1.Application of intelligent information system of transfusion reactions in the supervision and management of transfusion reactions

【Objective】 To analyze the application effect of intelligent information system of transfusion reactions(referred to as the system) in the supervision and management of transfusion reactions. 【Methods】 A retrospective analysis was conducted on the clinical data of a total of 67 914 blood transfusion patients in our hospital from January 2018 to August 2023. The patients were divided into the control group (n =36 367, with conventional transfusion management) and the observation group (n =31 547, with management by the system).The incidence of adverse reactions, the supervision and management scores, reporting time and processing time of adverse reactions, and patient satisfaction with blood transfusion in the two groups was observed. 【Results】 The incidence of transfusion reactions in the observation group and the control group was 0.23% (72/31 547) and 0.24% (86/36 367), with no statistically significant difference (P>0.05). The quality scores of the reporting, registration, processing and supervision, and the total scores of the observation group were higher than those of the control group:(21.46±2.76) vs (17.38±2.74) points, (21.65±2.80) vs (16.98±3.20) points, (22.30±2.42) vs (18.80±3.15) points, (22.85±2.10) vs (18.55 ± 3.06) points, and (90.38±5.96) vs (75.26 ± 6.10) points, with statistically significant difference(P<0.05). The reporting time and processing time of transfusion reactions in the observation group were shorter than those in the control group: (3.26±0.45) vs (7.25±1.26) minutes, (10.38±2.96) vs (20.26±2.10) minutes, with statistically significant differences (P<0.05). The satisfaction rate with blood transfusion of the observation group was 99.95% (31 532/31 547), higher than 99.62% (36 229/36 367) of the control group, with significant difference (P<0.05). 【Conclusion】 The application of the system in the supervision and management of transfusion reactions has achieved ideal results, including improving the quality of monitoring and management, shortening the reporting and processing time of transfusion reactions, and improving patient satisfaction with transfusion, which is worth popularizing.
2.Realization of Long Bone Stitching Technology in DR Chest Radiography System.

OBJECTIVE:
Using a common DR chest radiography system to realize a long bone stitching technology.
METHODS:
Introduce the role of long bone stitching technology in medical diagnosis and treatment, and the principle of long bone stitching technology to make a long bone stitching radiographic device, and combine with the chest radiography system to take the long bone stitching image experiment.
RESULTS:
The hospitals of class Ⅱ (or more lower levels) can realize the long bone stitching technology using a common DR chest radiography system.
CONCLUSIONS
Using this technology can save the hospital costs, reduce the burden on patients, achieve good social and economic benefits.

3.A case of pulmonary aspergillus infection in underground coal mine workers.

The underground environment is dark and humid, and it is easy to breed pathogenic microorganisms. A lump in the right lung of a coal mine underground transport worker was found druing occupational health examination. CT examination showed that the lump was located in the posterior segment of the upper lobe of the right lung, with point strip calcification, liquefaction necrosis, and proximal bronchial stenosis and occlusion. MRI examination FS-T(2)WI and DWI showed "target sign", annular low signal around the central high signal, and low mixed signal around the periphery, and annular high signal in the isosignal lesions on T(1)WI. Then the pulmonary aspergillus infection was confirmed by pathology.

4.Medication therapy management of a diabetic patient complicated with cardiovascular disease

OBJECTIVE To provide reference for medication therapy management (MTM) of diabetic patients complicated with cardiovascular disease. METHODS A 63-year-old male diabetic patient who suffered from temporary headache every morning after percutaneous coronary intervention (PCI) visited the neurology department of our hospital, and then was recommended to the pharmaceutical outpatient department. The pharmacists thought that the patient’s symptoms of headache, severe constipation and hyperuricemia were more likely induced by the medication used. The pharmacists further found that his atherosclerotic cardiovascular disease (ASCVD) influencing factors such as blood pressure, heart rate, blood glucose and blood lipids did not reach standard. The pharmacists provided MTM services for the patient through pharmacy inquiry and adverse drug reactions judgement, medication evaluation, medication reconciliation, medication education and pharmacy follow-up. RESULTS Through fifteen MTM services for thirteen weeks, the pharmacists reconciliated and optimized the medication therapy plan, discontinued the use of Isosorbide mononitrate sustained-release tablets, Nifedipine controlled-release tablets, and Indapamide tablets, which caused adverse drug reactions; the number of drugs was adjusted from fifteen to seven, and the symptom of headache disappeared; severe constipation had also been significantly improved, and hyperuricemia dropped to normal range. The ASCVD influencing factors of blood pressure, heart rate, fasting plasma glucose, glycosylated hemoglobin, low-density lipoprotein cholesterol and uric acid were reduced from >140/90 mmHg(1 mmHg=0.133 kPa), 70-80 beats per minute, 7.71 mmol/L, 7.2%, 2.13 mmol/L and 494 μmol/L before MTM services to <130/80 mmHg, 55-60 beats per minute, 6.22 mmol/L, 6.3%, 1.55 mmol/L and 348 μmol/L after MTM services. CONCLUSIONS The pharmacists providing MTM services to the patients can improve their quality of life and therapeutic efficacy, reduce medication risks, and enhance the level of rational drug use in hospitals and pharmaceutical service capabilities.
5. Effect of mindfulness-based stress reduction on surgical stress of bronchoalveolar lavage in patients with pneumoconiosis

