of the experimental group were significantly reduced at each time from day 3, and the difference was statistically significant. The NRS and AIS scores of the control group were reduced at each time from day 7, and the difference was statistically significant. Compared with control group, the NRS scores of the experimental group on the 3rd, 5th, and 7th days and the AIS scores on the 3rd and 5th days after treatment were lower than those of the control group, and the difference was statistically significant. There were no serious adverse reactions in the two groups.Conclusions:External application of Qingpeng ointment can quickly relieve early neuralgia in patients with herpes zoster, improve the patients' sleep, and there is no obvious adverse reaction.
6. Analysis of Rhizosphere Soil Fungal Community Composition and Diversity of Salvia miltiorrhiza Based on ITS Sequences

Objective: To study rhizosphere soil fungal community composition and diversity of Salvia miltiorrhiza,in order to explain the changes of the rhizosphere microenvironment of S.miltiorrhiza,and provide theoretical basis for revealing the reasons for the formation of continuous obstacles.Method: Based on the high-throughput sequencing technology,three kinds of soil samples were collected,namely soil of non-plant (CK),soil for cultivating for one year (Y1) and soil for cultivating for two years (Y2),respectively.According to the sequencing of the internal transcribed space of nuclear ribosomal DNA (ITS) region of fungal rRNA gene,14 153,19 024 and 7 622 valid sequences were obtained and annotated as 1 027,1 095 and 712 varieties of OTUs.The diversity of fungal communities showed a trend of decreasing first and then increasing.Agaricomycetes,Leotiomycetes and Tremellomycetes were the dominant bacterial communities at the class level,their orders in three treatments were Y1 > CK > Y2,Y2 > Y1 > CK,CK > Y2 > Y1.At the genus level,Guehomyces was dominant species in CK,and both Y1 and Y2 had a decrease of 96.13% and 90.21%,respectively.Wickerhamomyces had a significantly increase in Y2(6.64%),250.59% and 564.00% compared with CK and Y1,respectively.Result: The results of cluster analysis showed that CK and Y1 had a higher similarity at the genus level in soil fungal community composition compared with Y2.Conclusion: Cryptosporiopsis and other pathogenic fungal communities were dominant genuses according to the heatmap analysis in Y2,suggesting a correlation with the incidence of S.miltiorrhiza.The findings provide theoretical basis for the formation of continuous cropping obstacle.
7.Ginkgo biloba extract protects against depression-like behavior in mice through regulating gut microbial bile acid metabolism.

Depression is a mental disorder with high morbidity, disability and relapse rates. Ginkgo biloba extract (GBE), a traditional Chinese medicine, has a long history of clinical application in the treatment of cerebral and mental disorders, but the key mechanism remains incompletely understood. Here we showed that GEB exerted anti-depressant effect in mice through regulating gut microbial metabolism. GBE protected against unpredictable mild stress (UMS)-induced despair, anxiety-like and social avoidance behavior in mice without sufficient brain distribution. Fecal microbiome transplantation transmitted, while antibiotic cocktail abrogated the protective effect of GBE. Spatiotemporal bacterial profiling and metabolomics assay revealed a potential involvement of Parasutterella excrementihominis and the bile acid metabolite ursodeoxycholic acid (UDCA) in the effect of GBE. UDCA administration induced depression-like behavior in mice. Together, these findings suggest that GBE acts on gut microbiome-modulated bile acid metabolism to alleviate stress-induced depression.