OBJECTIVE: To evaluate the intervention effect of mindfulness-based stress reduction(MBSR) on surgical stress of bronchoalveolar lavage(BAL) in patients with pneumoconiosis. METHODS: A total of 102 patients with pneumoconiosis who undergoing selective BAL were randomly divided into control group and MBSR group with 51 cases in each group by a convenient sampling method. The patients in the control group received routine nursing, the MBSR group received 5 times of MBSR treatment. All patients were investigated using the Self-Rating Anxiety Scale(SAS), Self-Rating Depression Scale(SDS) and Chinese Version Perceived Stress Scale(CPSS) before the first BAL and after the last BAL. RESULTS: Before the first BAL, the scores of SAS, SDS and CPSS in these two groups were compared and showed no statistical significance(P>0.05). After the last BAL, the scores of SAS, SDS and CPSS were lower in the MBSR group than that of the control group and the same group before the first BAL(P<0.01).CONCLUSION: The MBSR could effectively reduce the pressure perception level of patients with pneumoconiosis who undergoing selective BAL, improve their anxiety and depression and promote the smooth implementation of BAL.
6. Effect of mindfulness-based stress reduction on negative emotion and coping style of patients with newly diagnosed silicosis

OBJECTIVE: To investigate the effects of mindfulness-based stress reduction(MBSR) on anxiety, depression and coping style of patients with newly diagnosed silicosis. METHODS: A total of 156 cases of patients with newly diagnosed silicosis were randomly divided into the control group and the intervention group by convenient sampling method, with 78 cases in each group. Patients in the control group were given routine care only, patients in the intervention group received MBSR intervention for 8 weeks on the basis of routine care. The Self-Rating Anxiety Scale, Self-Rating Depression Scale and Medical Response Questionnaire investigated the scores of anxiety, depression and coping styles in the two groups before and after the intervention. RESULTS: Before intervention, there was no significant difference in the scores of anxiety, depression and each dimension of medical response such as facing, avoidance and resignation dimensions in the two groups(P>0.05). After intervention, the anxiety and depression scores of patients in the intervention group decreased [(54.9±3.8) vs(45.5±3.8) and(57.5±3.9) vs(48.7±3.3), respectively, P<0.01] compared with the control group. The facing dimension scores of medical response increased [(19.0±1.3) vs(21.8±1.2), P<0.01], and the scores of avoidance dimension and resignation dimension of medical response decreased [(13.0±2.9) vs(12.2±2.1) and(10.3±1.6) vs(8.0±1.5), respectively, P<0.05]. CONCLUSION: The MBSR training can improve the negative emotion such as anxiety and depression in patients with silicosis, and encourage them to cope with the disease in a positive way.
7.Effect analysis of the colposcopy quality control in 19 hospitals in Zibo, Shandong Province

ObjectiveTo find out the problems and deficiencies in colposcopy through the quality control of colposcopy in 19 hospitals in Zibo City, so as to put forward constructive opinions and suggestion to improve the accuracy and standardization of colposcopy by the doctors in local hospitals. MethodsA total of 21 hospitals with colposcopy examination in Zibo City were selected as the research object. The quality control standards were formulated and the case data in the early stage were evaluated. Two hospitals had less than 10 cases completed each year and no on-site quality control.A total of 19 hospitals had on-site quality control with the operation process of colposcopy instrument on site, the configuration of colposcopy, the proportion of colposcopy examination reagents used in the department of pathology, and the operation period of colposcopy examination, as well as the on-site observation report. The qualified rate of each hospital were counted and analyzed. ResultsThe quality control results of 19 hospitals showed that the qualifications and the clinical practice of obstetrics and gynecology for more than 3 years reached 100.00%. Those who complete more than 100 new diagnosis cases each year and those who have been trained for more than 3 months are gradually increasing. The pathological specimens met the needs and the positive predictive value of high-grade squamous intraepithelial lesion was significantly improved in the quality evaluation indicators of colposcopy (all P<0.05).In the basic elements of the colposcopy report, there were statistically significant differences in 5 aspects:adequacy of the examination, the description of specific images, the interpretation of key images, the diagnosis of the colposcopy, and the post-examination processing through 2 colposcopy quality control comparisons (all P<0.05). ConclusionThe overall level of colposcopy in Zibo City has been significantly improved through the comparison of the quality control results of two colposcopy. We can improve the quality of colposcopy effectively through regular quality control.
8.Study on the nerve fiber layer thickness and other parameters of optic disc in patients with retinitis pigmentosa

AIM: To study the nerve fiber layer thickness and other parameters of optic disc in patients with retinitis pigmentosa.
METHODS: Totally 40 patients(80 eyes)with retinitis pigmentosa from April 2015 to April 2016 in our hospital were selected as the observation group and 40 healthy subjects as control group. All the subjects were examined by OCT. Disc parameters of retinal nerve fiber layer(RNFL)thickness, rim volume, cup volume, cup vertical diameter ratio, cup level diameter ratio, cup area ratio, rim area, cup area, disc area in temporal, superior, nasal and inferior quadrant were compared.
RESULTS: The thickness of the whole circle, nasal, temporal, superior and inferior RNFL in the eyes of RP group was significantly higher than those in the normal control group, the difference was statistically significant(P<0.05); RP eyes group with rim volume, cup volume, cup vertical diameter ratio, cup level diameter ratio, cup area ratio, rim area, cup area, disc area parameters were significantly higher than those in normal control group, the difference was statistically significant(P<0.05).
CONCLUSION: The mean thickness of RNFL in patients with retinitis pigmentosa at temporal, nasal, the whole circle compared with healthy subjects are thicker, while cup volume, cup vertical diameter ratio, cup level diameter ratio, cup area ratio, rim area and cup area increased.
9.Practice and experience of standardized management of radiation hygiene in medical institutions

More than 90% of artificial irradiation comes from medical irradiation. New radiation technologies are constantly emerging in the medical field, bringing benefits to patients. At the same time, the harm of medical irradiation has attracted more and more attention. There are many problems in the supervision and management of radiation health in medical institutions, such as many standards and specifications involved in radiation health in medical institutions, uneven professional ability of personnel in primary health supervision institutions, inadequate implementation of the main responsibility for the safety of radiation diagnosis and treatment in medical and health institutions, and non-standard service of radiation health technical service institutions, etc. In view of the above problems, the implementation plan of standardization of radiation health supervision, radiation diagnosis and treatment behavior, and radiation technical service behavior has been set. After the pilot operation, the effect is obvious.
10.Application of two phacoemulsification modes in cataract surgery with different hard nuclear

AIM: To compare the clinical efficacy of the Balanced energy system versus the conventional torsional ultrasound system in phacoemulsification surgeries for cataracts with varying nuclear hardness.METHODS: In this study, 120 patients(122 eyes)with age-related cataracts scheduled for surgery between November 2021 and November 2022 at our hospital were randomly divided into two groups: 58 patients(59 eyes)in the experimental group underwent surgery using the Balanced energy system, while 62 patients(63 eyes)in the control group were treated with the conventional torsional ultrasound system. Intraoperative cumulative dissipated energy(CDE), case time(CT), aspiration time(AST), and estimated fluid used(EFU)were recorded. Patients were followed-up for 3mo to examine and record the best-corrected visual acuity(BCVA)and corneal endothelial cell density(ECD), and to calculate the rate of endothelial cell loss.RESULTS: Comparing the intraoperative parameters between the two groups, there was no significant difference in CT(P>0.05), but the CDE, AST and EFU of the patients in the experimental group were lower than those of the control group(P<0.05), and the CDE of patients with grade III nuclear hardness in the experimental group was lower than the control group(P<0.05), CDE, AST and EFU in patients with grade IV nuclear hardness were lower than those in the control group(P<0.05). After 3mo of follow-up, BCVA in both groups improved significantly, and the experimental group recovered faster than the control group. At 3mo after surgery, the ECD of the two groups of patients was reduced compared with that before surgery(P<0.01), but there were no significant differences in ECD and endothelial cell loss rates between the experimental and control groups before and at 3mo after surgery(P>0.05). In grade IV nuclear hardness cataracts, the rate of endothelial cell loss in the experimental group was significantly lower than that in the control group(4.63%±4.10% vs. 6.63%±4.49%, P<0.01).CONCLUSION: The Balanced energy system and the conventional torsional ultrasound system both show high safety and efficiency in phacoemulsification of cataracts with different nuclear hardness. However, the former demonstrates substantial advantages in cases with dense nuclei, offering lower ultrasound energy, shorter aspiration and infusion times, and reduced volume of infusion fluid.
